The purpose of this study was to investigate the effect of low intensity combined kinesitherapy on type 2 diabetes mellitus and to compare the effects of exercise and methodology aspects, the difference in the post - test scores between fasting glucose and blood lipids in the control group (exercise group) and the control group (exercise group) that prescribed the dance - exercise and low intensity combined exercise for the elderly diabetic patients for 12 weeks sample t-test. the main results of the study are summarized as follows. first, fasting blood sugar showed difference in pre - post - difference of group compared to control group. second, the total cholesterol was found to have a statistically significant difference in the experimental group from pre - post - difference. third, the neutral region showed no statistically significant differences in both experimental and comparator groupsfrom pre - post - difference. fourth, high-density lipoprotein cholesterol levels showed no statistically significant difference in both experimental and comparator groups from pre - post - difference. fifth, low density lipoprotein cholesterol showed statistically significant difference in the experimental group from pre - post - difference group. in conclusion, this study is significant in that the combined effects of dance - motion activities and low intensity combined exercise for elderly with type 2 diabetes were quantitatively proved by using physiological index who had not been treated previously.
This study was conducted to identify the effects of exercise therapy applied in an efficacy expectation promoting program that was based on Bandura's self efficacy model on self-efficacy, cardiopulmonary function and metabolism in type 2 diabetes mellitus patients. The study design was nonequivalent pre-test post-test control design. 34 type 2 diabetes mellitus patients who received follow-up care regularly through the diabetic out-patient clinic were randomly sampled for this study. Twenty patients were assigned to the experimental group and fourteen patients were assigned to the control group. To the experimental group, exercise therapy applied in an efficacy expectation promoting program that is composed of individualized exercise prescription for 12 weeks was provided. In case of the control group, they were instructed to continue their usual lives. Data collection period was from March 1998 to June 2000 Data were analyzed using SPSS/WINDOW 10.0 program. The results were as follows. In experimental group, The score of self efficacy has increased from 64.20 to 66.65 after exercise therapy applied in an efficacy expectation promoting program and it was statistically significant(t=2.07, p=.04). The anaerobic threshold has increased from $18.20\;m{\ell}$/kg/min to $19.07\;m{\ell}$/kg/min and it was statistically significant(t=2.05, p=.04). Level of fasting blood sugar has decreased from $188.20\;mg/d{\ell}$ to $155.55\;mg/d{\ell}$ after exercise therapy applied in an efficacy expectation promoting program and it was statistically significant.(t=-2.69, p=.01). For the lipid metabolism, percent body fat has decreased from 27.16% to 26.57% after exercise therapy applied in an efficacy expectation promoting program. In conclusion, the exercise therapy applied in an efficacy expectation promoting program showed positive effect of self-efficacy, cardiopulmonary function and glucose and lipid metabolism.

Purpose Anti-Glutamic acid decarboxylase antibody test (GAD Ab) has been used as a predictor of type 1 diabetes. GAD Ab has also been shown to be highly potent in cerebrospinal fluid (CSF) of patients with suspected diabetic peripheral neuropathy. Recently, it has been known that clinical significance of GAD Ab using CSF is useful for the neurological disorders. However, the reference value of anti-GAD Ab has been provided only for serum. In this experiment, we estimated the reference value of anti-GAD antibody for CSF in neurological patients. Materials and Methods A total of 211 neurological patients were enrolled. Serum and CSF were analyzed by radioimmunoassay (RIA) using commercial RIA anti-GAD Ab kit (RSR, London, United Kingdom). Normal saline was used as the normal CSF control because CSF is most similar to 0.9% normal saline. Results The mean value of normal CSF control was 1.97 U/mL, and two standard deviations (2SD) was 1.44 U/mL. Based on this data, the expected reference range of CSF could be estimated from 0.54 U/mL to 3.40 U/mL Conclusion The reference range of normal CSF control using normal saline obtained with Hoffmann's method. However, there will be a need to validate the CSF reference values using human normal CSF.
This study has been carried out to understand the effect of SP3 and GV6 acupuncture on the hyperglycemic rat induced with Streptozotocin (STZ). Diabetes was induced in experimental groups by inraperitoneal injection of STZ (50mg/kg of body weight) twice by 24 h interval and the additional 100mg/kg 3 days after the earlier treatment. Control group was treated with tail-non acupoint, and experimental groups were treated SP3, GV6 and SP3+GV6 after hyperglycemic induction for 6 weeks. The body weight of control was lower than the experimental groups. The blood glucose was decreased significantly in the experimental groups. Glucose tolerance in acupuncture treated groups was improved. Blood cholesterol level and transaminase activites were lower in the experimental groups than in the control. In the SP3 or GV6 treated rats, hepatocyte degeneration were apparently decreased and the organelles were properly arranged. Furthermore, decrease in liver IGF-I mRNA expression was improved by the acupuncture in STZ-induced diabetic rats. In conclusion, our observation indicate that SP3 or GV6 acupoints treatment can exert beneficial effects in diabetes, with preservation of ${\beta}-cell$ function and liver function.
This study was carried out to provide information on the effect of nutrition education program for diabetic patients at the Guri City's Public Health Center. Subjects of this study were 31 persons(male 7, female 24) who attended all courses of "2002 Diabetes Education Class". They were indicated as the 'education group'(EG). Eating and living habits of EG were investigated before the education. EG's weight and blood glucose (post prandial 2 hours, PP2) were examined as well. EG's PP2 reduction was compared with a 'control group'(CG) who didn't join any course in that class. All of the subjects were non-insulin-dependent diabetes mellitus(NIDDM) patients. EG's average age was 62.4$\pm$8.8. Before taking the course, EG's PP2 was 251.5$\pm$29.6mg/dl, and body mass index(BMI) was 26.3$\pm$2.3 on average. Most of them were stressed out from their daily lives and usually had no exercise. Most people of EG ate meals rapidly and liked sweet and fatty foods. After the course of training, EG's weight and BMI before the training were not decreased significantly. However, all of the EG's PP2s, which were measured 4 times(before the meal at the special lunch session, after 2 hours at this meal, after 2 weeks and 4 weeks dietary assembly), were decreased in comparison with the PP2 which was checked prior to joining the training. EG's average PP2 was more reduced than CG's one. In addition, all groups' PP2s were decreased for 8 weeks. After all, this nutritional education at the public health center was effective in glycemic control for diabetes mellitus patients. Especially, when the dietary assembly as practical training was included in the educational process, the patient's dietary intake and PP2 was improved more effectively. Therefore, this study suggests that nutrition work at public health centers is necessary for the Health Promotion Policy.

Diabetes has been one of major health risks in industrialized countries. Allium hookeri is a wild herb distributed in India and Myanmar. The root of the plant has been used as food and medicine in Southeast Asia. We investigated Allium hookeri extract improves type 2 diabetes mellitus in C57BL/KSJ db/db obese mouse. C57BL/KSJ db/db obese mouse arise out of Type 2 diabetes and we treated Allium hookeri methanol extract 400 mg/kg (AH 400), 800 mg/kg (AH 800), positive control group (thiazolidinedine;TZDs) were administered orally for 8weeks. AH treated group normalized lipid enzyme system (triglyceride, total cholesterol, HDL-cholesterol and LDL-cholesterol) and serum glucose, HbA1c and plasma insulin level. AH treated group recovered β-cell damage by hyperglycemia and fatty liver disease. AH treated group significantly up regulated expression of Peroxisome proliferator-activated receptor gamma (PPAR-γ), pyruvate dehydrogenase kinase4 (PDK4), Sterol regulatory element-binding protein 1c (SREBP 1) and fork head box O1 (FOX 01) proteins in C57BL/KSJ db/db obese mouse liver. And we found that AH treated group decreased hepatic malondialdehyde formation in C57BL/KSJ db/db obese mouse liver. These results indicate that Allium hookeri methanol extract might be a potential anti-diabetic agent and could be useful in the treatment of type 2 diabetes mellitus.
Purpose : We describe the changes of rat glomerular epithelial cells when exposed to high levels of glucose and advanced glycosylation endproducts(AGE) in the in vitro diabetic condition. We expect morphological alteration of glomerular epithelial cells and permeability changes experimentally and we may correlate the results with a mechanism of proteinuria in DM. Methods : We made 0.2 M glucose-6-phsphate solution mixed with PBS(pH 7.4) containing 50 mg/mL BSA and pretense inhibitor for preparation of AGE. As control, we used BSA. We manufactured and symbolized five culture dishes as follows; B5 - normal glucose(5 mM) + BSA, B30 - high glucose(30 mM) + BSA, A5 - normal glucose(5 mM) + AGE, A30 - high glucose(30 mM) + AGE, A/B 25 - normal glucose(5 mM) + 25 mM of mannitol(osmotic control). After the incubation period of both two days and seven days, we measured the amount of heparan sulfate proteoglycan(HSPG) in each dish by ELISA and compared them with the B5 dish at 2nd and 7th incubation days. We observed the morphological changes of epithelial cells in each culture dish using scanning electron microscopy(SEM). We tried the permeability assay of glomerular epithelial cells using cellulose semi-permeable membrane measuring the amount of filtered BSA through the apical chamber for 2 hours by sandwich ELISA. Results : On the 2nd incubation day, there was no significant difference in the amount of HSPG between the 5 culture dishes. But on the 7th incubation day, the amount of HSPG increased by 10% compared with the B5 dish on the 2nd day except the A30 dish(P<0.05). Compared with the B5 dish on the 7th day the amount of HSPG in A30 and B30 dish decreased to 77.8% and 95.3% of baseline, respectively(P>0.05). In the osmotic control group (A/B 25) no significant correlation was observed. On the SEM, we could see the separated intercellular junction and fused microvilli of glomerular epithelial cells in the culture dishes where AGE was added. The permeability of BSA increased by 19% only in the A30 dish on the 7th day compared with B5 dish on the 7th day in the permeability assay(P<0.05). Conclusion: We observed not only the role of a high level of glucose and AGE in decreasing the production of HSPG of glomerular epithelial cells in vitro, but also their additive effect. However, the role of AGE is greater than that of glucose. These results seems to correlate with the defects in charge selective barrier. Morphological changes of the disruption of intercellular junction and fused microvilli of glomerular epithelial cells seem to correlate with the defects in size-selective barrier. Therefore, we can explain the increased permeability of glomerular epithelial units in the in vitro diabetic condition.
Measurement of hemoglobin A1c is used as an objective indicator of long-term blood glucose control in diabetic patients. We evaluated recently introduced Norudia$^{(R)}$ HbA1c (Daiichi Pure Chemical Co. Ltd, Tokyo, Japan) test reagent using enzyme method for HbA1c assay. Linearity, precision and correlation with VARIANT$^{TM}$ II Turbo HbA1c analyzer (BIO-RAD, Hercules, CA, USA) were evaluated. The reference range was determined from 201 healthy subjects. The Norudia$^{(R)}$ HbA1c test reagent was founded to be linear in a range of 5.6% to 14.0% ($r^2=0.9885$). The within-run and between-day precision were 0.954% and 1.03% for low level (HbA1c 5.24%), 0.67% and 1.28% for high level (HbA1c 9.01%), respectively. Comparison study between Norudia$^{(R)}$ HbA1c test reagent and VARIANT$^{TM}$ II Turbo showed good correlation with a slope of 1.0489. an intercept at -0.9717, and coefficient of correlation was 0.9907. The reference range of HbA1c obtained from this reagent was 4.07-5.50%. The Norudia$^{(R)}$ HbA1c test reagent showed good linearity, precision and correlation with HbA1c analyzer with HPLC method. In addition, the exclusive analyzer is not required for assay and then this kit may be useful for HbA1c assay in clinical laboratory.

In this study, the inhibitory effect on advanced glycation end products (AGEs) formation of 43 Korean herbal medicines has been evaluated. Among them, 16 Korean herbal medicines were showed to have significant effect ($IC_{50}$; <50 ${\mu}g/ml$) compared to positive reference, aminoguandine ($IC_{50}$: $76.47{\pm}4.81{\mu}g/ml$). Especially, five herbal medicines, Rubus coreanus (leaves, $IC_{50}$: $4.49{\pm}0.03{\mu}g/ml$), Rubus coreanus (twigs, $IC_{50}$: $3.80{\pm}0.34{\mu}g/ml$), Ampleopsis brevipedunculata (stems, $IC_{50}$: $7.43{\pm}0.09{\mu}g/ml$), Lindera erythrocarpa (leave, $IC_{50}$: $8.14{\pm}0.20{\mu}g/ml$), and Lindera erythrocarpa (stems, $IC_{50}$: $3.69{\pm}0.14{\mu}g/ml$) showed more potent inhibitory activity (approximately 9-20 fold) than the positive control aminoguanidine.

Insulin has crucial roles in energy metabolism in all mammals but has been less studied in ruminants. An experiment was conducted to investigate the effects of hypoinsulinemia induction on appetite, performance, carcass composition and blood metabolite levels in sheep. Treatments were intravenous injection of four doses of streptozotocin; 0, 25, 50 and 75 mg/kg BW named C, L, M and H, respectively. Twenty male lambs were divided into four treatment groups. Animals in group H could not continue the experiment because of abnormalities. The duration of the experiment was eight consecutive weeks, and injection was performed at the end of week 3. Feed and water intakes were measured weekly and weight changes of animals were recorded and used for calculation of other growth parameters. Blood samples were collected weekly via venipuncture at fasting and 2.5 h post-prandial and analyzed for hormones and blood metabolites. Results showed a marked hypoinsulinemia in group M with significant decrease in fasted and postprandial insulin concentrations and also fasted leptin concentrations vs. the control group C (p<0.05). Group M showed significant increases in blood glucose, triglycerides, cholesterol, total protein, blood urea nitrogen and ketone body levels vs. group C (p<0.05). After injection, animals in group M showed diabetic hyperphagia and enhanced water intake as compared to group C (p<0.05) but, despite increased feed intake, they did not gain more weight than controls (p<0.05), and consequently, their feed conversion ratio was greater. Protein and fat contents of meat and liver were not significantly different among groups (p>0.05). In conclusion, the results suggested a regulatory role of insulin in energy metabolism of ruminants by exerting two opposing effects; central catabolic and peripheral anabolic.
