Purpose: This study was conducted to investigate alloys spectrophotometrically including yttrium of nickel-chromium (Ni-Cr) alloys, which are used as substitutes for the regulation of beryllium and provide helpful improvements in Ni-Cr alloys. Methods: Four groups of specimens (ZN, ZY, SN and EM) were prepared for analysis. Color parameters were measured with a spectrophotometer, and color difference (∆E*) was calculated. The t-test and one-way analysis of variance test were used to determine significant difference, and the Tukey test was used to identify where the differences were. To measure the spectroscopic reflectivity, the spectroscopic reflectance was measured and converted into CIE L*, a*, b* color system. Results: The ∆E* value of each metal ceramic group after opaque firing of Ni-Cr alloy with and without yttrium was <2, and the total group color difference (∆E*) was below 1 in the dentin ceramic all experimental group. However, the a* and b* values of the metal ceramic groups were higher than that in the lithium disilicate all ceramic group, and the chroma was higher than the natural tooth. The brightness of all experimental groups was similar to that of the shade guide sample. Conclusion: Yttrium added to Ni-Cr alloys showed similar CIE L*, a*, b* values to Ni-Cr alloys that did not contain yttrium, indicating that yttrium had no effect on color in metallic ceramic systems.
Porcelain is the first ceramic material to be introduced into dentistry. Porcelain jacket crown was introduced by Dr. Charles H Land in 1886, which was an excellent aesthetic dental restoration but has not been widely used due to high firing shrinkage and low tensile strength. Then metal-ceramic system, which combines the esthetic properties of ceramics and the mechanical properties of metals, was introduced and nowadays it is still used in dental clinical field. However, the metal-ceramic system has shown some problems, such as increased lightness by reflection of light at opaque layer, shadow beneath the gingival line due to the block-out of light by metal coping, exposure of metal in margin part, bond failure between metal and porcelain, oxidation of metal coping during firing the porcelain, etc. Recently, along with the advance of fabrication methods of dental ceramics, the all-ceramic restorations with high esthetic and mechanical properties has increased and gradually replaced metal-ceramic restorations. Especially, CAD/CAM technology has opened a new era in fabricating the dental ceramic restorations. This overview will take a look at the past, present and future possibility of the dental ceramic materials.
The requirements for the successful treatment of all-ceramic restorations are not so different from the ones of conventional restorations. "The provisional restoration followed by an adequate tooth reduction" and "the accurately fitting prostheses with corresponding to final impression" can be the examples of them. Nevertheless, the one which all-ceramic restorations are distinguished from conventional restorations is the additional procedure of so called "bonding". In addition to the application of resin cement between "inner surface of restoration and outer surface of abutment", bonding technology can be also applied to the treatment process of "Post and Core" in particular if the abutments are non-vital teeth. Core build-up for all-ceramic crown is conducted with fiber post and tooth colored composite by considering the properties of the restorations transmitting light. I would like to share my clinical experience about "silica based ceramic and non silica based ceramic restoration.
PURPOSE. To evaluate the wear of computer-aided design/computer-aided manufacturing (CAD-CAM) dental ceramic materials opposed by enamel as a function of increased chewing forces. MATERIALS AND METHODS. The enamel cusps of healthy human third molar teeth (n = 40) opposed by materials from CAD-CAM dental ceramic groups (n = 10), including Vita Enamic® (ENA), a polymer-infiltrated ceramic network (PICN); GC Cerasmart® (CERA), a resin nanoceramic; Celtra® Duo (DUO), a zirconia-reinforced lithium silicate (ZLS) ceramic; and IPS e.max ZirCAD (ZIR), a polycrystalline zirconia, were exposed to chewing simulation (1,200,000 cycles; 120 N load; 1 Hz frequency; 0.7 mm lateral and 2 mm vertical motion). The wear of both enamel cusps and materials was quantified using a 3D laser scanner, and the wear mechanisms were evaluated by scanning electron microscopy (SEM). The results were analysed using Welch ANOVA and Kruskal Wallis test (α = .05). RESULTS. ZIR showed lower volume loss (0.02 ± 0.01 mm3) than ENA, CERA and DUO (P = .001, P = .018 and P = .005, respectively). The wear of cusp/DUO [0.59 mm3 (0.50-1.63 mm3)] was higher than cusp/CERA [0.17 mm3 (0.04-0.41 mm3)] (P = .007). ZIR showed completely different wear mechanism in SEM. CONCLUSION. Composite structured materials such as PICN and ZLS ceramic exhibit more abrasive effect on opposing enamel due to their loss against wear, compared to uniform structured zirconia. The resin nano-ceramic causes the lowest enamel wear thanks to its flexible nano-ceramic microstructure. While zirconia appears to be an enamel-friendly material in wear volume loss, it can cause microstructural defects of enamel.
Fabricating a crown to retrofit an existing abutment tooth for a partial removable dental prosthesis (PRDP) is one of the most time-consuming and labor-intensive clinical procedures. In particular, when the patient is concerned with esthetic aspects of restoration, the task of fabricating becomes more daunting. Many techniques for the fabrication of all-metallic or metal-ceramic crowns have been discussed in the literature. This article was aimed to describe a simple fabrication method in which a retrofitting crown was fabricated for a precise fit using a ceramic-pressed-to-metal system.
Purpose: Physical and chemical properties of gold is most suitable to be restored of teeth to its original state. Recently zirconia was used instead of gold because of esthetical and intimacy of human body. Because of high strength and high abrasion resistance of zirconia, all zirconia artificial tooth lead to wear the original tooth of opposite site. To preserve this original tooth, zirconia artificial tooth covered with dental ceramic glass was used. When joining the zirconia core and dental ceramic glass, difference of their thermal expansion coefficient and wetting ability is generated the residual stress at interface lead to crack. In order to solve this problem, intermediate layer what is called zir-liner was imported to decrease the residual stress and increase the bonding strength. Methods: In this study, to identify the optimum conditions for manufacturing process, various methods to rough the surface of zirconia core were adopted, and vary the thickness of interlayer, and analyzed bond strength. Results: Bond strength of sanding specimens group showed higher than that of non-sanding specimens group, and once applied intermediate layer with sanding specimens showed highest bond strength with 28 MPa. SEM photomicrographs of zirconia cores fired at $1500^{\circ}C$ showed parallel straight lines in sanding and pockmarked surface in blasting surfaces as abrasion traces. Observation of the destruction section after shear test by SEM were carried out. Liner applied non-sanding group and non-liner applied sanding group all showed interfacial crack. Sandblasting group with non-liner showed remained dental ceramic glass on the surface of zirconia. Sandblasting group with once applied liner showed partially remained liner and dental ceramic glass on the surface of zirconia. XRD analysis revealed that sandblasting group showed higher monoclinic peaks than other specimens group and this result was due to the high collision energy for stress induced phase transformation. Conclusions: A study on the improvement of bonding strength between zirconia and dental ceramic glass steadily carried out for the future to practical use.
Objectives : The purpose of this in vitro study was to evaluate the effect of firing cycles on the marginal and internal fit of metal ceramic crown. Methods : Ten same cases of stone models (abutment teeth 11) were manufactured. Ten cobalt-chrome cores were made per each models and the marginal and internal fit was evaluated through a silicone replica technique. The marginal and internal fit of specimens was measured twice. The first measurement was done after manufacturing cobalt-chrome alloy core and the second measurement was done after porcelain firing. T-test of paired sample for statistical analysis was executed with SPSS 12.0K for Windows (${\alpha}$=0.05). Results : Mean(SD) marginal and internal fit were 77.1(23.3) ${\mu}m$ for the cobalt-chrome alloy core group and 84.4(21.9) ${\mu}m$ for the metal-ceramic crown group. They were statistically significant differences between groups for marginal and internal fit (p<.05). Conclusions : All metal ceramic crowns showed marginal and internal fit ranged within the current clinical recommendations.
Objectives: The purpose of the study was to investigate the effect of CAD/CAM ceramic thickness on shade. Methods: 24 disk-shaped ceramic specimens (E.Max CAD & Empress CAD, $12mm{\times}12mm$) were prepared and divided into 4 groups (n=6). Ceramic specimens in 2 thicknesses (0.8 mm, 1.0 mm) were made low translucency(LT) shade A2. All specimens were measured as L*, a*, b* using spectrophotometer. The color differences(${\Delta}E$ values) between the specimen and a control target block ($12{\times}12{\times}1.2mm$) were calculated. Two-way ANOVA performed between 2 groups(material and thickness). Results: As the thickness increased, the $L^*$ (for all groups) and $b^*$ value (for all groups) increased and the $a^*$ value(for LR group) decreased. The mean color differences caused by thickness was acceptible (${\Delta}E$ > 2) for group LD1. But the mean color differences by thickness were not selected (${\Delta}E$ < 2) for group LD2, LR1, LR2. Conclusions: The different thickness can influence the final color of CAD/CAM ceramic.
