혈액투석 환자의 사전의료의향서 지식, 연명치료중단에 대한 태도 및 삶의 질의 관계 (The Relationships between Knowledge on Advance Directive, Attitudes towards the Withdrawal of Life-sustaining Treatment and Quality of Life in Hemodialysis Patients)

  • Purpose: This study aims to investigate the relationships between knowledge on advance directive, attitudes towards the withdrawal of life-sustaining treatment and quality of life among hemodialysis patients. Methods: A descriptive correlational study was conducted with 103 hemodialysis patients. Data were collected using a structured questionnaire from May to September of 2016, and analysed by using descriptive statistics, t-test, ANOVA and Pearson correlation coefficient. Results: The participants' knowledge level on advance directive was $5.47{\pm}2.08$ out of 9, the attitudes towards the withdrawal of life-sustaining treatment was $3.22{\pm}0.49$ out of 5, and the quality of life was $3.35{\pm}0.92$ out of 6. The knowledge on advance directive was positively correlated with attitudes towards the withdrawal of life-sustaining treatment (r=.21, p=.037) and quality of life (r=.21, p=.036). Conclusion: According to the results of this study, the level of knowledge on advance directive is preferred to improve the quality of life of hemodialysis patients. It is needed to support and maintain ongoing education opportunities in order to improve the level of knowledge on advance directive among patients undergoing hemodialysis.

입원환자의 수면 방해 요인이 수면의 질에 미치는 영향 (Factors Influencing Quality of Sleep of Disturbance Factors Sleep by Inpatients)

  • Purpose. The purpose of this study was to identify the factors affecting quality of sleep and sleep disturbance among inpatient. Methods. A descriptive correlational study was conducted Participants were 200 inpatient in a hospital located in B city. A structured questionnaire was used for data collection. Collected data were analyzed using descriptive statistics, t-test, one-way ANOVA, Pearson's correlation, and stepwise multiple regression. Result. Hospitalized patient's score of sleeping quality was 38.26 on an overall scale of 60, indicating it to be higher than 'Medium (30 points). Almost hospitalized patient in this study suffer from sleep disturbances. As results of univariate analyses, Educational level, Departments, Why hospitalized, Insomnia / depression, Chronic Disease, Scale of pain, physical symptoms, environmental factors, emotional factors, and sleep promoting behavior, disturbed sleep among hospitalized. However, the result of stepwise multiple regression analysis identified that physical symptoms, environmental factors, emotional factors, sleep promoting behavior, and Why hospitalized disturbed sleep and were significant score of sleeping quality for hospitalized and these sleep disturbance factors accounted 46.8% of variance of sleeping quality among hospitalized. Conclusion. These findings suggest that hospitalized patients with poor sleep quality should have their health carefully screened for physical symptoms, environmental factors, emotional factors, sleep promoting behavior, and why hospitalized. In addition, we recommend the development of a nursing program for improving sleep quality.

일 중소병원 입원노인환자의 수면양상에 영향을 미치는 요인 (Factors Affecting Sleeping Patterns among Hospitalized Elderly)

  • Purpose: The purpose of this study was to identify the factors affecting sleeping patterns among hospitalized elderly. Methods: A descriptive correlational study was conducted. Participants were 121 hospitalized elderly in a hospital located in a county. A structured questionnaire was used for data collection. Collected data were analyzed using descriptive statistics, t-test, one-way ANOVA, Pearson's correlation, and stepwise multiple regression. Results: Almost a half of hospitalized elderly in this study suffer from sleep disturbances. As results of univariate analyses, physical symptoms, anxiety, depression, and environmental factors disturbed sleep among hospitalized elderly. However, the result of stepwise multiple regression analysis identified that anxiety and environmental factors were significant predictors of sleeping patterns for hospitalized elderly and these two factors accounted 32.5% of variance of sleeping patterns among hospitalized elderly. Conclusion: Sleep assessment should be conducted on admission to identify sleep difficulties and thereby to improve quality of nursing care. Nurses and other health care personnels should make efforts to decrease anxiety and to eliminate environmental barriers of sleep among hospitalized elderly. Educational programs for nursing staff that offer information about sleep and sleep promoting interventions for the elderly patients are critical. Moreover, sleep promotion intervention program should be developed, applied and evaluated.

미혼임부의 사회적 지지, 건강증진행위 및 우울과의 관계 연구 (A Study on the Relationship between Social Support, Health Promoting Behaviors and Depression among Unmarried Pregnant Women)

  • Purpose: The purposes of this study were to examine the level of social support, health promoting behaviors and depression among unmarried pregnant women and to identify the relationship between social support, health promoting behaviors and depression. Method: A descriptive correlational study was conducted. The participants were 102 unmarried pregnant women receiving shelter services from four facilities in two metropolitan cities. Data was collected using a self-administered questionnaire. Descriptive statistics, ANOVA and Pearson correlation were used for data analysis. Results: The level of social support and health promoting behaviors were relatively lower and the level of depression was relatively higher than those of married pregnant women. The participants received especially low social support from their unmarried partner. There was a positive relationship between social support and health promoting behaviors. Moreover, there were negative relationships between social support and depression and between health promoting behaviors and depression. Conclusions: To promote physical and emotional health of unmarried pregnant women, more attention is necessary to increase their social support. A nursing intervention program to increase social support among unmarried pregnant women in needed.

요양병원 진폐 환자의 심폐증상, 수면의 질 및 우울에 관한 연구 (Cardiopulmonary Symptoms, Quality of Sleep, and Depression in the Hospitalized Patient with Pneumoconiosis)

  • Purpose: This study was to examine the cardiopulmonary symptoms, quality of sleep, and depression, and to identify the influencing factors in the hospitalized coal worker's pneumoconiosis patient. Methods: 137 hospitalized patients who received pneumoconiosis treatment were conveniently selected. A descriptive correlational study was conducted. Data were collected using structured questionnaires and were analyzed using descriptive statistics, t-test, ANOVA, Pearson's correlation coefficients and multiple regression analysis. Results: The mean score of cardiopulmonary symptoms was relatively high ($3.97{\pm}0.56$, out of 5). The 5 and over score of quality of sleep, sleep disturbance was 94.2%, and the reported depression was 95.6%. The levels of depression depended on the oxygen therapy and family/acquaintance visiting. The depression was significantly correlated with the cardiopulmonary symptoms and quality of sleep. The influential factors affecting depression were cardiopulmonary symptoms and quality of sleep, which explained about 34.1% of the variance. Conclusion: The results of this study indicate that nursing interventions are needed to reduce depression, and to improve cardiopulmonary symptoms and quality of sleep in the hospitalized pneumoconiosis patient. These results can provide for nursing intervention to facilitate reduction of depression.

임상간호사의 가족지지도가 직무 스트레스에 미치는 영향 (Family Support and Job Stress of Clinical Nurses)

  • Purpose: This study was done to investigate the level of family support, job stress in clinical nurses end identify the participant's individual characteristics that influenced each of them. Methods: This study was a descriptive correlational study. The participants were 192 clinical nurses working in local middle range hospitals. The data were collected from May to June, 2013 and analyzed using descriptive statistics, t-test and Pearson's correlation by PASW statistics20. Results: The item mean score for family support was 21.01 and job stress was 98.89. There was a statistically significant negative correlation between family support and job stress (r=-.12). The level of job stress differed according to age (p=.014), duration of career (p=.013), department area (p<.001), position (p=.005), work duty form (p<.001), family member (p=.021), burden (p=.031), marital status (p=.001) and satisfaction (p<.001). Conclusion: Family support is needed to reduce job stress. Also, job satisfaction improvement programs and effective working systems should be developed to decrease the nurse's workplace stress.

급성관상동맥증후군 환자의 건강상태에 영향을 미치는 요인 (Factors Influencing Health Status in Patients with Acute Coronary Syndrome)

  • Purpose: This study was done to investigate the correlation of illness perception, coping strategy and health status in patients with acute coronary syndrome who had undergone a coronary angioplasty. Methods: A descriptive correlational design was used in this study. A total of 102 patients with acute coronary syndrome was recruited. Data were collected using structured questionnaires including scales for the illness perception, the coping strategy, and health status. Data were analyzed using descriptive statistics, t-test, one-way ANOVA, Pearson correlation coefficient, and multiple regression analysis. Results: Health status showed significant correlations with illness perception (r=-.44, p<.001) and avoidance coping (r=-.33, p=.001); illness perception, avoidance coping explained 21% of variance in health status (F=8.58, p<.001). The factors that influenced health status were illness perception (${\beta}=-.37$, p<.001), and avoidance coping (${\beta}=-.21$, p<.001). Conclusion: The findings of this study indicate that illness perception and avoidance coping were significant variables for health status in patients with acute coronary syndrome who had undergone a coronary angioplasty.

내분비요법 중인 폐경 전 유방암 환자의 디스트레스 영향 요인 (Factors Influencing Distress in Premenopausal Patients with Breast Cancer Undergoing Endocrine Therapy)

  • Purpose: The purpose of this study was to investigate relationships among menopausal symptoms, functional status, and distress and to identify factors influencing distress in premenopausal breast cancer patients who had been on endocrine therapy. Methods: A descriptive correlational study was conducted. Data were collected using questionnaires from 140 patients with breast cancer undergoing endocrine therapy at a general hospital. Data were analyzed using descriptive statistics, t-test, ANOVA, Tukey HSD test, Pearson's correlation analysis and hierarchical regression analysis. Results: The mean scores for menopausal symptom, functional status, and distress were $19.65{\pm}7.86$, $2.67{\pm}0.33$ and $3.69{\pm}2.19$, respectively. The menopausal symptoms and distress were positively correlated (r=.76, p<.001). The menopausal symptoms and functional status (r=-.43, p<.001) and functional status and distress (r=-.31, p<.001) were negatively correlated. The most influential factor for distress was menopausal symptoms (${\beta}=.79$, p<.001). Conclusion: Based on the findings of this study, developing nursing intervention programs focusing on decreasing menopausal symptoms and distress are recommended.

Do depression, anxiety, emotional intelligence, and sleep quality affect diabetes management self-efficacy in Korean women with gestational diabetes in pregnancy?: a descriptive correlational study

  • Purpose: This study aimed to identify factors associated with diabetes management self-efficacy in pregnant women with gestational diabetes mellitus (GDM) in Korea. Methods: A total of 173 pregnant women with GDM in Korea were recruited by posting announcements at two Korean online communities focusing on pregnancy and GDM. Participants completed a structured online survey from July to September 2018. Thirteen inappropriate responses were excluded and a total of 160 questionnaires were used in the final analysis. Descriptive statistics were calculated and multiple regression with the enter method was done to identify the associations of depressive mood, anxiety, emotional intelligence, and sleep quality with diabetes management self-efficacy. Results: Respondents reported a moderately depressive mood (mean, 10.36), low to moderate anxiety (mean, 41.65), above-average emotional intelligence (mean, 78.04), moderate sleep quality (mean, 42.01), and above-average diabetes management self-efficacy (mean, 52.29). The major factor associated with diabetes management self-efficacy of pregnant women with GDM was emotional intelligence (β=.51, p<.001). Other factors, in descending order of influence, were sleep quality (β=.22, p<.001) and exercise (β=.18, p=.004). Taken together, the aforementioned factors explained 34.6% (F=39.53, p<.001) of the total variance. Conclusion: The results of this study suggest that to improve the diabetes management self-efficacy of pregnant women with GDM, it is necessary to develop an education program that can also enhance emotional intelligence, sleep quality, and exercise.

뇌졸중 환자의 불확실성, 생리적 위험지표, 자기효능감이 자기관리에 미치는 영향 (Influence of Uncertainty, Physiologic Risk Factors, Self-efficacy on Self-management in Stroke Patients)

  • Purpose: The purpose of this study was to investigate the level of uncertainty, physiological risk factors, self-efficacy, and self-management among stroke patients and to identify factors influencing their self-management. Methods: A descriptive correlational design was used for this study. A convenience sample of 149 patients with stroke were enrolled at C national university hospital from February to April in 2016. Data were collected using a structured questionnaire and electronic medical record. Collected data were analyzed using descriptive statistics, t-test, ANOVA, Pearson correlations, and multiple regression analysis with the SPSS/WIN 21.0 program. Results: There were significant negative correlations between uncertainty and self-efficacy (r=-.56, p<.001); between uncertainty and self-management (r=-.56, p<.001); and between total cholesterol and self-management (r=-.23, p=.005). There were significant positive correlations between self-efficacy and self-management (r=.78, p<.001); between uncertainty and total cholesterol (r=.24, p=.003). The significant factors influencing self-management were uncertainty and self-efficacy. Theses variables explained 62.7% of the variance in self-management. Conclusion: The results suggest that intervention programs to reduce the level of uncertainty and to increase the level of self-efficacy among patients would improve the self-management of stroke patients.
