CADIA(Computer-assisted densitometric image analysis) method is used to analyze bone density changes around the implants. The usefullness and reproducibility of the method was assessed. We tried to find out if there is any possibility to quantitiate and qualitify peri-implant bone density change as time passes. And we concluded that this newly developed linear analysis is efficient for analyzing peri-implant bone density change non-Invasively. In this study, 2152 machined $Br{\aa}nemark$ fixtures installed from 1994 to 2002 in the department of Periodontics, Dental hospital of College of Dentistry, Yonsei University were included. Of these fixtures 22 radiographically analyzable failed fixtures were used as experimental group, and 22 successful implants placed in the same patient were used as control group. 1. 57 out of 1635 machined $Br{\aa}nemark$ standard and Mk II implants system failed, the survival rate was 96.5%. And 11 out of 517 machined $Br{\aa}nemark$ Mk III and Mk IV implants system failed, the survival rate was 97.9%. Total survival rate was 96.8%. 2. 22 failed implants were used for the analysis, 10 of which failed before prosthetic treatment due to infection and overheating. 12 failed due to overload after prosthetic treatment, 63.6% of which failed during the early phase of functional loading, i, e. before 1 year of loading. 3. Bone density change values around coronal region of the failed implants were $-6.54{\pm}6.35$, middle region were $-3.53{\pm}5.78$, apical region were $-0.75{\pm}10.33$, resulting in average of $-3.71{\pm}8.03$. 4. Bone density change values around coronal region of the successful implants were $4.25{\pm}4.66$, middle region were $6.33{\pm}5.02$, apical region were $9.89{\pm}4.67$, resulting in average of $6.27{\pm}5.29$. 5. There was a statistically significant difference between two groups (p<0.01). In conclusion, the linear analysis method using computer-assisted densitometric image analysis could be a useful method for the analysis of implants, and could be used for future implant researchs.

The purpose of this study was to make the stress distribution produced by simulated different load under two types of internal connection implant system (stepped and tapered type) by means of 3D finite element analysis, The finite element model was designed with the parallel placement of the one fixtures ($4.0mm{\times}11.5mm$) with reverse buttress thread on the mandibular 1st molar. Two models were loaded with 200 N magnitude in the vertical direction on the central position of the crown, the 1.5 mm and 3 mm buccal offset point from the central position of the fixture. The oblique load was applied at the angle of $30^{\circ}$ on the crown surface. Von Mises stress value was recorded and compared in the fixture-bone interface in the bucco-lingual dimension. The results were as follows; 1. The loading conditions of two internal connection implant systems (stepped and tapered type) were the main factor affecting the equivalent bone strain, followed by the type of internal connections. 2. The stepped model had more mechanical stability with the reduced max. stress compared to $11^{\circ}$ tapered models under the distributed oblique loading. 3. The more the contact of implant-abutment interface to the inner wall of implant fixture, the less stress concentration was reduced.
Edentulous patients with severe alveolar bone resorption have trouble with using traditional complete denture. In order to overcome these problems, implant-retained overdenture was developed. SFI-bar$^{(R)}$ system can save time and cost compared to other existing bar systems which need complicated laboratory procedures because it can be adjusted directly in a patient's mouth. A 55-year-old male, who had experienced a fractured lower old implant-retained overdenture, wanted a durable and painless denture. The fractured Locator$^{(R)}$ attachments were removed and edentulous mandible was restored with SFI-bar$^{(R)}$. A 77-year-old female with a medical history of the Parkinson's disease and severely absorbed alveolar bone of mandible, wanted to wear a retentive mandibular denture without pain. After placing two implants in front of mental foramen, two adaptors were connected to two implants and a tube bar was connected to the adaptors. A female part fitted to the bar was attached to the new denture. These clinical reports describe two-implant-retained overdenture using the SFI-bar$^{(R)}$ system in mandibular edentulous patients. Since the patients were satisfied esthetically and functionally during 2 years' observation, we would like to report cases.

The aim of this study was to evaluate effect of implant abutment screw coating treatment on joint stability, investigating mechanical properties of these. For this study used $ExFeel^{(R)}$ external hexed implant system and $15mm{\times}1mm$ discs. Experimental group was $1{\mu}m$ TiN, TiCN, TiC coated abutment screws and discs. To know mechanical property, i evaluated adhesion strength, surface hardness, using disc, corrosion test using screw. The results were as follows : rotation angle of coated screws increased than that of non-coated screw because of lower friction coefficient, especially TiC coated screw group had the largest value, but removal torque decreased in all coated screws (p<0.05). Torque loss before and after fatigue test was the smallest in TiC-coated screws, and the largest in non-coated screws (p<0.05), and there was no statistically significant difference between dry condition and wet condition of screws because of higher surface hardness and lower friction coefficient. From the above results, TiN, TiCN, TiC coating group had high abrasion resistance, especially TiC coated group which had low torque-consuming, high rotation angle as low friction coefficient will be considered to influence on implant abutment screw joint stability positively.

Purpose: Clinicians must examine tooth wear to make a proper diagnosis. However, qualitative methosds of measuring tooth wear have many disadvantages. Therefore, this study aimed to develop and evaluate quantitative parameters using the cusp area and volume of virtual dental models. Methods: The subjects of this study were the same virtual models that were used in our former study. The same age group classification and new tooth wear index (NTWI) scoring system were also reused. A virtual occlusal plane was generated with the highest cusp points and lowered vertically from 0.2 to 0.8 mm to create offset planes. The area and volume of each cusp was then measured and added together. In addition to the former analysis, the differential features of each cusp were analyzed. Results: The scores of the new parameters differentiated the age and NTWI groups better than those analyzed in the former study. The Spearman ${\rho}$ coefficients between the total area and the area of each cusp also showed higher scores at the levels of 0.6 mm (0.6A) and 0.8A. The mesiolingual cusp (MLC) showed a statistically significant difference (P<0.01) from the other cusps in the paired t-test. Additionally, the MLC exhibited the highest percentage of change at 0.6A in some age and NTWI groups. Regarding the age groups, the MLC showed the highest score in groups 1 and 2. For the NTWI groups, the MLC was not significantly different in groups 3 and 4. These results support the proposal that the lingual cusp exhibits rapid wear because it serves as a functional cusp. Conclusions: Although this study has limitations due to its cross-sectional nature, it suggests better quantitative parameters and analytical tools for the characteristics of cusp wear.
Titanium (Ti) is a bioinert material and has lower elastic coefficient and better strength/volume property than other metals. Ferroelectric materials show alignment of positive and negative charges by poling treatment. This study was purposed to develop a new implant system by combining the advantages of Ti and ferroelectric property of $BaTiO_3$ (BTO). It was performed with the assumption that the $Ca^{2+ }$ ions would be easily attracted on negatively charged surface and the attracted cation might behave as nuclei for bone-like crystal growth in biological solutions. A ferroelectric BTO thin film on Ti was fabricated and the effect of poling treatment on the improvement of calcium phosphate (Ca-P) formation in biological solutions was evaluated. After immersion in Eagle’s minimum essential media (MEM) solution, NaCl was formed on Ti, and Ca-P layer containing NaCl was formed on Ti-O. Weak and sparse Ca-P layers were formed on BTO, while thick, homogeneous, and dense Ca-P layer was formed on negatively polarized BTO (N-BTO), which was confirmed by FE-SEM and EDX. In summary, these results demonstrate that poling the ferroelectric BTO surface negatively is effective for the formation of Ca-P layer in MEM solution, and that N-BTO coating on Ti could be used as a possible alternative method for enhancing the osseointegration of the implants.
Statement of problem. The intial stability for osseointegration of implant has been an interesting factor. Especially, in the case of poor bone quality or immediately loaded implant, various strategies have been developed focusing on the surface of materials to improve implant fixation to bone. The microscopic properties of implant surfaces play a major role in the osseous healing of dental implants. Purpose. The aims of this study are to perform a histologic and histomorphometric comparison of the healing characteristics of three different surfaces and the comparison of resonance frequency analysis (RFA) values measured by $Osstell^{TM}$ and perio-test values (PTV) measured by Periotest. Material and methods. A total of 24 screw titanium implants (Dentium Co., Seoul, Korea) with 6mm in length and 3.4mm in diameter, were placed in the mandible of 4 beagle dogs. Implants were divided into three groups following the surface treatment methods: Group I is machined(control group). Group II is anodically oxidized. Group III is coated 500nm in thickness with hydroxyapatite(HA) by ion beam assisted deposition(IBAD) on the anodized oxidization. Bone blocks from 2 dogs were caught after 3 weeks of covered healing and another blocks from 2 dogs after 6 weeks. RFA values and PTV were measured right after insertion and at 3 and 6weeks. Histomorphometric analysis was made with Kappa Image Base System to calculate bone-to-implant contact (BIC) and bone area inside the threads. Pearson's correlation analyses were performed to evaluate the correlation between RFA and PTV, BIC and bone area ratio of three different surfaces at 3 and 6 weeks. Results. 1) In all surface treatment methods, the RFA values decreased and the PTV values increased until 6 weeks in comparison to initial values. 2) At 3 weeks, no significant difference was found from bone-to-implant contact ratio and bone area ratio of three different surface treatment methods(P>0.05). However, at 6 weeks, different surface treatment methods showed significantly different bone-toimplant contact ratio and bone area ratio(P<0.05). 3) In the implants with the IBAD on the anodic oxidization, significant difference was found between the 3 weeks and the 6 weeks bone area ratio(P<0.05). 4) Correlation was found between the RFA values and the bone area ratio at 3 and 6 weeks with significant difference(P<0.05). Conclusions. These results indicate that the implants with the IBAD on the anodic oxidization may have a high influence on the initial stability of implant.

The aim of this study was to compare the retention and stability of implant overdenture according to the shape and the number of magnetic attachment. The experimental groups were designed for the number of implants(1, 2, 4) and shape of magnetic attachments(flat, cushion, dome type) resulting in 9 subgroups. 45 attachments were tested attached to $Br{\aa}nemark$ system implants which were planted on a mandibular model. Each attachment was composed of the magnet assembly embedded in a overdenture sample and the abutment keeper screwed into the implants. Dislodging tensile forces were applied to the overdenture samples using an Instron(cross-head speed 50.80mm/min) in 3 directions simulating function: vertical, oblique, and anterior-posterior. The loading was repeated 10 times in each direction for 45 samples. The values of maximum dislodging force of each subgroup were processed statistically using SPSS V. 12.0 at the 0.05 level of significance. The results of this study were as follows: 1. Flat type magnetic overdenture was the most retentive when subjected to vertically directed forces and dome type was the lest retentive when subjected to obliquely directed forces(p<0.05). 2. In case of planting one implant, flat type had a higher vertically retentive force than anterior-posteriorly retentive force. In case of planting two implants, flat type and dome type had a higher vertically retentive force and in case of planting four implants, flat type and cushion type had a higher vertically retentive force than anterior-posteriorly retentive force(p<0.05). 3. The incremental number of dental implant, without regards to the three types of magnetic attachment shapes, showed higher retention of overdenture(p<0.05). From the results, if a patient need much more retention of implant overdenture, flat type magnetic overdenture would be a good treatment. In case of the bruxism where excessive lateral forces are already present, dome type could be expected to produce better results. In case of planting one implant, flat type is more stable than the other shape of magnet and in case of two implant, flat type and dome type are more stable and in case of four implants, flat type and cushion type are more stable. Planting more than two implants and using flat type magnetic attachment would provide better retention and stability of implant overdenture
For the success of dental implant, accurate radiographic evaluation is prerequisite for planning the location of the osseointegrated implants and avoiding injury to vital structures. CT/MPR(computed tomography/multiplanar reformation) shows improved visualization of inferior alveolar canal. In order to obtain cross-sectional images parallel to the teeth, the occlusal plane is used to orientate for the axial plane. If the direction of axial plane is not parallel to the occlusal plane, the reformatted cross-sectional scans will be oblique to the planned fixture direction and will not show the actual dimension of the planned fixture's location. If the available bone height which measured in the cross-sectional view is much greater than the actual available bone height, penetration of canal may occur. The aim of this study is to assess the effect of the axial plane to measurement of available bone height for dental implant in computed tomography of the mandible. 40 patients who had made radiographic stents and had taken CT were selected. The sites that were included in the study were 45 molar regions. In the central panoramic scan, the length from alveolar crest to superior border of inferior alveolar canal(available bone height, ABH) was measured in direction of reformatted cross-sectional plane(uncorrected ABH). Then, length from alveolar crest to superior border of canal was measured in direction of stent(corrected ABH). The angle between uncorrected ABH and corrected ABH was measured. From each ABH, available fixture length was decided by $Br{{\aa}}nemark$ system. The results were following ; the difference between two ABHs was statistically significant in both first and second molar(p< 0.01). The percentage of difference more than 1 mm was 8.7% in first molar and 15.5% in second molar. The percentage of difference more than 2 mm was 2.0% in first molar and 6.6% in second molar. The maximum value of difference was 2.5 mm in first molar and 2.2 mm in second molar. The correlations between difference of 2 ABHs and angle was positive correlations in both first and second molar. The correlation coefficient was 0.534 in first molar and 0.728 in second molar. The second molar has a stronger positive correlation. The percentage of disagreement between 2 fixture lengths from two ABHs was 24.4% in first molar and 28.9% in second molar.
Purpose: To compare the measurements of the mandible and the detectability of the mandibular canal on reformatted images using a newly developed 3-dimensional implant simulation program with traditionally used CT multiplanar reconstruction program and true measurements. Materials and Methods: Ten dry dog mandibles were used in this study. Occlusal templates for CT examination were fabricated and marked with gutta perch a at ten sites. Axial CT scans were taken and reconstructed using DentaScan (D group) and Vimplant program (V group), and each mandible was sectioned at the previously marked sites (R group). Maximum vertical height (H) and maximum width (W) of the mandible, the distances from buccal border of the mandibular canal to the most buccal aspect of the mandible (X), and the distance from the superior border of the mandibular canal to the alveolar crest (Y) were measured, and the mandibular measurements in each group were compared. Detectability of mandibular canal was evaluated using a 3-point scale in both V and D groups by three oral radiologists and compared. Results: H in the V group was slightly greater than that in the D group, and Wand X in the V group was slightly less than those in the D group. H in the V group was less than that in the R group, and Wand X in the V group was larger than those in the R group. The detectability of the mandibular canal did not show statistically significant differences between V and D groups. Conclusion: The results of the experiment show that the newly developed, inexpensive Vimplant/TM/ simulation program can be used as an alternative to the traditionally used, and more expensive CT multiplanar reconstruction program.
