$TcpO_2$ 이용한 당뇨병성 신경병증 환자의 조기진단 (A new diagnostic method for diabetic neuropathy using $TcpO_2$)

  • 당뇨병성 신경병증은 당뇨병성 신증, 당뇨병성 망막병증과 함께 당뇨병의 가장 흔한 합병증 중의 하나로 임상에서는 신경전도검사(nerve conduction velocity: NCV)를 실시하여 신경병증의 유무를 진단한다. 그러나 신경전도검사는 피부에 직접적으로 최대 100mA의 전기자극을 가하므로 피검자가 놀라거나 고통을 호소하는 경우가 많다. 본 논문에서는 $TcpO_2$를 이용하여 비관혈적으로 신경병증이 있는 그룹과 정상인 그룹 간의 $TcpO_2$$SpO_2$의 차이를 확인하고, $TcpO_2$$SpO_2$가 어떠한 관계인지 살펴보았다. 또한 $TcpO_2$ 측정방법을 당뇨병성 신경병증의 새로운 조기 진단 방법으로 제안하고자 하였다. 실험은 정상인 50명과 NCV를 통해 신경병증으로 확진 받은 당뇨병 환자 50명을 대상으로 하였으며, 분석 파라미터는 $TcpO_2$, $TcpCO_2$, $SpO_2$이었다. 측정결과 $TcpO_2$는 정상인과 당뇨병성 신경병증 환자 그룹 간에 손과 발에서 모두 통계적으로 유의한 차이가 있었다(p<0.01). $SpO_2$는 검지에서는 정상인과 당뇨병성 신경병증 환자 간에 통계적으로 차이가 없었지만(p>0.05), 엄지발가락에서는 유의한 차이가 있었다(p<0.01). 손가락의 $SpO_2$와 손의 $TcpO_2$는 상관관계가 0.400(p<0.01), 발가락의 $SpO_2$와 발의 $TcpO_2$는 0.471(p<0.01)로 통계적으로 유의한 상관관계를 발견하였다. 정상인과 당뇨병성 신경병증 환자 그룹 간에 손과 발의 $TcpO_2$ 방법의 민감도는 66%, 특이도는 92%로 나타났다. 본 연구를 통해서 $TcpO_2$ 방법이 신경병증의 유무를 확인할 수 있는 사전검사 도구로 사용이 가능할 수 있으며, 주기적인 $TcpO_2$ 검사를 통해 혈관폐색과 신경병증을 조기에 발견할 수 있어 당뇨병으로 인한 혈관폐색과 신경병증을 조기에 진단 할 수 있는 유용한 방법이 될 것으로 사료된다.

  • During long standing hyperglycaemic state in diabetes mellitus, glucose forms covalent adducts with the plasma proteins through a non-enzymatic process known as glycation. Protein glycation and formation of advanced glycation end products (AGEs) play an important role in the pathogenesis of diabetic complications like retinopathy, nephropathy, neuropathy, cardiomyopathy along with some other diseases such as rheumatoid arthritis, osteoporosis and aging. Glycation of proteins interferes with their normal functions by disrupting molecular conformation, altering enzymatic activity, and interfering with receptor functioning. AGEs form intra- and extracellular cross linking not only with proteins, but with some other endogenous key molecules including lipids and nucleic acids to contribute in the development of diabetic complications. Recent studies suggest that AGEs interact with plasma membrane localized receptors for AGEs (RAGE) to alter intracellular signaling, gene expression, release of pro-inflammatory molecules and free radicals. The present review discusses the glycation of plasma proteins such as albumin, fibrinogen, globulins and collagen to form different types of AGEs. Furthermore, the role of AGEs in the pathogenesis of diabetic complications including retinopathy, cataract, neuropathy, nephropathy and cardiomyopathy is also discussed.

Motor and Somato Sensory Evoked Potentials During Intraoperative Surveillance Testing in Patients with Diabetes

  • Cerebral vascular surgery can damage patients' motor and sensory nerves; therefore, neuromonitoring is performed intraoperatively. Patients with diabetes often have peripheral neuropathy and may be prone to nerve damage during surgery. This study aimed to identify factors that should be considered when diabetic patients undergo intraoperative neuromonitoring during brain vascular surgery and to present new criteria. Methods: In patients with and without diabetes who underwent cerebrovascular surgery (n = 30/group), we compared the intraoperative stimulation intensity, postoperative motor power and sensory, glycated hemoglobin (HbA1c) and glucose levels, and imaging findings. Results: Fasting glucose, blood glucose, and HbA1c levels were 10%, 12.1%, and 9.7%, respectively; they were higher in patients with than in patients without diabetes. Two patients with diabetes had weakness, and 10 required increased Somato sensory evoked potential (SSEP) stimulation, while in 16, motor power recovered over time rather than immediately. The non-diabetic group had no weakness after surgery, but 10 patients required more increased SSEP stimulation. The diabetic group showed significantly more abnormal test results than the non-diabetic group. Conclusion: For patients with diabetes undergoing surgery with intraoperative neuromonitoring, whether diabetic peripheral neuropathy is present, their blood glucose level and the anesthetic used should be considered.

당뇨병성 족부 궤양의 치료 (Management of Diabetic Foot Ulcer)

  • In patients with diabetic foot, ulceration and amputation are the most serious consequences and can lead to morbidity and disability. Peripheral arterial sclerosis, peripheral neuropathy, and foot deformities are major causes of foot problems. Foot deformities, following autonomic and motor neuropathy, lead to development of over-pressured focal lesions causing the diabetic foot to be easily injured within the shoe while walking. Wound healing in these patients can be difficult due to impaired phagocytic activity, malnutrition, and ischemia. Correction of deformity or shoe modification to relieve the pressure of over-pressured points is necessary for ulcer management. Application of selective dressings that allow a moist environment following complete debridement of the necrotic tissue is mandatory. In the case of a large soft tissue defect, performance of a wound coverage procedure by either a distant flap operation or a skin graft is necessary. Patients with a Charcot joint should be stabilized and consolidated into a plantigrade foot. The bony prominence of a Charcot foot can be corrected by a bumpectomy in order to prevent ulceration. The most effective management of the diabetic foot is ulcer prevention: controlling blood sugar levels and neuropathic pain, smoking cessation, stretching exercises, frequent examination of the foot, and appropriate education regarding footwear.

제 2형 당뇨병 및 당뇨 합병증의 발생과 Peroxisome Proliferator-Activated Receptor-$\gamma2$ C161T 유전자 다형성과의 관계 (The Association between Peroxisome Proliferator-Activated Receptor-Gamma C161T Polymorphism and Type 2 Diabetic Complications)

  • Objective : Peroxisome proliferator-activated receptor (PPAR)-gamma, a transcription factor in adipocyte differentiation, has important effects on insulin sensitivity, atherosclerosis, endothelial cell function and inflammation. Through these effects, PPAR-gamma2 might be involved with type 2 diabetes and vascular disease, including diabetic complications. Recently, it has been reported that the C161T polymorphism in the exon 6 of PPAR-gamma is associated with type 2 diabetes interacting with uncoupling protein 2 (UCP2) gene, and is associated with acute myocardial infarction. We studied the association of this polymorphism with type 2 diabetes and its complications, such as retinopathy, ischemic stroke, nephropathy and neuropathy in Korean non-diabetic and type 2 diabetic populations. Methods : Three hundred and thirty eight type 2 diabetic patients (retinopathy: 64, ischemic stroke: 67, nephropathy: 39 and neuropathy: 76) and 152 healthy matched control subjects were evaluated. The PPAR-gamma C161T polymorphism was analyzed by PCR-RFLP. Results : PPAR-gamma C161T genotype and allele frequency did not show significant differences between type 2 diabetic patients and healthy controls (T allele: 17.0 vs. 14.5, OR= 1.21, P=0.3188). In the analysis for diabetic complications, T allele in diabetic nephropathy was significantly higher than controls (P=0.0358). T allele in the ischemic stroke patients was also higher than healthy controls, although it had no significance (P=0.1375). Conclusions : These results suggest that the C161T polymorphism of the PPAR-gamma gene might be associated with diabetic nephropathy in type 2 diabetes.

당뇨병성 말초신경병증 통증환자에서 사암침법의 유효성: 무작위 배정 대조군 사전예비연구 (Efficacy of Saam Acupuncture for Diabetic Peripheral Neuropathy - A Pilot, Randomized Controlled Study)

  • Objectives : Diabetic peripheral neuropathy(DPN), generally considered to be the most symptomatically distressing complication of diabetes, affects more than 50% of people with diabetes. However, no consistently effective treatment for DPN is available and patients are forced to struggle with medications that provide only partial relief. In this pilot study, we evaluated the clinical effects of Saam acupuncture for the treatment of painful DPN. Methods : A total of 10 patients with painful DPN were included in the study; 6 subjects with Saam acupuncture treatment and 4 subjects without it. Subjects were defined as having painful DPN if they had at least 2 points using total symptom score(TSS). Treatments were delivered three times a week for 4 weeks. Vitamin $B_{12}$ was orally administrated in the all subjects. At initial(0 week) and follow-up after 4 weeks and 8 weeks, all subjects underwent TSS, Michigan Neuropathy Screening Instrument(MNSI), and nerve conduction test. Results : After initial(0 week) and follow-up(8 weeks), TSS and MNSI were not significantly different between the two groups(p=0.400 and p=0.830, respectively). However, in both two groups, according to time, there was a significant difference in TSS as well as MNSI(p=0.001 and p=0.004, respectively). Conclusions : Saam acupuncture may be considered as the effective treatment for the patients of DPN although the changes of the symptoms were of limited significance in this study. Further investigations are required to elucidate the role of Saam acupuncture for the pain control of DPN.

당뇨병 환자에서의 안쪽 발바닥 감각신경의 신경전도검사: 세 가지 다른 방법의 비교 (Medial Plantar Sensory Nerve Conduction Studies in Diabetics: Comparision of Three Different Methods)

  • Background: The medial plantar nerve (MPN) is a distal branch of the posterior tibial nerve, and various methods of nerve conduction study for MPN have been introduced so far. Hemmi et al described a new method (Hemmi's method) for recording medial plantar sensory nerve action potentials (SNAPs), which is considered as a simple and reliable method for measuring medial plantar SNAPs. This study was aimed to establish the normal values for the MPN conduction study among Koreans and to compare the sensitivities of three different methods for MPN conduction study (Hemmi, Oh, and Saeed's method) in detecting evidence of peripheral neuropathy among diabetic patients. Methods: In 27 healthy subjects, MPN conduction study using Hemmi's method was performed and normal values were calculated. In 54 diabetic patients who showed normal routine nerve conduction studies, three different methods for MPN conduction study were performed and diagnostic sensitivity of each method were compared. Results: In normal subjects, the mean medial plantar SNAP amplitude and conduction velocities measured by Hemmi's method were $4.3{\pm}1.0$ uV and $38.3{\pm}6.8$ m/s respectively. Among 54 patients with diabetes who showed normal routine nerve conduction studies, medial plantar SNAP was not obtainable in 28, 31, and 6 patients by Hemmi, Oh and Saeed's method respectively. Conclusions: In terms of the diagnostic sensitivity for detecting diabetic neuropathy, there had been no significant statistical difference between three different methods. Our study suggested that MPN conduction study using Hemmi's method is simple and useful screening test for early diabetic neuropathy, and is comparable with Oh's method.

말초혈관 혈류에서 진폭-스펙트럼-밀도 분석에 의한 당뇨병에서의 신경병증 및 갑상선 기능 유무 분류 (The classification of neuropathic and thyroid function of the diabetic using amplitude-spectrum-density analysis in peripheral blood vessels)

  • 최근에 당뇨병이 말초혈관 혈류흐름에 치명적인 영향을 준다는 의학보고에 따라 기존의 혈액체취를 통한 혈액 속의 당치를 측정하는 방식 대신에 고분해능을 가진 LDF(Laser Dopller Flowmeter)를 이용하여 말초혈관 혈류를 측정하여 정상인과 당뇨병환자와의 차이점에 대하여 연구하였다. 여러 가지 분석 방법중, 본 연구에서는 주파수영역에서의 ASD방법을 통하여 그 차이를 분석하였다. 실험대상은 음성 신경병증, 양성 신경병증, 갑상선 기능 항진증, 갑상선 기능 저하증 등 4개의 당뇨병환자군과 당뇨병에 대한 정상군에 대하여 LDF를 측정하였다. 그 결과 모든 당뇨환자군은 정상인에 대하여 모든 진동수영역에서 낮은 ASD를 가져 당뇨병이 말초혈관 혈류운동에 손실을 가져옴을 확인하였고, 신경병증을 가진 환자와 갑상선기능저항증 당뇨환자군은 거의 모든 진동수 영역에서 낮은 ASD를 가졌다.

당뇨환자의 불안정 족관절에 있어서의 전방 T자형 금속판을 이용한 족관절 고정술 (Ankle Arthrodesis Using an Anterior T Plate in treating the Charcot joint of Ankle in a diabetic patient - A Case Report -)

  • It has been known that the ankle arthrodesis is a common surgical procedure for treating the ankle arthrosis and deformity that do not respond to the non-operative treatment. To date, various surgical techniques for the ankle arthrodesis have been reported. Clinical and biomechanical trials have shown that the rigid internal fixation leads the increased rate of the union. The ankle arthrodesis may be complicated with the nonunion, delayed union, malunion, and infection. In cases of the Charcot joint of the ankle in diabetic patients, however, arthrodesis could reduce the disadvantage of the nonoperative treatment, such as the loss of the reduction, progressive collapse, multiple ulcerations and infection. The object of this case report is to report our experience of a successful ankle arthrodesis using an anterior T plate in treating the unstable ankle of a diabetic patient, associated with the ankle fracture and the neuropathy. The surgical approach of this technique is simple so that it would allow less soft tissue injury, and this procedure would be regarded as one alternative to provide the rigid internal fixation in the ankle arthrodesis.

당뇨병성 말초신경병증 환자에서 통증 정도와 당화혈색소 간의 상관성 연구 (A Study on the Correlation between Pain Intensity and HbA1c for Diabetic Peripheral Neuropathy Patients)

  • Objectives : The purpose of this study is to evaluate the correlation between HbA1c and pain intensity in diabetic peripheral neuropathy patients, and to compare the difference between two groups divided by the risk of complications. Methods : The participants were 46 men and women suffering from neuropathic pain diagnosed with diabetes mellitus who visited the Hospital of Traditional Korean Medicine, Semyung University from June, 2014 to August, 2015. Age, duration of diabetes mellitus, numeric rating scale(NRS), and Michigan neuropathy screening instrument(MNSI) were used as evaluation tools. Results : 1. In all 46 cases, there were no significant correlations between HbA1c and age, duration of diabetes mellitus, NRS, MNSIQ, or MNSIE. 2. Among 23 cases with a high risk of complications (more than 7.0 % of HbA1c, group A), there were no significant correlations between HbA1c and age, duration of diabetes mellitus, NRS, MNSIQ, or MNSIE. 3. Among 23 cases with a low risk of complications(less than 7.0 % of HbA1c, group B), there were no significant correlations between HbA1c and age, duration of diabetes mellitus, NRS, MNSIQ, or MNSIE. 4. Means of duration of diabetes mellitus, NRS, MNSIQ, and MNSIE were higher in group A than group B, but there were no significant correlations in statistics. Conclusions : This study could not find statistically significant correlations between pain intensity and HbA1c in diabetic peripheral neuropathy patients, so more studies are required in the future.
