A Case Study of Type-II Diabetes Mellitus: Two Patients with Taeumin's Dry-febrile Symptom (태음인(太陰人) 조열증(燥熱證)으로 진단된 2형 당뇨병 환자 치험(治驗) 2례)

  • Objectives: This study describes how two patients diagnosed with type-II diabetes mellitus were successfully treated using herbal medicine. Methods: The patients were given Yuldahanso-tang to reduce the effects of Taeumin's dry-febrile symptom. Results: Type-II diabetes mellitus symptoms experienced by patients with Taeumin's dry-febrile symptom included thirst, fatigue, weight loss, and numbness in the hands and feet. After administering Yuldahanso-tang, all numerical levels for blood sugar, glycated hemoglobin (HbA1c), glycosuria, and proteinuria recovered to within normal levels. Conclusions: Treatment of patients with Taeumin's dry-febrile symptom and type-II diabetes mellitus with Yuldahanso-tang was effective, and this new evidence supports the use of herbal medicine for treating such patients.

Methylene Tetrahydrofolate Reductase C677T Mutation and Left Ventricular Hypertrophy in Turkish Patients with Type II Diabetes Mellitus

  • This study was designed to investigate, in the Turkish population, the association of methylene tetrahydrofolate reductase (MTHFR) C677T polymorphism and left ventricular hypertrophy (LVH) in patients with type II diabetes mellitus. Our study included 249 patients with type II diabetes mellitus (102 men, 147 women) and 214 healthy volunteers as controls (91 men, 123 women). MTHFR C677T genotypes were determined by polymerase chain reaction, restriction fragment length polymorphism techniques. No differences were observed in the distribution of MTHFR genotypes or allele frequencies in the cases versus the controls. The frequency of the MTHFR-mutated allele (T) was 31.7% in the type II diabetes mellitus versus 31.1% of the controls. The homozygous mutation (T/T) in the MTHFR gene was identified in 12% of the type II diabetes mellitus versus 9.3% of the controls. Patients with the TT genotype showed a higher prevalence of LVH when compared to patients with the CC and CT genotypes (p = 0.01). The MTHFR gene C677T mutation may be a possible risk factor for the development of LVH in the type II diabetic patients.

Clinical Characteristics of Coronary Artery Disease Patients by Comorbidity of Diabetes Mellitus (당뇨병을 동반한 관상동맥질환자의 임상적 특성)

  • Purpose: The study was conducted to find out clinical characteristics for coronary artery disease patients with diabetes mellitus. Methods: We retrospectively reviewed the electronic medical records which included the data of 6,792 patients, who had been diagnosed coronary artery disease (CAD) such as angina or acute myocardial infarction and admitted to a university hospital in Seoul from January, 2005 to November, 2010. Results: Of the 6,792 patients, 43% had been diagnosed diabetes as comorbidity. The CAD patients with diabetes had lower left ventricular ejection fraction, stayed longer at hospital, and spent on more time from the first symptom to hospital visit than those without diabetes. In addition, they were more likely to have multi vessel coronary artery disease. Conclusion: The CAD patients with diabetes lay on the various factors which can make more worsen condition. Hence, we need to pay attention to specialized nursing care and patient education for the CAD patients with diabetes.

  • The Republic of Korea (ROK) has been putting much effort to deal with diabetes which is being increasingly found in all age groups due to inadequate diet. Diabetes is closely related to one's eating habits and lifestyle that often cause hyperglycemia so that it is essential for the diabetic patients to change them to improve or control the major symptoms of diabetes. This study introduces a software engineering solution to support these patients. The software designed for the solution lets the patients to easily recognize their sugar levels and current treatment schemes and then advises a more effective approach along with the essential information through the embedded push service. The major goal of this study is to support diabetes patients by providing a convenient but effective means to prevent or improve diabetic symptoms by patients themselves who will in turn change their lifestyles in a positive manner.

Factors Influencing the Level of Diabetic Retinopathy in Patients with Type 2 Diabetes Mellitus (제2형 당뇨병 환자의 당뇨병성망막증 정도에 영향을 미치는 요인)

  • Purpose: This study was a descriptive survey research to identify whether characteristics of patients with type 2 diabetes mellitus, their knowledge about diabetes, and self-care behavior impacted on the level of diabetic retinopathy. Methods: Participants were 133 patients who had type 2 diabetes mellitus and were being seen at a hospital in Korea. The scale for knowledge about diabetes had 24 items, the scale for self-care behavior, 20 items, and the level of diabetic retinopathy was classified according to the international clinical diabetic retinopathy severity measurement standards. Results: The influence of the independent variables on the level of diabetic retinopathy showed that age, job, time since onset of Diabetes Mellitus, regular ophthalmologic examinations, and systolic blood pressure were identified as factors affecting the level of diabetic retinopathy. The explanation power of this regression model was 23.0% and it was statistically significant (F=5.42, p<.001). Conclusion: Early education about occurrence of diabetes related diseases, specifically diabetic retinopathy should be provided for patients from younger ages. Moreover, for disease management, social support is needed from co-workers and friends. Efforts to encouraged prevention and delay of diabetic retinopathy should include control of blood sugar and blood pressure.

Evaluation of Nutrition Education for Diabetes Mellitus Management of Older Adults (당뇨병 노인을 대상으로 한 영양교육의 효과 평가)

  • Diabetes mellitus is the prevalent disease among older adults. The purpose of this study was to implement and evaluate the nutrition education program for diabetes mellitus patients aged 60 and over. The one group pretest and posttest design was employed to evaluate the program effectiveness. Nutrition education program for diabetes mellitus patients was carried out at the public healthy center in Guri city. The 38 out of 63 patients completed education program. They received four sessions of group education during four weeks. Nutrition education materials (booklet, leaflet) for older adults were provided to participants. Data about blood glucose, blood pressure, nutrition and diabetes mellitus knowledge, dietary behavior, dietary intake by 24-hour recalls were collected before and after nutrition education to evaluate the program effectiveness. All data were statistically analyzed using SAS package (ver.8.2) and significant difference was evaluated by $X^2$-test, paired t-test and Wilcoxon signed rank test. Study results showed that blood pressure and blood glucose were slightly decreased after nutrition education but they did not reach statistical significance. There were positive changes in nutrition knowledge and dietary behavior. The total score of nutrition and diabetes knowledge increased significantly (p < 0.001), and the total score of dietary behavior was improved (p < 0.05) after nutrition education. Dietary intakes of most of nutrients examined were not significantly different between pre- and post-test. Based on study results, it appears that nutrition education program for the aged diabetes mellitus patients might effectively increase nutrition knowledge, dietary behavior and diet quality. This nutrition education program can be used at the public health centers or senior centers for the management of diabetes mellitus for older adults.

Accessible Strategy of the Registration & Management of Hypertension and Diabetes Mellitus Patients through the Public-Private Partnership: Policy Implications Fron the Hongcheon-gun Case (민관협력을 통한 고혈압.당뇨병 등록.관리사업의 접근 전략: 홍천군 사례의 정책적 함의)

  • Objectives: This study aimed to share with experiences of a demonstration program based on a community for prevention and management of hypertension and diabetes mellitus, and to supply the evidence of accessible strategies within the community through the public-private partnershipin the near future. Methods: This study case was "the program of registration and management of hypertension and diabetes mellitus patients" which was conducted in Hogncheon-gun in Gangwon-province, 2012. Results: The infrastructure of this center was constructed with the public-private sector partnership according to the basic model of demonstration program since November, 2012. So, the total registered rate of hypertension and diabetes mellitus patients were 26.6% in comparison with suspected patients (to the result of 2011 Korea National Health and Nutrition Examination Survey), 37.8% in comparison with the real number of outpatient (to the claims data of 2011 Branch Honcheon-gun, National Health Insurance Corporation), and 107.8% in comparison with the project goal, sequentially. To the patients who were not treated for 30 and 60 days among the registered patients, a recall service was conducted. Through this intervention, it was monitored that this program has enhanced the consecutive treatment rate of the registered patients. Conclusions: To improve the continuous management of hypertension and diabetes mellitus patients, we are gotten to know that the community need the joint participation and mutual cooperation with public-private sector partnership.

An Effect of the Health Camp Program for Promoting Self-efficacy in Juvenile Diabetes Mellitus Patients (소아 당뇨병 환자의 자기효능증진을 위한 건강캠프 프로그램의 효과)

  • The purpose of this study was to determine an effect of the health camp program on self-efficacy, sick-role behavioral compliance and glucose metabolism in juvenile diabetes mellitus patients. The quasi -experimental study was designed using a nonequivalent control group pre-posttest design with the framework of Bandura's self-efficacy theory. Forty-two juvenile diabetes mellitus patients participating in this study were selected from outpatients. Of these, twenty-one were assigned to the experimental group and twenty-one to the control group. The period of data collection was from August 8 to December 9 in 1994. The health camp program for the experimental group was carried out over 6 days and the control group did not participate in the program. Collection data was analyzed by means of chi - square test, t - test, ANCOVA, Pearson correlation with SPSS /PC+. The result were summarized as follows: 1. The health camp program was effective in in-creasing the scores of self-efficacy for juven-ile diabetes mellitus patients. 2. The health camp program was effective in increasing the scores of sick - role behavioral compliance for juvenile diabetes mellitus patients. 3. The health camp program was not effective in decreasing the levels of glucose metabolism. 4. The more self-efficacy increased, the more sick - role behavioral compliance increased. 5. Boys showed the higher self-efficacy than girls and the group with diabetes patients whose family members are also patients, showed the higher self-efficacy and sick-role behavioral compliance than the group without diabetic patient among the family members. Thus, it can be concluded that the health camp program was a useful health intervention for juvenile diabetes mellitus patients.

A study on the Polymorphisms in Salivary Parotid Middle-band Protein of the Patients with Diabetes Mellitus (당뇨병 환자의 이하선 타액내 Pm의 다형현상에 대한 연구)

  • The purpose of this study was to evaluate the polymorphism in parotid middle-band protein(Pm) of the patients with diabetes mellitus. Saliva from the parotid glands was collected from 60 healthy Korean who were live in Kwan-ju and from 33 diabetes mellitus patients who had more than 140mg/dl of fasting blood sugar for on week. In the saliva collected from parotid glands, Pm was analyzed to evaluate the distribution of phenotype using acid-urea starch gel elecrophores is The following results were obtained : 1. The phenotypes of parotid middle band protein(Pm) observed in parotid saliva of the control group(60 people) were Pm(+) in 38 people (63.3%) and Pm(-) in 22 people (36.7%). The gene frequency of Pm+ was0.394, and that of Pm- was 0.606. 2. The phenotypes of parotid middle band protein(Pm) observed in parotid saliva of the diabetes mellitus patient group(33 patients) were Pm(+) n 21 patients(63.6%) and Pm(-) in 12 patients (36.4%). The gene frequency of Pm+ was 0.397, and that of Pm- was 0.693 3. Pm dose not have significant differences between phenotypes on both the control group and the diabetes mellitus patient group.

  • Objectives : This report aimed to study of paralytic patient associated with Diabetes Mellitus. Methods : Diabetes Mellitus has one of the highest death rate, because of it induces various complications. Especially paralytic patients who associated with diabetes mellitus are slow in recovery and easy to induce many complications. So control of the diabetes mellitus is the most important thing to treatment. We observed and treated with Cheunhwasangamibang and acupunture. Results : The paralytic patient with Diabetes Mellitus was treatment with Cheunhwasangamibang and improvement was seen. Conclusions : Results in this case support a role for Cheunhwasangamibang in controlling Diabetes Mellitus.
