Physical Functions in Follow-up Convergency Care by Discharge Locations (퇴원장소에 따른 융합적 돌봄을 위한 신체기능연구)

  • Discharge plans should be considered during rehabilitation depending on the location specified by the patients. This study aims to compare the demographics and rehabilitation function according to discharge location in people with rehabilitation one month after discharge. Among 300 discharged patients, 146 were discharged to their homes, 154 were transferred to facilities such as nursing homes. The results showed that rehabilitation function that scored using the modified Barthel Index were different according to discharge location. That is, the modified Barthel Index scored greater in home discharged patients than counter group. However, the home discharged patient's physical function and daily activities showed scores that required rehabilitation. Differentiated strategies should be needed for home and facility visit rehabilitation programs for rehabilitated patients.

A Study on the Home Nursing Care Need Trajectory of the patients with chronic illnesses after discharged from Hospital (만성질환자의 퇴원후 가정간호 요구변화 추이에 관한 조사 연구)

  • The traditonal inpatient acute hospital setting is organized primarily for the intensive management of disease, but not well-suited for continuity of care for the chronically ill patients after being discharged from hospital. For the planning of the continuity of care, firstly, it is necessary to assess the home care needs of the discharged pateints in the context of the nursing diagnosis. Therefore, this study is designed to identify the home nursing care need trajectory of the patients with chronic illness after discharged from one of the the General Hospitals in Seoul, Korea. The subjects are the patients with chronic illness such as stroke, musculoskeletal disease, hypertension, cancer etc., in average age of 52. 2 years old. The findings of this study are as follows : 1) The limitaion of ADL has been constantly facing to the subjects and has not been changed 4 weeks after being discharged. And the sense of with-drawal was getting worse at 4th weeks than the 1st week after being discharged. 2) The lists of the patient's problems are the impairment of mobility, elimination pattern, inactivity, impairment of skin integrity, ineffective airway clearance, and potential anxiety, self concept deficit, ineffective family coing, etc. Those problems were diminished in quantity at the first week after discharged, but at the 4th week, those problems were getting worse. 3) The need of specialized nursing care such as tube feeding, ostomy care, $O_2$ inhalation, IV therapy, teaching and exercise are considered as the most consisting problems facing to the subjects. 4) In general, the chronically ill patients and their caregivers have not been adapted well even at the 4th weeks after being discharged. 5) Considering those findings, the basic care for patients should be given and the trainging for process of the adaptation after discharged should be encouraged prior being discharged from hostital. For this suggestion, the systematic discharge planning should be carried and the hospital based home nursing model should be implemented at the general hospital for the chronically ill patients.

  • A forty-eight-year-old female patient with ascending aortic aneurysm with aortic insufficiency underwent a modified Bentall operation. The ascending aorta and the aortic valve were replaced with a composite graft containing a St. Jude valve. The coronary orifices were anastomosed to the tubular Dacron prosthesis by means of a second smaller Gore-Tex tube, and a fistula between the aneurysmal sac and the right atrial appendage was created to drain oozing from the prosthesis. The postoperative course was uneventful and the patient was discharged without complication. She is doing well on the 14 months follow-up.

Thoracoscopic Pneumonectomy (흉강경을 이용한 전폐절제술)

  • We reported a successful right pneumonectomy using video assisted thoracoscopy for endobronchial tuberculosis. Thoracoscopic surgery has many advantages in spite of its technical difficulties. These advantages include good cosmetic effects, short hospital stay, decreased postoperative morbidity, and good postoperative pulmonary function.The patient was young unmarried female, and her right lung was nearly total obstructed by endobronchial tuberculous dissemination and secondary pulmonary infection. The procedure was a little complicated by inadequate placements of the thoracoscopic ports and 5cm length utility thoracotomy incision. Minimal pain killer was needed only two days after operation and scar of operative wound was very satisfying. Postoperative course was unusual and the patient was discharged 7 days after operation.

Immature Teratoma at Anterior Mediatinum - Report of one case - (종격동에 발생한 미성숙 기형종: 1례 보고)

  • We experienced one case of immature teratoma at anterior mediastinum. the patient was Syears old female whose complain were cough and chest pain. Chest film showed mass density St the lower half of the Rt chest. Chest CT showed inhomogenous mass which have some calcified area. At the time of operation, 12x10x13cm sized mass have smooth surface and its stump elongated to the thymus. a-FP level, preoperative 22.5ng/ml, was decreased to 9.7ng/ml after operation. Postoperative adjuvant chemotherapy was performed with Vinblastin, Bleomycin and Cisplatin combination. The patient had an uneventful postoperative or postchemotherapy course and was discharged in a good condition.

One Case Report of Primary Choriocarcinoma of the Lung (폐에 발생한 원발성 융모막 상피종 치험 1례)

  • Primary choriocarcinoma of the lung is extremely rare. The patient was 28-year-old female and had no specific signs and symptoms except right chest pain for 5 years. On simple chest film, 8X8 cm sized, well demarcated, homogeneous ovoid mass was found on right lower lung field. The qualitative urine \ulcorner-HCG was 17140 mIU/ml. The result of percutaneous needle biopsy highly suggested choriocarcinoma. Under the impression of primary choriocarcinoma of the lung, right middle and lower lobectomy was done. On 33 postoperative days, serum \ulcorner-HCG level was within normal limit, the patient was discharged without complications after one-cycle chemotherapy.

  • Congenital esophageal atresia without tracheoesophageal fistula which called isolated esophageal atresia is a very rare entity and the neonate classified as category C by Waterston`s classification has high mortality rate. We experienced a case of isolated esophageal atresia. This patient was a male with 1,750gm in body weight and had been suffered from bilateral pneumonia. The patient was managed with staged operation. Feeding gastrostomy was made as the first intervention and delayed primary anastomosis was performed 3 months later. The postoperative course was uneventful and he was discharged on the 22nd postoperative day.

Total Anomalous Pulmonary Venous Connection in Adult -A Case Report- (성인 총폐정맥 환류이상 -1례 보고-)

  • We experienced one case of TAPVC patient who was 39 years old man with mild dyspnea on exertion and easy fatigability. He was the oldest patient among TAPVC which was reported in Korea. Preoperative echocardiogram, cardiac catheterization and angiogram revealed supracardiac type TAPVC drained through left innominate vein. An anastomosis between common pulmonary venous trunk and left atrium, pericardial patch closure of ASD and ligation of left vertical vein were performed with extracorporeal circulation. The postoperative course was uneventful and discharged with excellent general condition and has been well during 8 months follow-up.

Surgical Treatment of Coronary Artery Aneurysm - A case report - (관상동맥류의 외과적치료: 1례보고)

  • We experienced one case of the coronary artery aneurysm. The patient was 67 years old female, who complained of dyspnea on exercise[NYHA functional class III] for two years. Coronary angiogram demonstrated a localized saccular aneurysm on left main coronary artery, just above bifurcation of left anterior descending coronary artery and circumflex artery. On the operative field, the aneurysm was 2cmx2cmx 1.5cm in size with 0.5cm in neck. Resection and aneurysmorrhaphy was done. The patient was recovered and discharged uneventfully. During 8 months follow-up period, she lives in good physical activity and absence of chest pain.

  • We experienced 2 years and 5 months old male patient with partial anomalous pulmonary venous return of the left lung into the coronary sinus without atrial septal defect. After incising the atrial septum and the wall between the left atrium and the coronary sinus, we made the roof of the coronary sinus and closed the artificial atrial septal defect, with using patch, then we could change the direction of the blood flow from the coronary sinus into the left atrium. The patient was discharged on the 13th postoperative day after uneventful postoperative course.
