A case of removable dentures using digital method (디지털 방식을 이용하여 제작한 양악 가철성 의치 수복 증례)

  • Generally dentures are manufactured by conventional method, however the frequency of fabricating denture using digital method is increasing due to the recent development of digital technology in dentistry. The digital method of manufacturing denture is classified into two systems; 3D scan of the impression to arrange the artificial teeth on the CAD (Computer-aided design) and 3D printing to produce the resin-based complete denture, or 3D scan of the model to design of the framework using CAD, resin pattern formation by 3D printing and casting of metal framework of complete denture or removable partial denture. In this case report, electronic surveying and design the metal framework of the dentures were performed using CAD program, and plastic resin patterns fabricated by 3D printing were casted for upper full denture and lower removable partial denture. During follow-up periods, dentures using digital method have provided satisfactory results esthetically and functionally.

Effects of Critical Thinking Disposition, Digital Literacy, and Nursing Informatics Competence on the Competency in Evidence-Based Practice among Clinical Nurses (간호사의 비판적 사고성향, 디지털리터러시, 간호정보역량이 근거기반실무역량에 미치는 영향)

  • Purpose: This study aimed to investigate the competency in evidence-based practice, critical thinking disposition, digital literacy, and nursing informatics competence, and to identify the factors influencing the competency in evidence-based practice among nurses. Methods: The participants in this study were 196 nurses from a university hospital in J-city. Data were collected from December 1 to December 30, 2023. The collected data were analyzed with the SPSS 26.0 program using descriptive statistics, independent t-test, one-way analysis of variance, Scheffé test, Pearson's correlation coefficients, and multiple regression analysis. Results: The results showed that the factors influencing the competency in evidence-based practice included nursing informatics competence, critical thinking disposition, digital literacy, and having a master's degree. The participants with a master's degree demonstrated higher competency in evidence-based practice compared to those with a three-year degree or a four-year degree, and a total explanatory power was 53.1%. Conclusion: In this study, critical thinking disposition, digital literacy, and nursing informatics competence were influential factors in competency of evidence-based practice. Therefore, a variety of intervention programs should be developed to enhance the impact of these factors on nurses' competency in evidence-based practice.

Regional Hospital Medical Equipment Operation of Adequacy Assessment (지역 의원 의료장비운영에 따른 적정성 평가)

  • Due to the increasing quantity of health services demand a wide range of growing medical markets and hospital through advanced, diversification can meet the needs of the patient. This hospital is having the introduction of expensive medical equipment. Because of this, hospitals are struggling to operate the hospital management. Therefore, when operating at the hospital before and after introduction of the most important factor in the interests of medical equipment through the analysis is directly related to hospital operations. CT study of the local clinics, or CR 40 where members of the medical equipment with 54 points against selected practitioners and hospital visits and interviews to investigate the expected benefits of each of the members raised an average of seven months to identify revenue performance caused by interests. Raised against the expected benefits resulting benefits to the overall average 87.43% did not meet the expected benefits. The hospital plans to introduce future when reviewing the medical equipment in consideration of the characteristics of the equipment requires a more solid plan, and whether the plan is properly implemented will be needed for the institutional verification capabilities.

Effects of Korean value incentive program on the in-hospital mortality in acute hemorrhagic stroke patients (가감지급사업 실행 후 급성기 출혈성 뇌졸중의 원내 사망률 변화 분석)

  • Purposes: This study purposed to evaluate the effect of a value incentive program(VIP) on the in-hospital mortality of acute stroke. Methodology: Study period was from January 2010 to December 2018. This study included 63 hospitals for acute hemorrhagic stroke that the mortality rate per month was more than one during study period. Independent variables were time variables and hospital characteristics such as hospital type, district and bed number. Interrupted time series analysis was applied to analyze the data. Findings: In case of general hospitals, the in-hospital mortality rate per month for acute hemorrhagic stroke tends to be increased by 0.03% in overall study periods but decreased by 0.32% after the implementation of the policy. On the other hand, tertiary hospital changes are not statistically meaningful. Conclusion: This study provides evidences how the VIP was effective in improving quality of acute hemorrhagic stroke care. General hospitals showed higher policy effect compare to that of tertiary hospitals.

  • Digital extensor hypoplasia (DEH) is a rare malformation that presents with loss of active finger extension at the metacarpophalangeal (MCP) joints. Descriptions of optimal treatment and outcomes in this population are sparse. We describe successful operative treatment of a child with DEH involving the extensor digitorum communis, extensor digiti minimi, and the extensor indicis proprius tendons. The 5-year-old male patient was referred for severe limitation on bilateral finger extension since birth. He had been previously diagnosed with arthrogryposis and managed conservatively. Due to lack of improvement, magnetic resonance imaging was done evidencing hypoplasia/aplasia of the extensor tendons. The patient underwent successful tendon transfers using extensor carpi radialis longus to the common extensor tendons, and one hand required an additional tenolysis procedure. 2 years postoperatively, his MCP position and finger extension are markedly improved, and he is able to grip objects without limitation or difficulty. The patient returned to full activity without restriction.

  • The conventional approach of looking down a microscope to perform microsurgical procedures is associated with occupational injuries, anti-ergonomic postures, and increased tremor and fatigue, all of which predispose microsurgeons to early retirement. Recently, three-dimensional (3D) visualization of real-time microscope magnification has been developed as an alternative. Despite its commercial availability, no supermicrosurgical procedures have been reported using this technology to date. Lymphovenous anastomoses (LVAs) often require suturing vessels with diameters of 0.2-0.8 mm, thus representing the ultimate microsurgical challenge. After performing the first documented LVA procedure using 3D-augmented visualization in our unit and gaining experience with this technique, we conducted an anonymized in-house survey among microsurgeons who had used this approach. The participants considered that 3D visualization for supermicrosurgery was equivalent in terms of handling, optical detail, depth resolution, and safety to conventional binocular magnification. This survey revealed that team communication, resident education, and ergonomics were superior using 3D digital hybrid visualization. Postoperative muscle fatigue, tremor, and pain were also reduced. The major drawbacks of the 3D visualization microscopic systems are the associated costs, required space, and difficulty of visualizing the lymphatic contrast used.

  • This study, was conducted to find out the correlation factors of anxiety of the patients during MRI exam on the claustrophobia. This study conducted a survey targeting those korea auxiliary police and combat auxiliary police patients who were admitted to the National Police Hospital over a period from January 2 to December 24, 2013 to investigate their degree of claustrophobia. In study result, sufficient explain before exam was helpful and decrease anxiety. However, they felt more anxious about limited spare, and when the entered a bore. Symptoms of instability and fear were stuffy, sweat, dyspnea, palpitation those are ascending rank of frequency.

  • Rapidly developed de novo aneurysm is very rare. We present a rapidly developed and ruptured de novo anterior communicating aneurysm 8 days after the rupture of another aneurysm. This de novo aneurysm was not apparent in the initial 3-dimensional computed tomography and digital subtraction angiography. We reviewed the literature and discussed possible mechanisms for the development of this de novo aneurysm.
