장애노인의 특성과 장애로 인한 추가소요 의료비 지출간의 관련성 연구 (A Study of the Relations between Characteristics of the Elderly with Disabilities and Extra Health Care Costs Caused by Disability)

  • This study found a relationship between the characteristics of the elderly with disabilities and extra health care monthly costs caused by disability and suggests methods for effectively managing health care spending of the elderly with disabilities. This study used data from the 2011 Survey on the Disabled conducted by the Ministry of Health and Welfare and the Korea Institute for Health and Social Affairs. The results of the analysis indicate that the relations between the socio-demographic characteristics of the elderly with disabilities and extra health care monthly costs caused by disability of the elderly are influenced by factors such as gender, age, members of the household, average monthly income and type of health insurance. Elderly people with disabilities spend too much for their medical care. To solve this problem, the government needs to find a variety of policy approaches to improve administrative procedure, establish a medical delivery system and enhance health care services.

구강건강증진프로그램이 노인의 구강건강에 미치는 영향 (Effects of Oral Health Promotion Program on Oral Function in the Elderly)

  • Objectives : The purpose of this study was to examine the effects of an oral health promotion program. And suggest that education on oral health should be given by a professional. Methods : Sixty-four people over the age of 65 years were selected from three elderly care facility located in Gyeongsangbuk-do, A city. From June 7 to August 19, 2016, an oral health promotion program was given to two experimental groups (N=21 and 20) for a total of eight times. One program was conducted by a professional and the other by a non-professional although both were considered experts. Additionally, a control group(N=23) was a observed. Results : While both experimental groups showed a positive change from the oral health promotion program, the experimental group that was given the oral health promotion program by a professional showed more positive changes in oral cavity function. Conclusions : By using professionals, it is possible to provide a health promotion program that actively monitors the oral cavity of the elderly as well as provide elderly care services for oral hygiene. Appropriate elderly health policies and oral health business model for elderly are needed.

의료서비스경험조사의 신뢰도 및 타당도 검토: 의료기관 특성별 차이를 중심으로 (Review of Reliability and Validity of Medical Service Experience Survey: Focused on the Differences by Type of Medical Institutions)

  • Background: The efforts to build more "people centered," "patient centered" health system has been emerging all over the world. Aligning with it, the Korean government is conducing the survey called "Medical Service Experience Survey (MSES)." There are critics, however, that MSES is not scrutinizing the medical experiences of patients in various healthcare settings. For this reason, this study aims to perform an empirical analysis of the differences in answers of patients responding to various healthcare settings. Methods: There are two steps in this study. First, explanatory analysis is conducted to compare the tendency of statistical concentration on questionnaires by divided healthcare settings. Second, confirmative analysis is carried out to evaluate the construct validity, reliability, and discriminant validity of the questionnaire in each healthcare setting. The raw data of MSES, which was conducted in 2020 by the Ministry of Health and Welfare in Korea and the Korean Institute for Health and Social Affairs is used. Results: As a result of exploratory factor analysis for all outpatients, the items were classified into four factors statistically: "doctor experience," "nurse experience," "outpatient service experience," and "patient satisfaction." It was confirmed that the reliability of all factors extracted was secured. However, for patients who visited hospitals, questionnaires related to personal privacy, such as "experiences on medical staffs considering physical exposure" or "experiences related to personal information exposure," were answered in conjunction with items of "nurse experience." Besides, patients responded that administrative elements of medical services, such as "experiences of comfort in medical institutions" and "experiences of satisfactory administrative services," were related to the items of "nurse experience." The answers of patients who visited traditional medical hospitals and clinics about "doctor experience" and "nurse experience" were not discerned statistically, and the answers to "doctor experience," "nurse experience," and "medical institution experience" were entangled with the responses of patients who visited dental hospitals and clinics. On the other hand, as a result of the confirmatory factor analysis, it was found that the inquiries of MSES generally had intensive validity. Conclusion: The collection of objective and scientific data is the prominent component to enlighten the patient-centered healthcare system alongside with change of the worldwide paradigm of measuring the healthcare system performance as follows the transition of perspective of health care from provider-centered to patient-centered. This study empirically shows that the patient experience can vary as the healthcare settings. Furthermore, to make an advance in measuring the experience of patients with medical services, this article proposes the deliberate consideration of the different kinds of healthcare settings and articulate design of the survey.

부산지역 양호교사의 업무분석에 관한 연구 (A Study on the analysis of activities of t?e 5.H.T. (5.H.T. in Pusan City))

  • The purpose of this study was to find out the general characteristics of school health teachers, the status of school health resources and the degrees of self-confident performance for the 124 school health teachers in Pusan City. Data was collected by means of questionaires from Aug. 1986 to Mar. 1987. The data were analyzed by use of percentage, mean, standard deviation, t-test, ANOVA and Pearson-correlation coefficient. The results of this study were as follows: 1 . General Characteristics of School Health Teachers (SHT) 1) The average of age of the SHT was 32.8 years old and 39.5% of them were from 30 to 34 years old. 2) The average for school nursing experience of the SHT was 7.9 years and 37.9% of them were from 5 to 9 years. 3) The 45.2% of them have not the clinical experience. 4) The 74.2% of them were graduated from the 3 years college of nursing. 2. Status of school health resources and nursing activities. 1) The average of student number was 2497.3 and class number was 45.2. 2) The average of school health budget was 1039000 won and 27.7% of school health budget expended on examination cost. 3) Only 29.0% of all schools have organization for school health. 4) The 84.7% of all schools have health clinic separately and 69.1% of schools have less than $33m^2$ sized. 5) The average of clinic visitor number was 2111.8 for 1 year. 6) Major problem was on digestive system. And other problems were skin, respiratory, musculo-skeletal system and dental problem. 7) The number of literal message was 14.4 times for 1 year. 3. The degree of the school health teachers' self-confidence. 서울대학교 치과병원 소아치과에 내원한 신환 분포 및 내원 동기에 관한 연구 (A STUDY OF NEW-PATIENT DISTRIBUTION AND THE MOTIVES FOR VISITING IN THE DEPARTMENT OF PEDIATRIC DENTISTRY AT SEOUL NATIONAL UNIVERSITY DENTAL HOSPITAL)

  • 거주지 주변 소아치과 전문 의원의 수가 빠르게 증가하고 있는 요즘 대학병원의 소아치과에 내원하는 환자들의 내원 경로와 내원 동기는 의원급 소아치과에 내원하는 환자의 내원 경로, 동기와는 다를 것으로 예상된다. 2010년 5월 1일부터 7월 31일까지 서울대학교 치과병원 소아치과에 신환으로 내원한 환자의 자료를 조사 분석하고, 보호자를 대상으로 한 설문 조사결과를 바탕으로 대학병원에 신환으로 내원한 환자의 내원 경로와 내원 동기 등에 관하여 알아보고, 환자 및 보호자가 어떠한 기준으로 병원을 선택하고 내원하는지 살펴보았다. 1. 조사기간 동안 내원한 신환의 평균 연령은 6.2세였다. 2. 전체 신환 중 남자 환자는 54.5%, 여자 환자는 45.5%로 남자가 여자보다 더 많은 비율을 차지하였다. 3. 보호자를 대상으로 한 설문조사에서 서울대학교 소아치과에 내원하게 된 경위는 타 치과에서 대학병원으로의 권유가 가장 많았으며, 그 다음은 주변의 추천, 예전에 치료 받던 병원이라서, 거주지와의 접근성이 좋음, 브랜드 네임, 인터넷 검색 순으로 나타났다. 4. 내원 전 타 치과를 방문한 경험이 있는 환자는 75%로 나타났다. 1) 타 치과 방문 후 내원한 환자의 71%는 1군데 방문 후 내원하였으며 3군데 이상 방문 후 내원한 환자도 약 4% 정도로 나타났다. 2) 이전에 방문한 치과의 전문성 여부를 살펴보았을 때 소아 전문치과가 아닌 곳을 방문 후 내원하는 경우가 약간 많게 나타났다. 3) 타 치과를 방문 후 본원에 다시 내원하게 된 이유는 진단의 정확성을 확인하기 위해서라는 경우가 60.7%로 가장 많았으며, 이전 병원에서 대학병원으로 의뢰, 비용상의 문제, 기타 의견, 이전 치과의 접근성 불편 순으로 나타났다. 90% 이상의 환자들이 진단의 정확성을 재확인하거나 이전 병원에서 대학병원으로 의뢰하여 재내원한 것으로 나타났다. 4) 본원에서 치료를 원치 않는 경우, 치료를 원치 않는 가장 큰 이유는 교통 불편, 접근성이 어렵다는 점이었다. 어린 연령의 환아와 보호자가 동행해야 하며 잦은 간격으로 여러 번 내원해야 하는 소아치과 진료의 특성상 접근성이 재내원 여부를 주저하는 가장 큰 요인으로 작용한 것으로 보인다.

고객가치가 관계품질 및 고객충성도에 미치는 영향: 치과병·의원을 중심으로 (A Study of the Impact of Customer Value on Relationship Quality and Customer Loyalty)

  • Purpose - Recent rapid environmental changes in the hospital industry are accelerating the spread of customer satisfaction management. Customers' desires have become diversified and advanced; in the past, customers tended to preferred popularized and standardized care, whereas they now prefer individualized and differentiated care, based on an increase in income. Specifically, this study tries to analyze the mediating effects of factors that affect the configuration portion of customer value and relationship quality (customer trust and relationship commitment) by investigating the impacts and configuration factor of customer value on relationship quality and determining how these factors impact customer loyalty directly or indirectly. Research design, data, and methodology - This study aims to determine the customer value factors that impact the perceptions of dental hospital customers, how these factors impact relationship quality and customer loyalty, and the causal relationship of these factors, and to verify the research model based on previous research. To increase the validity and reliability of the questionnaire, the authors of this study constructed basic questions using measurement tools already verified for reliability and validity in existing studies. In this study, customer value is defined as customers' recognizing value by exchanging goods or services and is measured using a five-point Likert scale using 19 questions about the 4-Ds, such as convenience value, quick service, response value, and trustworthiness. For each question, "very low" was set at 1 point and "very high" at 5 points. Customer trust, relationship commitment, and customer loyalty are also measured using a five-point Likert 5-point scale (1 = very low, 5 = very high) based on previous studies. Results - For customer value, trustworthiness and quick service are shown to have direct significant positive impacts on customer loyalty. For customer value and quality of the relationship (customer confidence and commitment), trustworthiness, response value, confidence value, and quick service are shown to have a significant positive impact on customer truth, in order of impact. For the relationship between customer value and commitment, quick service and response value are shown to have significant positive impact. Customer confidence has a very high positive influence on commitment. For the relationship between the quality of the relationship (customer confidence and commitment) and customer loyalty, customer confidence is shown to have more of an impact than commitment, in terms of a direct influence of customer loyalty. Commitment showed a positive impact on customer loyalty. For the relationship between customer confidence and customer loyalty, commitment showed a mediating effect. Conclusions - Many additional variables could apply; this study focused on customer value, quality of the relationship, and customer loyalty. In particular, there will be significant value in identifying the relationships among customer value, relationship quality, and customer loyalty by using impact factors for customer value; ensuring external validity by expanding denotation and applying the findings to other service industries; and undertaking continuous research. This study has limited generalization potential because the target for this survey was located only in the Seoul area.

Comparison of Parallel and Fan-Beam Monochromatic X-Ray CT Using Synchrotron Radiation

  • Monochromatic x-ray CT has several advantages over conventional CT, which utilizes bremsstrahlung white x-rays from an x-ray tube. There are several methods to produce such monochromatic x-rays. The most popular one is crystal diffraction monochromatization, which has been commonly used because of the fact that the energy spread is very narrow and the energy can be changed continuously. The alternative method is the use of fluorescent x-ray, which has several advantages such as large beam size and fast energy change. We have developed a parallel-beam and a fan-beam monochromatic x-ray CT, and compared some characteristics such as accuracy of CT numbers between those systems. The fan beam monochromatic x-rays were generated by irradiating target materials by incident white x-rays from a bending magnet beam line NE5 in 6.5 GeV Accumulation Ring at Tukuba. The parallel beam monochromatic x-rays were generated by using a silicon double crystal monochromator at the bending magnet beam line BL-20BM in Spring-8. A Cadmium telluride (CdTe) 256 channel array detector with 512mm sensitive width capable of operating at room temperature was used in the photon counting mode. A cylindrical phantom containing eight concentrations of gadolinium was used for the fan beam monochromatic x-ray CT system, while a phantom containing acetone, ethanol, acrylic and water was used for the parallel monochromatic x-ray CT system. The linear attenuation coefficients obtained from CT numbers of those monochromatic x-ray CT images were compared with theoretical values. They showed a good agreement within 3%. It was found that the quantitative measurement can be possible by using the fan beam monochromatic x-ray CT system as well as a parallel beam monochromatic X-ray CT system.

지역주민의 신체활동 수준에 따른 직접 의료비 지출의 차이 (Differences in the Direct Medical Expenditures among Physical Activity Levels of the Community Residents)

  • 본 연구의 목적은 지역 주민들의 신체활동 수준이 직접 의료비 지출에 미치는 영향을 분석 하는데 있다. 연구 대상자는 경상북도에 있는 G시의 청소년 수련원, 노인 복지회관, 문화회관, 그리고 체육센터 이용자들을 모집단으로 설정한 후, 각각의 시설별로 이용자들을 250명씩 할당표집 하여 총 773부의 설문지들을 수집 하였으며, 이들 중에서 불충분한 내용의 57부를 제외한 총 716부를 최종분석 하였다. 연구도구는 인구사회학적 요인, 신체활동 수준, 개인의 직접 의료비 지출 항목으로 구성된 설문지를 이용하였다. 연구 결과, 신체활동 수준이 매우 높은 집단은 한 달에 7,500원을, 신체활동이 높은 수준의 집단은 26,299원을, 신체활동 수준이 보통인 집단은 47,517원을, 신체활동 수준이 낮은 집단은 9,314원을, 그리고 신체활동 수준이 매우 낮은 집단은 9,997원을 직접 의료비로 지출한 것으로 나타났다. 지역주민들의 신체활동 수준에 따른 직접 의료비용 지출 차이를 분석한 결과, 외래진료에 대한 지불 비용에는 유의미한 차이가 없는 것으로 나타났다. 이러한 결과는, 신체활동 수준이 높다고 하여도 개인이 지불하는 직접 의료비용은 감소되지 않음을 나타내고 있다.

엔도크라운 디지털 인상을 위한 구강스캐너 3종의 정확도 평가: 실험실 연구 (Evaluation of the accuracy of three different intraoral scanners for endocrown digital impression: An in vitro study)

  • 목적:본 연구의 목표는 다른 유형의 엔도크라운(endocrown) 와동 형태를 세 가지 다른 구강스캐너로 디지털 인상을 채득하였을 때의 정확성을 평가하는 것이다. 재료 및 방법: 두 개의 인체 하악 대구치를 협측벽이 있는 것(Class 2)과 협측벽이 없는(Class 3), 두 가지 엔도크라운 지대주 디자인으로 치아형성 하였다. 와동 디자인 2종을 레퍼런스로 탁상용 스캐너(E3, 3shape)와 세 개의 다른 구강스캐너, Trios 3 (3shape, TRI group), Cerec Omnicam (Dentsply Sirona, CER group), i500 (Medit Corp., I5 group)로 스캔하였다. 표준 테셀레이션 언어(.stl) 데이터 세트를 얻어, 계측 소프트웨어에서 불러들였다. 각 구강스캐너로 획득한 반복된 스캔 데이터 사이의 편차에 기초하여 정밀도(precision)를 평가하였다. 기준 데이터와 반복하여 얻은 구강스캔 사이의 편차로서 진도(trueness)를 평가 하였다. 상호작용을 탐지하기 위해 데이터는 일변량 분산분석(ANOVA)을 사용하여 통계적으로 분석하였고, 실험군의 비교 분석을 위해 데이터는 .05의 유의 수준에서 일원 분산분석 및 사후 Tukey 테스트로 분석하였다. 결과: I5 군의 두 와동 형태에 대한 편차값은 진도의 측면에서 다른 구강스캐너 군에 비해 낮았다. 두 와동 디자인 모두에서 TRI 군은 다른 구강스캐너군 보다 우수한 정밀도를 보였다. 결론: 구강스캐너의 다양한 기술과 다양한 엔도크라운 치아형성 디자인이 디지털 스캔의 정확도에 영향을 미쳤다.

초.중학생을 위한 보건교육의 영역 및 주요개념 선정을 위한 일 연구 (A Study on the Selection of Health topic areas and major concepts for Health Education in Primary and Junior High Schools)

  • In Korean education, the health contents are scattered in various course subjects throughtout the primary and junior high school curriculum. So it is very difficult to provide systematic health education. The purpose of this study was to provide a guide for health education using health topic areas and major concepts that represent the scope of material that should be covered in health instruction. The steps used in selecting these health topic areas and major concepts were as follows: 1. A review of the literature related to health and health education was done to develop the rationale underlying this study. 2. Health topic areas basic to the growth and development characteristics of children, to human needs and to societal needs for healthful living were indentified. 3. The major concepts for each health topic area based on health sciences and children's growth and development levels were selected. 4. The major concepts selected were organized in sequence to guide health education from grade one to grade nine. The results of this study were as follows: 1. The identification of eleven health topic areas essential for health education. These include: personal habits and health healthy growth and development nutrition and health prevention of disease and disorders drugs and health mental health family life and health sex education accident prevention consumer health community health 2. The identification of the major concepts(generalizations) for each health topic area: 33 major concepts were identified as a guide in determining the health content of health education programs. These are 1) body cleaniness, 2) health of the sensory organs, 3) dental health, 4) exercise and rest, 5) growth and development, 6) body structure and function, 7) developmental tasks, 8) balanced nutrition, 9) eating habits, 10) food preparation and food storage, 11) sources of disease and disorders, 12) disease preventive behavior, 13) care during illness, 14) drug use and misuse, 15) drug addiction, 16) emotional responses, 17) human relationship, 18) self concept, 19) social adjustment, 20) health habits of the family, 21) interdependence of family members, 22) origin of life, 23) characteristics of man and woman, 24) sexual instinct, 25) safety behavior, 26) emergency measures, 27) criteria for selection of health products, 28) proper use of health information, 29) utilization of health and medical services, 30) environmental conservation, 31) environmental pollution, 32) population control, 33) function of public health services. 3. The organization of the concepts(generalizations) in sequence and for continuity in health instruction at the primary and junior high school level.
