Radiographic Bone Density Around Immediately Placed Titanium Implant on the Extraction Socket of Diabetic and Insulin-Treated Rat Maxilla (당뇨쥐 상악에서 발치후 즉시 식립 임플란트 주위골의 방사선 골밀도)

  • Purpose: Although it is generally accepted that patients with controlled diabetes have similar rates of success for dental implants as healthy individuals, the use of dental implants in diabetic patients is controversial. In addition, the impact of diabetes on the healing of bone associated with immediately place dental implants is not completely understood. The purpose of this study was to measure bone response to implants radiologically in uncontrolled and insulin-controlled diabetic rats. Materials and Methods: Twenty rats were divided into control, insulin-treated and diabetic groups. The rats received streptozotocin (60 mg/kg) to induce diabetes; animals in the insulin-treated group also received three units of subcutaneous slow-release insulin. Two titanium implants ($1.2{\times}3$ mm) were placed in the extraction socket of the maxillary first molars of the animals and were harvested at 3 days, 1, 2 and 4 weeks. The bone density was measured by digital radiography using gray-level analysis (histogram) in the regions of interest (ROI) at four points: two mesial and two distal to both sides of the implant. Results: The results showed that the osseointegration of the implants was impaired in the diabetic rats compared to the control and the insulin-treated rats. The radiographic evidence demonstrated marked destruction of bone around the implants in the diabetic group. Both the control and the insulin-treated groups had a significantly higher bone density on radiograph than the diabetic group from the 1 week of the experiment (P<0.05 for each comparison). Conclusion: The present study revealed that the immediate placement of titanium implants in the maxilla of diabetic rat lead to delay in the maturation of bone adjacent to implants. It is expected that the reduced predictability of success of immediate implantation in patient with the uncontrolled diabetes.

Factors Influencing Health Status by the salutogenic Theory of Radiologists (방사선사의 건강생성이론에 의한 건강상태 영향요인)

  • This study is based on the theory of salutogenesis, and investigates factors affecting the health status of radiologists who experience high stress and health problems in the rapidly changing medical environment. The study enrolled 379 radiologists working at hospitals in Daegu. A health structure model was established by applying the theory of salutogenesis. Sense of coherence had a direct effect on the health status of radiologists. Psychological stress, social support, and drinking were also influential factors, due to their mediating effect on the sense of coherence. Moreover, social support was also found to be lower when job stresses were higher. Hence, increased psychological stress resulted in lower levels of sense of coherence, social support and exercise. Taken together, results of this study indicate that effects on the sense of coherence have maximum influence on the health status of radiologists, and social support among general resistance resources is an important factor for enhancing sense of coherence. In addition to social support and health behaviors, researches involving various resources and the development of organizational strategies and health promotion intervention programs are required in future, to enable radiologists to cope with their stresses and improve health.

A Phantom Study for the Optimal Low-dose Protocol in Chest Computed Tomography Examination (흉부 전산화단층촬영검사를 위한 최적의 저선량 프로토콜에 관한 팬텀연구)

  • The purpose of this study was to evaluate optimal CT scan parameters to minimize patient dose to the irradiation and maintain satisfactory image quality in low-dose chest computed tomography (CT) scans. In a chest anthropomorphic phantom, chest CT scans were performed at different kVp and mA within reference of 3.4mGy in volume CT Dose Index (CTDIvol). The following quantitative parameters had been statistically evaluated: image noise, signal-to-noise ratio (SNR), contrast-to-noise ratio (CNR), and figure of merit (FOM). Nine radiographers conducted the blind test to select the optimal kVp-mA combination. Results indicated that the kVp-mA combination of 80kVp-90mA, 100kVp-50mA, 120kVp-30mA and 140kVp-30mA were obtained high SNR and CNR. The 120kVp-30mA combination offered good compromise in the FOM, which showed the quality and dose performance. In the blind test, an image of 80kVp-90mA obtained a high score with 4.7 points, and 120kVp-10mA or 140kVp-10mA with a low tube current were observed severe noise and poor image quality, thus resulting in decreased diagnostic accuracy. On the other hand, in the combination of high kVp and high mA(140kVp-90mA), the image quality was improved, but the radiation dose was also increased. the FOM value of 140kVp-90mA was lower than 120kVp-30mA. The application of appropriate scan parameters in low-dose chest CT scans produced satisfactory results in dose and image quality for the accuracy of the clinical diagnosis.

  • Digital imaging for general rediography has many advantages over the film/screen systems, including a wider dynamic range and the ability to manipulate the images produced. The wider range means that acceptable images may by acquired at a range of dose levels, and therefore repeat exposures can be reduced. Digital imaging can result in the over use of radiation, however, because there is a tendency can be reduced. We investigated the actual exposure dose conditions on general radiography and a questionnaire survey was conducted with radiotechnologiest at medical institutions using digital radiology system. As a results, the dose of exposure was not controlled with patient's figure and dose optimization but was controlled by worker's convenience and image quality. Radio-technologiests often set up the exposure dose regardless of patient figure and body part to be examined. Many organizations, such as the International Commission on Radiological Protection, recommend to keep the dose as low as possible. In addition, they strongly recommend to keep the optimal but minimal dosage by proper training programs and constant quality control, including frequent patient dose evaluations and education.

A Study on Intracavitary Therapy in Cervix Cancer Using Needle for Interstitial Therapy (조직 내 삽입용 바늘을 이용한 자궁경부암의 강내치료에 관한 연구)

  • Based on the data of cervical cancer patients who were treated by the radiotherapy with concurrent chemotherapy at the radiation oncology department of National Cancer Center from January 2002 to February 2003, we have studied the method recommended by ICRU 38 to maximize the prescription dose to the planing target volume (PTV) with minimizing the dose to surrounding normal organs. Clinical stage of the patients are 1 patient for stage IB, 3 patients for IIA, 19 patients for IIB, 3 patients for IIIA, 3 patients for IIIB and 1 patient for IV. All patients took the MRI before treatment and the maximum size of the gross tumor volume were under 4cm for 17 patients and from 4 cm to 6 cm for 12 patients and above 6 cm for 1 patient. The results show that while the irradiated volume can be reduced with optimized dose distribution using PTV treatment planning (p<0.0001) when the remained tumor size is small, the surrounding normal organs will receive unnecessarily large dose when the irradiated tumor volume is relatively large. This is because there is some limitation in controling the intensity of radiation in Fletcher Williamson Applicator. To overcome the limit of applicator and to achieve the optimal dose distribution, we have virtually applied 4 needles with Fletcher Williamson Applicator for 10 patients who have relatively large tumor and studied the change in dose distribution before and after application. The results show that this new virtual treatment plan reduces the volume covered by 100 % isodose (p=0.0608, p=0.0607) and reduces the dose of normal organs (p=0.0162, p=0.008). This evidence suggest that this method is superior than the currently used method such as PTV treatment and ICRU treatment.

Relative ratio about dose value of thermoluminescence and optical stimulated luminescence dosimeter according to exposed condition in diagnostic radiation (진단방사선의 노출 조건에 따른 열형광선량계와 광자극형광 선량계의 선량값 상대비)

  • The purpose of this study was to find out the difference of radiation dose value through energy, exposure number, fluoroscopy time, the number of days of exposed scatter X-ray when TLD and OSLD is used in diagnostic radiology. The difference of value were measured by relative ratio and interval. Energy makes high relative ratio of TLD($1.81{\pm}0.41$) than OSLD($1.40{\pm}0.26$), exposure number makes high of OSLD($1.40{\pm}0.26$) than TLD($2.10{\pm}0.10$). There are no significant differences between relative ratio of TLD and OSLD in fluoroscopy time and the number of days of exposed scatter X-ray. But interval of relative ratio in the number of days of exposed scatter X-ray was narrowed in less 0.2. That means, the measurement of scatter X-ray could more confident in TLD and OSLD than the measurement of direct ray. In conclusion, we have to recognize the relative ratio of TLD and OSLD could be vary depending on exposed condition of radiation. And in some cases, double test of TLD and OSLD get more creditable results of dose value.

  • Patients diagnosed with the serious disease of cancer may have anxiety and fear of closed spaces while receiving radiation therapy. This study investigated patients receiving treatment based on Linac and Tomotherapy to look into this anxiety and fear. Study method was survey. The survey was performed according to treatment duration (treatment within 5 minutes, 10 minutes, over 20 minutes, and over 30 minutes). The patients were also surveyed about any experience of changing treatment rooms or machines due to machine failure. A total of 200 survey questionnaires with full answers were researched in this study. As a result, it was found that the less the patients' experience on radiation therapy, the higher their anxiety was. The dominating reason for this result was because the patients expected possible pain during the treatment process. In terms of treatment machine, Linac showed the highest anxiety of all for the openness of its patient stand. The most stable status was found in the case of treatment between 20 minutes and 30 minutes using Tomotherapy. The reason was the coziness of Tomotherapy machine. In the case of receiving the treatment for over 30 minutes, patients felt anxious for the isolation from the outside. The study findings are expected to serve as the necessary data for quality medical service with enhanced patient satisfaction in the clinical field.

Factor Analysis of Decreased Score on Coronary Artery Calcium Score (관상동맥 석회화점수 감소 요인 분석)

  • The purpose of our study was to retrospectively evaluate the cause of a decreased calcium score of follow-up studies on coronary artery calcium scores (CACs) computed tomography (CT). The subjects were healthy 100 people(85 males $60.6{\pm}6.9$ years, 15 females $67.2{\pm}7.3$ years). The subjects decreased CACs were divided into 4 subgroups depending on Agatston classification, minimal (1-10), mild (11-100), moderate (101-400), severe (400<). As a result of decreased CACs were scan location disagreement 51%, motion artifact 26%, equipment changes 14%, operator mistakes 5%, input miss 2%, image loss 1%, arrhythmia 1%. In the mild group, the most common decreased CACs were 49 people. In the minimal group, the most significant variation reduction has occurred to 6 people. Scan location disagreement was considered a partial volume effects due to the scan starting position. It showed less than 100 CACs a high variation (19.7%) in more than 100 CACs, a lower variation (2.2%), these could be seen that the variation range is different that can be tolerated according to the calcification score. Motion artifact factor was found in 26%, which is so closely related to the preceding tests that affect the higher heart rate like this pulmonary function test, exercise stress test.

A Study on Dose and Image Quality according to X-ray Photon Detection Method in Digital Radiography System (Digital Radiography System에서 X선 광자 검출 방식에 따른 선량 및 화질 특성에 관한 연구)

  • The purpose is a comparative evaluation in the DR System according to the dosimetry and image quality of the quantitative and objective via Direct digital radiography, Indirect digital radiography, Image intensifier (Charge Coupled Device type) digital radiography. The experimental method used rando phantom and measured the entrance surface dose. And through using the measured entrance surface dose and then using the PCXMC program were evaluated risk due to irradiation and the effective dose. SNR and NPS and CNR were measured and analyzed by using 21cm acryl phantom. Significance of measured value was evaluated by statistics method. Entrance surface dose, major organ dose, effective dose all of them were measured the lowest rated in direct DR when it is on the basis of direct DR dose, high-dose ratio were measured in I.I DR approximately 1.3 times, indirect DR approximately 2.4 times. Risk in accordance with radiation also was measured same as dose ratio. On the conclusion that SNR measurement result based on direct DR SNR measurements, low-SNR ratio were measured in I.I DR approximately 7.25 times, indirect DR approximately 1.48 times. On the conclusion that CNR measurement result based on direct DR CNR measurements, high-dose ratio were measured in I.I type DR approximately 1.16 tims and low-dose ratio were measured in indirect DR approximately 0.87 times. Therefore Direct DR system using a-selenium sensing element to detect x-ray photon is thought effectively at the examination such as infant to sensitive irradiation and the genital gland. Because quality image is built by low dose. Also when it is necessary that image test requiring many diagnosis information, indirect DR system is thought effectively.

Feasibility of Two Dimensional Ion Chamber Array for a Linac Periodic Quality Assurance (선형가속기의 품질관리를 위한 2차원이온전리함배열의 유용성)

  • Aim of this study is to investigate the feasibility of 2D ion chamber array as a substitute of the water phantom system in a periodic Linac QA. For the feasibility study, a commercial ion chamber matrix was used as a substitute of the water phantom in the measurement for a routine QA beam properties. The device used in this study was the I'm RT MatriXX (Wellhofer Dosimetrie, Germany). The MatriXX consists of a 1,020 vented ion chamber array, arranged in $24{\times}24\;cm^2$ matrix. Each ion chamber has a volume of $0.08\;cm^3$, spacing of 0.762 cm. We investigated dosimetric parameters such as dose symmetry, energy ($TPR_{20,10}$), and absolute dose for comparing with the water phantom data with a Farmer-type ionization chamber (FC65G, Wellhofer Dosimetrie, Germany). For the MatriXX measurements, we used the white polystyrene phantom (${\rho}:\;1.18\;g/cm^3$) and also considered the intrinsic layer (${\rho}:\;1.06\;g/cm^3$, t: 0.36 cm) of MatriXX to be equivalent to water depth. In the preliminary study of geometrical QA using MatriXX, the rotation axis of collimator and half beam junction test were included and compared with film measurements. Regarding the dosimetrical QA, the MatriXX has shown good agreements within ${\pm}1%$ compared to the water phantom measurements. In the geometrical test, the data from MatriXX were comparable with those from the films. In conclusion, the MatriXX is a good substitute for water phantom system and film measurements. In addition, the results indicate that the MatriXX as a cost-effective novel QA tool to reduce time and personnel power.
