A case of bisphosphonate-related osteonecrosis of the jaw with a particularly unfavourable course: a case report

  • Bisphosphonates are drugs used to treat osteoclast-mediated bone resorption, including osteoporosis, Paget disease, multiple myeloma, cancer-related osteolysis, and malignant hypercalcemia. The use of these drugs has increased in recent years as have their complications, especially bisphosphonate-related osteonecrosis of the jaw (BRONJ), which more frequently affects the mandible. Here we report a case of BRONJ with a particularly unfavorable course due to cervical inflammation that developed into necrotizing fasciitis, followed by multiorgan involvement leading to septic shock and death.

  • 구순 구개열 환자의 악교정 성형수술로 구강내 Le Fort II 골절단술이나 비중격 성형술이 많이 이용되는데, 이때 비골의 외측골절단술을 요하며, 이에 대한 술 후 합병증으로 비루관의 폐쇄나 비골의 분쇄골절 등이 발생할 수 있어, 악안면기형 환자를 다루는 구강악안면외과의사에게 비루관의 웅용해부학적 연구는 중요하다. 본 연구의 목적은 교합면을 기준으로 촬영된 컴퓨터 단충 촬영에서 비루관의 위치와 크기를 조사하는데 있다. 2000년 7월부터 2003년 2월까지 서울대학교병원 구강악안면방사선과에서 컴퓨터 단층촬영을 시행한 환자 62명을 대상으로 비상악봉합선에서 비루관까지의 최단거리와 비루관의 최대반경과 최소반경을 측정하였다. 우측비상악 봉합선에서 비루관까지의 거리는 5.68mm이고, 좌측은 5.67mm였다. 좌우 및 성별간의 차이는 없었다. 이의 해부학적 지견은 악기형 수술시의 비루관 폐쇄라는 합병증의 예방책으로 기여하리라 생각된다.

  • 편측 왜소증의 얼굴을 상하악과 하악 모두 신장할 필요가 있는 경우 하악을 신장하여 상악도 신장시키는 방법을 최근 소개한 바 있으나 본 논문에서는 오히려 상악을 신장시킬 때 하악도 같이 신장시키는 방법을 개발하여 좋은 결과를 얻었다. 두 명의 편측안면 왜소증 환자에서 양악 동시에 골 신장술을 시행하였다. Ortiz Monasterio의 방법과 달리 상악은 구내 르포트씨 제일 골절단술을 시행하였고, 하악은 구내 시상분할 골 절단술을 이용하여 골신장시 저항을 최소한으로 줄이려 하였다. 편방향 구내 골 신장기와 스플린트를 이용하여 양악 골신장술을 시행하였다. 5일의 잠재기 후 매일 1mm의 골신장을 시행하였고 6주에서 8주간의 경화기를 둔 후, 골 신장기와 스플린트를 제거하였다. 골신장 길이는 각각 13mm와 15mm이었고, 특이할 만한 부작용은 관찰되지 않았다. 이에 본 교실에서는 상악신장을 바탕으로 양악을 신장시키는 새로운 술식을 보고하는 바이다.

Development and application of the intraoral tracer for the record of centric relation (중심위 채득을 위한 구내묘기장치의 개발과 응용)

  • It was clinically important to substitute the physiologic centric relation to the therapeutic position of the patients who needed the oral rehabilitation or occlusal treatment. There were several methods for recording the centric relation. One of the known methods was to use the gothic arch tracer. However the existing intraoral device was difficult to adjust the three dimensional angulation of the recording plate and recording stylus depending on the hinge movement arch of the individual. The purpose of this study was to develop new intraoral tracer which had adjustable stylus within hinge movement arch for the record of centric relation and to evaluate the clinical application of this device. The results were as follow; 1. A stylus of new developed intraoral tracer was so adjustable that the recording of mandibular positions could be reproducible within the hinge movement arc. 2. A record plate of new developed intraoral tracer was so adjustable to parallel with the occlusal plane that lateral recording of mandibular position was able to obtain stably. This study showed that new developed intraoral tracer allowed the determination of the treatment position which can be used in the full mouth rehabilitation and occlusal treatments.

Foreign body aspiration and ingestion in dental clinic: a seven-year retrospective study

  • Background: This retrospective study investigated the incidence rate of accidental foreign body aspiration and ingestion according to patient sex, age, and dental department. This study aimed to verify whether the incidence rate is higher in geriatric than in younger patients and whether it is different among dental departments. Methods: Accidental foreign body aspiration and ingestion cases were collected from electronic health records and the safety report system of Yonsei University Dental Hospital from January 2011 to December 2017. The collected data included patients' age, sex, medical conditions, treatment procedures, and foreign objects that were accidentally aspirated or ingested. The incidence rate was calculated as the number of accidental foreign body aspirations and ingestions relative to the total number of patient visits. Differences depending on the patients' sex, age, and dental department were statistically identified. Results: There were 2 aspiration and 37 ingestion cases during the 7-year analysis period. The male to female incidence ratio was 2.8:1. The incidence rate increased with age and increased rapidly among those aged 80 years or older. Seven of the 37 patients with accidental foreign body ingestion had intellectual disability, Lou Gehrig's disease, dystonia, or oral and maxillofacial cancer. The incidence rate was highest in the Predoctoral Student Clinic and the Department of Prosthodontics. The most frequently swallowed objects were fixed dental prostheses and dental implant components. Conclusion: The incidence rate of accidental foreign body aspiration and ingestion differed according to patient sex, age, and dental department. Dental practitioners must identify high-risk patients and apply various methods to prevent accidental foreign body aspiration and ingestion in dental clinics. Inexperienced practitioners should be particularly careful.

Long-term Evaluation of Occlusal Adjustment in Patients with Temporomandibular Disorders (측두하악장애환자의 교합교정에 관한 장기평가)

  • 18 TMD patients who received occlusal adjustment in PNUH though Jan.1984 to 1985 were followed up for short-term(2-6yrs.) and long-term(1-2yrs.) evaluation. The obtained results were as follows : 1. Pain index showed gradual decrease after occlusal adjustment and significant change on long-term evaluation. 2. Noise index had no significant change throughout the all follow-up evaluation. 3. Opening limitation index showed gradual decrease after occlusal adjustment and significant change on both long-term and short-term evaluation. 4. Maximum comfortable opening exhibited more and more increase and significant change on long-term evaluation.

  • Recently, an advanced workflow has been introduced to finish implant surgery and prosthetic treatment in one-day. However, Because of 1. Patient's physical condition, 2. Surgical technique, 3. Digital technical limitations, the complete completion of one-day implant treatment is practically difficult. Therefore, this paper proposes a "two-days implant digital workflow" that short-time implant surgery and restores prosthetics the next day. Even though it takes more than one day, this workflow is a realistic implant treatment protocol that can reduce the chair time in the clinic.

  • This study is about the basis of satisfactions by patients : One is 'what factors of the marketing by dental medical service have an effect on consumers dental clinic' The other is 'what is the most important part when consumers choose the dental medical ser Seoul and Gyeonggi area unintentionally. Finally 446 people were analyzed. 6 general questions, 5 selective form questions when consumers choose the dental service, 11 satisfactions questions after treating and thought of reuse the dental service 6 (Likert scale) questions. Whether the choice of hospital dental marketing by dental analysis, both male and female hospital medical marketing and use of selected highly suggests that it does not respond. The resulting satisfaction analysis using the Hospital Dental Marketing consumer access to medical care, and then, a full explanation, comfort, quality and level, health care costs, treatment management, and symptom improvement were higher satisfaction with the item, select the dental healthcare after the analysis of the marketing of recycled doctors are otherwise subject the person selected from all entries equal to or higher than the average consumer satisfaction showed a higher medical doctor also higher reuse. Consequently, Through the use of marketing to choose the best dental healthcare need to providing quality care.

  • The study was performed to evaluate the Parent's knowledge and attitudes concerning the dental health of their children. A questionnaire consisting of 17 questions was given to each of the 664 parents by their children who are attending in primary school and was completed by them. The collected information from the questionnaire was analyzed by the visits to dental clinic, age and education. The results obtained from this study are as follows: 1) The more educated parents were more interested to the dental health of children. 2) In the know ledge concerning the dental health of their children, the parents who were more educated and had visited the dental clinic gave the better. responses. 3) There were no clear differences in age groups in the knowledge and attitudes on their children's dental health. 4) The reason for first visiting the dental clinic was dental treatment 43.2%, toothache 35.4%, for the purpose of oral examination 21.2%, and others were 9.2%. 5) 13.1% of the parents who had visited the dental clinic couldn't or didn't follow the recommended treatment because of economic difficulty 53.3%, lack of time 25%, lack of understanding of child's dental needs 18.3%, and other reasons were 3.3%.

By the way, this final prosthesis is not made in the clinic but is made in the dental lab and comes to the clinic with some time difference. In the clinic, the only medium to deliver the patient's oral information to the dental laboratory is the impression. However, many errors occur in this process. Dentists and dental technicians should try to identify the cause and make an accurate prosthesis to reduce this error.
