의료시장의 개방과 더불어 의료기관의 경쟁력확보를 위한 핵심전략으로 환자관리의 중요성이 강조되고 있다. 이 연구는 치과위생사의 업무능력향상과 전문성을 높이기 위해 수행업무인식과 교육요구주제를 조사 분석하여 Dental Hygienist Coordination System을 개발하는데 기초자료로 활용하고자 수행하였다. 비례표본추출법을 이용하여 광주광역시 85개 치과의원에 근무하는 치과위생사 156명을 대상으로 직접 방문하여 설문법을 이용하여 조사 분석한 바, 다음과 같은 결론을 얻었다. 1. 코디네이터교육경험 유무에 따른 교육과정별 우선순위 요구과목으로 서비스분야에서는 코디네이터이론과 실무(유무:상담 및 교육실무), 코디네이터를 위한 기초(유무:의학용어), 코디네이터를 위한 원무관리(유무:건강보험), 시설 및 환경관리(유:환경관리, 무:감염관리)이고, 매니저분야에서는 의료서비스마케팅(유무:의료환경과 고객특성, 의료서비스마케팅), 조직인사관리(유무:병원조직), 회계 및 재무관리(유무:병원회계), 건강보험실무(유:진료비청구관리, 무:진료비산정)으로 조사되었다(p>0.05). 2. 코디네이터교육경험 유무에 따른 교육요구과목 차이를 분석한 결과, 코디네이터이론과 실무 0.14, 건강보험실무와 의료서비스마케팅 0.13이고, 인사관리 - 0.23, 코디네이터를 위한 의료기초 -0.19로 조사되었다(p>0.05). 3. 코디네이터교육경험 유무에 따른 교육요구주제는 경험자군과 비경험자군 모두 상담 및 교육실무로 나타났으며, 교육경험자군과 비교육경험자군의 차이를 보면 고객접점관리 0.25, 고객응대기법 0.24로 조사되었다(p>0.05). 이상과 같은 결론에서 우선순위가 높은 교육과목인 코디네이터 이론과 실무, 건강보험 실무에서 진료비 청구관리를 중심으로 체계적인 교육과정을 포함한 Dental Hygienist Coordination System(D.H.C.S)개발에 필요한 근거자료로 활용될 수 있을 것으로 사료된다.
치과위생사의 감염관리 실천도를 측정하는 척도를 개발하고 타당화하기 위해 254명의 치과위생사를 대상으로 설문조사를 실시하였다. 예비척도는 리커트 4점 척도로서 21개의 문항으로 구성되었으며, 신뢰도 분석, 탐색요인분석, 확인요인분석을 통해 척도의 차원과 항목을 축소하고 신뢰도와 타당도를 검증하였다. 1. 탐색요인분석과 신뢰도 분석을 통해 4개 요인과 12개의 항목으로 척도를 수정하였다. 수정된 척도의 요인은 '예방접종 및 정기검진'(2항목), '기구 소독 및 멸균'(3항목), '손위생 관리'(4항목), '개인 보호'(3항목)가 추출되었으며, 전체분산 중 58.296%를 설명하고 있다. 예비척도 중 '환경 관리' 요인이 제외되었다. 2. 확인요인분석을 통해 한 문항을 제외하였다. 최종적인 측정모형은 4개 요인과 11개 항목으로 구성되었으며, 모형의 적합도는 $x^2$= 79.593(df = 38, 0 = 0.000), RMR= 0.045, GFI = 0.940, CFI = 0.904, AGFI = 0.896, NFI = 0.837, TLI = 0.861, RMSEA = 0.67로 나타나 대체로 기준을 충족하였다. 모든 측정변수의 요인부하량은 유의하였으나(p < 0.001), 세 변수만이 0.7 이상으로 나타났다. 평균분산추출값과 구성개념신뢰도는 대체로 기준을 충족하지 못하였으나, 모든 요인의 평균분산추출값은 각 요인간 상관관계제곱값 보다 크게 나타났다. 3. 요인분석을 통해 추출한 네 개의 요인이 치과위생사의 '감염관리 실천률 인식'에 미치는 영향을 알아보기 위해 네 요인을 독립변수로 선정하고 실천률 인식을 종속변수로 선정하여 회귀분석을 실시하였다. $R^2$는 0.304, 수정된 $R^2$은 0.431, F = 25.813, p = 0.000이며, '손위생 관리'를 제외한 세 변수의 회귀계수는 통계적으로 유의하게 나타났다.
This case reports describe a new approach to the use of polymers in orthodontics, using a fiber reinforced composite(FRC). FRC was successfully used in a periodontal splints, fiber post for endodontic use, orthodontic retainer and space maintainers, implant prosthesis, large span bridge, management of cracked tooth, anchorage reinforcement in orthodontics. FRC has highly favorable mechanical properties, and its strength-to-weight ratios is superior to those of most alloys. FRC has potential for use in many applications in dentistry and is expected to gain increasing application and popularity in dentistry. These case reports show that FRC is a promising anchorage reinforcement material for use in orthodontic practice.
Objectives: The purpose of this study is to investigate awareness and performance of infection control by dental hygienist. Methods: A self-reported questionnaire was answered by 239 dental hygienists working in dental clinics from September 7 to 18, 2016 in Gwangju city & Jeonnam region after explaining the purpose of the study and receiving the informed consent. The questionnaire consisted of sociodemographic characteristics and awareness and performance of infection control using Likert 4 point scale. Results: Average of Infection control awareness was $3.33{\pm}0.38$, average of infection control Performance was $3.04{\pm}0.37$ (p<0.05). The awareness and practice of dental equipment management showed the largest difference. The awareness of infection control by general characteristics varied according to position, number of dental units, number of employees, Infection control needs, Received infection control training experience, infection experience, infection control guideline (p<0.05). The relationship between infection control awareness and Infection control performance was r = 0.624 (p<0.001), indicating positive correlation. Conclusions: To improve awareness and performance of dental hygienist's infection control, regular received infection control training experience is needed. It is considered that the dentist's active cooperation is needed in shortening the life span of the dental equipment due to the Infection control performance and securing the quantity.
Objectives: The purpose of the study is to investigate the subjective oral status perception and OHIP according to the oral health managementamong international and domestic university students. Methods: self-reported questionnaire was completed by 176 domestic and 175 international university students in Daegu and Gyeongbuk from August 25 to November 1, 2014. The informed consent was received after explanation of the purpose of the study by the researcher. The questionnaire consisted of general characteristics of the subjects, toothbrushing behavior, oral health management, subjective oral status perception. and oral health impact profile (OHIP). The subjective oral status perception and OHIP was measured by Likert 5 point scale. Results: The international students tended to have higher subjective oral status perception when they had longer stay with the roommate in Korea. The female international students tended to have higher OHIP than the male students. The domestic students with lower grades and roommates showed higher OHIP. The international students had a poorer practice of oral health management than domestic students. The overall mean of subjective oral status perception was 3.13 in domestic students, and 3.09 and 3.22 in international students. The overall mean of OHIP was 4.21 in domestic students and 4.25 and 4.16 in the international students, Conclusions: International students had a higher subjective oral status perception than domestic students, but their wrong oral health management lowered their quality of life. It is necessary to provide the oral health management for the international students continuously.
Background: Recently the use of sedation by pediatric dentists in Korea is increased. This study describes training programs about sedation practices in Korean pediatric dentistry residency program. Methods: A questionnaire was filled in by participants of Korean Academy of Pediatric Dentistry on 17th-18th August, 2008. Also the data about sedation practices of the training institution is collected by phone call. Results: Seventy two percent of respondents used sedation. Most of them used sedation with agents under 25% of their patients. Distribution of ages in patients sedated with agents was 3 years, 4-5 years, under 2 years, 6-10 years, and more than 10 years. Determinative factors of using sedation were behavior management, number of visiting, amount of treatment and general condition, and oral route was the most favorable route. Sixty six percent of them have failed on sedation, and thity percent of them have rarely failed on sedation. Only fifty percent of dentists using sedation completed the cardiopulmonary resuscitation course. Conclusions: For safety, dentists using sedation need to be educated about emergency equipment and management. Especially medication dose, use frequency and the detail related to treatment procedure should be discussed carefully. Also putting a patient under general anesthesia and taking emergency measures should be discussed with Korean Dental Society of Anesthesiology.
Objectives : The aim is to examine what a selective factor of dental institution in the elderly is, which can be used as a strategy for vitalizing hospital management, and which difference there is in selection of dental institution according to health-promotion lifestyle. Methods : Questionnaire survey of individual interview was carried out targeting 177 old people in over 65 years old of Jeonju, Iksan, and Gunsan in Jeollabuk-do Province by convenience sampling from August 16, 2010 to August 27. The analysis was performed by using SPSS WIN 12.0 program. Results : 1. The average mark of health-promotion lifestyle in the elderly was 2.69 points. The health responsibility was the highest with 2.91 points. 2. The mark of health-promotion lifestyle was higher in the higher educational level of the elderly(p<0.01), when monthly income is more than 2 million won(p<0.01), and in the better subjective awareness of oral condition(p<0.01). 3. The group with high mark of health-promotion lifestyle regarded reliability(p<0.001), economic efficiency(p<0.01), and facilities & cleanliness(p<0.01) as important factors when selecting dental institution. Conclusions : The development and expansion in health-promotion program targeting the elderly are seen to be necessary that reflect difference by sphere in health-promotion lifestyle. There will be necessity for pursuing even a strategy for the elderly in the group who have relatively low characteristic in the practice level of health-promotion lifestyle. Also, given understanding characteristics on the elderly and focusing on management proper for the needs of medical consumers, the successful medical management is thought to be likely possible.
Stevens-Johnson syndrome (SJS) is a severe adverse cutaneous drug reaction seen rarely in clinical practice. Although relatively rare, the condition can be fatal. Mainly, it is caused by side effects of certain medications. Previous reports have associated Stevens-Johnson syndrome with abnormal root development, but the other long-term dental complications have rarely been reported. In this case, the patient developed SJS at the age of 5, and abnormal root development of the maxillary and mandibular first molars and mandibular incisors was observed, as well as impaction of the mandibular canine and enamel hypomineralization of multiple teeth. Accordingly, appropriate restorative treatment and orthodontic treatment were performed, and the clinical characteristics of this symptoms and its treatment were discussed in more detail. We aim to highlight the need for dentists to be aware of the potential dental complications of SJS and to enable early diagnosis and management of the condition to avoid undesirable sequelae.
Background: The management of the behavior of handicapped children when providing required dental care is often a problem, whether in the dental office or in a hospital setting. Because of the high incidence of poor cooperation, many of these patients are scheduled for dental care under general anesthesia with preoperative medical assessment. The purpose of this study was to carry out a clinico-statistical survey on dental treatment for handicapped children under general anesthesia. Methods: After approval from the institutional review board, the medical records of 64 handicapped children between 1997 and 2002 were reviewed to determine the patient profiles, anesthesia management, and complications. The charts of these patients, who underwent dental examination, scaling and prophylaxis, and restoration and extraction of teeth under general anesthesia, were reviewed. Results: The mean age was 12.8 years old, and males (53%) predominated females (47%). Twenty-four patients had mental retardation, twelve had autism, six had cerebral palsy, 4 had behavior disorder, others had heart disease, convulsive disorder, etc. Sixty-two had intravenous thiopental with neuromuscular blocker, 2 had intravenous ketamine induction. Nasotracheal intubation was uneventful in 55 patients, nine had orotracheal intubation because of difficult visualization of the larynx. Twenty-one patients experienced postoperative complications in the recovery room, including epistaxis, nasal obstruction, vomiting, airway obstruction, respiratory depression. Conclusions: General anesthesia is a very effective way of completing the dental treatments for disabled children. We emphasize the need to train anesthesiologists in the care of disabled patients.
This study aimed to investigate and compare the perception of personal protective equipments and instrument sterilization of students who are in attendance at colleges and universities and which have bedside training as a regular subject in their curriculum for third-year students. 'Completely True' is the highest response to the question about the importance of wearing gloves, mask and medical gown from all groups. 'Completely True' is the highest response to the question about the necessity for replacement of gloves from all groups. 'Moderate True' is the highest response to the question about the necessity for replacement of mask from the group without practical training and 'Completely True' is the highest response to the question about the importance of hair cleanliness from all groups regardless of practical training experience. For the question about the perception of instrument disinfection and sterilization, the group with inside and outside practical training showed the highest perception level. Consequently, the right education for prevention of infection by setting inclusive guidelines to enhance the level of perception and practice seems continuously necessary.
