Dental hygienists' perspective and coping measures towards medical market opening (의료시장 개방에 대한 치과위생사의 인식과 대응방안)

  • Objectives : This research seeks to present the data needed for the development of coping strategy, following medical market opening by identifying dental hygienists' perspective and coping measures towards the opening. Methods : One hundred eighty-eight dental hygienists were targeted to identify their level of perception towards medical market opening, attitude towards medical market opening, question of whether they agree or not with the opening and reasons, and coping measures and benefits of the medical market opening. t-test, chi-square test and cross-tabulation analysis were used for the analysis Results : First, team leaders are more aware of the medical market opening and hold greater sense of crisis towards opening compared to the rank and file. Second, the reasons cited for agreeing with the medical market opening included improvement of medical services' quality and diversification of services. As for the reasons for disagreeing, they cited the increase medical expenses paid by public. Third, limitation of the hospital management technique was cited the most when it comes to the scope of Korean hospitals' management crisis, followed by the limitations of the diagnosis procedure, limitations of the medical services, limitations of the medical techniques and increase in the number of large hospitals, in the order cited. Fourth, team leaders perceive greater need to seek coping measures from the aspect of realizing medical insurance fee from the policy development, service and system level aspects when it comes to the coping measures depending on their ranks. Conclusions : Therefore, Dental Hygienist has a comparatively low awareness of medical market opening, coping measures need to be explored to cope with the medical market opening by ensuring the dissemination of accurate knowledge through the education on the fees for dental hygienist and seminars in relation to the medical market opening.

Delayed bleeding after implant surgery in patients taking novel oral anticoagulants: a case report

  • The use of novel oral anticoagulants (NOACs) has increased in recent times in an effort to overcome the shortcomings of warfarin. They are being used primarily for the prevention of thrombosis caused by atrial fibrillation and offer the advantages of having fewer drug interactions than warfarin, no dietary restrictions, and no requirement for regular blood tests. Although there is reportedly less postoperative bleeding even if the drug is not discontinued during procedures that can cause local bleeding, such as dental procedures, no well-designed clinical studies have assessed postoperative bleeding associated with the use of these drugs. This article reports a case of a 74-year-old male patient who was taking rivaroxaban. The patient underwent a dental implant procedure after discontinuing rivaroxaban for one day and subsequently suffered delayed bleeding on postoperative day 6. Accordingly, this article also reports that the use of NOACs may also lead to delayed bleeding.

Awareness and satisfaction on tooth whitening (치아미백에 대한 인식 및 만족도 조사)

  • Objectives : The purpose of this study is to establish guideline for tooth whitening by investigating the awareness towards tooth whitening. Methods : The subjects were recruited in Seoul and Gyeonggi-do from June to August, 2011. A total of self-reported 395 questionnaire data were analyzed except for 5 incomplete answers. Results : 1. Smoking and coffee had no significant influence on the intention to whiten the tooth according to gender(p<0.001), age(p<0.001) and marital status(p<0.001). 2. Seventy persons (64.8%) tried to whiten tooth for color change. Half of the respondents (50.0%) bleached teeth by themselves. Of 295 persons, 66 persons (61.1%) were satisfied with self-whitening of teeth. Conclusions : Majority of the respondents wanted to whiten their teeth and they tried whiten by themselves because the whitening procedure was easy to do. It is necessary to establish the guideline for whitening teeth for safety.

Immediate loading on mandibular edentulous patient with SFI Bar$^{(R)}$ overdenture

  • Despite the greater retention and low maintenance of bar attachment system, the longer clinical time and increased number of visits were the assignments to overcome in bar attachment system. This case report describes SFI-Bar$^{(R)}$ (Cendres et Me$\'{e}$taux, Biel/Bienne, Switzerland) to be solve those problems. A 65-year-old female, who had severely absorbed mandible, hoped to wear a stable mandibular denture without pain. As soon as two implants were placed on mandible, a tube bar was connected to two adaptors connected to each implant. The length of the tube bar was adjusted considering inter implant distance, and reconnected to the adaptors. Finally a female part was seated beneath the denture. This case report showed that a satisfactory clinical result was achieved by delivering bar overdenture immediately after implant placement without laboratory procedure.

THE LITERATURE REVIEW ON THE SINUS BONE GRAFT USING DEPROTEINIZED BOVINE BONE MINERAL WITH LATERAL APPROACH (탈단백 우골 (Deproteinized Bovine Bone Mineral)을 이용한 상악동 골이식술: 측면 접근법의 문헌 고찰)

  • As the uses of dental implants are prevailing, the need for sinus bone graft is increasing. Deproteinized bovine bone mineral (DBBM) was not mentioned in 1996 Sinus Bone Graft because of the deficit of the available data. Since then, many clinical and laboratory reports support the use of DBBM in the sinus bone graft procedure. In this report, the histological and clinical successes of sinus bone grafting with DBBM is discussed with available literatures. After sinus bone grafts with DBBM, the proportion of new bone formed was similar or superior to natural maxillary posterior alveolar bone after healing period of 6 months to 1 year. It seems that the grafted DBBM is not be either resorbed nor replaced with bone, but this may not disturb the osseointegration of dental implants installed into it. The clinical survival rates of dental implants installed on the sinus grafted with DBBM was similar to those installed on the ungrafted posterior maxillary alveolar ridge or grafted with autogenous bone. So, it can be concluded that DBBM can be used successfully in the sinus bone graft.

Pain Control after Dental Surgery: Myprodol Versus Ibuprofen Versus Codeine (치과 수술후의 통증 관리: Myprodol 대 Ibuprofen 대 Codeine)

  • Background: Myprodol, a newly introduced combination analgesics with codeine, ibuprofen and paracetamol was evaluated in the dental surgery patients for its efficacy and side effects. Methods: Total 60 ASA I or II outpatients dental surgical patients were randomly assigned into three groups(n=20 each). After various oral procedures, one of three oral analgesics, myprodol, ibuprofen or codeine, was prescribed to each patients in double blind fashion for three days. Each patient was followed carefully by daily phone calls for verbal analog pain scale, side effects and patient's satisfaction level. Results: Demographic data and duration of surgery revealed no statistical differences in all three groups. Myprodol group showed better verbal analgesic scores consistently for the study period than ibuprofen or codeine group. Adverse effects were minimal. Patients' satisfaction level was high in all three groups. Conclusion: We conclude that myprodol is an excellent oral analgesic for day-surgery based dental procedure patients. This effect appears to be synergistic among different analgesics rather than additive.

  • Ankylosis of joint is defined as limited movement due to infection, trauma, or surgical procedure. A 59-year-old female with a chief complaint of limited movements during mouth opening had a positive history of trauma to her right TMJ area about 5 years ago. From that time, progressive mouth opening limitation and intermittent pain have occurred. At the time of admission the patient showed mandibular deviation to the right side during mouth opening, with a maximum opening limited to 5 mm. On plain radiographs, right condylar enlargement and joint space reduction by newly formed bony tissues were observed. CT scans showed right condylar enlargement, cortical sclerosis, and thickening of the condyle, articular fossa and articular eminence.

A Study on Hybrid material of Making Dental restorations by CAD/CAM System (치과 CAD/CAM용 복합소재를 이용한 치과보철물의 제작에 대한 연구)

  • In recent years, perhaps the biggest driver in new material development is the desire to improve crown and bridge esthetics compared to the traditional PFM or all-metal restorations. As such, zirconia, leucite-containing glass ceramic and lithium disilicate glass ceramic have become prominent in the dental practice. Each material type performs differently regarding strength, toughness, ease of machining and the final preparation of the material prior to placement. For example, glass ceramic are typically weaker materials which limits its use to single-unit restorations. On the other hand, zirconia has a high fracture toughness which enables multi-unit restorations. This material requires a long sintering procedure which excludes its use for fast chair side production. Developed hybrid material of CAD/CAM is contained nano ceramic elements. This new material, called a Resin Nano Ceramic is unique in durability and function. The material is not a resin or composite. It is also not a pure ceramic. The material is a mixture of both and consists of ceramic. Like a composite, the material is not brittle and is fracture resistant. Like a glass ceramic, the material has excellent polish retention for lasting esthetics. The material is easily machined chair side or in a dental lab, polishes quickly to an esthetic finish and if necessary, can be useful restoratives.

  • This research aims to improve oral health knowledge through oral health education by investigating and analyzing the effect of subjective oral health knowledge and oral health behavior on personal oral hygiene management ability targeted for adults who visited a dental hygiene laboratory at B University in Busan for comprehensive dental hygiene management and procedure from September 23, 2013 to December 12, 2013, and provide basic data to improve adults' personal oral hygiene management ability by inducing behavior on oral health. Results derived from the research are as follows. Oral health education is a prerequisite to improve personal oral hygiene management ability through improvement in oral health knowledge and oral health behavior, which leads to improved personal oral health and furthermore promotion of national oral health through not just simply transmitting oral health knowledge, but desirable change in oral health behavior based on oral health knowledge.

Endodontic management of mandibular first premolar with C-shaped canals by using cone-beam computed tomography and dental operating microscope (CBCT와 치과용 현미경을 이용한 C형 근관을 가지는 하악 제1소구치의 근관치료)

  • Aberrant anatomy of mandibular premolars is very rare in Korean, but aberration can contribute the endodontic failure as it makes difficult to remove the irritants during cleaning and shaping procedure. This case report describes the successful root canal treatment of a rare mandibular first premolar with C-shaped root canal as using a cone-beam computed tomography to understand the internal shape of root canal system and a dental operating microscope to improve the magnification and illumination.
