Background: Attention deficit hyperactivity disorder (ADHD) is characterized by a persistent pattern of inattention and/or hyperactivity-impulsivity that interferes with functioning or development. It has a worldwide pooled prevalence of 5.29%. The characteristics of ADHD can increase the probability of dental treatment, while special behavior management can be required to allow proper treatment. In South Korea, the use of sedation in dental treatment has rapidly increased in recent decades. The present study aimed to investigate the trend and effects of sedation in patients with ADHD undergoing dental treatment in South Korea. Methods: The study used customized health information data provided by the Korean National Health Insurance Service. Among patients with the record of sedative use during the period from January 2007 to September 2019, those with International Classification of Diseases-10 codes for ADHD (F90, F91) were selected; the data of their overall insurance claims for dental treatment were then analyzed. The patients' age, gender, sedative use, and dental treatment were analyzed per year. The annual number of general anesthesia or sedation cases was also analyzed, and changes in the method of behavior management with increasing age were examined. Results: The study involved 7,654 patients with ADHD (6,270 males; 1,384 females). The total number of dental treatments was 137,778, while the number of sedation cases was 16,109, among which 13,052 involved male patients and 3,057 female patients. The number of general anesthesia cases was 631, among which 538 involved male patients and 93 female patients. The most frequently used sedation method in the dental treatment of patients with ADHD was N2O inhalation. The percentage of sedation cases was highest in patients aged 4 years, and it decreased with increasing age. Conclusion: In South Korea, both sedation and dental treatments were slightly more common in patients with ADHD than in the general population. With increasing age, the frequency of dental treatments and the percentage of sedation cases decreased.
Objectives: This study aims to provide basic data for high-quality dental services. In addition, we will promote the operation of preventive dentistry that implements preventive measures. It was conducted to study the change of patient's treatment behavior and treatment cost due to the discontinuation of preventive dentistry in university dental hospitals. Methods: This study collected data using the integrated medical information system of the C University Dental Hospital. From September 1, 2017 to August 31, 2019, data were analyzed using frequency, percentage, mean, standard deviation, chi-square test using SPSS version 24.0 statistical program, and T-test. Results: There was a significant difference in the number of preventive dental treatment cases from 58.3% of preventive dental operation periods to 41.7% of preventive dental operation periods. As a result of comparing the medical expenses, the total medical expenses during the preventive dental operation period decreased from 521,308,872 won to 379,724,995 won during the discontinuation period, 141,583,877 won. The number of medical treatments by treatment behavior decreased 3,835 (28.4%) from a total of 13,520 preventive dental operation periods to 9,685. Conclusions: This study is meaningful as the first study to confirm the change in the treatment behavior and the change in the cost of treatment due to the discontinuation of the operation of preventive dentistry at university dental hospitals. In conclusion, it is thought that there is a possibility of the lack of accessibility and the limitation of professional preventive care due to the discontinuation of preventive dentistry.
A questionnaire-based survey was done to evaluate the patients' perception on the use of Rubber Dam (RD) for the tooth isolation during dental treatment. Total 106 questionnaire were gathered and the data were analyzed according to the patients' gender, age, previous experience of RD. The 96% of total patients reported no experience of RD in their previous treatment. The first advantage of RD was selected as the protection of irrigant and medication into the oral cavity and the disadvantage was selected as saliva gathering during treatment. 98% of patients want to use RD for the next treatment and 99% remarked that the RD is essential for the dental treatment. Based on the surveyed data, majority of the patients highly satisfied with the RD use and they want to use RD and may consider the use of RD for the selection of dental clinic.
본 연구는 편의 추출된 30대~50대 성인 남녀 266명을 대상으로 치과치료비 본인부담금 스트레스, 비급여 치과치료 건강보험 확대, 민영 치과단독의료보험 가입 의사를 설문조사하였다. 건강보험 비급여 본인부담금 스트레스가 급여 본인부담금 스트레스에 비해 높으며, 교정, 임플란트, 틀니, 보철, 비급여재료 치아우식증 치료 순으로 스트레스 수준이 높았다. 치과치료에 있어서 건강보험확대는 필요하며 필요 순으로는 비급여재료 치아우식증 치료, 보철, 교정 순이었다. 연령제한이 있는 급여항목에 대해서는 적절하다는 응답이 치석제거 57.1%, 틀니 23.3%, 임플란트 14.3%였다. 구강상태가 나쁘다고 응답한 대상자는 급여 본인부담금과 비급여 본인부담금 모두에서 스트레스 정도가 유의하게 높으며 보철, 치아교정에 대한 건강보험 확대요구가 유의하게 높았다. 민영의료보험 가입자 중 치과치료를 보장하는 대상자는 18.3%, 민영 치과단독의료보험에 가입한 가입자는 8.3%였으나, 민영 치과단독의료보험 가입의사는 68.4%였다. 일반적 특성에 따른 민영 치과단독의료보험 가입의사 차이는 나타나지 않았고, 치과치료비의 부담정도와 민영 치과단독의료보험 가입필요 정도의 상관분석 결과, 임플란트 0.408, 비급여 치아우식증 치료는 0.404, 틀니 0.394, 보철치료 0.375, 치아교정 0.313 순으로 나타났다. 따라서, 적기치료가 가능하도록 급여 치과치료 본인부담금에 대한 부담 감소를 위해 치과치료 건강보험 급여 확대, 민영 치과단독의료보험 개발과 이에 따른 정부의 감독이 필요할 것으로 생각되었다.
Dental care is becoming more available on the NHI(National Health Insurance) in Korea. Especially, complete denture, partial denture, dental scaling, and dental implant has been applied by NHI from 2012 to 2014. Although, the entire nation is not eligible for the benefit now, the more dental coverage of NHI is extended, the more regulaition is tightened. Essential documents for proof of correctness of dental treatment covered by NHI are dental records and the receipt book. Summary of regal regulation about them is as follows 1. Chief complaints of patients, diagnosis, progress, and act of treatment, drugs and materials of treatment, doctor's sign, date and hour should be placed accurately on dental record 2. Dental clinic should collect patients sharing of the dental cost covered by NHI. 3. Dental clinic should keep the receipt as proof of purchase of dental drugs or materials.

Background: The number of patients with Alzheimer's disease is growing worldwide, and the proportion of patients requiring dental treatment under general anesthesia increases with increasing severity of the disease. However, outpatient anesthesia management for these patients involves great risks, as most patients with Alzheimer's disease are old and may show reduced cardiopulmonary functions and have cognitive disorders. Methods: This study retrospectively investigated 43 patients with Alzheimer's disease who received outpatient anesthesia for dental treatment between 2012-2017. Pre-anesthesia patient evaluation, dental treatment details, anesthetics dose, blood pressure, duration and procedure of anesthesia, and post-recovery management were analyzed and compared between patients who underwent general anesthesia or intravenous sedation. Results: Mean age of patients was about 70 years; mean duration of Alzheimer's disease since diagnosis was 6.3 years. Severity was assessed using the global deterioration scale; 62.8% of patients were in level ${\geq}6$. Mean duration of anesthesia was 178 minutes for general anesthesia and 85 minutes for intravenous sedation. Mean recovery time was 65 minutes. Eleven patients underwent intravenous sedation using propofol, and 22/32 cases involved total intravenous anesthesia using propofol and remifentanil. Anesthesia was maintained with desflurane for other patients. While maintaining anesthesia, inotropic and atropine were used for eight and four patients, respectively. No patient developed postoperative delirium. All patients were discharged without complications. Conclusion: With appropriate anesthetic management, outpatient anesthesia was successfully performed without complications for dental treatment for patients with severe Alzheimer's disease.
IV Deep sedation is useful for Waardenburg syndrome Child undergoing extensive dental treatment. We experienced a case of dental treatment under W deep sedation in a 3 years old boy with Waardenburg syndrome. He was treated on an outpateint basis. He was diagnosed hypothyroidism and received thyroid hormone. Premedication was done using ketamine and atropine IM. Induction using $N_2O$-enflurane mask ventilation was uneventful and intravenous cathetering was carried out. IV deep sedation was maintained with midazolam-$N_2O$ combination. After monitoring the patient for 2 hours and confirming his recovery, he was discharged from the day care unit. In summary, we report this successful IV Deep sedation management of Waardenburg syndrome child during dental treatment in as out patient.
For dental pulp treatment of immature permanent teeth, direct pulp capping or partial/cervical pulpotomy (apexogenesis) procedures can be used if the dental pulp is vital. MTA (Mineral Trioxide Aggregate) is regarded as the first choice dressing material for these procedures because its higher success rate. It can be also used successfully for devitalized dental pulp which has been treated by calcium hydroxide. This apexification procedure with MTA has a few advantage such as short treatment period and increase of resistance against root fracture. Recently, regenerative endodontic treatment was introduced for devitalized immature pulp. It can maintain pulp vitality and lead to continuing root development although the dental pulp was devitalized.
Anxiety and fear is two main factors that keep patients from going to dental clinic. Especially, patients may feel implants operations are more traumatic. Intravenous conscious sedation for dental treatment can make patient comfortable and relaxable. Midazolam is more popular for sedation for dental treatment, but target-controlled infusion (TCI) of propofol and remifentanil is gaining wide popularity. A 54-year-old female patient who had severe dental phobia was referred to our dental hospital. She had past history of 2 times of hyperventilation and syncope during dental treatment. The patient showed a lot of dental anxiety and fear to dental treatments and stress reduction protocol was needed. We administered intravenous conscious sedation using target controlled infusion system with remifentanil and propofol. During sedation, we monitored the status of consciousness with bispectral index and vital signs. Dental treatment could be finished successfully without any problems.
In order to develop the school dental health care in rural area, the author collected data about the population of all 6-17 year students living in Young dong-gun county, and Surveyed their dental health cares during one year of 1982. From the collected data, several dental health indices such as percentage of students of all population, percentage of students who visited dentists once or more during one year, average annual dentist visit and average annual dental treatment case were calculated and discussed. The obtained results were as follows; 1. The percentage of students of all population in Young dong-gun county was 29.65%. 2. The percentage of students who visited dentists once or more during one year was 4.67%. 3. The average annual dentist visit per student was 0.11. 4. The average annual dental treatment case per student was 0.16. 5. The oral examination case was 0.05, intraoral radiograph 0.01, oral prophylaxis 0.00, filling of dental carious lesion 0.02, pulp treatment 0.02, extraction of teeth 0.04, and others 0.02 annually in the average. In comparison with detectable need for dental treatment cases, oral prophylaxis was not supplied at all, filling of dental carious lesion was supplied about 1% and extraction of teeth was supplied about 10% of detectable need. 6. It was recommended that school incremental dental care project should be developed for school dental health programme in order to supply all of the detectable need for dental treatment.
