Depression, Anxiety, ADHD, and Impulsivity with Fire Officers (소방관의 우울, 불안, ADHD, 충동성)

  • Objective : This study aimed to evaluate the association of exhaustion, ADHD, self-esteem, impulsivity, suicidal tendencies and quality of life as well as depression and anxiety in fire officers in Korea. Methods : This study was undertaken for the first time in Korea, from September 2016 to December 2016. A set of questionnaires was provided to a total of 113 fire officers and 284 other participants as a comparison group. All subjects were evaluated using the Beck's Depression Inventory, Beck's Anxiety Inventory, Malach Burnout Inventory, Korean Adult Attention Deficit Hyperactivity Disorder Scales, Rosenberg's Self-esteem Inventory, Baratt's Impulsivity Scale, Beck's Scale of Suicide Ideation, and Quality of Life Scale. Furthermore, the odds ratio of depression, anxiety, exhaustion, ADHD, self-esteem, impulsivity, suicidal tendency and quality of life were evaluated the fire officer group and control group were compared through logistic regression analysis. Result : The results of this study indicated that the fire officers were more likely to show depression, anxiety, exhaustion, ADHD, low self-esteem and higher impulsivity. Also, the association between the fire officer group and depression, ADHD and impulsivity was confirmed through logistic regression analysis. Conclusion : Timely and effective evaluation and treatment of impulsivity, as well as depression is required for fire officers.

Status of Antepartum Depression and Its Influencing Factors in Pregnant Women (임신 여성의 산전우울 실태 및 영향요인)

  • The purpose of this study was to identify the rate and influencing factors on antepartum depression in pregnant women. A self-administered questionnaire-based study was performed in 160 pregnant women who visited outpatient clinics or attended special lectures of the general hospital in D metropolitan city. All participants were interviewed using the Korean version of Edinburgh Postnatal Depression Scale(EPDS). The data were analyzed using descriptive statistics, t-test, ANOVA, Pearson's correlation coefficients, and stepwise multiple regression. On the results, the mean score of antepartum depression was $7.80{\pm}4.86$ scores(0-30 scores). The percentage of all participants for EPDS scale were 56.3% with normal level, 20.6% with a boundary line and 23.1% with serious level. Influencing factors on antepartum depression were accounted for 36.7% of the total variance which consisted of perceived health status, pregnancy stress and self esteem. The results indicate that these variables should be considered in preventing depression of pregnant women.

Depression, Attachment and Addiction Problems in Runaway Youths (가출청소년에서 우울증상과 애착 및 중독문제)

  • Objectives: This study investigated the relationship between depression, attachment and addiction problems in runaway youths. Methods: We sent a questionnaire package to 187 runaway youths in 18 adolescent shelters. The questionnaire package collected their sociodemographic data and included the Patient Health Questionnaire-9, Inventory of Parent Attachment (IPA), Korean Internet Addiction Self-Scale (K-Scale), Korean version of Alcohol Use Disorders Identification Test, and Korean version of the $Fagerstr{\ddot{o}}m$ Test for Nicotine Dependence. Results: The percentage of youth with depression was 28.8% (N=54). In the depression group, there were significantly more problematic alcohol users (53.06%, p=.0199) and significantly lower IPA scores (p=.0064). There was a significant negative correlation between depression and a IPA, and significant positive correlation between depression and the K-Scale. Conclusion: These results suggest that runaway youths with depression have more problematic alcohol drinking and attachment problems.

The relationship between workplace violence, depression, burnout, subjective health status, job and life satisfaction of physical therapists in South Korea

  • Objective: To investigate the physical therapists' actual experience of workplace violence and examine its relationship with depression, burnout, subjective health status, and job and life satisfaction. Design: Cross-sectional study. Methods: The level of workplace violence was assessed using the Korean Workplace Violence Scale composed of 24 questions in the following 4 subscales of workplace violence. Depression was assessed using the Korean version of the Patient Health Questionnaire-9. Burnout was assessed using the Korean version of the Maslach Burnout Inventory-Human Services Survey, which is a 5-item scale. Subjective health status was measured on a 5-point scale. Job and life satisfaction was measured based on the assessment tool used in the World Values Survey. Results: There was a significant correlation between the three subscales of workplace violence and health indicators, including depression, subjective health status, job and life satisfaction (p<0.05). The organizational protective system subscale showed a significant negative correlation with depression and a positive correlation with subjective health status as well as job and life satisfaction (p<0.05). The area of 'psychological and sexual violence from customers' showed a significant moderate correlation with depression and job satisfaction (p<0.05). Also, there was a negative correlation that was significant between depression and subjective health status, job satisfaction, and life satisfaction (p<0.05). Conclusions: This study suggested that future studies should continue to investigate and reveal the causal relationships between workplace violence and physical therapists' health indicators.

A Study of Psychological Distress, Anxiety and Depression on Motor Recovery of Acute Bell's Palsy Patients' Facial Muscle (불안 및 우울이 급성기 벨마비 환자의 안면근 운동기능 회복에 미치는 영향)

  • Objectives : The aim of this study is to investigate the influence of anxiety and depression on motor recovery of acute Bell's palsy to estimate how much psychological factors affect the clinical prognosis. Methods : A total of 20 inpatients with acute unilateral Bell's palsy within 1 week of onset participated in this study. The severity of participants' facial palsy was measured by Yanagihara(Y-system) score, FDI and House-Brackmann scale at the time of 1 week and 3 weeks from the onset. The motor recovery of acute Bell's palsy is defined as ${\Delta}Y$-system during 2 weeks. Beck anxiety scale(BAI) and the center for epidermiologic studies depression scale(CES-D) were adopted to assess anxiety and depression, respectively. Correlation analysis and linear regression analysis were conducted between ${\Delta}Y$-system and prognostic factors including anxiety and depression. Results : Significant associations were found between ${\Delta}Y$-system and depression(CES-D) but no significant associations were found between ${\Delta}Y$-system and other prognostic factors, hypertension, diabetes, postauricular pain, disgeusia, age, degree of initial palsy and anxiety(BAI). And a regression equation with 0.295 for coefficient of determination was obtained. Through this analysis, the ${\Delta}Y$-system can be predicted using regression equation which cover 29.5 % of depression index(CES-D). Conclusion : Depression is a significant clinical prognostic factor on motor recovery of acute Bell's palsy. So, Bell's palsy treatment should be combined with psychological care and support.

Associations Between Resilience and Gratitude in Patients With Depression and/or Anxiety Disorders (우울 및 불안장애 환자에서 리질리언스와 감사 성향의 관련성)

  • Objective : The levels of resilience and gratitude were examined in psychiatric patients with depression and/or anxiety disorders and compared to those of the general population. In addition, we analyzed the associations of positive affect, negative affect, depressive and anxiety symptoms, and gratitude on resilience in this patient population. Methods : The participants included a general population sample (n=155) and psychiatric outpatients with depression and/or anxiety disorders (n=108). All participants completed self-report questionnaires, which included demographic variables, the Connor-Davidson Resilience Scale (CD-RISC), the Gratitude Questionnaire-Six-Item Form (GQ-6), the Positive Affect Negative Affect Scale (PANAS), and the Hospital Anxiety and Depression Scale (HADS). Results : The CD-RISC and GQ-6 scores of patients with depression and/or anxiety disorders were significantly lower than those of the general population. In the patient population, hierarchical regression analysis showed that gratitude was significantly associated with resilience while controlling for a range of demographic and clinical variables. Conclusion : Patients with depression and/or anxiety disorder generally had lower resilience and gratitude. Moreover, our findings showed that gratitude could influence the levels of resilience in patients with depression and/or anxiety disorders.

Relationships between Health, Depression, Memory Self-Efficacy and Metamemory in Adults (성인의 건강, 우울, 기억, 자기효능과 메타기억과의 상관관계 연구)

  • Defining prediction variables related to metamemory for the adults in aging process has worthwhile meaning from the perspective that the produced results can be helpful to reducing the difficulty of memorizing efforts and it can also enhance quality of life of aged. This study attempted to analysis relationship between perceived health status, depression, memory self-efficacy and meta memory for the subjects of middle age and old age adults. This study was designed by adopting descriptive correlational analysis method for the 468 middle and old age adults who are living in Seoul. Samples were selected by convenience sampling. Data collection was done over 1 month period in june 1998. The instruments used in this study were health status measuring scale including depression measuring scale, memory self efficacy measuring scale and metamemory measuring scale which were verified for reliability. Data collected were analized by using SPSS for frequency, Peason correlation, t-test and ANOVA according to the variables character and the study purposes. Results of the study were as follows. 1. Relationship between perceived health status, depression, memory self-efficacy and metamemory. Relational analyses between perceived health status, depression, memory self-efficacy and metamemory supported the hypotheses of 1st, 2nd and 3rd(p < .01). These results suggested that the aged perceived great health status then their memory self-efficacy, and metamemory showed the high scores. In the case of depression when its level became decreased metamemory was inclined to increased. Thus, it is identified that strong relationship exists between these variables. 2. Perceived health status, depression, memory self-efficacy by subject's general characteristics. Scores of perceived health status were high in the group of man compared to the group of women, and also highly educated group showed great perceived health status. Group of persons having occupation showed high score of perceived health status and low depression score. The score of memory self-efficacy and metamemory showed higher in the middle aged than the old aged. The high scores of memory self-efficacy and metamemory were found in the group of highly educated people and who have continuing education. The high scores of memory self-efficacy were found in the group of persons having their job and high metamemory scores found in the group of persons having religion. In summary, the greater perceived health status and memory self-efficacy, the more metamemory scores were likely increased and the more depression level was decreased, the more metamemory was likely increased. Also it was found that general characteristics like educational level, continuing education and religion influenced the metamemory of the aged. Therefore, prevention the aged from getting depression and activation of health promotion are needed to delay time of memory loss.

The Effect of Medical Exercise Therapy Program on Ankle pain, Range of Motion, Stress After Traumatic Injury, and Depression in a Stroke Patient with Inflammation on Subcutaneous Bursa of Ankle Joint: Case Study (발목관절 피하밑주머니에 문제가 있는 뇌졸중 환자에게 의학적 운동치료가 발목 통증, 관절가동범위, 외상 후 스트레스 증상, 우울증에 미치는 영향: 사례연구)

  • Background: This study was to investigate the effect of 8-weeks medical exercise therapy on ankle pain, range of motion, stress symptom after traumatic injury, and depression, in a 51 years old stroke patient with right ankle joint inflammation. Method: The 8-weeks medical exercise therapy program was applied to 4 grades of Dosage 1 (1-3 weeks), Dosage 2 (4-5 weeks), Dosage 3 (6-7 weeks), and Dosage 4 (8 weeks) on right ankle joint inflammation in a female with right hemiplegia admitted to D hospital located in Gyeonggi-do. Result: The findings showed that visual analogue scale (VAS) scores improved from 8 to 0-1 scores, passive range of motion (ROM) increased to 5 degrees more than before, Korean-version impact of event scale-revised (IES-R-K) scores increased from 61 to 31 scores, and Korean-version beck depression inventory II (BDI-II-K) scores decreased from 51 to 17 scores. As such, the 8-week medical exercise therapy program may decrease the pain, increase ROM, improve stress after traumatic injury, and improve depression symptom. Conclusion: The presented evidence suggests that exercise and physical activity have beneficial effects on depression symptoms. It is possible to apply the medical exercise therapy for modulating pain experience and treating pain. Also, it may be effective methods to treat the psychological aspects of pain.

Long-term Results of Stereotactic Psychosurgery (뇌정위적 정신수술의 장기 추적 결과)

  • Objective : Stereotactic psychosurgery is known as one of effective means of treating in some medically intractable psychiatric illness. However, it is unfamiliar and it's long-term clinical result has not reported in our country. The authors evaluated its long-term results of pscychosurgery and discussed its neuroanatomical basis. Methods : Since 1993, eight patients underwent stereotactic psychosurgery for medically intractable psychiatric illnesses. All were referred from psychiatrist of these disorders, one was aggressive behavior, five were obsessive-compulsive disorders(OCD), and two were depression with anxiety disorders. Bilateral amygdalotomy and subcaudate tractotomy were done for aggressive behavior, and limbic leukotomy was done for OCD and depression with anxiety. The results of OCD were evaluated with with YBOCS(Yale-Brown obsessive compulsive scale) and VAS (visual analogue scale), CGI(clinical global impairment) in OCD, and OAS(overt aggression scale), MMS, WAIS were checked for the evaluation of aggressive behavior. Hamilton depression scale(HAMD) was used for evaluation of depression. Ventriculography was used in the first five patients and MR-guided stereotaxy was used in recent three cases for localization of target. The lesions were made with radiofrequency lesion generator. Results : With long-term follow up(mean 45 months) in five OCDs, mean YBOCS declined from 34 to 3(n=5). All returned to previous social life. In OAS scores of aggressive behavior during six-year follow up, scores declined from 8 to 2 with clinical improvement. In two patients with depression with anxiety, HAMD declined from 28.5 to 16.5(n=2). There was no operative mortality and no significant morbidity except one case of mild transient urinary incontinence. Conclusion : With these long-term results, authors assumed that stereotactic psychosurgery could be one of safe and effective mtherapeutic methods in several medically intractable psychiatric illness.

Factors Influencing Successful Aging in Elders (노인의 성공적 노화와 영향요인)

  • Purpose: This study was conducted to evaluate successful aging (SA) in elderly individuals and to determine the factors that influence SA. Methods: The subjects included 207 elderly individuals from D city, Korea. The data were obtained between July 1 and August 30, 2008. The participants were assured of anonymity and confidentiality. For the evaluation, Kim & Shin's SA scale, Lawton's instrumental activities of daily living (IADL) scale, Jon's self-esteem scale, Kee's geriatric depression scale short form-Korean (GDSSF-K), and Hong's productive activities scale were used. The data were analyzed using descriptive statistics, the t-test, analysis of variance (ANOVA), and Pearson correlation coefficients; multiple regression analysis was performed using the SPSS/WIN 10.0 program. Results: The maximum score for SA was 34, and the mean score was 22.62. It was found that SA was significantly correlated to IADL, self-esteem, depression, and productive activity. Depression was the most powerful predictor of SA and it accounted for 40.6% of the total variance in SA. Self-esteem, productive activity, living situation, economic status, and age together accounted for 62.1% of the total variation in SA. Conclusion: Depression, self-esteem, and productive activity were identified as variables that influence SA. On the basis of these results, we conclude that nurses should assist elderly individuals in order to facilitate SA. Moreover, individualized nursing management strategies must be developed in order to facilitate SA.
