One of the major deficiencies of composite restorative resins is their insufficient resistance to wear. Of the multitude of factors that have been associated with wear, subsurface degradation within the restoration is considered to be one. The aim of this study was to evaluate the resistance to degradation of four commercial composite resins in an alkaline solution. The brands studied were Z100(3M), Clearfil AP-X(Kuraray), Tetric Ceram(Vivadent), Aelit flo(Bisco). Resistance to degradation was evaluated on the basis of the following parameters: (a) mass loss(%) - determined from pre-and post-exposure specimen weights: (b) Si loss(ppm) - obtained from ICP-AE analysis of solution exposed to specimens; and (c) degradation depth(${\mu}m$) - measured microscopically (SEM) from polished circular sections of exposed specimens. The results were as follows: 1. The sequence of the mass loss was in ascending order by AE, EL, TC, Z100. There was statistically significant difference of mass loss between AE, CL group and TC, Z100 group(p<0.05). 2. The sequence of the degree of degradation layer depth was in ascending order by AE, CL, TC, Z100. But there was no statistically significant difference of degree of degradation layer depth between AE and CL(p<0.05). 3. For the Si concentration, Z100 was the highest of all. 4. The correlation coefficient between mass loss and degradation depth was relatively high(r=0.71 p<0.05).
Objectives: The aim of the study was to evaluate the 5-year clinical performance of occlusal carious restorations using nanofill and microhybrid composites, in combination with 3-step etch-and-rinse adhesives, in patients who were going to commence orthodontic treatment. Materials and Methods: A total of 118 restorations for occlusal caries were conducted prior to orthodontic treatment. Occlusal restorations were performed both with Filtek Supreme XT (3M ESPE) and Filtek Z250 (3M ESPE) before beginning orthodontic treatment with fixed orthodontic bands. Restorations were clinically evaluated at baseline and at 1, 2, 3, 4, and 5-year recalls. Results: None of the microhybrid (Filtek Z250) and nanofill (Filtek Supreme XT) composite restorations was clinically unacceptable with respect to color match, marginal discoloration, wear or loss of anatomical form, recurrent caries, marginal adaptation, or surface texture. A 100% success rate was recorded for both composite materials. There were no statistically significant differences in any of the clinical evaluation criteria between Filtek Z250 and Filtek Supreme XT restorations for each evaluation period. Conclusions: The composite restorations showed promising clinical results relating to color matching, marginal discoloration, wear or loss of anatomical form, recurrent caries, marginal adaptation, and surface texture at the end of the 5-year evaluation period.

Excessive tooth wear can lead to decrease in occlusal vertical dimension and can cause pathological changes in the oral environment and masticatory system. When recovering occlusal vertical dimension and occlusion, accurate diagnosis and analysis are essential. This clinical case describes a 75-year-old woman with severely worn dentition due to loss of the posterior support. Full mouth rehabilitation with occlusal vertical dimension increment was planned. Clinical and radiographic examinations, occlusal vertical dimension evaluation, and diagnostic wax-up were performed and patient adaptability was evaluated using provisional restorations. As for definitive restoration, considering economic condition of the patient, removable partial denture was fabricated and solitary implants were placed in the mandibular left and right posterior region to increase support and retention of the removable partial denture. During one year of follow-up, functional and esthetic outcomes were observed satisfactory.

With the development of digital dentistry, it is being applied in various ways of dental treatment. This case report presents the definitive prosthesis designed in advance with a re-established vertical dimension and the digital technology, which determined the amount of tooth preparation, in order to preserve as much tooth structure as possible in a patient with pathological wear of the posterior teeth and loss of vertical dimension. For accurate tooth preparation, the guides of the occlusal and axial surfaces were digitally and additively manufactured. Then, aesthetics and anterior guidance were established at the provisional stage. The information of the provisional restoration was delivered to the definitive stage by double scanning. The digital technology, including the virtual planning and the guided tooth removal, produced the definitive restorations satisfactory to both the patient and clinician.

Loss of posterior support may cause extrusion of antagonistic teeth and excessive wear of remaining teeth which often leads to the destruction of the occlusal plane. In such cases, it is critical to verify the need to increase vertical dimension of occlusion (VDO). Should you increase VDO, provisionalization is crucial in evaluating function and esthetics. Double scanning technique is a useful method when fabricating definitive restoration that mimic provisional restoration. In this case, a patient with apparently no loss of VDO and insufficient interocclusal space for dental materials due to loss of posterior support and extrusion of antagonistic teeth was rehabilitated using double scanning technique.

In a case of multiple posterior teeth loss, antagonistic teeth extrude to the edentulous space and compensatory occlusion on the remained anterior teeth leads to occlusal trauma. Extrusion of antagonistic teeth breaks down occlusion plane and loss of posterior support bring about severe wear of remained teeth. In this situation, it is needed to restore remained teeth and edentulous space by increasing vertical dimension to obtain prosthodontic rehabilitation space and to correct occlusion plane. In this case report, the patient had a masticatory problem with loss of posterior teeth support and an esthetic problem of shortened anterior teeth. Before the tooth preparation for the prosthodontic restoration, the patient used removable device for 2 months to increase vertical dimension reversibly. After that, he got provisional fixed restoration with irreversible tooth reduction and used it for 3 months. It had spent 5 month to evaluate the adaptation state on final restoration with incresed vertical dimension. The increasing amount was 3 mm, which was relatively in less degree and masticatory system adapted to the increased vertical dimension without any pathologic changes. Final restoration was made to have equal-intensity contacts on all teeth in a verifiable centric relations and immediate disclusion of all posterior contacts the moment the mandible moves in any direction from centric relation. In addition, metal occlusion surface on posterior teeth was applied to prevent excessive muscle activation, occlusal trauma and the porcelain fracture.

Purpose: The purpose is to test and evaluate the physical properties of commonly used temporary restoration materials and newly emerged materials. Materials and Methods: Four groups of polymer materials were evaluated: Polymethyl methacrylate (PMMA) 2 groups, Polyetheretherketone (PEEK), Polycarbonate. Four physical properties were tested: surface hardness, bending strength, abrasion resistance during wear, wear behavior. The 3-axis bending strength and Vickers hardness test were measured using a universal testing machines respectively. The microstructure was observed with a scanning electron microscope and weight comparison was evaluated after 100,000 chewing tests using a chewing simulator. Kruskal wallis test was performed to evaluate statistical significance. Results: The four groups showed the highest flexural strength and Vickers hardness of PEEK, followed by PC, PMMA-H, PMMA-T. Microstructure observation also showed the least surface roughness in the PEEK group, followed by PC, PMMA-H, PMMA-T. Conclusion: PC is considered to have sufficient mechanical properties that can be applied to the manufacture of temporary teeth. However, further studies, such as biocompatibility, are considered to be necessary for practical clinical applications.

The difference of composition of composite resin may affect the mechanical properties of composite resin and the environment is important for the properties of materials. The composite resin restoration is always exposed to fluid in oral cavity and the composite resin matrix is able to absorb water, which is accompanied by some swelling of the composite The uptake of water by composites has been correlated with decreases in surface hardness and wear resistance. The purpose of this study was to investigate the effects of water storage in $37^{\circ}C$ distilled water after 7days, 30days, 60days, 120days on compressive strength and flexural strength of dental composite resin, Z-100(group 1) Spectrum(group 2), Clearfil AP-X(group 3), Pyramid(group 4), Heliomolar(group 5). The compressive and flexural strength were measured by instron machine. The following results were obtained: 1. There were significant reduction of compressive strength as water storage time increased, 7days, 30days, 60 days, 120days(p<0.05). 2. There were significant reduction of flexural strength as water storage time increased, 7days, 30days, 60days, 120days(p<0.05). 3. Group 1, 2, 3 -hybrid type showed higher compressive and flexural strength than group 5-microfine type which had lower filler contents.
This study reported the treatment of a patient with excessive worn dentition and limited maxillo-mandibular space for restoration, utilizing the computer-aided design and computer-aided manufacturing (CAD/CAM) technology. After the thorough examination of the patient's occlusal vertical dimension (OVD), full mouth rehabilitation was planned with increase of the OVD. The patient was satisfied with the provisional restorations establishing the increased OVD. The horizontal and vertical data of the patient's jaw relation that the provisional restorations contained were transferred to the definitive metal ceramic fixed prostheses by double scanning and three-dimensional printing. After the fixed restorations were cemented to the abutments, electronic surveying and three-dimensional printing were used to fabricate metal frameworks for the patient's removable partial dentures. The mandibular definitive removable prostheses were delivered to the patient's mouth and the full mouth rehabilitation procedures were completed. The digital technologies used for this case produced fixed and removable restorations satisfactory in masticatory, phonetic and aesthetic functions to both the patient and the dental clinician.

The composite restorative resins have their insufficient resistance to wear. The subsurface degradation within the restoration is considered to be associated with wear. The aim of this study was to evaluate the resistance to degradation of six commercial composite resins in an alkaline solution. The brands studied were Clearfil APX(Kuraray), Heliomolar(Vivadent), Surefil(Dentsply), TPH(Dentsply), Tetric Ceram(Vivadent), and Z100(3M). Preweighed discs of each brand were exposed 0.1N NaOH solution at $60^{\circ}C$. After 14 days they were removed, neutralized with HCL, washed with water and dried to constant mass at $60^{\circ}C$. Resistance to degradation was evaluated on the basis of the following parameters: (a) mass loss(%) - determined from pre-and post-exposure specimen weights; (b) Si loss (ppm)-obtained from ICP-AE analysis of solution exposed to specimens; and (c) degradation depth$({\mu}m)$ - measured microscopically (SEM) from polished circular sections of exposed specimens. The results were as follows: 1. The mass loss was in descending order by Z100, TC H, S, CL, TPH and in the range of $0.45\sim3.64%$ 2. The degradation layer depth was in descending order by H, Z100, S, TC, TPH, CL and in the range of $10.85\sim73.38{\mu}m$ 3. For the Si concentration, Z100 was the highest of all 4. The highly significant correlation(r=0.81, p<0.05) was observed between mass loss and degradation depth. 5. Under scanning electronmicroscopy, the degradation of connection between resin matrix and fillers was observed 2 weeks after soaking in NaOH solution.
