코로나바이러스감염증-19 상황에서 임상실습 중 치위생(학)과 학생의 감염관리 인식과 치과의료기관의 감염관리 수행정도 (Dental infection control in clinical practice institutions experienced by dental hygiene students in the COVID-19 situation)

  • The aim of this study was to investigate the level of dental infection control experienced by dental hygiene students in clinical practice institutions to identify problems and improve infection control in dental institutions. This study conducted online surveys targeting 269 dental hygiene students from universities that conducted clinical practice to determine the students' level of awareness regarding dental infection control in dental institutions and the reality of infection control in dental institutions. The results showed that dental hygiene students recognized the need for infection control and education about infection control at a high level. However, only 47% of the students were accurately informed about COVID-19. Basic instruments, periodontal instruments, and implant surgical instruments were sterilized after use for each patient, mostly by the institution, but 3-way syringe tips, preservation instruments and prosthetic instruments were more frequently reused without sterilization immediately after use. For dental infection control to be practiced at dental institutions, it is necessary to establish a systematic and safe infection control system, including infection control education, designation of infection managers, and provision of infection control guidelines.

일부 직장근로자들의 건강증진생활양식에 따른 치과의료기관 선택 요인 (Health promotion lifestyle to the selection factors of dental institutions among some office workers)

  • Objectives : The purpose of this study was to examine the influential factors for the choice of dental institutions among adult office workers who were one of major medical consumer groups, the relationship between their health promotion lifestyle and their choice of dental institutions, ways of boosting the efficiency of hospital management and differences in differentiation strategies among dental institutions. Methods : The subjects in this study were 160 office workers who were aged 20 and over and worked in Seoul, North Jeolla Province and South Jeolla Province. They were selected by convenience sampling, and a survey was conducted from February 1 to July 20, 2011. The answer sheets from 149 respondents were analyzed by the statistical package SPSS WIN 12.0 except for 11 incomplete ones. Results : As for the selection factors of dental institutions by general characteristics, they attached a statistically and significantly different importance to convenience according to gender(p<0.05), to reliability, facilities and cleanness according to age(p<0.05), to reliability and convenience according to academic credential(p<0.05) and to facilities and cleanness according to academic credential(p<0.01). Concerning the links between the form of using dental institutions and the selection factors of dental institutions, they gave a statistically and significantly different weight to reliability according to that(p<0.05). Convenience(p<0.001) was the primary selection factor of dental institutions among those who scored higher in health promotion lifestyle. Conclusions : Given the above-mentioned findings, it seemed that the regular implementation of oral health plans geared toward workers and the development of educational programs are required to promote the oral health of workers. But this study examined the workers in several regions only, and the findings might not be generalizable.

청주시 소재 치과의료기관의 장애인 접근성 확보를 위한 편의시설 실태조사 (A Study on the State of Convenience Facilities for the Accessibility of a Disabled Person in Dental Institutions in the City of Cheongju)

  • 본 연구는 치과의료기관을 대상으로 장애인 편의시설의 설치 실태를 파악하여 향후 장애인의 치과의료기관 이용 편의성 개선을 위한 기초자료로 삼고자 2011년 3월부터 4월까지 충청북도 청주시에 개설신고 된 치과의료기관을 대상으로 조사를 실시하였다. 장애인 편의시설 기준항목을 선정하여 항목별 시설물의 설치 실태를 조사한 결과 다음과 같은 결론을 얻었다. 조사대상 의료기관은 치과의원이 대부분을 차지하였고 의료기관 개설 시기는 1990년 이전부터 2011년까지 전체적으로 폭넓게 분포하고 있었으며 건물 내 치과진료실의 위치는 대부분 2-3층에 위치하는 것으로 조사되었다. 치과의원의 장애인 편의시설은 주출입구 접근로 및 승강기의 설치율이 가장 높았고 장애인 주차구역과 장애인용 화장실의 경우 편의시설 중 설치율은 매우 낮게 나타났으나 병원 및 보건소는 조사대상 편의시설이 모두 설치되어 있는 것으로 조사되었다. 또한, 2006년 이후에 개설된 의료기관이 1990년 이전에 개설한 의료기관과 비교하여 편의시설 항목 전체에서 높은 설치율을 보였고, 1-2층에 위치한 경우보다 3층 이상에 위치한 치과의원의 설치율이 모두 높게 나타났다. 장애인 편의시설 설치율은 병원 및 보건소가 높았고 치과의원의 경우 대체로 낮게 조사되어 향후 편의시설의 의무사항을 일차 의료기관까지 확대하는 법적 개선이 필요한 것으로 나타났다. 이러한 결과는 장애인들의 치과의료기관 접근 편의성 확보를 위한 기초자료로 활용하는 데 도움이 될 것으로 사료된다.

  • The Government is going to enforce assessment of dental institutions in addition to assessment of medical institutions so that it can provide good-quality medical service to people having much interest in quality of medical service. But the empirical research on the assessment of dental institutions which is still in a model assessment stage, is insufficient. Accordingly, the present research aims to help dental institutions preparing the main assessment by researching an acceptance level of workers of dental institutions in a process performing assessment of dental institutions, based on the preceding researches that the perceived utility has influence on implementation intention. The present research proved the influence that job relevance, result demonstration, usability and education & training of workers and manager's leadership of an infection management part affects perceived utility and implementation intention. As a research result, all the job relevance, result demonstration, usability have a positive(+) influence on perceived utility and implementation intention, and the influence of job relevance most affects especially. The leadership and education & training have influence on what workers get to have implementation intention after perception of utility of infection management according to the order. According to the above results, it can be understood that medical institutions preparing for assessment of dental institutions take charge of the assessment part having high relevance with work of workers and need to make a manager ordering performance of its assessment be able to foster a leadership for improving effectiveness of assessment performance.

대도시 지역 일부 어머니들의 치과의료 이용행태에 영향을 미치는 관련요인 분석 (Factors Affecting Mother's Dental Utilization in Metropolitan Area)

  • The present study attempted to analyze the influence of the mother's demographic characteristics and knowledge on oral health as well as dental care behaviors on her practice of using dental care institutions. A self-report survey was conducted among 172 mothers of children aged 5-6 attending on three preschools located in Daegu metropolitan city. Statistical analysis was conducted using the SAS 8.01 with $X^2$-test, t-test and logistic regression. The obtained results were as follows: 1. No significant association was found among the demographic characteristics of the mother, her past experience as well as the purpose of using dental care institutions. 2. There was significant relationship between the mother's knowledge and behavior concerning oral health and her past experience of using dental care institutions; the mothers having a higher level of knowledge and who thought they themselves didn't have healthy teeth and gum tended to have more visits to dental care institutions(pE0.05). There was also significant relationship between their purpose and their actual practice of using dental care institutions: the mothers having a higher level of knowledge and who thought their own oral health was good tended to visit dental institutions more for the preventive purpose than for treatments(pE0.05). 3. As a result of logistic regression, the variables formed as models were their knowledge on oral health and their values for oral health. The standardized coefficients for the two variables were -0.19 and 0.19, respectively. For the mother's purpose of visiting dental care institutions, the variables formed as models were their knowledge on oral health and their values for oral health. The standardized coefficients for the two variables were -0.40 and -0.37, respectively. The relative contributions of the two variables to the mother's practice of using dental care institutions were on the similar level.

서울시내 일부 근로자들의 치과 선택요인에 관한 연구 (A study on the factors related to the choice of dental clinics for worker in Seoul)

  • Objectives : The purpose of this study was to examine the awareness of company employees. The importance of the selection factors of dental clinics by demographic characteristics. Methods : The subjects of this study was 218 employees. In this study, structured were used. Results : As for perception of the importance of traffic convenience by general characteristics, the company employees who were male, who were married, who had children, who continued to work for a longer period and who were better paid placed more significance on traffic convenience among the selection factors of dental institutions, and the gaps between them and their counterparts were significant. Regarding perception of the importance of the physical environments of dental institutions by general characteristics, those who were in their 20s and in their 50s and up, who were less educated, who worked for a smaller period and who were less paid gave more weight on the importance of physical environments, and the gaps between them and their counterparts were significant. As to perception of the importance of the image of dental institutions by general characteristics, monthly pay made a significant difference to that. Concerning the external qualifications of dental personnels, they took a significantly different view according to their age and monthly pay. In relation to perception of the importance of dental service by general characteristics, the women attached more importance to dental service than the men, and the intergroup gap was significant. In regard to perception of the importance of the basic components of dental institutions by general characteristics, those who were in their 40s and up, who had children, who were better paid and who were assistant directors were significantly different from their counterparts in their opinions on the importance of the basic components of dental institutions. Conclusions : The findings of the study suggest that dental institutions should make efforts to provide patients with the best medical service to gain their confidence.

장애인 입소시설과 특수학교의 구강건강관리 실태에 대한 조사 연구 (ORAL HEALTH CARE STATUS IN INSTITUTIONS AND SPECIAL SCHOOLS FOR THE DISABLED PEOPLE IN KOREA)

  • 전국의 123개 장애인 입소시설과 102개 특수학교를 대상으로 구강건강관리 실태를 파악하여 장애인의 구강건강관리 계획을 수립하는 데 사용할 수 있는 근거를 마련하고자 하였다. 설문지 조사법으로 연구하였다. 정기구강검진은 특수학교에서 연 2회 실시하고 있었으나, 장애인 입소시설에서는 56.8%에서 시행하였다. 우식 예방 프로그램은 장애인 입소시설의 69.1%, 특수학교의 86.3%에서 시행하였으며 프로그램의 종류는 입소시설에서 규칙적 칫솔질이 58.1%, 특수학교에서 예방교육이 65.7%로 가장 많았다. 불소를 이용한 우식예방은 입소시설과 특수학교에서 각각 8.6%, 11.8%에서 시행하고 있었으며, 열구전색은 각각 6.8%, 6.9%에서 시행하였다. 치과치료를 시행하고 있는 입소시설과 특수학교에서는 각각 84.2%, 39.2%이었고, 입소시설에서 치료를 받기 위해 이용하는 곳은 치과의원(60.2%), 보건소(16.8%), 자원봉사자(15.3%), 대학병원 (1.0%) 등이었다. 치료를 위해 치과의원에 방문하여 치료를 거부당한 경험이 있는 응답자는 17.7%이었다.

치과진료기관의 고객 불만처리에 관한 실태분석 (Analysis of Grievance Handling for Medical Consumers: the Case of Dental-Care Institutions)

  • 본 연구는 치과진료기관의 고객 불만처리 실태를 파악하고, 불만처리 서비스 및 불만 예방서비스 시행 정도를 조사하기 위하여 2007년 1월 20일부터 2월 20일까지 치과진료기관을 대상으로 총210부의 설문지를 수거하였으나 이 중 4부는 응답자료에 결측치가 있어 분석에서 제외시키고 총 206(98%)부의 설문지를 SPSS WIN 12.0 프로그램을 이용하여 분석한 결과 다음과 같은 결과를 얻었다. 1. 치과진료기관에서 불만처리서비스에 대해 32.5%가 인지하고 있는 것으로 조사되었고, 64.6%는 불 만처리서비스를 시행 하지 않는 것으로 조사되었으며, 94.7%가 불만을 경험한 것으로 나타났다. 2. 의료소비자의 불만 내용으로는 '진료 및 대기시간이 길다'가 40.3%로 가장 높게 나타났으며 '의료보험 미적용'이 30.6%, '진료에 대한 설명부족' 6.3%, '진료비 과다청구' 5.8% 순으로 나타났으며, 월 불만건수는 1~10건이 91.3%로 가장 높은 것으로 조사되었다. 불만표현방법에서는 '직원에게 직접 말한다'가 88.2%로 가장 높게 나타났으며, 주된 불만처리 담당자는 '치과위생사'가 56.8%로 높게 나타났다. 치과진료기관의 불만대처방법으로는 '즉각적인 조치'가 34.5%로 가장 높게 나타났으며, '환자의 성향을 분석 후 불만을 처리' 30.0%, '불만에 대해 변명으로 대처' 11.1%, '불만처리센타를 설치해 적극적으로 대처' 7.0% 순으로 나타났다. 3. 불만처리서비스 시행 정도는 총평균 3.02로 나타났으며, 불만처리 담당자가 '치과위생사'일 경우 접수담당자 보다 의료소비자 불만처리서비스 시행 정도가 높은 것으로 나타나 통계적으로 유의한 차이를 보였다(p < .01). 월 불만건수가 '없다'고 응답한 경우가 '1~10건'과 '11건 이상'의 경우보다 불만처리 시행 정도가 높은 것으로 나타나 그룹 간에 통계적으로 유의한 차이를 보였다(p < .05). 4. 불만예방서비스 시행 정도는 총 평균 2.59로 나타났으며, 서울, 인천 경기의 경우 지방보다 불만예방서비스 시행정도가 높은 것으로 나타나 통계적으로 유의한 차이를 보였으며(p < .01), 치과위생사 가 불만처리를 담당했을 경우 접수담당자 담당한 경우 보다 불만예방서비스 시행 정도가 높은 것으로 나타나 통계적으로 유의한 차이를 보였다(p < .05).

Analysis of the annual changes in dental institutions that claimed dental sedatives in Korea and the types of sedatives using health care big data

  • Background: Dentists make various efforts to reduce patients' anxiety and fear associated with dental treatment. Dental sedation is an advanced method that dentists can perform to reduce patients' anxiety and fear and provide effective dental treatment. However, dental sedation is different from general dental treatment and requires separate learning, and if done incorrectly, can lead to serious complications. Therefore, sedation is performed by a limited number of dentists who have received specific training. This study aimed to investigate the proportion of dentists who practice sedation and the main sedatives they use in the context of the Republic of Korea. Methods: We used the customized health information data provided by the Korean National Health Insurance. We investigated the number of dental hospitals or clinics that claimed insurance for eight main sedatives commonly used in dental sedation from January, 2007 to September, 2019 at the Health Insurance Review and Assessment Service. We also identified the changes in the number of dental medical institutions by region and year and analyzed the number and proportion of dental medical institutions prescribing each sedative. Results: In 2007, 302 dental hospitals prescribed sedatives, and the number increased to 613 in 2019. In 2007, approximately 2.18% of the total 13,796 dental institutions prescribed sedatives, increasing to 3.31% in 2019. In 2007, 168 institutions (55.6%) prescribed N2O alone, and in 2019, 510 institutions (83.1%) made claims for it. In 2007, 76 (25.1%) hospitals made claims for chloral hydrate, but the number gradually decreased, with only 29 hospitals (4.7%) prescribing it in 2019. Hospitals that prescribed a combination of N2O, chloral hydrate, and hydroxyzine increased from 27 (8.9%) in 2007 to 51 (9%) in 2017 but decreased to 38 (6.1%) in 2019. The use of a combination of N2O and midazolam increased from 20 hospitals (6.6%) in 2007 to 51 hospitals (8.3%) in 2019. Conclusion: While there is a critical limitation to the investigation of dental hospitals performing sedation using insurance claims data, namely exclusion of dental clinics providing non-insured treatments, we found that in 2019, approximately 3.31% of the dental clinics were practicing sedation and that N2O was the most commonly prescribed sedative.

국내 거주 중국인 유학생의 치과의료 이용 실태조사 (A Study on the Use of Dental Institutions among Chinese Students in Korea)

  • 본 연구는 중국인 유학생을 대상으로 치과의료 이용 실태에 대해 조사함으로써 외국 유학생의 치과의료 이용에 대한 문제점을 개선하고 치과의료서비스 확충을 위한 기초자료를 제공하고자, 2011년 4월 1일부터 7월 15일까지 국내 8개 대학에 재학 중인 중국인 유학생을 대상으로 조사를 실시하여 다음과 같은 결과를 얻었다. 1. 국내 치과의료기관 이용 경험에 관한 질문에서 응답자의 14.4%가 국내에서 치과치료를 받은 경험이 있었으며, 이용한 치과의료기관 분류로는 치과의원이 52.5%로 가장 많았고, 치료받은 내용으로는 충치(충전)치료가 52.8%로 가장 많은 것으로 나타났다. 2. 국내 체류 중 치과치료를 받지 못한 경험에 대한 이유는 '비용이 부담스러워서'가 38.7%로 가장 많았으며, '시간이 없어서'라는 응답도 29.8%로 조사되었다. 3. 건강보험에 가입되어 있는 경우가 57.3%로 가입되어 있지 않은 경우보다 약간 많았으며, 건강보험의 필요성을 묻는 질문에서는 대부분 필요하다고 응답하였다. 또한, 건강보험에 가입한 경우 가입하지 않은 경우 보다 치과치료를 더 많이 받은 것으로 조사되었다(p<.001). 4. 치과의료 이용 만족도에서는 한국어 실력이 좋을수록 치료 만족도가 높은 것으로 조사되었으며(p<.01), 건강보험에 가입되어 있는 경우 가입하지 않은 경우보다 치료 만족도가 높았다(p<.05). 이러한 결과는 중국인 유학생의 효율적인 치과의료 이용을 위해서 유학생의 건강보험 가입에 관한 제도를 개선하고, 보건의료 관련 행정 지원이 필요할 것으로 사료된다.
