산 처리가 상아질 접착에 미치는 영향 (EFFECT OF ACID-TREATMENT ON DENTIN BONDING)

  • 산 처리방법에 따른 상아질 접착을 평가하고 표면처리 후의 상아질 표면의 성상변화 및 파단면의 양상을 관찰하기 위해 발거된 구치 에 10% polyacrylic acid (GC Dental Industrial Corp., U. S. A.), 10 % 인산(Bisco Inc., U. S. A.) 및 10-3 용액 (Parkell Bio-Materials, U. S. A.)을 사용하여 상아질 표면을 처리하고 모든 시편에 상아질 접착제인 All Bond 2$^{(R)}$(Bisco Inc., U.S.A.)를 도포하였으며 복합레진 (Silux Plus$^{(R)}$, 3M Co., U.S.A.)을 첨가한 후 접착강도를 측정하고 상아질 표면 및 파단면을 주사 전자현미경으로 관찰, 비교하여 다음과 같은 결과를 얻었다. 1. 접착강도는 상아질 표면을 산 처리한 군이 무처리한 대조군에 비해 높게 나타났고, 산처리를 시행한 군들 간에는 10-3 용액으로 처리한 군이 가장 높았으며 10% 인산으로 처리한 군, 10% polyacrylic acid로 처리한 군의 순이었다(p<0.01). 2. 상아질 표면의 관찰에서, 무 처리의 상아질 면은 상아세관이 개방되지 않은 채 도말층으로 덮혀 있었고, 10% polyacrylic acid로 처리한 상아질 면은 도말충이 제거되고 상아세관이 개방되어 있었으나 상아세관 내에 smear plug이 일부 남아 있었으며, 10% 인산또는 10-3 용액으로 처리한 상아질 면은 도말층 및 smear plug이 모두 제거되고 상아세관이 완전히 개방되어 있음이 관찰되었다. 3. 파단면의 관찰에서, 무 처리군에서는 도말층 내에서 파단이 일어났으며, 10% polyacrylic acid로 처리한 군에서는 상아세관 내 잔존 레진 tag이 관찰되지 않았으나 10% 인산 및 10-3 용액으로 처리한 군에서는 상아세관 내의 잔존 레진 tag이 관찰되었다. 4. 파단 양상에 있어서, 접착강도가 큰 군일수록 응집성 파단의 빈도가 증가되어 나타났으며, 각 시편에서 구한 접착강도와 응집 파단율의 상관 계수는 0.71이었다.

상아질과 접착제 간의 계면양상 (INTERFACIAL MORPHOLOGY BETWEEN DENTIN AND ADHESIVES)

  • The purpose of this study was to evaluate the effect of smear layer management on the interfacial morphology between dentin bonding system and dentin. Clearfil New Bond, Scotchbond Multipurpose, Prisma Universal Bond 3 and X-R Bond were used on the cervical dentinal surfaces of bovine incisor teeth. All of the dentin bonding systems were labeled with fluorescene in primer and rhodamine B in adhesive. Specimens of 2~3mm thichness were prepared by longitudinal and labiolingual sectioning. The interface between dentin bonding system and dentin was observed by flouresence imaging with a confocal laser scanning microscope. Following results were obtained. 1. In the specimen of Clearfil New Bond, dentinal tubules were widened by destruction of peritubular dentin in the course of treatment with phosphoric acid of high concentration. 2. Hybrid layer was observed in the specimen of Scotchbond Multipurpose and X-R Bond. 3. In the specimen of Prisma Universal Bond 3, the penetraton of adhesive was not observed clearly.

Update on dentin hypersensitivity: with the focus on hydrodynamic theory and mechanosensitive ion channels

  • Dentin hypersensitivity is an abrupt intense pain caused by innocuous stimuli to exposed dentinal tubules. Mechanosensitive ion channels have been assessed in dental primary afferent neurons and odontoblasts to explain dentin hypersensitivity. Dentinal fluid dynamics evoked by various stimuli to exposed dentin cause mechanical stress to the structures underlying dentin. This review briefly discusses three hypotheses regarding dentin hypersensitivity and introduces recent findings on mechanosensitive ion channels expressed in the dental sensory system and discusses how the activation of these ion channels is involved in dentin hypersensitivity.

Effects of Hybrid Coat on shear bond strength of five cements: an in-vitro study

  • PURPOSE. To evaluate the sealing performance of Hybrid Coat and its influence on the shear bond strength of five dentin surface cements. MATERIALS AND METHODS. Six premolars were pretreated to expose the dentin surface prior to the application of Hybrid Coat. The microscopic characteristics of the dentinal surfaces were examined with scanning electron microscopy (SEM). Then, 40 premolars were sectioned longitudinally, and 80 semi-sections were divided into a control group (untreated) and a study group (treated by Hybrid Coat). Alloy restoration was bonded to the teeth specimen using five different cements. Shear bond strength was measured by the universal testing machine. The fracture patterns and the adhesive interface were observed using a stereomicroscope. RESULTS. SEM revealed that the lumens of dentinal tubules were completely occluded by Hybrid Coat. The Hybrid Coat significantly improved the shear bond strength of resin-modified glass ionomer cement (RMGIC) and resin cement (RC) but weakened the performance of zinc phosphate cement (ZPC), zinc polycarboxylate cement (ZPCC) and glass ionomer cement (GIC). CONCLUSION. Hybrid Coat is an effective dentinal tubule sealant, and therefore its combined use with resin or resin-modified glass ionomer cements can be applied for the prostheses attachment purpose.

상아질표면처리제에 의한 상아질표면의 형태변화에 관한 연구 (A STUDY ON MORPHOLOGIC CHANGES OF DENTINAL SURFACES BY DENTIN CONDITIONING AGENTS)

  • The purpose of this study is to observe the morphological changes of prepared dentin surfaces by 5 dentin conditioning agents. Freshly extracted 48 healthy human molars were used in this study. The teeth were stored at $4^{\circ}C$ physiologic saline solution befor experiment. The teeth were cross-sectioned to expose dentin below 3.0mm at the cusp tip and above 2.0mm at the cemento-enamel junction with Crystal Cutter (MC411 D, Maruto Co., Japan). The specimens were then divided into 12 groups. The sectioned dentin surfaces in group 1, 3, 5, 7, 9, and 11 were prepared with No. 301 diamond point under air-water spray and those in group 2, 4, 6, 8, 10, and 12 were prepared with No. 700 carbide bur. The prepared dentin surfaces were conditioned with Nitric acid, Citric acid, Poly acrylic acid, EDTA, and Phosphoric acid. All the specimens were gold-coated with Eiko ion coater (Eiko-engineering Co.) and observed in Hitachi S-2300 Scanning electron microscope at 20 KV. The following results from this study were obtained; 1. The dentinal smear layers prepared with diamond point were compacted than those prepared with the carbide bur. 2. The dentinal smear layers prepared with diamond point or carbide bur were thick but after treatment of dentin conditioning agents smear layers were removed almost. 3. Irrespective of the uses of the diamond point or the carbide bur the morphological changes of dentin surfaces treated with the same conditioning agents were similar. 4. Treatment of nitric acid and EDTA was a little effect in removing dentinal smear layer. 5. Treatment of citric acid and phosphoric acid removed the smear layer very effectively and showed dissolution of peritubular dentin and opening of dentinal tubules.

Er:YAG 레이저를 이용한 치아삭제 효과 (EFFECTS OF ER:YAG LASER ABLATION ON THE DENTIN)

  • Er:YAG 레이저는 파장이 물의 주흡수 정점과 일치하는 $2.94{\mu}m$의 방출파장을 가지기 때문에, 물분사를 하면서 사용했을 때, 다른 레이저보다 열손상은 더 작으면서 치아경조직을 보다 효과적으로 삭제한다고 알려져 있다. Er:YAG 레이저의 상아질 삭제 효과를 평가하고자 발거된 소구치와 대구치를 대상으로 상아질 시편을 제작하고 $2.94{\mu}m$의 pulsed Er:YAG 레이저(SDL-300EN, 삼성전자, 한국)를 handpiece형의 전달계를 이용한 비접촉식 방법으로, 물에 담궈진 시간과 레이저 조사시간에 따른 삭제량과 형태를 조사, 분석하여 다음과 같은 결과를 얻었다. 1. 건전 상아질에 대한 조사 결과물에 담군 시간이 증가할수록 삭제율이 증가하였다. 2. 건전 상아질과 우식 상아질에 150mJ, 10Hz의 조건으로, 30초간 레이저를 조사했을 때, 우식상아질에 비해 건전 상아질에서 surface irregularity가 증가하고 상아세관이 열리는 경향이 있었으며 crack이 관찰되었다. 또한 우식 상아질에 비해 많은 탄조각(ash flecks), crack이 관찰되었고 상아세관의 불연속성 (inconsistency)이 관찰되었다. 3. 우식 상아질에서 30초로 조사한 경우에서는 dark zone이 와동에 국한되어 나타났으며 작은 fleck들이 관찰되었고, 1분간 조사한 경우에서는 30초의 경우보다도 와동이 더 넓었으며, crack이 상아세관 주행방향과 직각으로 형성된 것을 관찰 할 수 있었다. 2분간 조사한 경우는 와동이 가장 넓었으며 crack이 더 많이 형성된 것을 볼 수 있었다.

  • Root resorption is conventionally divided into internal and external varieties. Internal resorption occurs where there is loss of the internal wall of the root canal. and is usually associated with a localized area of necrotic pulp(Trope '||'&'||' Chivian 1984). External resorption occurs where there is loss of the external surface of the root and the resorption may be either transient or progressive(Tronstad 1988). External inflammatory resorption usually occurs following damage to the periodontal ligament or where there is communication between the periodontal ligament and a necrotic pulp. via open dentinal tubules or accessory canals. This type of resorption is usually progressive until root canal treatment is instituted (Barclay 1993).

레진접착제를 도포한 상아질에 대한 합착용 시멘트의 전단결합강도 (SHEAR BOND STRENGTH OF LUTING CEMENTS TO DENTIN TREATED WITH RESIN BONDING AGENTS)

  • The purpose of this study was to confirm the formation of hybrid layer and resin tags in dentin tissue and the possibility of bonding between luting cements used for the prosthesis and the resinous surface coated with resin bonding agents to prevent the dentin hypersensitivity after abutment preparation. Some resin bonding agents, which may have the possibility of bonding with polyacrylic acid as a liquid ingredient of polycarboxylate and glass ionomer cements, were selected. All-Blond desensitizer containing NTG-GMA and BPDM, Scotch-Bond Multipurpose plus containing HEMA, and XR-bond containing organophosphate were selected as a coating agent. Dental cements were zinc phosphate, polycarboxylate, and glass ionomer cement. After the exposed dentin surface of premolars was ethced with 10% phosphoric acid and coated with resin bonding agents, the morphology of treated surfaces and the resin tags and hybrid layers on sectioned surfaces were observed by SEM. Shear bond strength between the resin bonding agents and 3 kinds of cements was measured 24 hours after bonding. On the debonded surfaces of the shear bond strength tested specimens, the cement tags and the bonding sites between the resin materials and cements were examined by SEM. Following conclusions were drawn : 1. Coating of dentin with resin bonding agents had no effect on the shear bond strength of zinc phosphate cement. 2. Both of polycarboxylate and glass ionomer cements showed the increased shear bond strength by the dentinal coating with Scotch-Bond Multipurpose plus containing HEMA. However, in the case of dentinal coating with some agents containing NTG-GMA and BPDM or organophosphate, polycarboxylate cement exhibited the lowered shear bond strength, and glass ionomer cement showed the unchanged shear bond strength. 3. Complete obstructions of dentinal tubules were observed on the dentin coated with All-Bond desensitizer or XR-bond, but distinct shape of the orifices of dentinal tubules was observed consistently on the dentin coated with Scotch-Bond Multipurpose plus. 4. The hybrid layer was thickest on the dentin coated with All-Bond desensitizer, and the length of resin tags was longest on the dentin coated with Scotch-Bond Multipurpose plus. 3. On the debonded specimens which had been bonded with polycarboxylate cement or glass ionomer cement after coating with Scotch-Bond Multipurpose plus, the cement tags and the bonding sites between the resinous surface and the cements could be examined.

In vivo와 in vitro에서 상아세관을 통한 색소침투의 차이 (DIFFERENCE IN DYE PENETRATION INTO DENTINAL TUBULES IN VIVO AND IN VITRO)

  • Penetration degree of several dyes into dentinal tubules in vivo was compared with in vitro, and experimental sensitivity of those dyes was investigated in 64 canines of 16 cats. Dentin 1mm below cusp tip was exposed by cross-sectioning with diamond disc. Pulp of 32 canines were extirpated through cervical cavities. In the presence of smear layer or after add-etching with 37% phosphoric acid to the exposed dentin surface, acrylic collar and resin cap was attached to keep dyes. 52 mmol/$\ell$ Evans' blue, 2% Methylene blue, 10 % Silver nitrate and 5% Fluorescene were then applicated on each 4 canines respectively for 30 minutes. After rinsing, the canines were ground-sectioned longitudinally and linear dye penetration was measured under microscope. The results obtained were as follows ; 1. Evans' blue and Methylene blue penetrated significantly(P<0.05) more in vitro than in vivo only in the cases that exposed dentin surfaces were not etched with acid. 2. Silver nitrate penetrated significantly(P<0.05) more in vitro than in vivo in both cases that exposed dentin surfaces remained intact and were etched with acid. 3. The penetration degree of Fluorescene did not show statistical significance between in vivo and in vitro or in acid-etched and in not acid-etched cases.

인공타액 오염이 수종 상아질접착제와 상아질간의 결합에 미치는 영향 (INFLUENCE OF ARTIFICIAL SALIVA CONTAMINATION ON BONDING OF DENTIN ADHESIVES TO DENTIN)

  • The purpose of this study was to evaluate the influence of artificial saliva contamination on bonding of several dentin adhesives to dentin. Sixty - three human molar teeth extracted within a month were used. Each tooth was sectioned longitudinally in a buccolingual direction to obtain 126 specimens. These specimens were randomly divided into three groups and were treated by Scotchbond 2, Gluma and All bond. Each group was subdivided into three subgroups; normal group not contaminated with artificial saliva, contaminated with artificial saliva and dried group, and contaminated with artificial saliva and washed and dried group. Enamel/dentin bonding agent(Dental Adhesive of Scotchbond 2) was applied and light cured on the treated dentin surfaces. Thereafter P - 50 were cured on them, and specimens were stored in $37^{\circ}C$ artificial saliva for 24 hours before measuring shear bond strength. Shear bond strengths were determined using an universal testing machine with cross head speed 1mm/min and SEM examinations were conducted to evaluate the resin - dentin interface and degree of penetrating resin string into the dentinal tubules. The following results were obtained. 1. Normal groups not contaminated with artificial saliva showed greater shear bond strength than any other group contaminated with artificial saliva(P<0.01). 2. The shear bond strengths showed no significant difference between washed groups with distilled water and not washed groups after contamination with artificial saliva(P>0.05). 3. In normal groups, the shear bond strength of A group was significantly greater than in any other group(P<0.01). 4. In Sand G groups, fractures after shear bond strength tests occured adhesively on resintooth interface in all specimens. But in A groups, fracture of the normal group occured cohesively in dentin and fracture of the contaminated groups occured adhesively and cohesively. 5. On SEM examination, the number of resin strings penetrated into dentinal tubules were the greatest in normal groups, followed by, in descending order, washed groups and not washed groups after contamination with artificial saliva.
