  • Lichen planus is an immune-mediated mucocutaneous disease, affects more frequently middle-aged Caucasian women and makes signs and symptoms in the oral mucosa. Cutaneous lichen planus lesions cause itching but they are self-limiting, oral lichen planus lesions are usually chronic, recalcitrant to treatment and potentially premalignant in some cases. Although, oral lichen planus is non-plaque related disease, they possess particular problems because plaque control is complicated by pain and bleeding and might cause plaque-related disease. The resulting condition comprises accumulations of plaque, which again can influence the progress of oral lichen planus with burning sensation, spontaneous gingival bleeding. Thus, it should be noted that both medication and supportive periodontal treatment are essential for the remission of the lesions. This case report introduces topical corticosteroid therapy and supportive periodontal treatment including intensive oral hygiene procedures to obtain an improvement of subjective symptoms and objective changes and to prevent relapse the lesions.

  • This clinical study was conducted to evaluate the clinical effects of a concave neck of external connection type implant fixture designed for platform switching on the peri-implant tissue responses. Two types of implants with different neck designs were implanted in 20 patients. For the experimental group, the bioseal(BS) implant fixtures with 's' shaped concave profile on the neck were used, and non-bioseal(NBS) implant fixtures with a straight profile on the neck were used as control(Total of 40 implants, NBS: n = 19, BS: n=21). During the one-year period after implant placement, implant survival rate, marginal bone resorption, bleeding, plaque, and complications were evaluated. The survival rate of NBS and BS group was 94.74% and 90.48%, respectively. There was no significant difference on marginal bone resorption, bleeding and plaque between the two groups (P>.05). Within the limits of the present study, implants with a concave neck design showed similar clinical results to implants with a straight neck design on the peri-implant tissue responses. Longitudinal clinical studies are necessary to confirm more effective clinical results.

  • Objectives: This purpose of this study was to examine the trends in interventional research and analyze the contents and results of intervention through a systematic literature review of the domestic research literature, suggesting the effect of an oral care program on elementary school students. Methods: A total of 262 articles were retrieved initially, and the duplicate articles were then removed, excluded after screening titles, abstracts, and full-text articles. Finally, a total of 16 papers were used in the review. Results: The study included 9 articles with the nonequivalent control group pretest-posttest design and 7 with the one-group pretest-posttest design. All articles were of non-randomized studies. The most common index of the interventional effect was dental plaque. Further, oral health behaviors and knowledge, perception, self-efficacy, and DMFT index were common indices. In the quality assessment, in the "blinding of outcome assessment" among six items of ROBANS, 12 studies showed a high risk of bias. Conclusions: In order to develop an interventional oral health care program for elementary school students and evaluate the effectiveness, a strict research design and qualitative improvement of research reports are required, and continuous research should be conducted to develop a systematic protocol.

  • The purpose of this study is to use basic data of dental hygiene curriculum with a new technique called motivational interviewing of communication skill to demonstrate the effectiveness of the method of oral health care. In this study, we performed oral health care program that has been made in dental hygiene department to university students. It was assigned to the control group and 66 and 32 experimental group based on the date of the first visit time. It conducted motivational interviewing of a total of three times in the experimental group. The analytical results of the measurements obtained in the oral examination and questionnaires. The results were as follows: The experimental group measured value was reduced after the intervention compared to before the PSR to evaluate the state of periodontal, gingival index, calculus index, plaque control record (PCR; O'Leary plaque index), simple plaque scor of Quantitative Light Induced Fluorescnece Digital measurement value (p<0.05). Experimental group decreased more and more the result of changes in the reduction of the average of the PCR. But control group was reduced to 3 weeks and increased back to the middle 16 weeks. There was also support interaction between the measurement point and the groups (p<0.05). Re-visit adherence of fit, 12.1% in the control group, the experimental group was 43.7% in the period of participation in the oral health care program. Thus, visit adherence of the experimental group was higher. In this study, a group that has motivational interviewing, It was able to confirm the improvement of oral health state. Discussion of the motivational interviewing can be applied to oral health care program.

  • Purpose : The purpose of this clinical study was to evaluate the effect of chelating and deproteinizing agent containing dental conditioning gel on alleviation of peri-implant mucosa inflammation. Methods: 36 patients with functionally loaded implants for at least 1 year and have clinical signs of peri-implant mucositis were recruited. At baseline, all implants received subgingival prophylaxis with ultrasonic scaler. In the test group, patients were provided a chelating and deproteinizing agent dental conditioning gel (Clinplant$^{(R)}$) and were given instructions to applicate it around the implants using an interdental brush for 2 weeks. Chlorhexidine and saline were provided to the positive control group and negative control group, respectively. The modified sulcus bleeding index (mSBI), modified plaque index (mPI), and probing pocket depth (PPD) were evaluated at baseline, 1 week, and 2 weeks. Results: In the Clinplant$^{(R)}$ and chlorhexidine group, mSBI (-0.81, -0.85 respectively; p<0.01), mPI (-0.46, -0.5 respectively; p<0.01), and PPD (-0.58, -0.48 respectively; p<0.01) at 2 weeks were significantly reduced from baseline. In the saline group, all the clinical parameters were reduced but there was no statistical significance. The saline may be attributed to the influence of prophylaxis at baseline. Conclusions: The present study demonstrated the beneficial clinical effects of chelating and deproteinizing agent containing dental conditioning gel to decrease peri-implant mucosa inflammation equivalent to chlorhexidine. This dental conditioning gel might be useful for alleviation of peri-implant mucosa inflammation.

Methods : This research was conducted from November of 2011 to February of 2012, in Chilgok-gun in the Kyeongbuk Province, South Korea. One hundred and twenty five (125) elderly women among the beneficiaries of the Visiting Oral Care Program within the area were randomly selected for the research. The research subjects would regularly visit the area senior citizen center, and they were divided into two groups, with 64 of them belonging to a control group, and the rest (61 people) belonged to a intervention group. The research subjects were examined for their dental health condition, and we investigated the effect of the Visiting Oral Care Program through this research. Results : 1. When they were asked about how many times they wash their dentures, the majority of the control group, 57.4% of them answered that they do it "once" a day. The majority of the intervention group answered "more than 3 times", with 35.2% of group mentioning that they wash their dentures more than 3 times a day(p<.001). 2. When the research subjects were asked to scale their overall quality of life before and after the Visiting Oral Care Program, the control group had rated 49.3 before the Service and 56.8 after the Service (p<.01), and scores for the individual factors to determine the overall quality of life has significantly improved as well (p<.01, p<.01, p<.01, p<.01, p<.01, p<.01, p<.01). 3. The oral hygiene condition after the Visiting Oral Care Program has shown improvement as well. The control group scored 64.2 for the severity of the symptoms of dental plaque before the dental care, and scored 46.8 after the dental care (p<0.1). The control group was diagnosed for the symptoms of coated tongue, and the severity of the symptoms were scaled as 3.7 before the Program and 2.0 after the Program (p<.01). Furthermore, the overall functionality of the oral cavity was improved as well. The control group showed 1.0 time increase in repetitive voluntary swallowing test (p<0.1), increased amount of sublingual saliva from 0.8mm to 1.4mm, and from 1.6mm to 2.0mm in dorsum linguae(p<0.1). Also, the control group showed an improvement in mouth opening as well, increased from 3.9 to 4.0cm after the oral care program (p<.05). When compared the result with the examination after the Visiting Oral Care Program, the two groups showed a significant difference with the control group showing a significant improvement compared to the intervention group(p<.01, p<.01, p<.01, p<.01, p<.05). 4. The correlation between the dental hygiene condition and the functionality of oral was made for the result of examining the beneficiaries for Visiting Oral Care Program. The amount of saliva showed a negative correlation with the severity of dental plaque (p<.05), and a positive correlation with the repetitive voluntary swallowing count (p<0.1). Also, mouth opening showed a positive correlation with repetitive voluntary swallowing count (p<.05). Conclusions : After the Visiting Orall Care Program, the beneficiaries of the program experienced improvement in their quality of life related to oral health, overall oral hygiene and functionality of oral.

  • Dental caries is initiated by the acid accumulated in dental plaque. Streptococcus mutans, one of a major causal agents of dental caries, is component of the dental plaque and produces various organic acids such as lactic acid as the end-product of glycolysis. As a consequence, we investigated the acid stress response of S. mutans KCTC 3065 in this study. The addition of lactic acid to the growth media had a concentration-dependent effect on the growth of S. mutans. S. mutans exhibited higher maximum culture OD compared with the more acidic growth pH values. At treatment of centration of 20 mmol/L lactic acid in the mid-log phage, cell growth was reduced to 40% relative to the control. The following results were obtained with the treatment of cells with a concentration of 20 mmol/L lactic acid in the mid-log phage for 2hrs: Analysis of fatty acids extracted from cells showed that growth at a concentration of 20 mmol/L lactic acid resulted in changes in $C_{14:0}$, $C_{16:0}$, $C_{18:0}$ and $C_{18:1}$ fatty acids. Protein profiles investigated by SDS-PAGE showed that approximately 70, 60, 45, 40 and 23 kDa proteins were highly expressed in S. mutans KCTC 3065.

  • The effect of xylitol and sorbitol chewing gums on dental caries levels among 5-year old kindergarteners was investigated. Three kindergartens the operations of which were partly subsidized by municipal government of Daegu city, Korea, were chosen as the study sites. The child base of the kindergartens were considered demographically and ethnically similar, representing middle-income families. The subjects were examined at their own kindergarten by the two dentists. Participation in the program was voluntary. Over 12 months, 123 participants chewed xylitol chewing gums(X group; 42 subjects), sorbitol chewing gum(S group; 42 subjects), or did not receive chewing gum as a control group(C group; 39 subjects). Consumption of xylitol and sorbitol was 4.5 to 5.0 g/day/subject, consumed in five daily chewing episodes of 5 min. Oral examination, Dentocult-SM test and interproximal dental plaque collection were completed at baseline and 12 months later. The dmfs of group C increased 59.2%, but group S increased 33.4% and group X increased only 31.3% during 12 months study period. The caries prevention ratio was 47.1% at group X and 43.6% at group S. There also appeared the reduction of caries activity at group X(1.39), group S(1.50) than control group(1.79). Compared with groups S and C, there was a statistically significant reduction of S. mutans in interproximal plaque in group X. The results suggest that xylitol chewing gum can prevent dental caries of deciduous dentition, and may be a little more effective than a sorbitol-containing product in controlling some caries-associated parameters in kindergarten-age subjects.

  • This research is conducted for 152 implant patients at the H dental clinic during the 2 years(from the January, 2007 to the December, 2008). After adjusting the following oral health care and analyzing the effects of plaque control scores(PCS), following results are obtained. The average PCS is higher at the 4th visit(65.37), which is the termination of education, than the 1st visit(32.89), which is before education(p = .000). Although following PCS decreases after 3 months(56.27) from the termination of education, it increase again after 6 months(60.44), 9 months(64.72) by following oral health care(p = .000). The female got a higher average PCS than the male, and also got a higher following PCS at 3 month(p = .000). According to the average PCS, patients are divided to group A who got lower grade than 69 and B who got higher grade than 70. Comparing with that results and the following PCS, group B get a higher grade than group A in the every period(p = .000). We can observe that the average PCS is improved as doing repeated education and constant management, and we think these results are occurred because people change their minds by following oral health care and obtain proper oral management habit.
