Dental Treatment of a Patient with Alzheimer Disease under Ambulatory General Anesthesia (알츠하이머병 환자의 외래전신마취 하 치과치료)

  • Background: Elderly patients with progressive dementia including Alzheimer's disease (AD) are more and more often scheduled to undergo general anesthesia for various pathologies including dental problem. But, there is high risk of deterioration of underlying mental diseases and other co-morbidities. So it is important to implement preventive strategies and take adequate measures to minimize negative perioperative events in these patients. Methods: We reviewed the 17 cases of 11 patients with AD who underwent ambulatory general anesthesia for dental treatment at the clinic for the disabled in Seoul National University Dental Hospital. Results: The mean age was 68 (57-81) years. All of them were diagnosed with AD and some had hypertsnsion, bronchiectasis, urinary incontinence. For anesthesia induction, 3 cases (1 patient) was needed physical restraint, but others showed good or moderate cooperation. Drugs used for anesthesia induction was thiopental (11 cases), propofol (3 cases) and sevoflurane (3 cases). All patients received nasotracheal intubation without difficulties. Mean total anesthetic time was 3 hour 44 min ${\pm}$ 60 min and staying time at PACU was 83 ${\pm}$ 34 min. All the patients except one who showed hypertension discharged without any complication. There was no death or long term hospitalization because of severe complications. Conclusions: If general anesthesia is needed, pertinent diagnostic tests and workup about other medical problems, and appropriate anesthetic planning are essential for safety.

  • In this paper planed Real-time Shopping System using Multipurpose Visual Language System(MVLS) with voice recognition remote controller. This system has a merit that using existing on-line & off-line shopping system with addition MVLS data. This can realization little modification existing shopping system. Also, customer's a point of view that has a merit to using easy device for shopping. That is no more using difficult device like that keyboard or mouse, and approach to easy device that voice recognition remote controller or smart phone. Especially, aspect of the old and the infirm and disabled persons that information minority group, can easy buy the product using this system. And, the sellers can more easily collection customer's data and using that future sales strategy.

A Study on Improvement of Barrier Free Door Standard (Barrier Free 출입문 규격기준 개선에 관한 연구)

  • Purpose: The Korean government has implemented a Barrier Free Certification System since 2008 to create a secure and convenient environment for the socially disadvantaged. The drastic increase in the number of BF-certification facilities is expected due to the revision of the system and increasing the number of certification institutions. An analysis of individual evaluation items needs to be made for the BF-Certification with public confidence. Method: Korean standard, International Standard(ISO/FDIS 21542, 2011), German Standard(DIN 18040-1, 2010), Austrian Standard(${\ddot{O}}NORM$ B 1600, 2017) and Swiss Standard(Norm SIA 500 / SN 521 500, 2009) were investigated and analyzed. A comprehensive improvement plan is proposed by comparing details of the aforementioned standards and the evaluation items of BF-Certification. Results: Many problems arise in applying existing Barrier-free standards due to changes in population structure, environmental change and the use of powered wheelchairs. International standards are being improved to solve these problems. The korean standards also require improving of the Barrier Free Law and Certification System, which reflect these trends. In korean cases, standards such as the size of the doors (width and height), the Unobstructed Manoeuvring Space and Clear Space at the Latch side of the Door are required to improve standards in accordance with international standards. In addition, the expression of laws and evaluation items of BF-Certification should be clearly defined. And the application of visual contrast standards for the enhancement of perceptions presented in international standards should be considered. Implication: Barrier Free related legal standards and evaluation items of BF-Certification that are used in Korea are required to be revised in consideration of social and environmental changes. Comprehensive improvements should be made through detailed review.

The Effect of Quota-Levy System on Disability Employment Outcome in Korea (장애인 고용부담금 부과 여부가 장애인 고용성과에 미치는 영향)

  • The object of this article is to examine the effect of the quota-levy system on employment outcome of people with disabilities in Korea. Merging the data from disability employment report of 16,246 businesses in 2011 and the macroeconomic indicators such as regional economic condition, the author analyzes the effect of quota-levy system on employment outcome of persons with disabilities by using Hierarchical Linear Model(HLM). The finding is that imposing the levy on businesses affects employment outcome of people with disabilities but regional economic condition does not. The rate of employees with disabilities of the levied business is 0.7%p higher than that of the other business. The result of analysis implies that employment outcome of people with disabilities is influenced by the quota-levy system rather than regional economic condition.

A Study on the determinants of intent to work of people with disabilities: Uncovering the roles of positive expressions from family members, social network, and disability identity (장애인 취업의사에 영향을 미치는 요인으로서 가족의 긍정적 의사표현, 사회적 관계망 그리고 장애정체감의 역할)

  • The purpose of this study is to analyze the cause-and-effect relationships among positive verbal expressions from family members, social network, and disability identity as the determinant factors of intention to work. To achieve the goal, 453 persons with disabilities and with experiences of using facilities for the disabled were studied, who were recruited for the national survey of domestic violence in 2010. For the research subjects, the cause-and-effect relationships among the major factors were examined by using a structural equation model. According to the results, disability identity and intent to work tended to be higher as they had experienced more of positive verbal expressions from their families. Moreover, a more participation in social activities increased disability identity, but not intent to work. Lastly, disability identity was found to be a determinant of intent to work. Based on the results, we provide suggestions to achieve social inclusion of people with disabilities by raising the intent to work.

The Proposal for Improvement of Legal System for the Persons with Disabilities (장애인복지 관련 법제의 발전방향)

  • In Korea, the evolution of welfare for people with disabilities can be divided largely four stages. The first phase is before 1981, the second is from 1981 to 1988, the third is from 1989 to 2006 and the fourth phase is from 2007 to now. In the first phase people with disabilities were considered as ones of charity. In the second period the welfare ideal for people with disabilities was appeared. In the third, the framework of welfare system for people with disabilities was formed and welfare services began to be expanded. The fourth phase has been the time to secure human right and social services for people with disabilities. In Korea, for people with disabilities, 2007 was the year of historical turning point. "The Act of Prohibition on Disability Discrimination and Remedy for Right"was built that year and became effective the next year. Also "The Special Education Act for People with disabilities" was enacted newly. "The Disabled Welfare Act" was amended for the change of welfare paradigm. The change is introduction of Independent Living. After 2007 Korean legal systems for people with disabilities have been improved largely.

Research on Possible Application of Person-centered Counseling and Handicapped Women Peer Counseling (인간중심 상담과 여성장애인 동료상담의 적용가능성 연구)

  • The purpose of this study is to analyze the applicability and effectiveness of the Person - centered counseling program in the field of counseling service provided by the Independent Living Center for Persons with Disabilities. Thirteen participants, operating peer counseling program at the Independent Living Center who finished 9 sessions of person - centered counseling program over 3 months were interviewed. The collected data were analyzed using Consensual Qualitative Research (CQR). The results were grouped into three themes as "Acceptance as oneself", "Counselor's variables", and "Limitations and unsatisfactory points". Eight categories such as "The relationship with myself and others", "Accepting oneself though with discomfort", "changed perspective about problems", "Recognizing the parts that oneself was not aware", "Accepting one's potentials as well as limitations", "Opportunity to reflect one's life", "Attitude about life ahead" were retrieved with 23 sub-categories. Based on the results, the possibility of applying person - centered approach on peer counseling was presented. Finally, several suggestions for the development of person-centered approach for peer counseling were discussed.

The relationship between disability and clinical outcomes in maintenance dialysis patients

  • Background: Dialysis patients are prone to having disabilities. We aimed to evaluate the association between disability and various clinical outcomes in Korean dialysis patients. Methods: This study consisted of 1,615 dialysis patients from 27 centers. We evaluated disability by using four questions on the activities of daily living (ADLs) concerning whether help was needed for feeding, dressing/undressing, getting in/out of bed, or taking a bath/shower. We divided the patients into three groups: no disability (Non-D, none of the four ADL domains required help; n=1,312), mild disability (Mild-D, one ADL domain required some/full help; n=163), or moderate to severe disability (MS-D, two or more ADL domains required some/full help; n=140). We evaluated falls, frailty, health-related quality of life (HRQoL), mortality, and hospitalization. Results: The numbers of participants with a fall during the last 1 year were 199 (15.2%), 42 (25.8%), and 44 (31.4%) in the Non-D, Mild-D, and MS-D groups, respectively (p<0.001). The numbers of participants with frailty in the Non-D, Mild-D, and MS-D groups were 381 (29.0%), 84 (51.5%), and 93 (66.4%), respectively (p<0.001). In both univariate and multivariate analyses, the physical component scale and mental component scale scores decreased as the grade of disability increased (p<0.001 for both scores). Hospitalization-free survival rate at 500 days was 64.2%, 56.7%, and 51.1% in the Non-D, Mild-D, and MS-D, respectively (p=0.001 for trend). Patient survival rate at 500 days was 95.3%, 89.5%, and 92.3% in the Non-D, Mild-D, and MS-D, respectively (p=0.005 for trend). Conclusion: Disability was associated with falls, frailty, HRQoL scales, and survival trends in Korean dialysis patients.

  • Most of the Internet users in Korea are issued certificates and use them for various tasks. For this reason, it is recommended that accredited certification authorities and security related companies and use public certificates on USB memory and portable storage devices rather than on the user's desktop. Despite these efforts, the hacking of the certificate has been continuously occurring and the financial damage has been continuing. Also, for security reasons, our military has disabled USB to general military users. Therefore, this study proposes a two-factor method using the unique information of the USB memory and the PC which is owned by the user, and suggests a method of managing the private key file secure to the general user. Furthermore, it will be applied to national defense to contribute to the prevention of important data and prevention of access by unauthorized persons.

  • The Coronavirus or COVID-19 is contagiousness virus that infected almost every single part of the world. This pandemic forced a major country did lockdown and stay at a home policy to reduce virus spread and the number of victims. Interactions between humans and robots form a popular subject of research worldwide. In medical robotics, the primary challenge is to implement natural interactions between robots and human users. Human communication consists of dynamic processes that involve joint attention and attracting each other. Coordinated care involves sharing among agents of behaviours, events, interests, and contexts in the world from time to time. The robotics arm is an expensive and complicated system because robot simulators are widely used instead of for rehabilitation purposes in medicine. Interaction in natural ways is necessary for disabled persons to work with the robot simulator. This article proposes a low-cost rehabilitation system by building an arm gesture tracking system based on a depth camera that can capture and interpret human gestures and use them as interactive commands for a robot simulator to perform specific tasks on the 3D block. The results show that the proposed system can help patients control the rotation and movement of the 3D arm using their hands. The pilot testing with healthy subjects yielded encouraging results. They could synchronize their actions with a 3D robotic arm to perform several repetitive tasks and exerting 19920 J of energy (kg.m2.S-2). The average of consumed energy mentioned before is in medium scale. Therefore, we relate this energy with rehabilitation performance as an initial stage and can be improved further with extra repetitive exercise to speed up the recovery process.
