Streptozotocin에 의해 유도된 당뇨 흰쥐의 고혈당과 고지혈에 미치는 오미자 추출물의 효과 (Effects of Schizandra Cchinensis Fruit Extract on the Hyperglycemia and Hyperlipemia in Streptozotocin-induced Diabetic Rats)

  • To investigate the antihyperglycemic and antihyperlipemic effect of 80% ethanol extract of Schizandra Chinensis fruit, we induced diabetes in the rats with streptozotocin (STZ) and administered schizandra extract or Acarbose to diabetic rats for 21 days by oral administration. Consequently, the groups treated using schizandra extract decreased blood glucose levels more 39% than no treatment group and the case of Acarbose group was decreased it about 21%. The concentration of cholesterol, triglyceride and LDL-C in blood was also decreased while treating schizandra extract, on the other hand, HDL-C concentration was significantly increased it about 26%. Those results induced that anti-atherogenic index (AAI) in blood was improved more than 82% level like normal condition, especially in treatment of schizandra extract 100 mg. The lipid profiled in feces was likewise showed apparent tendency to decrease and food efficiency ratio of diabetic rats was became higher for treatment with schizandra extract, but Acarbose group had low efficiency in compared with the result of glucose level and lipid profile in blood. As a result, schizandra extract is regard a good medicine for diabetes due to improve physical constitution, blood glucose and lipid level caused hyperglycemia and suggest that schizandra extract has real effects on the diabetes complication as atherosclerosis, coronary heart disease, high blood pressure.

Euglycemic diabetic ketoacidosis development in a patient with type 2 diabetes receiving a sodium-glucose cotransporter-2 inhibitor and a carbohydrate-restricted diet

  • Sodium-glucose cotransporter-2 (SGLT2) inhibitors have become increasingly prescribed because of their proven protective effects on the heart and kidneys, and carbohydrate-restricted diets are popular therapeutic approaches for patients with obesity and diabetes. A 28-year-old obese woman with recently diagnosed diabetes developed euglycemic diabetic ketoacidosis (DKA) while on dapagliflozin, an SGLT2 inhibitor, and following a carbohydrate-restricted diet. She presented with nausea, vomiting, and epigastric pain. Hospital tests showed a blood glucose of 172 mg/dL, metabolic acidosis, and increased ketone levels, confirming euglycemic DKA. Treatment involved discontinuing dapagliflozin and administering fluids, glucose, and insulin. She recovered and was discharged on the fourth day. This is considered a case of euglycemic DKA induced by SGLT2 inhibitors and triggered by a carbohydrate-restricted diet. This case highlights the importance of physicians in confirming the symptoms and laboratory results of DKA, even in patients with normal blood glucose levels taking SGLT2 inhibitors and following carbohydrate-restricted diets. It is also crucial to advise patients to maintain an adequate carbohydrate intake.

  • Objective : The present study was carried out to investigate the preventive effect of Sophorae Flos on streptozotocin - induced Diabetes mellitus. Method : Sophorae Flos was given to rats with oral administration. The experimental animals were divided into 3groups : normal group of rats, control group of Streptozotocin-induced diabetic rats, sample group with Sophorae Flos . The effect of Sophorae Flos on Streptozotocin-induced diabetes was observed by measuring the survival rate of rats, weight of rats, FER, blood glucose, each organ weight of rat, total cholesterol, HDL, GOT, GPT & creatinine. Result : Streptozotocin caused hyperglycemia and hypoinsulinemia by a selectively destroying pancreatic ${\beta}-cell$. Sophorae Flos treatment don't protected them from hyperglycemia and hypoinsulinemia. Organ weight liver, kidney, heart & spleen shows no significant changes. Sophorae Flos significantly don't recoverd the increase of several biochemical parameters such as blood glucose, total cholesterol, HDL, GOT, GPT, antioxidant & creatinine is vice versa. Conclusion : Sophorae Flos extract group did not show significant decrease than Streptozotocin control group.

  • Diabetes is a chronic disease which may lead to other life threatening health complications like heart disease, stroke and peripheral vascular disease that diminished quality of life. In order to assists diabetic patients, we develop a smartphone application to monitor the regular medicine intake and exercise routines through repetitive user friendly and motivational reminders. Health-practitioners including doctors and nurses can frequently view and analyze the daily routine of patients through a web-interface. This paper presents architecture to improve the quality of life with android smart phone as user terminal, and integration of webbased application through web services for health care professionals. We also explain how each of the application interacts with each other and internal integration.

청소년기 당뇨병 환자의 자율신경계 합병증에 관한 연구 (Autonomic Neuropathy in Adolescents with Diabetes Mellitus)

  • 목 적 : 심혈관계 자율신경 기능이상은 청소년기 당뇨병 환자의 7.7-28%에서 동반되며, 당뇨병성 자율신경병증은 심장 교감 신경계 이상으로 인한 돌발성 부정맥 및 돌연사를 초래할 수 있는 것으로 보고되었다. 본 연구는 소아 및 청소년기 당뇨병 환자에서 심혈관계 자율신경 기능이상의 빈도 및 이에 영향을 미치는 인자들을 파악하여 당뇨병성 자율신경병증의 조기진단 및 치료에 도움이 되고자 시행하였다. 방 법 : 8-26세의 1형 당뇨병 환자 80명(남자 27명, 여자 53명)과 2형 당뇨병 환자 12명(남자 4명, 여자 8명)을 대상으로, 발살바 동작시 R-R 간격의 변화(발살바 비), 심호흡시 심박수의 변화, 기립시 R-R 간격의 변화(30 : 15 비), 기립성 저혈압의 4가지 항목에 대하여 검사를 시행하여 환자의 연령, 유병기간, 당화혈색소, 소변 알부민 배설량, 신경전달속도 이상 및 당뇨병성 망막증 유무와의 연관성을 분석하였다. 결 과 : 검사 당시 연령 및 평균 유병기간은 1형과 2형 당뇨병 환자간에 의미있는 차이가 없었으나, 당뇨병의 발병연령은 2형에서 더 높았다. 체질량지수는 1형에 비해 2형에서 의미있게 높았으며, 혈압 및 당화혈색소는 두 군간에 차이가 없었다. 1형 당뇨병 환자의 22.5%에서 조기, 8.7%에서 중등도, 1.3%에서 심한 자율신경병증에 해당하였으며, 2형의 경우 16.7%에서는 조기, 8.3%에서 중등도, 8.3%에서 심한 자율신경병증에 해당하였다. 전체적으로, 심혈관계 자율신경 기능 이상은 1형의 32.5%에서, 2형의 33.3% 에서 나타나 빈도에 있어서 두 군간에 차이는 보이지 않았다. 1형에서는 발살바 비와 유병기간 간에 의미있는 상관관계가 있었으며, 2형에서는 발살바 비와 연령, 심호흡시 심박수 변화와 당화혈색소, 그리고 기립성 저혈압과 유병기간 간에 의미있는 상관관계가 관찰되었다. 1형과 2형 당뇨병 환자 전체를 대상으로 시행한 로지스틱 회귀분석상, 연령(OR=1.133(1.003-1.279), P<0.05) 및 유병기간(OR=1.148(1.009-1.307), P<0.05)만이 자율신경 기능 이상의 의미있는 예측인자로 나타났으며, 당화혈색소, 혈압, 소변 알부민 배설량, 신경전달속도 이상 및 당뇨병성 망막증 유무와는 관련이 없었다. 검사항목 중 발살바 비는 31.5%의 환자에서, 심호흡시 심박수의 변화는 41.3%의 환자에서 경계역 또는 이상소견을 보인 반면, 30 : 15 비 및 혈압변화는 각각 14.1%, 9.8%에서 경계역 또는 이상소견을 보였다. 발살바 비(OR=0.001(0.000-0.083), P<0.01)와 심호흡시 심박수의 변화(OR=0.840(0.754-0.934), P<0.01)는 자율신경 기능이상 유무에 의미있게 영향을 미쳤으나, 30 : 15 비 및 기립성 저혈압은 영향을 미치지 못하였다. 결 론 : 심혈관계 자율신경 기능이상은 소아 및 청소년 당뇨병 환자의 32.6%에서 동반되었으며, 중등도 이상의 뚜렷한 자율신경 기능이상도 10.8%에서 관찰되었다. 혈당 조절이 양호한 환자에서도 자율신경계 합병증의 가능성은 배제할 수 없으며 연령 및 유병기간이 증가할수록 그 위험은 더욱 증가하였다. 소아 및 청소년기 당뇨병 환자에서 자율신경 기능검사시, 발살바 비와 심호흡시 심박수의 변화가 가장 유용한 검사 항목으로 사료된다.

심탄도를 이용한 연속적인 심박수 모니터링 및 당뇨 예측 가능성 연구(파일럿연구) (Heart rate monitoring and predictability of diabetes using ballistocardiogram(pilot study))

  • 연구의 목적은 가정에서 안정 시 인체의 생리적 활력 정보를 센서와 ICT 정보 기술을 통해 연속적으로 수집하는 시스템과 수집된 정보를 이용하여 당뇨병증 유무를 예측하는 인공신경망 기계학습 방법과 필수적인 기본 변수 값을 제시하였다. 연구 방법은 정상인(DM-) 20명과 당뇨병(DM+) 15명을 대상으로 BCG와 ECG 센서의 심박수 측정값의 상관 관계를 분석하였으며 상관 계수는 R2=0.959이다. Artificial Neural Network(ANN) 기계학습 프로그램을 이용하여 당뇨병증 예측 가능성을 확인하였고 입력 변수는 심박변이도의 시계열정보와 심박수, 심박변이도, 호흡율, 박동량 정보, 최저혈압, 최고혈압, 년령, 성별이며 ANN 기계학습 예측 정확도는 99.53%이다. 그리고 향후 ANN 기계학습 방법을 활용하여 BMI 정보를 이용한 당뇨예측 모델, 심장 기능 장애 예측 모델, 수면장애 분석 모델 등의 계속적인 연구가 필요하다.

Streptozotocin 유발 당뇨 흰쥐에서 Tabebuia avellandae의 항당뇨 효과 (Hypoglycemic Effect of Tabebuia avellandae on Streptozotocin-Induced Diabetic Rats)

  • Hypoglycemic effect of Tabebuia avellandae was investigated in the streptozotocin(STZ)-induced diabetic rats. Diabetes was induced in male Sprague-Dawley rats by injections of STZ (45 mg/kg, i.v.). Rats weighing 200-250 g were divided into 6 groups: normal, STZ-control, hexane fr., CHCl$_3$fr., BuOH fr. and $H_2O$ fr. group. Normal and STZ-control rats received 3% Tween 80 only. Four groups of diabetic rats were administered orally at doses of 100, 400, 300 and 400 mg/kg/day of hexane, CHC1$_3$, BuOH and $H_2O$ fr. respectively. Fractions were administered orally to the rats for 7 days after STZ injection. All rats were anesthetized with ether, blood samples were taken by cardiac puncture for clinical chemistry and the rats were killed by exsanguination. Liver, kidney, heart and spleen were removed, weighed and analyzed. We measured glucose, protein, cholesterol and triglyceride levels in the plasma and glycogen, cholesterol and triglyceride levels in liver. The extent of blood glucose decrement in rats administered $H_2O$ fraction was greater than that in the STZ-control rats. The serum cholesterol and triglyceride levels were significantly lowered by administration of $H_2O$ fraction compared with those of STZ-control group. Treatment of rats with Tabebuia avellandae fractions caused decreases in STZ-induced elevation of cholesterol and triglyceride. Liver triglyceride level was significantly lowered hexane and BuOH fraction group compared with STZ-control group. These results suggest that $H_2O$ fraction of Tabebuia avellandae has the hypoglycemic action against STZ-induced diabetic rats.

문제해결식 간호상담과 강화된 걷기운동이 제2형 당뇨병 대상자의 자가간호, 대처기전 및 당대사에 미치는 효과 (The Effects of Problem Solving Nursing Counseling and Intensified Walking Exercise on Diabetic Self-care, Coping Strategies, and Glycemic Control among Clients with DM Type II)

  • Purpose: The purpose of the study was to identify the effects of problem solving nursing counseling and intensified walking exercise on diabetic self-care, coping strategies, and glycemic control among older adults with DM type II. Method: Ninety nine DM patients who were older than 50 were recruited from DM clinics or public health centers and conveniently assigned into three groups: the Polar(n=41), counseling(n=30) and control groups (n=28). Participants in both Polar and counseling groups attended weekly problem solving nursing counseling for 12 weeks. Polar heart rate monitors were used in the Polar group to intensify walking exercise. Data was collected from November 2003 to August 2004 and analyzed by ANOVA or ANCOVA using the SPSS WIN program. Result: After a 12 week intervention, participants in both the Polar and counseling groups reported increased diabetic self care behaviors and decreased blood glucose levels, which is significantly different from those in the control group. There were no distinctively different program effects between the Polar and counseling groups. Conclusion: Based on the findings, we concluded that problem solving counseling alone could have positive effects on diabetic self care and glycemic controls for older adults with DM. Future research is needed to identify long-term effects of the program.
