Multiloop edgewise Archwire 기법으로 치료된 전치 개교 증례의 두부방사선사진 계측학적 평가 (A CEPHALOMETRIC EVALAUATION OF ANTERIOR OPENBITE MALOCCLUSIONS TREATED BY MULTILOOP EDGEWISE ARCHWIRE TECHNIQUE)

  • The purpose of this study was to evaluate the change of before and after treatment of anterior openbite malocclusions treated by Multiloop Edgewise Archwire technique. The openbite sample consisted of 4 male and 12 female adults, treated with nonextraction or third molar extraction. The normal sample consisted of 58 subjects, which have pleasing facial profile and normal occlusion and no experience of orthodontic or prosthodontic treatment. The 58 subjects of normal sample were subdivided by cephalemetric vertical relationship of face. The 40 subjects, cephalometric vertical relationship of face was in normal range, classified as Normal Sample group 1. The 18 subjects, increased cephalometric vertical relationship of face, classified as Normal Sample group 2. The computerized cephalometric analysis was accomplished with 50 reference points for 22 skeletal measurements, 46 dentoalveolar measurements, 8 soft tissue measurements. Statistical analysis of the data was carried out with paired t-test, Student's t-test, and DUNCAN test using SAS(PC version), The results were as follows : 1. There were no statistically significant differences in skeletal measurement between before and after treatment. The major changes were in dentoalveolar region. 2. After treatment, the long axis of maxillary and mandibular posterior teeth were distally tipped-back, and uprighted to bisected occlusal plane. The interincisal angle was increased. 3. There were no statistically significant increase in the upper posterior dental height and statistically significant decrease in the lower posterior dental height. The upper anterior dental height was increased, but there was no statistically significant increase in the absolute upper anterior dental hight. The lower anterior dental height was increased. 4. After treatment, the maxillary occlusal plane to palatal plane angle and the mandibular occlusal plane to mandibular plane angle were statistically significant increased. Then, there were no statistically significant difference between after treatment group and normal sample group 2. 5. After treatment, the percentage of upper lip length to upper anterior dental height was decreased. Then, There were no statistically significant difference between after treatment group and normal sample group 2.

Identification of Enterococcus faecalis antigens specifically expressed in vivo

  • Objectives: Molecular mechanism of the pathogenicity of Enterococcus faecalis (E. faecalis), a suspected endodontic pathogen, has not yet been adequately elucidated due to limited information on its virulence factors. Here we report the identification of in vivo expressed antigens of E. faecalis by using a novel immunoscreening technique called change-mediated antigen technology (CMAT) and an experimental animal model of endodontic infection. Materials and Methods: Among 4,500 E. coli recombinant clones screened, 19 positive clones reacted reproducibly with hyperimmune sera obtained from rabbits immunized with E. faecalis cells isolated from an experimental endodontic infection. DNA sequences from 16 of these in vivo-induced (IVI) genes were determined. Results: Identified protein antigens of E. faecalis included enzymes involved in housekeeping functions, copper resistance protein, putative outer membrane proteins, and proteins of unknown function. Conclusions: In vivo expressed antigens of E. faecalis could be identified by using a novel immune-screening technique CMAT and an experimental animal model of endodontic infection. Detailed analysis of these IVI genes will lead to a better understanding of the molecular mechanisms involved in the endodontic infection of E. faecalis.

Evaluation of physical property and cytotoxicity of resin infiltrant based on a triethylene glycol dimethacrylate (TEGDMA)

  • Objectives: The resin infiltration technique is a promising alternative therapy for arresting the early dental caries. However, there are very few reports on the safety and biocompatibility of this technique. We evaluated various properties of resin infiltrant (RI) based on a triethylene glycol dimethacrylate (TEGDMA).The water sorption (Wsp) and water solubility (Wsl) was assessed. Additionally, the cytotoxicity of RI against both animal and human fibroblast cell lines was investigated. Methods: The RI of the $Icon^{(R)}$, the first product developed for resin infiltration, is mainly composed of TEGDMA in the resin matrix. The Wsp and Wsl for the RI were measured in accordance with ISO 4049 specifications. Fourier-transform infrared spectroscopy (FTIR) was used for analyzing the polymerization before and after curing of RI. The cytotoxicity of RI against the mouse fibroblasts (L929) and human gingival fibroblasts (hTERT-hNOF) was evaluated using MTT [3-(4,5-dimethylthiazol-2-yl)-2,5-diphenyltetrazolium bromide] assay and the data were analyzed using one-way analysis of variance. Results: Wsp and Wsl of the RI specimens were $53.37{\mu}g/mm^3$ and $10.6{\mu}g/mm^3$, respectively. FTIR analysis revealed a slightly higher degree of curing with longer irradiation time. The degree of conversion for RI was high (80.9%) after 40 seconds of light curing. There was a significant decrease in the viability of L929 and hTERT-hNOF cells at RI extraction solution concentrations above 50%, respectively, compared to that in the negative control (p< 0.05). Conclusions: Even though the RI exhibited positive effect on the early prevention of dental caries, the clinicians should also consider the toxicity of RI on periodontal tissues.

Anesthetic efficacies of buccal with palatal injection versus buccal with intra-septal injection in permanent maxillary first molars of pediatric patients

  • The high success rate of dental treatment is dependent on the cooperation of pediatric patients during procedures. Dental treatment often causes pain, particularly in children. The factors in providing treatment to pediatric patients include the characteristics and location of the tooth, profoundness of the anesthesia including the type of local anesthetic, and cooperation of the patient. Previous studies have examined several techniques to successfully achieve profound pulpal anesthesia in maxillary permanent teeth. The dentist should select the injection technique to be used based on patient needs. In children, either buccal with palatal injections or buccal with intra-septal injections may be used to anesthetize the permanent maxillary first molar. Buccal with palatal injections are commonly used prior to routine maxillary dental procedures. Currently, there are only a few studies on the employment of buccal with intra-septal injections to anesthetize permanent maxillary first molars in pediatric patients. This review will focus on efficacy of buccal with palatal versus buccal with intra-septal pulpal anesthesia of the permanent maxillary first molars in pediatric patients and aim to determine which technique should be used during routine dental procedures.

시멘트 공간이 적층 가공으로 제작한 지르코니아 하부구조물의 변연 및 내면 적합도에 미치는 영향 (Effect of cement space on marginal and internal fit of a zirconia core fabricated using by additive manufacturing)

  • Purpose: The goal of this study was to determine the clinical acceptability of various cement space settings for the marginal and internal fit of a zirconia core manufactured using additive manufacturing. Methods: The maxillary right incisor served as the master model. After scanning the maxillary right incisor with a dental 3D (three-dimensional) scanner, the stereo lithography file was created using different cement space settings of 40, 120, and 200 ㎛ using computer-aided design software (Dental System 2018; 3Shape). The marginal and internal fit of the 3 groups were determined using the silicon replica technique. Measurement points were divided into the following three categories: margin, axial wall, and incisal. To ensure more accurate measurements, these three measurement points were divided into 8 points. The Shapiro-Wilk, one-way ANOVA, and Tukey's honestly significant difference test (for all tests α=0.05) were the statistical analyses that were included in the study. Results: The CS (cement space)-200 group had better marginal and internal fit than the CS-40 and CS-120 groups, and there were statistically significant differences at the marginal and incisal points, except for the axial wall points. CS-200 group, both marginal and internal fit were within 120 ㎛, which is the clinically acceptable value. Conclusion: This study suggests that a 200 ㎛ cement space setting is ideal for optimal marginal and internal fit of 3D-printed ceramic crowns.

Maxillary reconstruction using tunneling flap technique with 3D custom-made titanium mesh plate and particulate cancellous bone and marrow graft: a case report

  • Background: Reconstructive surgery is often required for tumors of the oral and maxillofacial region, irrespective of whether they are benign or malignant, the area involved, and the tumor size. Recently, three-dimensional (3D) models are increasingly used in reconstructive surgery. However, these models have rarely been adapted for the fabrication of custom-made reconstruction materials. In this report, we present a case of maxillary reconstruction using a laboratory-engineered, custom-made mesh plate from a 3D model. Case presentation: The patient was a 56-year-old female, who had undergone maxillary resection in 2011 for intraoral squamous cell carcinoma that presented as a swelling of the anterior maxillary gingiva. Five years later, there was no recurrence of the malignant tumor and a maxillary reconstruction was planned. Computed tomography (CT) revealed a large bony defect in the dental-alveolar area of the anterior maxilla. Using the CT data, a 3D model of the maxilla was prepared, and the site of reconstruction determined. A custom-made mesh plate was fabricated using the 3D model (Okada Medical Supply, Tokyo, Japan). We performed the reconstruction using the custom-made titanium mesh plate and the particulate cancellous bone and marrow graft from her iliac bone. We employed the tunneling flap technique without alveolar crest incision, to prevent surgical wound dehiscence, mesh exposure, and alveolar bone loss. Ten months later, three dental implants were inserted in the graft. Before the final crown setting, we performed a gingivoplasty with palate mucosal graft. The patient has expressed total satisfaction with both the functional and esthetic outcomes of the procedure. Conclusion: We have successfully performed a maxillary and dental reconstruction using a custom-made, pre-bent titanium mesh plate.

임시 의치와 이중 디지털 스캐닝 기법을 활용한 전악 고정성 임플란트 수복 증례 (Full mouth rehabilitation with fixed implant-supported prosthesis using temporary denture and double digital scanning technique: a case report)

  • 임플란트 고정성 보철물 제작 시 디지털 시스템을 이용하여 수복할 때, 3차원적인 임플란트 위치의 구내 스캔과 임시 의치 혹은 임시 보철물 장착 후 구내 스캔을 이용한 이중 디지털 스캐닝(double digital scanning) 기법이 필요하다. 구내 스캔 시, 스캔 바디를 안정적인 랜드마크로 이용하면 다지털 인상의 정확도 향상과 기공과정이 효율적이고 단순화될 수 있다. 본 증례는 완전 디지털 시스템을 활용하여 맞춤형 치과용 임플란트 지대주 및 임시 보철물, 최종 보철물을 계획하고 제작하였다. 임플란트 식립 후, 임플란트에 체결한 구내 스캔 바디와 임시 의치 조직면에 인기 된 구내 스캔 바디의 스캔 영역을 중첩하였다. 중첩된 파일에서 임시 의치의 수직 고경에 맞는 맞춤형 치과용 임플란트 지대주와 임시 보철물을 제작하여 환자에게 시적 하였고, 임시 보철물을 토대로 최종 보철물을 제작하였다. 임시 의치 제작 시 설정한 수직 고경, 중심위 등을 최종 보철물까지 활용하여 기능 및 심미적으로 만족할 만한 결과를 얻어 이를 보고하고자 한다.

New virtual orthodontic treatment system for indirect bonding using the stereolithographic technique

  • The purpose of this article is to introduce a new virtual orthodontic treatment (VOT) system, which can be used to construct three-dimensional (3D) virtual models, establish a 3D virtual setup, enable the placement of the virtual brackets at the predetermined position, and fabricate the transfer jig with a customized bracket base for indirect bonding (IDB) using the stereolithographic technique. A 26-year-old woman presented with anterior openbite, crowding in the upper and lower arches, and narrow and tapered upper arch, despite having an acceptable profile and balanced facial proportion. The treatment plan was rapid palatal expansion (RPE) without extraction. After 10 days of RPE, sufficient space was obtained for decrowding. After a 10-week retention period, accurate pretreatment plaster models were obtained using silicone rubber impression. IDB was performed according to the protocol of the VOT system. Crowding of the upper and lower arches was effectively resolved, and anterior openbite was corrected to normal overbite. Superimposition of the 3D virtual setup models (3D-VSM) and post-treatment 3D virtual models showed that the latter deviated only slightly from the former. Thus, the use of the VOT system helped obtain an acceptable outcome in this case of mild crowding treated without extraction. More cases should be treated using this system, and the pre- and post-treatment virtual models should be compared to obtain feedback regarding the procedure; this will support doctors and dental laboratory technicians during the learning curve.

다양한 레진 의치상의 적합도와 기계적 특성 (Adaptation accuracy and mechanical properties of various denture base resins: a review)

  • This paper reviews the adaptation accuracy and mechanical properties of currently used denture processing systems with base resin materials and introduces the latest research on the development of antimicrobial denture base resins. Poly(methyl methacrylate) has been successfully used as a dental denture base resin material by the compress-molding method and heat polymerization for a long time, but recently, new processing techniques, injection molding-methods or fluid-resin technique are also used for fabricating denture base. However, studies indicated that there was no difference between the injectionmolding and the conventional compression-molding method in terms of adaption accuracy of denture base. The fluid-resin fabrication and one injection-molding systems exhibited better adaptation accuracy than the other processing methods. Resin denture bases in the oral cavity may undergo midline fractures due to flexural fatigue from repeated masticatory loading. For those patients, impact resistant denture base resins are recommended to prevent denture fracture during service. Thermoplastic denture base resins can be helpful for patients suffering from allergic reaction to resin monomers with a soft-fit, however, thermoplastic resins with low stiffness can irritate gum tissues and accelerate abnormal alveolar ridge resorption. Moreover, due to low chemical durability in oral cavity, those should be used for a limited period of time.

Evaluation of marginal discrepancy of pressable ceramic veneer fabricated using CAD/CAM system: Additive and subtractive manufacturing

  • PURPOSE. The purpose of this study was to evaluate the marginal discrepancy of heat-pressed ceramic veneers manufactured using a CAD/CAM system. MATERIALS AND METHODS. The ceramic veneers for the abutment of a maxillary left central incisor were designed using a CAD/CAM software program. Ten veneers using a microstereolithography apparatus (AM group), ten veneers using a five-axis milling machine (SM group), and ten veneers using a traditional free-hand wax technique (TW group) were prepared according to the respective manufacturing method. The ceramic veneers were also fabricated using a heat-press technique, and a silicone replica was used to measure their marginal discrepancy. The marginal discrepancies were measured using a digital microscope (${\times}160$ magnification). The data were analyzed using a nonparametric Kruskal-Wallis H test. Finally, post-hoc comparisons were conducted using Bonferroni-corrected Mann-Whitney U tests (${\alpha}=.05$). RESULTS. The $mean{\pm}SD$ of the total marginal discrepancy was $99.68{\pm}28.01{\mu}m$ for the AM group, $76.60{\pm}28.76{\mu}m$ for the SM group, and $83.08{\pm}39.74{\mu}m$ for the TW group. There were significant differences in the total marginal discrepancies of the ceramic veneers (P<.05). CONCLUSION. The SM group showed a better fit than the AM and TW groups. However, all values were within the clinical tolerance. Therefore, CAD/CAM manufacturing methods can replace the traditional free-hand wax technique.
