EFFECTS OF THE SINGLE AND FRACTIONATED IRRADIATION ON THE MICROVASCULATURE OF THE RAT SUBMANDIBULAR GLAND (단일 및 분할 방사선조사가 백서 악하선 미세혈관계에 미치는 영향)

  • The purpose of the study was to investigate the effects of the single and fractionated irradiation on the microvascular structure of the submandibular gland in rats. For this study, 90 Sprague-Dawley strain rats were irradiated to their neck region with equal split doses of 9Gy for a 4 hours interval and 15Gy single dose by 6MV X-irradiation and sacrificed on the 1st, 3rd, 7th, 14th and 27th day after irradiation. The author observed histological changes at Hematoxylin and Eosin staining and PAS staining under a light microscope, and also observed distribution and structural changes of the microvasculature in rat submandibular gland using a scanning electron microscope by forming vascular resin casting. The results were as follows: 1. In the light microscopic examination, the microvasculature was slightly dilated and decreased in number on the 1st day after irradiation, and increase in number of microvasculature was observed on the 3rd day after irradiation. And then distribution of microvasculature was markedly increased on the 7th day after iradiation, but decreased on th 14th day after irradiation again. Such changes were greater in the single irradiated group than in the fractionated irradiated group. 2. The reaction to PAS staining on glandular cell was decreased on the 1st and the 3rd day after irradiation, and recovered on the 7th day after irradiation. The reaction was decreased on the 14th day after irradiation again, and recovered on the 28th day after irradiation. Changes were more apparent in the single irradiated group. 3. In the scanning electron microscopic examination, early changes of microvasculature were decreased capillary density, dilation of conduits and meandering. Increased capillary dentsity or anastomosis due to vascular reproduction and smooth curved running were observed on the 7th and 14th day after irradiation. Decreased capillary and smooth running tendency were observed on the 28th day after irradiation again. Such changes were greater in the single irradiated group than in the fractionated irradiated group.

Emergency Carotid Artery Stent Insertion for Acute ICA Occlusion

  • Objective : An effective intervention has not yet been established for patients with acute occlusion of the internal carotid artery (ICA). The aim of our study was to investigate the feasibility, safety, and efficacy of emergent stent placement of carotid artery to improve neurologic symptoms and clinical outcome. Methods : Of 84 consecutive patients with severe ICA stenosis who were admitted to our institution from March 2006 to May 2009, 10 patients with acute ICA occlusion (11.9%) underwent emergency carotid artery stent placement. We reviewed their records for neurologic outcome using the National Institutes of Health Stroke Scale (NIHSS) score, before and at 7 days after stent placement; clinical outcome using the modified Rankin Scale score (mRS) and Glasgow Outcome Scale (GOS); frequency of procedure-related complications; and recurrence rate of ipsilateral ischemic stroke within 90 days. Results : Carotid lesions were dilated completely in all patients. Median NIHSS scores before emergency stent placement and at 7 days were 16.6 and 6, respectively, showing significant improvement. Eight patients (80%) had favorable outcomes (mRS score 0-2 and GOS 4-5). Complications occurred in two patients (20%): stent insertion failed in one and an intracerebral hemorrhage occurred in the other. Ipsilateral ischemic stroke did not recur within 3 months. Conclusion : Emergency carotid artery stent placement can improve the 7-day neurologic outcome and the 90-day clinical outcome in selected patients with acute cerebral infarction.

Protective Effect of Panax ginseng on TCDD-induced Liver Toxicity in Male Guinea Pigs

  • This study was carried out to investigate the preventive and therapeutic effect of Panax ginseng water extract (PG-WE) on the toxicity induced by 2,3,7,8-tetrachlorodibenzo-p-dioxin (TCDD), one of the most notorious toxic environmental pollutants belonging to the group of polyhalogenated aromatic hydrocarbons. Normal control (NC) group guinea pigs (180~200 g) received vehicle and saline, and TCDD-treated (TT) group was given TCDD and saline. P100 and P200 group animals received PG-WE for 28 days since 1 week before TCDD exposure at daily doses of 100 mg/kg b.w. and/or 200 mg/kg b.w., respectively. C100 and C200 group received PG-WE for 14 days starting 1 week after TCDD-exposure. Toxicity was induced by a single intraperitoneal injection of TCDD (1 $\mu\textrm{g}$/kg b.w.). Abnormal increase in AST and ALT activities in TT group was significantly improved by the administration of PC-WE. Microscopically, there were mild to moderate swelling of hepatocytes, hyperchromatism of individual cells, acidophilic cytoplasm and cytoplasm vacuolation of some hepatocytes, slight to moderate variations of staining density, occasional single cell necrosis, variable size and shape of some hepatocytes, small groups of degenerating hepatocytes surrounded by mononuclear cells, dilated sinusoids of centrilobular zone and some loss of lobular architecture in TT group liver. From these results, we could find the protective and therapeutic role of PG-WE in TCDD-induced liver toxicity by examining the blood chemical parameters and histopathological observation.

Transaxillary Subpectoral Placement of Cardiac Implantable Electronic Devices in Young Female Patients

  • Background The current indications of cardiac implantable electronic devices (CIEDs) have expanded to include young patients with serious cardiac risk factors, but CIED placement has the disadvantage of involving unsightly scarring and bulging of the chest wall. A collaborative team of cardiologists and plastic surgeons developed a technique for the subpectoral placement of CIEDs in young female patients via a transaxillary approach. Methods From July 2012 to December 2015, subpectoral CIED placement via an axillary incision was performed in 10 young female patients, with a mean age of 25.9 years and mean body mass index of $20.1kg/m^2$. In the supine position, with the patient's shoulder abducted, an approximately 5-cm linear incision was made along one of the deepest axillary creases. The submuscular plane was identified at the lateral border of the pectoralis major, and the dissection continued over the clavipectoral fascia until the subpectoral pocket could securely receive a pulse generator. Slight upward dissection also exposed an entrance to the subclavian vein, allowing the cardiology team to gain access to the vein. One patient with dilated cardiomyopathy underwent augmentation mammoplasty and CIED insertion simultaneously. Results One case of late-onset device infection occurred. All patients were highly satisfied with the results and reported that they would recommend the procedure to others. Conclusions With superior aesthetic outcomes compared to conventional methods, the subpectoral placement of CIEDs via a transaxillary approach is an effective, single-incision method to hide operative scarring and minimize bulging of the device, and is particularly beneficial for young female or lean patients.

Surgical Correction of Single Extrahepatic Portosystemic Shunt Using Ameroid Constrictor in a Dog (Ameroid Constrictor를 이용한 개에서의 단순 간외성 간문맥전신단락증의 치료)

  • A 2-year-old 4.0-kg female Shih Tzu with history of hematemesis and melena was referred to Veterinary Medical Teaching Hospital, Seoul national University for further evaluation and treatment. During physical examination, the dog revealed mild depression, dry mucous membrane and abdominal pain. Hematologic values were normal and serum chemical values showed increased serum bile acid (53.47 umol/l, preprandial), fasting serum ammonia concentration (184 g/dl), alanine transferase (98 U/L), alkaline phosphatase (871 U/L) and gamma glutamyl transpeptidase (21 U/L), and decreased blood urea nitrogen (4 mg/dl), total protein (4.1 g/dl) and albumin (1.2 g/dl). Microhepatica was shown in abdominal radiography. During the ultrasound examination, dilated tortuous vein communicating with caudal vena cava ws observed near the stomach. Intraoperative jejunal vein portography was performed during laparotomy to confirm the location and size of shunt vessel. According to history taking, physical examination, hematologic and serum chemical examination and radiographic study, it was diagnosed as single extrahepatic portosystemic shunt. The anomalous vessel (7 mm, o.d.) that enter the caudal vena cava from the left gastric vein, near the level of the diaphragm, was identified. A Ameroid constrictor (5 mm, i.d.) was applied to the shunting vessel near the caudal vena cava. Hematologic and serum chemical values recovered gradually and were revealed normal values 4 months after surgery. Four month after surgery serum bile acids concentrations were 0.56 $\mu$mol/l (preprandial) and 18.45 umol/l (postprandial). Abdominal radiograph showed normal gastric axis and it revealed normal size of the liver. Fine texture and increased echogenecity of liver and enlargement of portal vein were shown in ultrasonography. Single extrahepatic portosystemic shunt might be treated surgically using Ameroid constrictor.

  • Cardiac allograft vasculopathy (CAV) is a major factor that limits the long-term survival after cardiac transplantation. Because the main feature of CAV is a diffuse stenosis that predominantly develops in the distal arteries, reperfusion therapy has shown poor outcomes. The results of cardiac retransplantation for CAV are better than that for acute resection and the survival is identical to that of patients who undergo primary transplantation. We describe a case of performing cardiac retransplantation in a 28 year-old male patient with refractory CAV and who underwent primary transplantation due to dilated cardiomyopathy 8 years previously.

Light microscopic observations on the in vitro effects of praziquantel on Heterophyopsis continua (Heterophyopsis continua에 대한 praziquantel의 시험관내 효과의 광학현미경적 관찰)

  • This study was carried out to observe in vitro effect of praziquantel on the viability and internal organ changes of Heterophyopsis continua with light microscopy. Metacercariae were collected from the perch, Lateolabrax japonicus, by artificial digestion technique and fed to 2-week old chickens. Adult worms were recovered from the small intestines of chickens 8 days after infection. For working solutions, praziquantel was diluted with TC199 medium at the concentration of 0.01, 0.1, 1 and $10{\mu}g/ml$. To each petri dish containing 10ml of solution, 5~10 worms were introduced and incubated at $37^{\circ}C$. Motlity of worms was observed at 5, 15, 30, 60 minutes, 1, 2, 4 and 6 hours after incubation. For light microscopy, worms were fixed in 10% formalin under cover glass pressure and stained with Semichon's acetocarmine. The results were as follows: 1. In $0.01{\mu}g/ml$ praziquantel, the worms had their mobility until 6 hours post treatment. However, worms in over $0.1{\mu}g/ml$ of praziquantel contracted within 5 minutes and immobilized. 2. Intestine of the worm incubated in $0.001{\mu}g/ml$ praziquantel for 5 minutes was dilated and intestinal wall was thickened. 3. In incubated over $0.1{\mu}g/ml$ praziquantel, pharynx of the worm protruded out from oral sucker. 4. The lowest effective lethal concentration of praziquantel on H. continua was $0.1{\mu}g/ml$. The worms exposed to the drug were observed to be immobilized immediately after incubation in solutions of over $0.1{\mu}g/ml$ concentration. All of the worms in early period showed severe contraction and those in late period showed severe dilation.

Identification of intracerebral hematoma using ultrasonography in a dog

  • A 3-month-old male Maltese dog was presented with generalized trembling, ataxia, and seizure.The patient had traumatic injury on history takings. Physical examination revealed dome shaped craniumwith open fontanelle, nasal hemorrhage, and blepharoedema with hemorhage on the left side. On serumbiochemical profiles, creatine phosphokinase was severely elevated. There was no remarkable findingon radiography. Ultrasound images of brain were obtained via a persistence bregmatic fontanelle, andbilateraly dilated lateral ventricles with a hyperechoic mass on the left temporal lobe were detected.Based on clinical signs, history, physical examination, laboratory findings, and ultrasonographic findings,we suspected this intracranial mass to intracerebral hematoma induced by head trauma. Methylprednisoloneh after initial diagnosis. We performed necropsy and confirmed intracerebral hematoma. This case reportdescribes the identification of intracerebral hematoma using ultrasonography.

Computed Tomographic Findings of Segmental Caudal Vena Cava Aplasia in Two Dogs (개에서 컴퓨터단층촬영에 의해 진단된 후대정맥 분절 무형성증 2 례)

  • A 7-year-old intact female dachshund (Dog 1) and a 17-year-old intact male Yorkshire terrier (Dog 2) were presented for evaluation of mammary gland tumor and a regular checkup, respectively. Incidentally, segmental caudal vena cava aplasia was detected on CT images in both dogs. Absent postrenal segment (Dog 1) or discontinuation of caudal vena cava (Dog 2) were detected, but prerenal segment of caudal vena cava was continued to the dilated azygos vein. Segmental CVC aplasia should be considered when an abdominal vascular anomaly was evaluated with CT angiography in dogs.

Ultrastructural characteristics of the pancreatic acinar cells in the diabetes-prone BB rat (BB랫드의 췌장 샘포세포의 미세구조적 특징)

  • Functional and morphological characteristics of the exocrine pancreas in genetic model BB rat of insulin dependent diabetes medllitus(IDDM) were carried out. Wistar rat was used as control animal. Flow rate of pancreatic juice, output of amylase and protein, and plasma glucose and insulin levess were examined. Also light and ultrastructural characteristics of the exocrine pancreas were observed. Pancreatic flow rate, output of amylase and protein, and insulin level were lower;glucose level was higher comparing with those of the control Wistar rat. In Wistar rat, exocrine pancreas was typical light microscopically. Zymogen granules and cell organelles were well developed in fine structure. Cell size of the periinsular acini was larger, and number of zymogen granules were more than those of the teleinsular acini. Most acinar cells were dark cells which containe well-developed RER in their cytoplasm. On the other hand, some light cells which have the dilated RER cisterns were found. In BB rat exocrine pancreas, cell size of per-and tele-insular acini similar to that of Wistar rat. The number of light cells occupied 40-50% compairing with that of Wistar rat. Zymogen granules were lower in number than that of Wistar rat and divied into three types in morphological characteristics ; type I showing normal structure, type II showing the wide hallo and small electron dense core in center of the zymogen granule and type III not having the electron dense core in the zymogen granule. The present ratio of type I, type II and type III are less than 5%, 30-40% and more than 50%, respectively.
