비조절성 제2형 당뇨 환자의 치아 탈구 손상에 따른 치수 괴사: 증례 보고 (Pulp necrosis following luxated injury to teeth in a patient with uncontrolled type II diabetes mellitus: a case report)

  • Patients with diabetes mellitus show delayed wound healing and increased susceptibility to infection. Therefore, the effects of diabetes on pulpal and periodontal healing should be taken into consideration when treating diabetic dental traumatized patients. This case presents the treatment for dental traumatized 20 yr old female with uncontrolled type II diabetes. The traumatized upper central incisors had showed pulpal healing in early days. However, 7 mon after the trauma, the teeth had been diagnosed with pulp necrosis with apical abscess. Eventually, non surgical root canal treatment on the teeth had been performed.

생활치수복탁조후의 치유 결과에 대한 임상적 고찰 (CLINICAL STUDY ON THE RESULTS OF PULP HEALING AFTER PULP CAPPING)

  • The authors have observed 207 cases on the results of pulp healing after pulp capping. The results are as follows: 1) We could have success-rate of 70% in 207 cases at the ages from 14 to 59. 2) The success-rate in male is 72.4% in 87 cases, and female is 67.5% in 120 cases. So we observed that success-rate was slightly higher in male than female. 3) In view of age group, it was observed that success-rate was 79% in 129 cases at 30 ages below, 62.5% in 40 cases at 30-40 ages, 47.3% in 38 cases at 40 ages over. So we found that the failure-rate was increased by aging.

Effect of hypoxia on angiogenesis-related proteins in human dental pulp cells

  • Dental pulp is a highly vascularized tissue with high regenerative potential. Revascularization of severed vasculature in the tooth is required for pulp healing during avulsed tooth treatment. In this study, the relative expression of angiogenesis-related proteins was determined in human dental pulp cells using a human angiogenesis proteome profiler array. The proteome profiler array detected differentially expressed angiogenesis-related factors under conditions of hypoxia, which enhances the angiogenic potential of dental pulp cells. We confirmed that hypoxia regulates the mRNA expression of angiogenesis-related factors, including CXCL16 in dental pulp cells. Furthermore, conditioned media of hypoxic pulp cells induced tube-like structures of vascular endothelial cells, which were reduced by the neutralization of CXCL16 function. In conclusion, our data show that angiogenesis-related factors are differentially expressed by hypoxia in dental pulp cells and suggest that CXCL16 may involve in the revascularization of hypoxic dental pulp.

흰 쥐 치아 재식 후 치수 치유 양상의 조직학적 관찰 (HISTOLOGY OF DENTAL PULP HEALING AFTER TOOTH REPLANTATION IN RATS)

  • 본 연구의 목적은 흰 쥐(rat)를 이용하여 미성숙 단계의 치아를 발치 후 즉시 재식 하였을 때 나타나는 치수의 치유 양상을 조직학적으로 관찰하고자 하는 것이다. 생후 4주된 암컷 Sprague-Dawley 계 흰쥐의 상악 우측 제1대구치를 발거 후 원래의 치조골와 내로 위치시켰다. 재식 후 3일째부터 국소적으로 치수 내 염증 소견이 관찰되었으나, 치근 부위에서는 이미 치수의 재혈관화 및 치유가 진행되고 있는 소견이 관찰되었다. 재식 후 5일째부터는 odontoblast-like cell이 관찰되기 시작하였다. 삼차 상아질의 형성은 재식 후 1주째부터 관찰되기 시작하였으며, 2주째부터는 확실히 관찰할 수 있었다. Odontoblast-like cell 및 삼차상아질 형성은 4주째까지도 계속 관찰되었다. 재식 후 4주째에는 bone-like tissue 및 cementum-like tissue이 형성되었음을 관찰하였다. 본 실험을 통하여 흰 쥐 치아 재식 시 석회화 과정은 초기에는 삼차상아질 침착에 의해서 진행되나, 시간이 경과하면서 점차 bone-like tissue 또는 cementum-like tissue가 차지하는 비율이 증가하는 것을 관찰하였다.

생활치수절단술에 사용되는 복탁제가 치수에 미치는 영향에 관한 실험적 연구 (EXPERIMENTAL STUDY ON EFFECTS OF PULP CAPPING AGENTS THAT ARE USED IN VITAL PULPOTOMY TO PULP TISSUE)

  • To compare the effects of various pulp capping agents that are usually applied to human pulp tissue, adult dogs were bred for a certain period and each capping agent was applied experimentally to pulp tissue after vital pulpotomy. Histological observations are as follows. 1) In comparison between methods of vital pulpotomy, one and two appointment method, different courses of healing were observed. In one appointment method, the granulation tissue formation at the amputation sur face of pulp tissue had a tendency to be transformed to scar tissue formation. In two appointment method, more transformation than that of one appointment method from scar tissue to dentin matrix formation were observed. 2) Histologic changes that have appeared in pulp tissue are a) fixation at outer layer b) degeneration at middle layer c) hyperemia and round cell infiltration at inner layer 3) With use of formocresol mixed zinc oxide powder in two appointment method complete formation of dentin matrix were observed. 4) Among the methods and aagents described above formocresol mixed zinc oxide powder in two appointment method appeared to be relatively effective.

Evaluation of reparative dentin formation of ProRoot MTA, Biodentine and BioAggregate using micro-CT and immunohistochemistry

  • Objectives: The purpose of this study was to assess the ability of two new calcium silicate-based pulp-capping materials (Biodentine and BioAggregate) to induce healing in a rat pulp injury model and to compare them with mineral trioxide aggregate (MTA). Materials and Methods: Eighteen rats were anesthetized, cavities were prepared and the pulp was capped with either of ProRoot MTA, Biodentine, or BioAggregate. The specimens were scanned using a high-resolution micro-computed tomography (micro-CT) system and were prepared and evaluated histologically and immunohistochemically using dentin sialoprotein (DSP). Results: On micro-CT analysis, the ProRoot MTA and Biodentine groups showed significantly thicker hard tissue formation (p < 0.05). On H&E staining, ProRoot MTA showed complete dentin bridge formation with normal pulpal histology. In the Biodentine and BioAggregate groups, a thick, homogeneous hard tissue barrier was observed. The ProRoot MTA specimens showed strong immunopositive reaction for DSP. Conclusions: Our results suggest that calcium silicate-based pulp-capping materials induce favorable effects on reparative processes during vital pulp therapy and that both Biodentine and BioAggregate could be considered as alternatives to ProRoot MTA.

Antioxidant therapy enhances pulpal healing in bleached teeth

  • Objectives: The purpose of this study was to evaluate the histopathological effects of an antioxidant therapy on the pulp tissue of rat teeth exposed to a bleaching gel with 35% hydrogen peroxide. Materials and Methods: Forty rats were subjected to oral ingestion by gavage of distilled water (DW) or ascorbic acid (AA) 90 min before the bleaching therapy. For the bleaching treatment, the agent was applied twice for 5 min each to buccal surfaces of the first right mandibular molars. Then, the animals were sacrificed at 6 hr, 24 hr, 3 day, or 7 day post-bleaching, and the teeth were processed for microscopic evaluation of the pulp tissue. Results: At 6 hr, the pulp tissue showed moderate inflammatory reactions in all teeth of both groups. In the DW and AA groups, 100% and 80% of teeth exhibited pulp tissue with significant necrosis and intense tissue disorganization, respectively. At 24 hr, the AA-treated group demonstrated a greater regenerative capability than the DW group, with less intense inflammatory reaction and new odontoblast layer formation in 60% of the teeth. For up to the 7 day period, the areas of pulpal necrosis were replaced by viable connective tissue, and the dentin was underlined by differentiated odontoblast-like cells in most teeth of both groups. Conclusions: A slight reduction in initial pulpal damage during post-bleaching was promoted by AA therapy. However, the pulp tissue of AA-treated animals featured faster regenerative potential over time.

  • The most desirable healing process for endodontic therapy is apical closure by hard tissue such as dentine or cementum. Then, we estimated hard tissue conductivity of tetracalcium phosphate (4CP)/dicalcium phosphate (2CP) compound using mandibular first molars of SD rats. Residual pulp responses to the calcium phosphate compound were examined at several amputation levels of pulp. 2CP was purchased and passed through a $32-\mu\textrm{m}$ sieve. 4CP was obtained from a stoichiometric mixture of 2CP and calcium carbonate (Mol ratio: Ca/P=2.0) by the dry synthetic method at 1, 400(C for 8 hours.(omitted)

성견에 있어서 Cobalt-60 조사가 치수치유과정에 미치는 영향에 관한 연구 (AN EXPERIMENTAL STUDY ON THE EFFECTS OF DOG'S PULPAL HEALING AFTER COBALT-60 IRRADIATION)

  • Irradiation is frequently employed as the sole therapy for oral cancer. These irradiated patients presents peculiar and progressive dental problems. But there is only scanty informations concerning specific approaches to endodontic treatment for head and neck cancer patients who have been subjected to tumorcidal doses of radiation therapy. The purpose of the present study was to determine the effects of cobalt-60 radiation on the pulpal healing of dogs after the direct pulp capping. As the experimental animals, 10 dogs (above 7-8 months after birth) were divided into 3 groups (Control, Group I, Group II). The cobalt-60 was irradiated to the Group I and Group II each 1,009 and 1,562.5 rads as single dose. As the capping material Dycal$^{(R)}$(L.D. Caulk company) was selected. After the direct pulp capping the dogs were sacrified 1, 2, 3, 4, week interval and made the original slides cut with a thickness of 8 microns and stained with hematoxylin and eosin. After examination and comparision of all specimen, the results of this study were drawn as follows; 1. The formation of reparative dentin was observed from the 1st week in the Control group, the 2nd week in the Group I & II. The few and irregular tuble structure was appeared in the 4th week in the Control group only, but failed in the Group I & II. 2. The continuity of dentin bridge was appeared in the 3rd week in all group and the degeneration of odontoblast in the 1st week of the Group II. 3. The congestion and hemorrhage in the pulp tissue were observed in all groups until 3rd week. The inflammation was appeared within the 2nd week in the Group I and especially marked in the Group II, but absent in the Control group. 4. In cases Dycal into the pulp tissue deeply, the local necrosis of pulp and decrease of dentin formation was observed.

치아탈구 시 처치 (THE TREATMENT OF TOOTH AVULSION)

  • When the tooth avulsion occur in accidents the drying damage to the periodontal ligament has extremely detrimental effects on healing. Pulp necrosis always occurs after an avulsion injury, but revascularization can only take place in teeth with immature apexes. Therefore complications after avulsion injuries are common, and treatment must be carried out in a timely and correct fashion to prevent or limit these complications. Every effort should be made to replant the tooth within the first 15 to 20 minutes. If doubt exists that the tooth can be replanted adequately, the tooth should quickly be stored in an appropriate medium until the patient can get to the dental office for replantation. A complication of inflammatory root resorption is occurred by bacterial infection of periodontal ligament and dental pulp. Therefore aseptic endodontic treatment must be carried out in a timely and systemic antibiotics given at the time of replantation and before endodontic treatment are effective in preventing bacterial invasion. Further studies are needed to establish the clinical importance of preparation of the socket and root.
