A Case of Polyradiculomyelitis Associated With Herpes Zoster (대상포진성 다발신경뿌리척수염 1예)

  • Shingles is a latent viral infection of the sensory ganglia. It can be accompanied by a variety of neurologic complications, including polyradiculitis and myelitis. A 66-year-old man with diabetes mellitus presented with progressive weakness, hypethesia and neuralgic pain in his right arm after herpes zoster infection in right C5 dermatome. He was diagnosed with zoster polyradiculomyelitis and treated with intravenous acyclovir and corticosteroid. It is a rare case of zoster neurologic complication in spite of oral acyclovir treatment.

  • Althogh spinal epidural lipomatosis is relatively rare, it is a widely recognized complication of excess exogenous glucocorticoids. We report 4 cases of epidural lipomatosis. None of the patients had definitive history of steroid treatment, obesity, or diabetes mellitus, but all had sciatica and radiating pain. In all cases, the authors removed excess amount of epidural fat with surgery after which significant improvement of symptoms was noted.

  • Herein, we report the case of a 60-year-old man, a smoker with a history of arterial hypertension and diabetes mellitus. After computed tomography (CT) for an episode of hemoptysis, the patient underwent elective thoracic endovascular aortic repair (TEVAR) because of a degenerative aneurysm of the descending thoracic aorta. The area of perianeurysmal pulmonary atelectasis reported on the CT scan was not considered. Three months later, he developed an aortopulmonary fistula without endoleaks. Although TEVAR is a relatively safe procedure, no detail should be overlooked in the preoperative evaluation in order to avoid life-threatening complications. Further, the effectiveness and modality of prolonged antibiotic prophylaxis and/or preoperative respiratory physiotherapy should be assessed in such cases.

  • The effects of various fractions from the whole plant of Pelvetia siliquosa Tseng et Chang (Fucaceae) on the $CCl_4$-induced hepatotoxicity as well as on streptozotocin (STZ)-induced diabetes in rats were investigated. The ether fraction exhibited a potent rat lens aldose reductase (RLAR) inhibition in vitro and showed a significant inhibition of not only serum glucose concentrations but also sorbitol accumulations in the lens, red blood cells and sciatic nerves in the STZ-induced diabetic rats. When administered orally in Sprague-Dawley rats, $H_{2}O$ fraction was found to cause a significant inhibition of the rise in the serum transaminase activities in $CCl_4$-intoxicated rats. These results suggested that this plant might possess constituents with hepatoprotective, anti-diabetic effects and those effects on diabetic complications.

  • Diabetic retinopathy (DR) is a prevalent complication of diabetes that can lead to vision impairment if not diagnosed and treated promptly. This study presents a novel approach for the automated grading of diabetic retinopathy in ultra-widefield fundus images (UFI) using deep learning techniques. We propose a method that involves preprocessing UFIs by cropping the central region to focus on the most relevant information. Subsequently, we employ state-of-the-art deep learning models, including ResNet50, EfficientNetB3, and Xception, to perform DR grade classification. Our extensive experiments reveal that Xception outperforms the other models in terms of classification accuracy, sensitivity, and specificity. his research contributes to the development of automated tools that can assist healthcare professionals in early DR detection and management, thereby reducing the risk of vision loss among diabetic patients.

  • Diabetic retinopathy is a threatening complication of diabetes, caused by damaged blood vessels of light sensitive areas of retina. DR leads to total or partial blindness if left untreated. DR does not give any symptoms at early stages so earlier detection of DR is a big challenge for proper treatment of diseases. With advancement of technology various computer-aided diagnostic programs using image processing and machine learning approaches are designed for early detection of DR so that proper treatment can be provided to the patients for preventing its harmful effects. Now a day machine learning techniques are widely applied for image processing. These techniques also provide amazing result in this field also. In this paper we discuss various machine learning and deep learning based techniques developed for automatic detection of Diabetic Retinopathy.

The Risk Factors for Infectious Complications after Elective Gastrectomy for Gastric Cancer (위암의 정규 위절제술 후 감염성 합병증의 위험인자)

  • Purpose: Postoperative Infectious complications are recognized as major complications that are associated with surgery. Although many studies have focused on the risk factors of postoperative complications, little is known about the risk factors of infectious complications after gastric cancer surgery, and especially after elective gastrectomy. There is now more and more interest in the risk factors of infectious complications in relation to controlling infection and as indicators of qualitatively assessing infectious complications. The aim of this study was to evaluate the risk factors related with infectious complications after performing elective gastrectomy for treating gastric cancer. Materials and Methods: We retrospectively reviewed a total of 788 patients who had undergone elective gastrectomy for gastric cancer between Jan. 2000 and Dec. 2007. The characteristics of the patients were divided according to the patients' factors and the operations' factors. Results: The patients' mean age was 58.9 (range: 24~91) years; 545 were male and 243 were female. The mean duration of the hospital stay was 20.3 days (range: 5~135 days), the mean operation time was 181.3 minutes (range: 65~440 minutes). The total complication rate was 17.1% (n=135) and the complication rate was 38.5% (n=52) among the 135 patients with infectious complications. The infectious complications were surgical site infection (59.7%), Pneumonia (19.3%), intra-abdominal abscess (11.5%), pseudomembranous colitis (5.7%), bacteremia (1.9%) and hepatic abscess (1.9%). On the univariate analysis, the significant risk factors were male gender, blood transfusion, smoking at the time of diagnosis, alcohol drinking, diabetes mellitus and previous cardiovascular disease (P<0.05 for all). On multivariate analysis that used a logistic regression model, the significant independent risk factors were smoking at the time of diagnosis (OR: 2.877. 95% CI: 1.449~5.713), blood transfusion (OR: 3.440, 95% CI: 1.241~9.534), diabetes mellitus (OR: 3.150, 95% CI: 1.518~6.538), and previous cardiovascular disease (OR: 2.784, 95% CI: 1.4731~5.2539). Conclusion: Pre- or post-operative blood transfusion and the patient's medical history such as previous cardiovascular disease, diabetes mellitus, smoking etc. are the risk factors for infectious complications after undergoing elective gastrectomy for gastric cancer. The patients that have these risk factors need to be treated with great care to prevent infectious disease after elective gastrectomy.

Acute Renal Failure after On-pump Coronary Artery Bypass Surgery (체외순환하 시행한 관상동맥우회술 후 발생한 급성신부전증)

  • Acute renal failure (ARF) is a common postoperative complication after the cardiac surgery. Postoperative ARF have various causes, and are combined with other complications rather than being the only a complication. It deteriorates the general condition of the patient, and makes it difficult to manage the combined complications by disturbing the adequate medication and fluid therapy. We have planned this study to evaluate the effects of postoperative ARF after the on-pump coronary artery bypass surgery (CABG) on the recovery of patients and identify the risk factors. Method and Material: We reviewed the medical records of patients who underwent CABG with cardiopulmonary bypass by a single surgeon from Jan. 2000 to Dec. 2002, We checked the preoperative factors; sex, age, history of previous serum creationism over 2.0 mg/㎗, preoperatively last checked serum creatinine, diabetes, hypertension, left ventricular ejection fraction, intraoperative factors; whether the operation is an emergent case or not, cardiopulmonary bypass time, aortic cross clamp time, the number of distal anastomosis, postoperative factors: IABP. Then we have studied the relations of these factors and the cases of postoperative peak serum creatinine over 2.0 mg/㎗. Result: There were 19 cases with postoperative peak serum creatinine over 2.0 mg/㎗ in a total 97 cases. Dialysis were done in 3 cases for ARF with pulmonary edema and severely reduced urine output. There were 8 cases (42.1%) with combined complications among the 19 patients. This finding showed a significant difference from the 5 cases (6,4%) in the patients whose creatinine level have not increased over 2.0 mg/㎗. The mortalities are different as 1.3% to 10.5%. The risk factors that are related with postoperative serum creatinine increment over 2.0 mg/㎗ are diabetes, the history of previous serum creatinine over 2.0 mg/㎗ and left ventricular ejection fraction. Conclusion: Postoperative ARF after the on-pump CABG is related with preoperative diabetes, the history of previous serum creatinine over 2,0 mg/㎗ and left ventricular ejection fraction. Postoperative ARF could De the reason for increased rate of complications and mortality after on-pump CABG. Therefore, in the patients with these risk factors, the efforts to prevent postoperative ARF like off-pump CABG should be considered.

Effecs of Soyangin Indongdeungjikolpitang on the Complication of Diabetes (소양인(小陽人) 인동등지골피탕(忍冬藤地骨皮湯)이 중소합병증(中消合倂症)에 미치는 영향(影響))

  • The purpose of this research was to investigate the effects of Indongdeungjikolpitang water extract(IJTE) on the complication of diabetes. IJTE did not affect the level of blood glucose in alloxan- or streptozotocin-induced hyperglycemic mice, but inhibited the motility of gastrointestine. IJTE inhibited the writhing syndrome induced by acetic acid, the permeability of evans blue into peritoneal cavity induced by acetic acid, the paw edema induced by histamine, and the formation of cotton pellet granuloma. IJTE increased the cell viability of thymocytes and splenocytes. IJTE decreased the release of ${\gamma}-interferone$(${\gamma}-IFN$) and interleukin-2(IL-2), but did not affect the release of interleukin-4(IL-4) from murine thymocytes. IJTE increased the release of IL-4 and decreased the release of tumor necrosis $factor-{\alpha}$($TNF-{\alpha}$) and $interleukin-1{\beta}$($IL-1{\beta}$), but did not affect of ${\gamma}-IFN$ and IL-2 from murine splenocytes. IJTE decreased the release of $TNF-{\alpha}$ and $IL-1{\beta}$ from murine peritoneal macrophages. IJTE decreased the production of niric oxide(NO) from murine peritioneal macrophages and increased the phagocytic activity of murine peritoneal macrophages. These results suggest that IJTE has an anti-inflammatory action via the inhibition of $TNF-{\alpha}$, $IL-1{\beta}$ and NO production from immune cells.

Effect of Dietary Supplementation of β-Carotene on Lipid Peroxide Level and Antioxidative Vitamins of Diabetic Rats (β-Carotene의 보충 급여가 당뇨쥐의 지질과산화물과 항산화비타민 함량에 미치는 영향)

  • Diabetic vascular complications such as atherosclerosis have been reported as one of significant obstructions in treatment of diabetes. There has been a significant increase in recognition of the importance of antioxidative nutrients such as vitamin E, for the prevention of diabetic vascular complication by oxidative stress. This study focused on the effect of dietary $\beta$-carotene on the levels of lipid peroxide and antioxidative vitamins of diabetic rats. The plasma glucose level, hepatic level of lipid peroxide and contents of antioxidants such as vitamins A and E were determined in STZ-induced diabetic rats. Dietary supplementation of B-carotene did not reduce the blood glucose in diabetic rats. Hepatic level of lipid peroxide tended to increase in diabetic rats, but $\beta$-carotene intake reduced the value. Plasma levels of retinol and retinol/lipid were not changed by dietary supplementation of $\beta$-carotene. There was no significant difference among experimental groups in plasma level of $\alpha$-tocopherol. Hepatic levels of retionl and retinyl palmitate were increased by dietary supplementation of $\beta$-carotene in diabetic rats. These results suggest that the supplementation of $\beta$-carotene to the normal diet of diabetics may reduce the incidence of the diabetic vascular complications through the improvement of antioxidants depletion.
