  • The purpose of this study was to clarify the dynamic relationships among risk factors of arteriosclerosis and to develop and examine a model which could explain this relationship clearly. Data were collected from medical records of 400 male clients who visited a university hospital located in Inchon for physical examinations, from May 1996 to December 1996. Data were analyzed using the LISREL (Linear Structural Relationship) 8 program. To test the fitness of the hypothesized model, chi-square, RMSR (root mean square residual), GFI (goodness of fit index), CN (critical number) and Q-plot were used. Most of the fitness measurements, except the chi-square showed that the hypothesized model complimented the real data. According to the results, there were trends that obesity and hyperlipidemia were prevalent in heavier smokers, higher alcohol intakers, and groups who excercised less. Also, hypertension was more prevalent in older age, higher alcohol intaker, and higher serum lipid level groups. In contrast to the hypothesis, alcohol intake did not significantly affect serum lipid levels. This might be due to the serum lipid measurements (total cholesterol and trigryceride) used in this study to estimate hyperlipidemia. The direct effect of smoking on hypertension was not significant. However, the total effect of smoking on the hypertension was significant since indirect effects of smoking on hypertension, such as obesity and hyperlipidemia, were significant. The total effect of obesity on hypertension was significant since the indirect effect of obesity on hypertension via hyperlipidemia was significant, although the direct effect of smoking on hypertension was not significant. The degree of explaining hyperlipidemia with smoking, exercise, and obesity was high (60%), however, the degree of explaining obesity with age, smoking, alcohol intake, and exercise was very low (7%). On the basis of these results, high risk factors of arteriosclerosis such as hypertension, hyperlipidemia, or obesity are either directly or indirectly correlated each other. Therefore, it is difficult to predict outcomes for increasing or decreasing the risk factors by simply modulating a factor. Smoking, alcohol, and exercise both directly and indirectly affected major risk factors of arteriosclerosis. Therefore, correcting these variables is required to decrease risk factors. Finally, the relationship among other risk factors which have been known to be related with arteriosclerosis (diet, stress or hereditary) should be clarified in further studies.

  • Zinc deficiency is known to be associated with insulin resistance in obese individuals. This study was performed to evaluate the effect of zinc supplementation on insulin resistance and metabolic risk factors in obese Korean women. Forty obese women (body mass index (BMI) ${\geq}25kg/m^2$) aged 19-28 years were recruited for this study. Twenty women of the study group took 30 mg/day of supplemental zinc as zinc gluconate for 8 weeks and 20 women of control group took placebo. Usual dietary zinc intake was estimated from 3-day diet records. Insulin resistances were measured using Homeostasis model assessment (HOMA) indices, and insulin sensitivities Matsuda indices, which were calculated using oral glucose tolerance test data. Metabolic risk factors, such as waist circumference, blood pressure, fasting glucose, triglyceride, high density lipoprotein (HDL) cholesterol, and adipocyte hormones such as leptin, and adiponectin were also measured. At the beginning of study, dietary zinc averaged 7.31 mg/day and serum zinc averaged $12.98{\mu}mol/L$ in the study group. Zinc supplementation increased serum zinc by 15% and urinary zinc by 56% (P < 0.05). HOMA values tended to decrease and insulin sensitivity increased slightly in the study group, but not significantly so. BMI, waist circumference, blood pressure, blood glucose, triglyceride, HDL cholesterol, and adipocyte hormones did not change in either the study or control group. These results suggest that zinc status may not affect insulin resistance and metabolic risk factors in obese Korean women. Further research is required on a larger cohort with a longer follow-up to determine the effects of zinc status on insulin resistance and metabolic variables.

  • The use of semiquantitative food frequency questionnaires to estimate dietary intake has become widespread in nutritional epidemiologic studies. It had been suggested that knowledge about a person's usual portion size of each meal in addition to consumption frequency will improve the accuracy of this method. This study was performed to investigate the dietary intake status(nutrient, food, and dish intake) and one portion size of commonly consumed food and dishes to develop a semiquantitative food frequency questionnaire that will be used to expore the relationship between the incidence of cardiovascular disease and diet in Korean women after menopause. Intake of food or dish in 123 elderly women were measured by 3-day food records in March to May, 1996. The results obtained in this study were summarized as follow : 1) Calcium, vitamin A, and vitamin E intakes were less adequate than Recommended dietary allowances(RDA). 2) The subjects who consumed more than 20% of the energy from fat were 35% and the subjects consumed dietary cholesterol more than 300mg were 18$\%$. 3) The most frequently consumed foods or dishes by the subjects were Korean cabbage kimchi〉cooked rice, well-milled〉laver〉apple〉strawberry〉mixed rice〉soybean soup. 4) The major dish groups which contributed to most daily nutrient intakes were cooked rice, Korean cabbage kimchi, soybean soup because of large amount and high frequency of intake although they do not contain large amount of most nutrients. 5) One portion size of commomly consumed foods and dishes in elderly women was different from one portion size suggested in the Korean nutrition society. This study provides information for the design of data-based semiquantitative food frequency questionnaire. for Korean elderly women. In addition, they may be useful to health care planners or nutrition educators. (Korean J Community Nutrition 2(4) : 578-592, 1997)

  • Purpose: The purpose of the this study was to examine the dietary knowledge and educational needs of stroke patients and analyze the related factors that are associated with dietary knowledge and educational needs. Methods: Our study analyzed data from 146 stroke patients. The survey was conducted by individual interviews using questionnaires and data collected from participants' medical records. Dietary knowledge and educational needs were measured by tools developed by the author. Results: The mean scores for the dietary knowledge and educational needs were 19.09±3.48 and 36.17±7.18, respectively. Many stroke patients had misconceptions about soy sauce, cholesterol, and fruits. The most needed items in dietary education was first identifying foods that are beneficial or harmful, and second, food interaction with medications. As for the results of multiple regressions, dietary knowledge was significantly associated with gender, educational attainment, monthly income, exercise, body mass index, and level of compliance to dietary therapy. Additionally, the educational needs were significantly related with educational attainment and employment. Conclusion: For secondary prevention of stroke patients, education strategy considering dietary knowledge and educational needs of stroke patients should be established. Special attention is needed for stroke patients with a lower level of dietary knowledge and educational needs.

  • Purpose: To report our experience of endoscopic botulinum toxin injection in patients who experienced severe delayed gastric emptying after pylorus-preserving gastrectomy (PPG). Materials and Methods: We reviewed the medical records of 6 patients who received the botulinum toxin injection. They presented with severe delayed gastric emptying in the early postoperative period. Endoscopic botulinum toxin was administered as 4 injections of 25-50 IU into each of the 4 quadrants of the prepyloric area. Results: All botulinum toxin injections were successful without any complications, enabling 5 patients to tolerate soft solid diets and one to tolerate a soft fluid diet within 10 days. The endoscopic criteria of 4 patients improved. Symptom recurrence caused 2 patients to undergo repeat injections that were successful. The median follow-up period was 27 months, and all patients could ingest normal regular diets at the last follow-up. Conclusions: Endoscopic botulinum toxin injection is a feasible treatment option for early delayed gastric emptying after PPG.

  • The purpose of this study was to investigate zinc and copper metabolism and risk factors of chronic diseases in 20 normal adults women. The daily intake, excretions in urine and feces, and serum levels of zinc and copper were determined by 24-hr food records and chemical analysis. The results were summarized as follows. mean age, body weight and BMI were 22.88 years, 54.65kg and 21.28kg/$m^2$ respectively. Mean daily intakes of energy and protein were 1578.84㎉(79% of RDA) and 52.05g (87% of RDA). The zinc and copper intake, excrestion in urine and feces were 7093.23$\mu\textrm{g}$(59% of RDA/2093.87$\mu\textrm{g}$, 203.50$\mu\textrm{g}$/39.87$\mu\textrm{g}$ and 3416.41$\mu\textrm{g}$/857.62$\mu\textrm{g}$, respectively. The serum levels of fasting glucose, insulin, total cholesterol, HDL-cholesterol, LDL-cholesterol, triglyceride, zinc and copper were 76.60mg/㎗, 15.66IU/㎗, 160.30mg/㎗, 50.95mg/㎗, 89.80mg/㎗, 89.79mg/㎗, 95.65$\mu\textrm{g}$/㎗ and 73.28$\mu\textrm{g}$/㎗ respectively. Dietary ratio of Zn to Cu was shown to have significant positive correlations with serum insulin, total cholesterol, LDL-cholesterol. The urinary ratio of Zn to Cu was shown to have significant positive correlations with triglyceride. The serum copper level was shown to have significant negative correlations with serum total cholesterol and LDL-cholesterol. In summary, Zn consumption of adult women in their normal diet does not meet the Zn requirement for Koreans. Also, intakes of Zn and Cu may effect the glucose metabolism and cardiovascular diseases. Therefore, to increase the Zn intake and to maintain an appropriate intake ratio of Zn to Cu, nutrition education needs to be implemented.

  • This study was conducted to provide insight into the management and care of the elderly in nursing homes. Eighty-six elderly subjects over 65 years old (38 male, 48 female) in 3 non-fee-paying nursing homes, located in Daejeon and Chungchong areas, were studied. Subjects'dietary intakes by estimated food intake records, and self-perceived health conditions, were studied using questionnaires from July 21 to August 1, 1997. Twenty-one % of the male subjects and 42% of the female subjects were over 80 years. Their predominant past occupation was farming. While 8.3% of the female and no male residents showed a BMI (Body Mass Index) of 27 and above, about 30% of the subjects were underweight and in poor health status in seeing, joint pains, lumbago and shoulder pains. Regarding overall health status, 72.1% of the subjects considered them to be in poor health, and female and male subjects suffered more difficulties from cloudy eyes, joint pains and lumbago and shoulder pains than any other. Neuralgia was the predominant chronic complaint and followed by hypertension in both sexes. Overall, female subjects felt worse off than the male subjects in terms of their health status, that can be attributed to higher average age of the female subjects compared to the male subjects. The elderly were eating a very low fat (about 15 g : 6% of total calorie) diet with low vitamin A and E. Intakes of calories, protein and iron slightly exceeded RDA. The phosphorus intake was more than double the RDA although calcium intake was close to the RDA. From these results, it seems important to doubling the fat intake of the elderly residents in the form of vegetable fat with the object of raising of vitamin E, antioxidant vitamin, and essential fatty acids for the elderly. It is also recommended that the elderly residents should be given adequate calcium and exercise for bone health.

  • This study was performed to investigate the effects of alcohol drinking frequency and foods consumed along with alcohol on anthropometry, serum lipid levels, and blood pressures in 73 male type 2 diabetic patients aged 30-59 years old. Dietary data for usual intake were obtained from the subjects by the 3-day food records. Separate data for foods consumed along with alcohol as accompaniment were collected and analyzed for energy and nutrient intakes. Both alcohol drinking frequency and/or the amount of energy consumed from accompaniment influenced clinical data as well as anthropometric measurements. The serum total- and HDL-cholesterol, triglyceride levels and systolic blood pressure were significantly higher in the group with a drinking frequency of ${\geq}$ 2/week than that of ${\leq}$ 1/week and also in the group whose daily energy intake from accompaniment was greater than the median (106.6 kcal/d) than that below the median. When the data were adjusted for age, amount of energy intake from alcohol and diet, the anothropometric measurements such as body weight, BMI, waist circumference were significantly higher in patients whose energy intake from accompaniment was greater than the median than that below the median. The results of our study suggest that both alcohol drinking frequency and the energy intake from foods consumed along with alcohol as accompanements are important contributing factors to clinical and anthropometric parameters whose associations with the cardiovascular complications are well established in patients with diabetes mellitus.

  • BACKGROUND/OBJECTIVES: There are various factors that affect metabolic abnormalities related to obesity. The purpose of this study is to analyze the differences in dietary intakes and body compositions of obese women according to metabolic risks and to classify them as metabolically healthy obese (MHO) or metabolically abnormal obese (MAO). SUBJECTS/METHODS: This study was conducted on 59 obese Korean women aged 19 to 60 years. NCEP-ATPIII criteria were applied and the women classified as MHO (n = 45) or MAO (n = 14). Body composition of each subject was measured by using dual-energy x-ray absorptiometry (DEXA). Three-day food records were used to analyze dietary intake. Eating habits and health-related behaviors were determined through questionnaires. Indirect calorimetry was used to measure resting metabolic rate and respiratory rate. RESULTS: The average age of the subjects was 43.7 years. The analysis of body composition according to phenotype revealed significantly higher body fat mass (P < 0.05), arm fat mass (P < 0.05), and android fat mass (P < 0.05), as measured by DEXA, in the MAO group than in the MHO group. There was no significant difference in the dietary intake of the two groups. However, eating behaviors differed. Compared to the MHO group, the MAO women had a shorter meal time (less than 10 minutes), a preference of oily foods, and a tendency to eat until full. Therefore, the eating habits of MHO women were more positive than those of MAO women. CONCLUSIONS: The results suggest that fat distribution in each body region affects various metabolic abnormalities. A high level of arm fat mass in obese Korean women may increase metabolic risk. In addition, eating habits of obese Korean women are considered to be environmental factors affecting the metabolic phenotype of obese Korean women.
