Regional comparison of dietary intakes and health related behaviors among residents in Asan

  • Inadequate dietary intakes and poor health behaviors are of concern among rural residents in Korea. This study is conducted to compare dietary intakes, dietary diversity score (DDS), mean nutrient adequacy ratio (MAR) and health related behaviors by rural, factory and urban areas in Asan. A total of 930 adults (351 men and 579 women) were interviewed to assess social economic status (SES), health related behaviors and food intakes by a 24-hour recall method. Mean age was 61.5 years with men being older (64.8 years) than women (59.3 years, p<0.001). Men in the factory area were older than rural or urban men while urban women were the youngest. Education and income of urban residents were higher than other area residents. There were more current drinkers in urban area while smoking status was not different by regions. Physical activity was significantly higher in rural or factory areas, whilst urban residents exercised more often (p<0.05). Rural or factory area residents considered themselves less healthy than others while perceived stress was lower than urban residents. Energy intakes were higher in urban residents or in men, however, after SES was controlled, energy intake did not show any differences. Energy-adjusted nutrient intakes were significantly higher in the urban area (p<0.05) for most nutrients except for carbohydrate, niacin, folic acid, vitamin $B_6$, iron and fiber. Sodium intake was higher in factory area than in other areas after SES was controlled. DDS of rural men and MAR of both men and women in the rural area were significantly lower when SES was controlled. In conclusion, dietary intakes, diversity, adequacy and perceived health were poor in the rural area, although other health behaviors such as drinking and perceived stress were better than in the urban area. In order to improve perceived health of rural residents, good nutrition and exercise education programs are recommended.

Zinc status and dietary quality of type 2 diabetic patients: implication of physical activity level

  • The purpose of this study was to analyze the relationships among zinc status, diet quality, glycemic control and self-rated physical activity level of type 2 diabetic patients. Dietary intakes for two non-consecutive days were measured by 24-hour recall method for seventy-six diabetic patients. Fasting blood glucose and HbA1c were measured for the assessment of glycemic control. We evaluated the extent of dietary adequacy by the percentage of subjects with a dietary intake of a nutrient less than the estimated average requirement(EAR), the dietary diversity score(DDS) and the dietary variety score(DVS). Zinc status was assessed from serum levels and urinary excretion. Dietary inadequacy was serious for five nutrients: riboflavin, calcium, thiamin, zinc and vitamin C. Dietary intakes from the meat, fish, and egg food groups and the milk food group were below the recommended level. 대구지역 유아들의 식사 및 식생활 형태 조사 - 식품 섭취패턴 및 식사다양성 평가 - (Study of Food Intakes and Eating Patterns among Preschool Children in Daegu Area - Assessment of Dietary Pattern and Dietary Diversity -)

  • This study was conducted to assess dietary pattern and dietary diversity of preschool children by investigating their food consumption. To accomplish this, dietary intake survey (24 hour-recall food diary) was administered to 508 children aged between 4 to 6 years old from 4 kindergartens in the Daegu area (415 were completed). By using CAN pro 3.0 for professionals, the dietary patterns and dietary diversity scores of each food group were examined by age and sex. Data were analyzed with SPSS 12.0, and $\chi^2$, t-test and Anova test, and Duncan's multiple range test used for verifying significance. For 4~5-year-old group, boys had significantly more fruit and meats than girls (p < 0.05), but girls had more dairy food than boys with no significance. For 6-year-old group, boys had significantly more foods, milk and dairy foods than girls (p<0.05), and also boys had more vegetables and fruit than girls with no significance. For DDS (Dietary Diversity Score), the score of 6-year-old group was significantly higher than that of younger children (p < 0.05) and the older the age was, the higher the score was. Dietary Pattern (GMVFD) was verified by 8 types, and the ratio of taking all five food groups (GMVFD = 11111) of the 6-year-old group was 48.8%. Children with older ages had more various foods than younger ones (p < 0.05). In addition, the ratio of taking less than the minimal intake of dairy foods of girls (48.1%) was higher than that of boys (41.3%). As results, unbalanced diet, overeating and not eating unbalanced diet were revealed as the problematic dietary habits among preschool children, and those habits may change positively with knowledge on nutrition grow on. Therefore, nutritional characteristics and importance of preschool children should be taught to parents and caregivers, and it must be practical and ready to use in cooking for these children to healthy dietary habits.

한국 청소년의 식사의 질 평가에 적절한 식품군의 조성 -Kant의 최소량에 기초하여- (Composition of Food Groups Appropriate for Evaluation of Diet Quality of Korean Adolescents -Based on Kant's Minimum Amount-)

  • The purpose of this study was to determine composition of food groups appropriate for the evaluation of the quality of total diet or food guide, using the data from the 1998 National Health and Nutrition Survey. Twenty-four-hour-dietary recalls of total 1,110 Korean adolescents aged 13 - 19 years (male 543, female 567) were analyzed for food group consumed (Dietary Diversity Score, DDS), intake patterns of food group, mean index of nutritional quality (mINQ) and nutrient adequacy ratio (NAR). Considering Korean food pattern different from the Americans, this study is to check if Korean 5 food groups with both of oil & sugar group and vegetable & fruit group, and a minimum amount of 30 g in grain group and 15 g in meat group modified from Kant's minimum amount, 15 g in grain and 30 g in meat, could also be appropriate for the diet evaluation of Korean adolescents. Combination of food group and minimum food intake per each food group used in the first trial were categorized into 6 kinds of types: 1) American 5 food groups with Kant's minimum amount as control or 2) American 5 food groups with modified Kant's minimum amount 3) Korean 5 food groups excluded by sugar with the modified amount 4) Korean 5 food groups with modified amount 5) American food groups added oil group with the modified amount 6) American food groups added oil & sugar group with the modified amount. In case of oil group or oil and sugar group, 5 g was quoted for the minimum amount. Correlation between DDS and indices of other diet quality in each type of food group composition were analyzed and the best food group composition was traced by comparing the correlation coefficients of DDS and mINQ or MARs at ${\alpha}\;<\;0.01$. There was no meaningful difference in weight length index of the subjects of eating or not-eating oil or oil & sugar, while the values of mINQ showed higher in the subjects eating oil but no difference in the subjects eating oil & sugar, compared to the group not-eating, respectively. In comparison of type 1 and type 2, correlation between DDS and indices of other diet quality showed higher in type 1 using Kant's amount. And comparing the type of a combined group of fruit & vegetable (type 3 and 4) and the type of separated group of fruit and vegetable (type 5 and 6). six groups of separated vegetable and fruit showed the higher correlation of DDSs and indices of diet quality. In case of being classified into six groups with oil, DDS showed correlation of r = 0.293 with INQ. While, being classified into six groups with oil & sugar instead of oil only, DDS showed correlation of r = 0.249 with INQ. Accordingly the type 5 with oil is regarded as the better food group type than the type 6 with oil & sugar. Since better result was shown in the composition of six groups with Kant's minimum amount, the comparative analysis on both of type1 with oil and oil & sugar was finally performed. Then oil added type 1 showed higher correlation of DDS and indices of diet quality. These result indicated that it would be more appropriate to use six food groups with separate vegetable and fruit including oil group (oil-added type 1) rather than oil & sugar group for evaluation of dietary quality of Korean adolescents using DDS.

한국 독거노인의 식사패턴에 따른 식사의 질 평가: 2016-2018 국민건강영양조사 자료를 이용하여 (Evaluation of Diet Quality Determined by Dietary Patterns of Elderly Koreans Living Alone: Data Retrieved from the 2016-2018 Korea National Health and Nutrition Examination Survey)

  • This study was undertaken to identify dietary patterns of Korean elderly people living alone, and to compare the quality of diet consumed. Data of 821 elderly people aged 65 years or older who were living alone, was obtained from the 2016~2018 National Health and Nutrition Survey. The percentage energy intake from 24 food groups was calculated, and a cluster analysis was applied to identify dietary patterns. General characteristics, energy and nutrient intake, nutrient adequacy ratio (NAR), mean adequacy ratio (MAR), and index of nutritional quality (INQ) were analyzed. Dietary variety score (DVS) and dietary diversity score (DDS) were subsequently calculated. The rice-centered diet group and mixed diet group were characterized as consuming higher intake of rice and a wider variety of food groups, respectively. The mean age and percentage of participants living in rural areas were higher, whereas education level and monthly income were lower, in the rice-centered diet group (P<0.001 for all). Intakes of energy and all other nutrients (except carbohydrate and thiamine) were lower in the rice-centered diet group (P<0.001 for all). The NAR for all nutrients (except thiamine), MAR, and INQs (except thiamine and iron) were significantly lower in the rice-centered diet group (P<0.001 for all). The rice-centered diet group had significantly lower DVS (P<0.001) and DDS (P<0.001), as compared with the mixed diet group (P<0.001). Due to the insufficient energy and nutrient intake in the rice-centered diet group, it is necessary to develop nutritional monitoring and customized nutrition policies for these individuals.

1998년도 국민건강영양조사자료를 이용한 한국인의 경제수준별 식품섭취현황 (Food Intake Patterns of Koreans by the Economic Status Using 1998 Korean National Health Examination Nutrition Survey)

  • The objective of this study was to investigate the differences in food intake patterns by the economic status. Data from the 1998 Korean National Health and Nutrition Survey were used. Using the poverty line based on the 1998 Korean minimum cost of living, the subjects (n = 10400) were classified into high class (36.1%), middle class (40.7%) and low class (23.1%). Mean intakes for other food groups except grains, potatoes and vegetables became higher as the eco-nomic levels were going up. By the family size, it was found that intakes of grains and fishes were different (p<0.05). By the economic status, intakes of fruits (p<0.05), meats (p<0.05) and fishes (p<0.01) were different. There were no significant differences in consumed types of food by the economic status. Rice was the most prevalent food in all clas-ses and its intakes increased as the economic status was going down. In the dietary patterns of main food group (GMDVF: Grain, Meats, Dairy, Vegetable and Fruit), the pattern except dairy (11011) revealed the highest proportion in high-middle class while the patterns without dairy and fruit food groups (11010) had the highest proportion in low class. The propor-tions of subjects with the low dietary diversity score (DDS) increased as economic status was going down (p<0.01). The food groups that most people do not consume were the dairy and fruit group. Proportions of people who missed one or more group increased as economic status was going down. The high dietary variety score (DVS) also was linked to a high score on the DDS and high class (p<0.01). Thus the intakes of the dairy and fruits product should be increased in all classes. The low class especially had worse quality of food intakes pattern than that of the high or middle class. The nutrition policy and education programs need to be established according to the economic status.

전라도 구곡순담 장수벨트지역에 거주하는 중노년층의 연령군에 따른 여름철 식품 섭취량과 식품 섭취 다양성 및 식이 패턴 평가 (Assessment of Food Consumption, Dietary Diversity and Dietary Pattern during the Summer in Middle Aged Adults and Older Adults Living in Gugoksoondam Logevity Area, Korea)

  • The purpose of this study was to investigate food intake, dietary diversity and dietary pattern during the summer in middle aged and older adults living in Gugoksundam (Gurye, Goksung, Sunchang and Damyang counties), Jeonla Province located in southern part of Korea and known as one of the representative Korean longevity areas. Food intake and dietary diversity were assessed by using the data from 2 day-24 hour recall of 1,051 subjects (394 males and 711 females) aged 45 years and older (45-93 years in male, 45-105 years in female). The average ages of males and females were 70.7 years and 71.1 years, respectively. Average total daily food intake was significantly decreased with aging in both genders, and the average animal food intake ratio to total food intake was ranged 10~14% in 3 different age groups, 45~64 years, 65~74 years and 75 years and older. The contribution of each food group, in weight, to total food intake showed the descending order of grains, fruits, vegetables and alcohols in males, and grains, fruits, potatoes and meats in females. On daily intake amount of each food item, rice, watermelon, soju, kimchi, and potato were ranked on top 5 in descending order in males, and rice, watermelon, potato, kimchi and ylmukimchi in females. On intake frequency, rice, kimchi, onion, green pepper and potato were ranked on top 5 in descending order in both genders. Dietary variety score (DVS) and dietary diversity score (DDS) for the assessment of dietary diversity and balance were significantly decreased with aging in both genders. In food group intake pattern (DMGFV), 01101 type without consumption of dairy and fruits was the most prevalent, and only 3.6% of male and 3.9% of female subjects showed 11111 type, consumed all the 5 food groups a day. Dietary pattern of subjects was analyzed by cluster analysis with 18 food groups intake. Overall dietary pattern was classified into two clusters, one was more desirable and the other was less desirable. The percentage of subjects with more desirable dietary pattern was about 15% in male and 32% in female. In conclusion, most of our subjects living in Gugoksundam area were taking very simple diet with low amount of dairy products and fruits. These results indicate that nutrition intervention and education for older people living in rural areas should be focused on various food intake including dairy products and fruits.

한국 성인 여성에서 스트레스 인지 정도에 따른 영양소 및 식품 섭취 상태 - 2014~2015년 국민건강영양조사 자료를 이용하여 - (Nutrient and Food Intakes of Korean Female Adults depending on Perceived Stress - Based on the 2014~2015 Korea National Health and Nutrition Examination Survey -)

  • Stress has been associated with poor eating behavior and diet quality. Yet, the relationship between nutrient and food intake and perceived stress in Korean female adults has not been validated. The purpose of this study is to identify the relationship between nutrient and food intake and perceived stress in Korean female adults. This is a cross-sectional study based on the 2014-2015 Korean National Health and Nutritional Examination Survey data. This study investigated 2,010 female adults age 19~64. In this study, according to subjects' perceived stress, we classified subjects into very low-stress recognition group (VLSRG) (n=211), low-stress recognition group (VSRG) (n=1,223), high-stress recognition group (HSRG) (n=488) and very high-stress recognition group (VHSRG) (n=88). Daily nutrients and food group intake, food groups consumed (Dietary Diversity Score, DDS) were analyzed using data from the 24-recall method. The VHSRG had a larger proportion of subjects that were younger (p<0.0001), single (p<0.0001) and smokers (p<0.0001) compared to the VLSRG. Energy and food intake did not differ between the four groups. Percentage of subjects with protein (p=0.0322), vitamin A (p=0.0364) and vitamin C (p=0.0115) intake lower than the EAR (estimated average requirement) in the VHSRG were significantly lowest in the four groups. Also, food intake percentage from fruits and seaweed groups in the VHSRG were lower than other groups (p=0.0034, p=0.0015). DDS in the VHSRG (3.87) was significantly lowest in the four groups (p=0.0022). Results suggest that perceived stress in Korean female adults may by significant to consider in nutritional education programs.

한국 청소년의 거주지역별 및 체격별 식사의 질 평가지수의 비교 (Comparison of Indices for Diet Quality Evaluation of Korean Adolescents by Residence Area and Body Size)

  • The purpose of this study was to assess several indices of diet quality based on nutrient, food and food group intake of Korean adolescents based on several indices on diet quality according to residence area and body size. Using the data from the 1998 National Health and Nutrition Survey, twenty-four-hour-dietary recalls of a total of 1,110 Korean adolescents aged 13 - 19 years (male 543, female 567) were analyzed for nutrient adequacy ratio (NAR) , index of nutritional quality (INQ) , the number of foods (Dietary Variety Score, DVS) and food group consumed (Dietary Diversity Score, DDS) . In doing that, it was attempted to apply only the minimum amount of solid foods of Kant's without inclusion of liquid foods because of the very limited variety in Korean foods. Based on weight length index, 13.1% of the subjects were categorized as obese, 14.2%, overweight, 44.4%, normal and 28.3%, underweight. Only vitamin B2 intake was higher in the obese group than in the underweight group. There was no meaningful difference in energy, protein and fat intakes according to the grade of the body size. In terms of residence area, intake of fat, niacin, vitamin $B_6$ and folic acid were lower in the rural areas than in the metropolitan city. Only vitamin E intake was higher in the rural areas. Mean value of NARs (MAR) and INQs (mINQ) was also higher in the metropolitan city than in the rural areas, but there was no significant difference of these two values according to body size of the subjects. Mean DVS was 21.02 for total subjects, and has no difference between male and female and between metropolitan city and other medium-small city. But, the rural areas showed the lowest DVS of 19.05. Mean DDS in which five is a maximum score was 3.3 with no significant difference by sex and by residence area in male subjects. However, in female subjects, DDS in the rural areas was the lowest. According to body size of the subjects, there was no meaningful difference in both scores of DVS and DDS. In conclusion, most indices of nutrient intake and food and food group intake were not significantly different by body size of the subjects, while most indices were significantly different by residence areas: higher in the metropolitan city than in the rural area.

성인 여성에서 비만 여부에 따른 식사섭취상태 및 식사의 질 평가 : 2007-2009 국민건강영양조사 자료를 이용하여 (Evaluation of nutrient and food intake status, and dietary quality in Korean female adults according to obesity : Based on 2007-2009 Korean National Health and Nutrition Examination Survey)

  • 본 연구에서는 성인 여성을 대상으로 비만 여부에 따른 영양소 및 식품 섭취의 질을 평가하고, 식사의 질과 비만지표와의 관련성에 대하여 분석하였다. 총 3,565명의 대상자 (정상군 2,841명, 비만군 724명)가 분석에 포함되었고, 그 결과를 요약하면 다음과 같다. 1일 총 섭취 열량에서는 정상군이 1,707.81 kcal로 비만군의 1,634.94 kcal에 비해 유의적으로 높게 나타났으며 (p = 0.0199), 또한 섭취 열량 1,000 kcal당 영양소 섭취량을 분석한 결과 정상군은 비만군에 비해 탄수화물 (p = 0.0298) 및 나트륨 (p = 0.0169)의 섭취량은 유의적으로 낮았던 반면 칼슘 (p = 0.0261)의 섭취량은 유의적으로 높게 나타났다. 칼슘의 경우 정상군에서 권장섭취량 대비 섭취율이 비만군에 비해 유의적으로 높았으며 (p = 0.0178), 평균필요량에 미달되게 섭취하는 대상자의 비율은 유의적으로 낮게 나타났다 (p = 0.0152). 영양소별 NAR 및 MAR을 분석한 결과, 정상군에서 칼슘의 NAR이 0.63으로 비만군의 0.60에 비해 유의적으로 높게 나타났으며 (p = 0.0158), 영양소별 NAR의 평균치인 MAR의 경우 정상군이 0.80, 비만군이 0.79로 두군간 유의한 차이를 보이지 않았다. 또한 1일 총 식품 섭취량은 정상군 1,297.36 g, 비만군 1,271.28 g으로 두군간 유의한 차이를 보이지 않았으나, 우유 및 유제품의 섭취량에서 정상군이 비만군에 비해 유의적으로 높게 나타났다 (p = 0.0162). DDS는 전체 대상자에서 3.753점으로 나타났으며, 정상군이 3.769점으로 비만군의 3.686점 보다 유의적으로 높은 것으로 나타났다 (p = 0.0253). 또한 전체 연구대상자의 DVS는 30.07개로 나타났으며, 정상군과 비만군 각각 30.41개, 28.63개로 나타나 유의적인 차이를 보였다 (p = 0.0003). 식품섭취와 관련된 식사의 질 지수 (DDS, DVS)와 비만과 관련된 신체계측치 (체질량지수, 허리둘레)와의 관계를 분석한 결과, DVS가 많은 대상자들의 허리둘레 및 체질량지수가 더 낮은 경향을 보였으며 (p for trend < 0.01), 이 결과는 연령, 음주 상태, 신체활동 정도 및 열량 섭취량을 보정한 후에도 같은 양상으로 나타났다 (p for trend < 0.01). 이상의 결과를 종합하면, 비만한 성인 여성의 경우 정상 체중인 여성에 비해 칼슘 섭취 및 유제품 섭취의 질이 유의적으로 낮았으며, DDS 및 DVS가 유의적으로 낮아 다양하지 못한 식품 섭취 양상을 보이는 것으로 나타났고, DVS는 체질량지수 및 허리둘레와 유의적인 음의 관계를 보였다. 따라서 이와 같은 결과를 비만한 성인 여성을 대상으로 한 비만 관리에 적용할 때 고른 영양 섭취를 위해 식품군의 구성, 섭취하는 식품의 가짓수 등 영양균형에 중점을 두어 식사지도를 해야 할 것으로 생각되며, 칼슘 섭취의 질과 비만과의 인과관계를 명확히 분석하기 위하여 한국인을 대상으로 한 전향적인 코호트 연구 및 추가 분석이 필요하다고 사료된다.
