Chronic osteitic rhinosinusitis as a manifestation of cystic fibrosis: A case report

  • A 28-year-old male patient with a history of cystic fibrosis (CF) was referred to the University of Connecticut School of Dental Medicine for an evaluation of a cystic lesion in the right maxilla using cone-beam computed tomography (CBCT). CF is an autosomal recessive disease characterized by an abnormal production of viscous mucus, affecting the mucociliary clearance. The CBCT scan revealed a large cystic lesion in the right maxilla extending from the right maxillary second molar to the midline in the region of the right central incisor with a significant buccal expansion. Further evaluation revealed complete opacification of the paranasal sinuses with medial bulging of the lateral maxillary sinus walls. The maxillary and sphenoid sinuses also appeared hypoplastic. The peculiar finding seen in this case was the presence of marked sclerosis and an increase in the thickness of the adjacent bony framework. This report aimed to describe the common sinonasal findings associated with CF and its underlying pathophysiology.

Open Lung Biopsy for Diffuse Interstitial Lung Disease (미만성 간질성 폐질환의 개흉 폐 생검)

  • Open lung biopsy was performed in thirty patients for the diagnosis and staging evaluation of interstitial lung disease during the period from January 1987 until December 1992. The age of the patients ranged from 14 to 71 years [mean 48 years], and the patients consisted of 14 males and 16 females. Preoperative FEV1`s were from 0.80 liter to 3.88 liters [mean 1.66]. Other non-invasive diagnostic studies such as PCNA, bronchoalveolar lavage, TBLB, and gallium scan were also done in addition to X-ray and high-resolution chest CT. Tweaty-eight were correctly diagnosed and 2 cases were not [diagnostic yield rate 93.3%]. Among the 28 cases,pathologic diagnosis influenced further treatment regimens and prognostic expectations in 23 cases [82.1%]. The diagnostic non-invasive studies other than open lung biopsy yielded a correct diagnosis without staging only in 5 cases. There was no mortality and only one complication, ARDS ; however, the patient recovered after 5 days ventilator support. Open lung biopsy, which is the gold standard for the diagnosis and staging evaluation of interstitial lung disease can be done safely and has value in clinical decision making. Also knowledge of the involvement of the lesion is important for proper selection of the biopsy site.

Application of alkaline phosphatase staining of cytology specimen for differential diagnosis of canine osteosarcoma (세포 검사시료에서 alkaline phosphatase 염색법을 활용한 개 골육종의 감별 진단)

  • Aspiration of lytic bone lesions is an excellent diagnostic test in the initial evaluation of primary bone tumor. However, cytologically, it can be difficult to differentiate osteosarcoma (OSA) from other bone neoplasms, including fibrosarcoma, chondrosarcoma, synovial cell sarcoma, malignant fibrous histiocytoma and malignant peripheral nerve sheath tumor. The purpose of this study is to introduce alkaline phosphatase (ALP) staining to differentiate OSA from other mesenchymal tumors. Tumors actively producing bone are specifically positive for ALP staining. Unstained, cytologic specimens were incubated for 10 minutes with nitroblue tetrazolium chloride/5-bromo-4-chloro-3-indolyl phosphate toluidine salt-phosphatase substrate. Among 20 cases of cytology specimen, 14 were positive for ALP staining and histopathology, 6 were negative for ALP staining and histopathology. ALP staining was 100% sensitive and specificity for the diagnosis of OSA. Aspirate cytology with ALP staining was a simple, fast, safe and accurate diagnostic test for the evaluation of suspected OSA lesions in dogs.

Performance Evaluation of In Vitro Diagnostic Reagents for Mycobacterium tuberculosis and Non-tuberculous Mycobacteria by FDA Approval (미국 FDA 허가사례를 통해 본 결핵균 및 비결핵 항산균 체외진단용 시약의 성능평가)

  • Tuberculosis (TB) is a bacterial infection disease caused by members of the species Mycobacterium tuberculosis (MTB) complex. Approximately one third of the world's population is infected with TB. In Korea, approximately 40,000 new patients are identified each year. Moreover, infections from non-tuberculous mycobacteria (NTM) have also increased. In the diagnosis of TB and NTM, traditional bacterial cultures are required for 3 to 4 weeks. Therefore, rapid and accurate diagnostic tests for TB and NTM are needed. To distinguish between TB and NTM, a range of diagnostic methods have been developed worldwide. In vitro diagnostic assays are constantly being developed to meet the increasing need for the rapid and accurate identification for TB and NTM. On the other hand, the performance evaluations of in vitro diagnostic reagents for TB and NTM are lacking. Recently, the Korea Food and Drug Administration (KFDA) issued a guideline for in vitro diagnostic reagents for MTB and NTM. Here, this study analyzed the performance of currently developed in vitro diagnostic reagents for TB and NTM in the US FDA. This analysis of US FDA approved molecular assays could serve as a useful reference for an evaluation of the reagent performance of TB and NTM.

  • Digital photographic analysis is currently considered as a routine procedure in clinic because periodic follow-up examinations can provide meaningful information for diagnosis. However, it is impractical to separately evaluate all suspicious lesions with conventional digital photographic systems, which have inconsistent characteristics of the environmental conditions. To address the issue, it is necessary for total diagnostic evaluation in clinic to integrate conventional systems. Previously, a multimodal digital photographic imaging system, which provides a conventional color image, parallel and cross polarization color images and a fluorescent color image, was developed for objective evaluation of facial skin lesions. Based on our previous study, we introduce a commercial product, "DermaVision-PRO," for routine use in clinical application in dermatology. We characterize the system and describe the image analysis methods for objective evaluation of skin lesions. In order to demonstrate the validity of the system in dermatology, sample images were obtained from subjects with various skin disorders, and image analysis methods were applied for objective evaluation of those lesions.

  • Background: This thesis aims at suggesting the direction for the introduction of public free treatment support system according to the establishment of [Special Education Law] and the right settlement of therapeutic support service. Method: It introduced the characteristics and the contents of school based PT & OT, diagnosis and evaluation, and operation method. It set up question items and presented intervention plan, and substantial intervention, beginning from the request of whole process. The diagnostic evaluation was described from 4 aspects, which are consideration matters in the time of document drawing and diagnostic evaluation, chiefly centering around SOAP. The flow of overall treatment support service, the allocation of 16 handicapped children for 1 therapist, and the weekly treatment frequency according to the treatment support location and environment were suggested in the concrete operation method. Result: The concrete method should be explored in order to provide handicapped students with requisite services, which are offered by various experts in the amended 'Special Education Law'. In addition, work condition and social welfare, which are equal to school teachers, should be provided for all experts. Conclusion: Along with these things, special education support center should establish the road-map for the education rehabilitation of the handicapped children from the evaluation of early diagnosis of the handicapped children to treatment support and lifelong education.

  • Simple chest radiography is the most frequently performed examination in radiologic practice. However, with transmitted dose through chest which has the problem of wide variations in absorption, conventional Film/Screen Combinations Method make it difficult to delineate lung field mediastinum & cardia portion and subdiaphragmatic structures. In order to overcome this problem, we have been tried several methods as follows. High Kilovoltage Technique, Rare Earth Screen/Wide Lattitude Film, Use of Compensating Filter and Digital Beam Attenuator and recently Insigh System has been developed. In this paper, we'd like to discuss Insight film through apperciation by measuring the difference of contrast. In order to evaluate insight film. We have done experiments using Insight System with Simple Phase, Picker 550. We could get an result which was able to overcome the limitted dynamic range of conventional Film/Screen System remarkably.

Development of On-line Performance Diagnostic Program of a Helicopter Turboshaft Engine

  • Gas turbine performance diagnostics is a method for detecting, isolating and quantifying faults in gas turbine gas path components. On-line precise fault diagnosis can promote greatly reliability and availability of gas turbine in real time operation. This work proposes a GUI-type on-line diagnostic program using SIMULINK and Fuzzy-Neuro algorithms for a helicopter turboshaft engine. During development of the diagnostic program, a look-up table type base performance module are used for reducing computer calculating time and a signal generation module for simulating real time performance data. This program is composed of the on-line condition monitoring program to monitor on-line measuring performance condition, the fuzzy inference system to isolate the faults from measuring data and the neural network to quantify the isolated faults. Evaluation of the proposed on-line diagnostic program is performed through application to the helicopter engine health monitoring.

  • Pelvic floor muscle is the main subsystem that maintains urinary continence. It is possible to diagnose the degree of the stress urinary incontinence(SUI) by evaluating the contraction pressure of the pelvic floor muscle. Bio-signal measurement system was developed to measure the contraction pressure. Diagnostic parameters were drawn out by analyzing the measured data. Statistical evaluations were done to classify the all subjects with five groups each has similar characteristics. SUI diagnostic algorithm was implemented to each group separately. The accuracy of the algorithm was about 78.9% and utility was confirmed by clinical trial.

DIAGNOSTIC ABILITY OF THE PERIAPICAL RADIOGRAPHS AND DIGITAL IMAGE IN THE DETECTION OF THE ARTIFICIAL PROXIMAL CARIES (인공적 인접면 치아우식증의 구내방사선사진과 디지털 영상의 진단능 평가)

  • Recently, the digital image was introduced into radiological image. The digital image has the power of contrast enhancement, histogram control, and other digitally enhancement. At the point of the resolution, periapical radiograph is superior to the digital image, but enhanced digital procedure improves the diagnostic ability of the digital image. The purpose of this study was to evaluate the diagnostic ability of artificial proximal caries in conventional radiographs, digital radiographs and enhanced digital radiographs (histogram specification). ROC (Receiver Operating Characteristic) analysis and paired t-test were used for the evaluation of detectability, and following results were acquired: 1. The mean ROC area of conventional radiographs was 0.9274. 2. The mean ROC area of unenhanced digital image was 0.9168. 3. The mean ROC area of enhanced digital image was 0.9339. 4. The diagnostic ability of three imaging methods was not significant difference(p>0.05). So, the digital images had similar diagnostic ability of artificial proximal caries to conventional radiographs. If properly enhanced digital image, it may be superior to conventional radiographs.
