Loosening torque of Universal Abutment screws after cyclic loading: influence of tightening technique and screw coating

  • PURPOSE. The purpose of this study was to evaluate the influence of tightening technique and the screw coating on the loosening torque of screws used for Universal Abutment fixation after cyclic loading. MATERIALS AND METHODS. Forty implants (Titamax Ti Cortical, HE, Neodent) (n=10) were submerged in acrylic resin and four tightening techniques for Universal Abutment fixation were evaluated: A - torque with 32 Ncm (control); B - torque with 32 Ncm holding the torque meter for 20 seconds; C - torque with 32 Ncm and retorque after 10 minutes; D - torque (32 Ncm) holding the torque meter for 20 seconds and retorque after 10 minutes as initially. Samples were divided into subgroups according to the screw used: conventional titanium screw or diamond like carbon-coated (DLC) screw. Metallic crowns were fabricated for each abutment. Samples were submitted to cyclic loading at $10^6$ cycles and 130 N of force. Data were analyzed by two-way ANOVA and Tukey's test (5%). RESULTS. The tightening technique did not show significant influence on the loosening torque of screws (P=.509). Conventional titanium screws showed significant higher loosening torque values than DLC (P=.000). CONCLUSION. The use of conventional titanium screw is more important than the tightening techniques employed in this study to provide long-term stability to Universal Abutment screws.

  • FDI World Dental Federation is organization representing more than one million dentists worldwide with its more than 100 years of long history. Now FDI headquarter is located in Geneve, Switzerland and serves for developing health policy and continuing education programs, speaking as a unified voice for dentistry in international advocacy and supporting member association in oral health promotion activities worldwide. Korean Dental Association has been a member of FDI since 1959 and participated actively in the FDI's worldwide activities. Past Dr. Heung-Ryul Yoon was a president of FDI from 2003 for 2 years and we also have an experience in 1997 of hosting FDI Annual World Dental Congress successively in Seoul. The image of a country reflects the country's culture as well as affects to its economical and social development and it decides the national brand toward the world. Currently, Korean people realized that and put their efforts for promoting diverse aspects of Korea to the world such as multimedia culture called Han-Ryu, electric industry, automotive industry, sports represented by soccer. We have advanced technique and skill in various fields and now it's time to show ourself better to the world. It's same to the dentistry. It's true that Korean dentistry is top-class in the world and the dental industry is prosperous and also has best technique in the world. I think and strongly hope that the 2013 FDI Seoul Annual World Dental Congress would be best opportunity for Korean dentistry and dental industry to promote ourselves and make a big step to the world. Not only for the dentistry, it will also be very good chance to all the Korean people to improve Korean national brand. I believe that we can accomplish if we, all dental societies, stand together and join our effort to use this good chance the best. I know we can make it.

A Study on the design waxup technique for maxillary molar occlusion (상악구치 교합면의 design 조각법에 관한 연구)

  • The first function of occlusion is mastication. Therefore the functional restoration of occlusal surface is very important. The restoration of occlusal surface is three method as wax bite technique, F.G.P. technique, cone technique. Many dental technician is using compound method. I have knew common point on each teeth during I have waxing up wax pattern. So I studied on the design waxup technique for maxillary molar occlusion. The results of the study were as follows ; 1. The dam wax up method can restore axial contour of teeth very easy and make short working time of wax pattern. 2. The height of dam must be same with cusp of adjacent teeth. 3. Automatically the contour of tooth is appeared if the contour of dam is relationship with cuspid line of adjacent teeth. 4. The height of contour of buccal, lingual surface is formed natural curve to add fluid wax by gravitation. 5. The development groove of Maxillary premolar is appeared V form. 6. The development groove of Maxillary molar is appeared W form. 7. The embrasure is formed to carve around contact point area as round convex. It affects to axial form of tooth. 8. I was knew that the lingual groove and stuart's groove of molar runs parallel with oblique ridge. 9. The buccal groove of molar is formed parallel with direction of teeth arrangement.

  • Purpose: The aim of this study was to make nanomesh on the surface of titanium by potentiostatic technique which was done at the suitable potential level. Methods: In order to find the suitable potential level, use a $25^{\circ}C$ NaCl, NaOH and NH4F solution of 1 M and 5 M as supporting electrolyte, working electrode(positive potential) was contact to the titanium specimen and counter electrode(negative potential) was contact to the Pt substrate. At the transpassive potential which was observed by potentiostatic technique, potentiostatic technique was done for 2hours. Results: As a result, 1 M NaOH solution was suitable as a supporting electrolyte, potentiostatic technique used a $25^{\circ}C$ NaOH solution of 1 M for 2hours, nanomesh was formed. Conclusion: The potentiostatic technique was used $25^{\circ}C$ NaOH solution of 1 M and 5 M as supporting electrolyte for 2hours. Nanomesh was built more uniform and fine in 1 M NaOH solution than 5 M NaOH solution.

  • Obturation is a important procedure of root canal treatment. Canal filling should be both provide a hermetic seal for the root canal system and eliminate leakage channel from the oral cavity. Gutta-percha have been the standard material of choice for root canal obturation. Canal filling has been aimed at maximizing the amount of gutta-percha and minimizing the amount of sealer. However recently, single cone technique has been introduced that include calcium silicate-based sealer and single gutta-percha cone. It is important to select an obturation technique that offers consistency and is easy to use. From the standpoint of appropriate technology, the single cone technique is thought to be useful for general dentist.

  • BACKGROUND. Even though excellent impression materials are now available for making accurate replication for hard and soft tissue, the numerous dentists have faced lots of obstacles in making simultaneous impressions of multiple abutments. CASE DESCRIPTION. This article describes a modified method of tray fabrication using auto-polymerizing acrylic resin and impression technique for multiple prepared teeth in cases with limitations and difficulties in taking dental impressions. CLINICAL IMPLICATION. This segmental tray technique has several advantages, including higher impression quality, fewer impressions, and being more comfortable for the patient and less stressful for the clinician.

  • Purpose: Prosthodontics for edentulous patients is a treatment technique using implant, which has impactful results in retention and support effects. Methods: As a retention technique, SCRP (screw and cement retained implant prosthesis) has reported in many studies as a beneficial method for both patients and curers, which can reduce errors in process of making abutment and top implant. Results: Prosthesis manufacturing, as polymerization method of hardened resin teeth with thermoplastic resin, is helpful for patients with aesthetic and financial situations regarding residual ridge and interocclusal relationship, also indicates reliable results in both retention and care. Conclusion: Using SCRP technique, we notably obtained a clinical and aesthetic outcome from five implants in anterior tooth, which are half fixable and detachable implants on screw of implant abutment by the technicians at anytime.

The Study on relation of dental hygienist's job-stress and self-conception (치과위생사의 업무스트레스와 자아개념과의 관계 연구)

  • The purpose of this study is to help dental hygienist's efficient work and 10 use this data as basic material for quality improvement of medical treatment by identify and analyze dental hygienist's job-stress and self-conception. This data was surveyed by 197 dental hygienists who working at Dental Clinic, Dentist's office, and Dental Dept on the General hospital in Daegu area. The result as follow. 1. General characters of dental hygienist who participated this survey was mostly less than 25years old and works in the Dentist's office. 2. The dental hygienist's job stress average 259, self-conception was average 3.06. 3. A stress degree by the primary factors appeared that the stress related to Role-conflict was the hygienist. On the other hand, stress related to employees was the lowest. 4. Concerning the stress of respondent's expert knowledge or technique, there exist statistically significant relations in age(p=0.000). Total period of work(p=0.000)), marriage(yes or no)(p=0.012), number of job movement(p=0.024). 5. Concerning the mutual analyze between stress by the primary factors and self-conception, there exist statistically significant relations in stress from expert knowledge or technique(p<0.05, r=0.l53), conflict with patient(p<0.001, r=0.347), conflict between dentist and patient(p<0.05, r=0.159), relationship with employee(p<0.05, r=0.l83). In conclusion, there were significant relations between job-stress and conception. The study of program development for the self-conceit, positive attitude and self-conception are highly required to help dental hygienist's efficient work.

Efficacy of kaleidoscope, virtual reality, and video games to alleviate dental anxiety during local anesthesia in children: a randomized clinical trial

  • Background: Distraction is a technique used to divert a patient's attention from unpleasant procedures. This study aimed to evaluate the effectiveness of kaleidoscopy, virtual reality, and video games in reducing anxiety and pain during invasive dental procedures in children. Methods: Sixty-six children aged 6 to 9 years were randomly assigned to three groups during local anesthesia administration: Group 1 (kaleidoscope), Group 2 (virtual reality), and Group 3 (mobile video games). The anxiety of the children was evaluated using physiological measures (heart rate) at three different time points: before, during, and after the procedure. The Raghavendra, Madhuri, and Sujata pictorial scale was used as a subjective measure before and after the procedure. Subjective measures of pain were assessed using the Wong-Baker Faces Pain Scale. The data were statistically analyzed using the Kruskal-Wallis and Wilcoxon signed-rank tests. Results: In the intergroup comparison, there were no statistically significant differences in the physiological measures of anxiety scores between the three groups before, during, and after distraction. Raghavendra, Madhuri, and Sujata pictorial scale scores were assessed before and after distraction, but no statistically significant differences were observed. Among the three groups, the children in Group 2 showed a significant reduction in pain scores. Conclusion: Compared with kaleidoscopes and video games, virtual reality is a promising distraction technique for reducing dental fear, anxiety, and pain during local anesthesia administration in children.

Inhalation Sedation with Nitrous Oxide in Dental Treatment (Nitrous Oxide를 이용한 흡입진정)

  • The usage of nitrous oxide is increased for the anxious patient to dental treatment. There are two methods to induce the sedation during dental treatment. One is sedation with drugs the other no need of drugs. We discussed here about sedation with drugs. The methods of drug administration are oral, intramuscular, intravenous, inhalation. The method of oral administration of drugs are convenient to patient and doctor but poor controllability. Intramuscular method is a parenteral technique that maintains several advantages over the enteral technique. However its pales in comparison to other parenteral technique. Intravenous method represents most effective method of ensuring predictable and adequate sedation in all patients. But it has inability to reverse the action of drugs after they have been injected except some drugs (e.g., narcotics and benzodiazepine). A variety of gaseous agents may be administered by inhalation to produce sedation. In dental practice, the inhalation administration of gas means use of nitrous oxide. There are many advantages of nitrous oxide administration. First, very short latent period and rapid onset of drug action which lead to possible titration of drug concentration. With nitrous oxide, clinical effects may become noticeable as quickly as 15 to 30 seconds after inhalation. Recovery from inhalation sedation is also quite rapid. In out patient dental practice rapid recovery is very important because it permit to discharge the patient without escort and the patient return to their ordinary life without limit. To success the conscious sedation with nitrous oxide, the administrator should be keep the mind that always titration of nitrous oxide concentration during induction and treatment. Careful observation need during treatment to prevent oversedation because the adequate nitrous oxide concentration to patients changed by environmental stress. Always begins with 100% oxygen and ends with 100% oxygen to prevent diffusion hypoxia which rare in clinical practice.
