Purpose: This study was a secondary data analysis aimed at identifying the predictors of health-related quality of life (HRQoL) of the home-dwelling disabled person by using EQ-5D which is a standardized instrument used as a measure of health outcome. Methods: Data were drawn from the 3rd Korea National Health and Nutritional Examination Survey conducted by the Ministry of Health and Welfare (MOHW). Subjects were 1,021 home-dwelling disabled persons over 19 years of age. Data were analyzed by using descriptive statistics, t-test, Pearson's correlation, one-way ANOVA, Scheff$\acute{e}$ test, and Stepwise multiple regression. Results: HRQoL (Health Related Quality of Life) is differentiated within age, genders, educational level, employment status, economic status, types and grades of disability, health problems associated with limited mobility, and the limited duration of time. HRQoL is correlated with age, number of cohabiters, educational level, grades of disability, activities of daily living (ADL), and instrumental ADL (IADL). HRQoL is significantly associated with ADL, health problems in connection with limited mobility, employment status, types of disability, marital status, grades of disability, and the limited duration of time. These factors explained 63.7% of variance in HRQoL. Conclusion: HRQoL among the disabled is related to their characteristics, ADL, and IADL. For this reason, it is necessary to develop health programs to promote those variables required to improve HRQoL.
Purpose: The purpose of this study was to identify health problem and joint bleeding, disability according to severity in patients with hemophilia. Background; In the event of an industrial accident, the appropriate choice of hospital is important for worker health and prognosis. This study investigates whether the choice of hospital by the employer in the case of industrial accidents affects the prognosis of injured employees. Methods; Data from the 2018 Panel Study of Workers' Compensation Insurance in Korea were used in an unmatched case-controlled study. The exposure variable is "hospital selection by an employer," and the outcome variable is 'worker's disability." Odds ratios (ORs) were estimated by modified Poisson regression and adjusted for age, gender, underlying disease, injury severity, and workplace size and stratified by industrial classification. The group at increased risk was analyzed and stratified by age, gender, and area. Results; In the construction industry, hospital selection by the employer was significantly associated with increased risk of disability (adjusted OR 1.26; 95% confidence interval [CI]; 1.20-1.32) and severe disability (adjusted OR 1.38; 95% CI; 1.08-1.76) among the injured. Female and younger workers not living in the Seoul capital area were more at risk of disability and severe disability than those living in the Seoul capital area. Conclusions; Hospital selection by employers affects the prognosis of workers injured in an industrial accident. For protecting workers' health and safety, workplace emergency medical systems should be improved, and the selection of appropriate hospitals to supply treatment should be reviewed.
Objectives: As the number of people with disabilities is increasing and their needs for care are varied, the continuum and comprehensiveness of their rehabilitative care are getting more important. This study was performed to understand the utilization of rehabilitation services and requirements of care among the people with severe disabilities in Korea. Methods: We interviewed 578 disabled persons who had severe extremity and cerebral impairment in the urban and rural areas of Korea. The questionnaire included questions on their general characteristics, the type of disability, their physical function (ADL, IADL), their use of rehabilitation services after discharge and their requirements for rehabilitation care Results: Only 12.6% of people with disability in the community continuously used the available medical rehabilitation care. The associated factors for utilization of rehabilitation services were pain and admission for rehabilitative treatment in the acute phase. There was a great need for rehabilitation services in community and this varied according to gender, the socio-economic status, the functional status and the geographic region. The gap between utilization and need for rehabilitation services was largest in the economic support. The gap of primary health care was larger in the rural area than in the urban area. Conclusions: The needs for rehabilitation service were diverse according to the individual functional status, the regional characteristics and other general characteristics of people with disability. Strategies should be considered to eliminate the barriers to obtain rehabilitation services for the people with disability in the community.
The purpose of this study was to examine the state of the articles in the Journal of Korean Association for Disability and Oral Health. All the papers in the Journal of Korean Association for Disability and Oral Health of 2005 to 2018 were analyzed. Objective: Patellofemoral pain syndrome (PFPS) is a condition that is characterized by patellar discomfort or pain that is aggravated during certain activities such as ascending/descending stairs. The Patellofemoral Disability Index (PDI) was developed to assess the effect of pain on functional activities in individuals with PFPS. The objectives of the current study were to determine the internal consistency, test-retest reliability, and validity of this index. Design: Cross-sectional study. Methods: Forty-one subjects who had PFPS with a mean age of $28.8{\pm}5.0years$ and a mean body mass index of $25.6{\pm}4.7kg/m^2$ participated in the study. All subjects were concurrently enrolled in a clinical trial for which they were instructed to complete hamstring-resistance exercises for 4 weeks. Over the course of the intervention, they completed both the PDI and the Oswestry Disability Index (ODI) at baseline after two weeks, and after four weeks. Pearson correlation coefficient was used to assess the criterion validity. Cronbach's ${\alpha}$ was used to examine the internal consistency. Intraclass correlation coefficients with 95% confidence interval were computed to examine test-retest reliability. Results: Subjects' responses within both the PDI and the ODI yielded Pearson correlation coefficient values that were positive and highly significant (range, 0.73-0.97; p<0.001). There was a high level of internal consistency (Cronbach's ${\alpha}{\geq}0.8$), with the exception of stair climbing (Cronbach's ${\alpha}=0.65$). Intraclass correlation ranged from 0.87 to 0.92, indicating high levels of test-retest reliability. Conclusions: The PDI is a valid, reliable, and feasible method of assessing pain and functional ability in patients with PFPS.
Objectives : To estimate the weighting for the disability caused by major cancers in Korea using the Delphi method. Methods : We selected 19 panelists to estimate the disability weighting of major cancers in Korea by using the Delphi method. To select the relevant kinds of cancers, we used National Death Certificate Data produced by the National Statistical Office in 1996. Then the stability of each delphi round was calculated by using the coefficient of variance. Results : The disability weight of major cancers for males was pancreas cancer(0.36), liver cancer(0.35), esophageal cancer(0.30), stomach cancer(0.27), lung cancer(0.26), and colorectal cancer(0.30). The disability weight of major cancers fer females was pancreas cancer(0.36), liver cancer(0.34), esophageal cancer(0.29), stomach cancer(0.28), lung cancer(0.26), and colorecial cancer(0.28). Conclusion : The results of this study will provide baseline data useful for the measurement of the burden of disease caused by cancers in Korea.
Objectives: The purpose of this study was to investigate the associated factors of self-reported dry mouth in adults. Methods: A self-reported questionnaire was filled out by 249 adults in Seoul and Gyeonggi-do from June to October, 2014. The questionnaire consisted of general characteristics of the subjects, age, monthly income, smoking, alcohol drinking, and systemic diseases including systemic diseases, medication, oral health status, and stress. The question for dry mouth consisted of dryness in skin, eyes, lips, and nasal mucosa. The subjective dry mouth consisted of 6 questions measured by visual analogue scale(VAS). Cronbach's alpha was 0.881 in the study. Oral health related quality of life (OHIP-14) was adapted from Yoon. The questionnaire for OHIP-14 included functional limitation, physical pain, psychological disability, social disability, and experience in hadicap measured by Likert 5 scale. Cronbach's alpha was 0.885 in the study. Data was analyzed for a t-test, one-way ANOVA and multiple regression analysis by using SPSS(SPSS 18.0, USA) program. Results: There were positive correlations between oral health-related quality of life and self-reported dry mouth (functional limitation r=0.288, physical pain r=0.219, psychological discomfort r=0.193, physical disability r=0.280, psychological disability r=0.205, social disability r=0.224 and handicap r=0.270). In the multiple regression analysis, variation of self-reported dry mouth were positively associated with dry eyes{very often(${\beta}=0.305$)), sometimes(${\beta}=0.186$)}, dryness on lips{very often(${\beta}=0.247$), sometimes(${\beta}=0.177$)}, handicap(${\beta}=0.152$), physical disability(${\beta}=0.128$) and alcohol drinking(1-2 times/week)(${\beta}=0.116$) (p<0.001). Conclusions: Self-reported dry mouth may cause deterioration of the entire body dryness(dryness on eyes and lips), low oral health-related quality of life(handicap and physical disability) and alcohol drinking. Thus, It is necessary to develop oral health education programs to prevent and manage dry mouth in adults.
Objective: Several muscles surrounding neck are vital not only for neck motion, but for upper extremities motions as well. Neck pain would affect neck and shoulder disability. The Neck Disability Index (NDI), Shoulder Pain and Disability Index (SPADI), and Visual Analogue Scale (VAS) are increasingly used to evaluate treatment effectiveness after chronic neck pain. The purpose of this study was to analyze the correlation of neck pain, shoulder pain, and quality of life in patients with chronic neck pain. Design: Cross-sectional study. Methods: Forty-three patients with neck pain participated in this study. Participants were instructed to place a mark on each item of three clinical measures that best represented their experience of his/her neck and shoulder problem over the last week. The outcome measures of the study were NDI, SPADI, and VAS for neck and shoulder pain and disability. Results: The NDI was significantly correlated with the VAS-Neck, SPADI-Pain, SPADI-Disability, SPADI-Total, and VAS-Shoulder (p<0.05). The VAS-Neck was significantly correlated with the VAS-Neck, SPADI-Pain, SPADI-Disability, SPADI-Total, and VAS-Shoulder (p<0.05). The SPADI-Total was significantly correlated with the NDI, VAS-Neck, SPADI-Pain, SPADI-Disability, and VAS-Shoulder (p<0.05). The SPADI-Pain was significantly correlated with the NDI, VAS-Neck, SPADI-Disability, SPADI-Total, and VAS-Shoulder (p<0.05). The SPADI-Disability was significantly correlated with the NDI, VAS-Neck, SPADI-Total, SPADI-Pain, and VAS-Shoulder (p<0.05). The VAS-Shoulder was significantly correlated with the NDI, VAS-Neck, SPADI-Pain, SPADI-Disability, and SPADI-Total (p<0.05). Conclusions: Neck disability in patients with chronic neck pain should be considered with disability and pain of the shoulder.
