Comparative evaluation of the effectiveness of a mobile app (Little Lovely Dentist) and the tell-show-do technique in the management of dental anxiety and fear: a randomized controlled trial

  • Background: Behavior guidance is a technique used to subdue inappropriate behavior by establishing communication that meets the needs of a child. This study aimed to measure the effectiveness of a mobile app (Little Lovely Dentist) compared to the tell-show-do (TSD) technique in managing anxious children during their first dental visit. Methods: Fifty children (30 boys and 20 girls) without any past dental experience, aged from 7 to 11 years, were randomly allocated into either the dental app group or the TSD group. The pre- and post-operative anxiety of children who underwent prophylactic cleaning was assessed both physiologically and subjectively using a heart rate measurement and the RMS pictorial scale, respectively. Results: The intragroup comparison of heart rate and RMS scores for children allocated to the dental app group was statistically significant (P value ≤ 0.001). However, a significant reduction only occurred in the RMS scores, but not the heart rate measurements, in the TSD group. Conversely, there was an increase in heart rates in the TSD group. Conclusion: Educating the child prior to a dental procedure using a smartphone application such as Little Lovely Dentist can significantly alleviate the anticipatory anxiety and engage children in dental treatment during their first visit.

Intraosseous anesthesia using a computer-controlled system during non-surgical periodontal therapy (root planing): Two case reports

  • Local anesthesia is administered to control pain, but it may induce fear and anxiety. Root planing is a non-surgical periodontal therapy; however, when it is performed in an extensive manner, some tissue removal is inevitable. Notably, this removal may be so painful that local anesthesia is required to be administered to the area scheduled for the treatment. Although patients tend to accept root planing easily, they frequently express a fear of local anesthesia. Intraosseous anesthesia (IA) is an intraosseous injection technique, whereby local anesthetic is injected into the cancellous bone supporting the teeth. A computer-controlled IA system (CIAS) exhibits multiple benefits, such as less painful anesthesia, reduced soft tissue numbness, and the provision of palatal or lingual, as well as buccal, anesthesia via single needle penetration. In this report, we present two cases of root planing that were performed under local anesthesia, using a CIAS.

The Clinical Effect with the Use of Gel Anesthesia within Gingival Sulcus during Scaling

  • Although scaling is the primary method for improving oral health, it is also associated with dental fear. The objective of this study was to empirically verify whether the use of gel anesthetic within the gingival sulcus during scaling relieves pain and improves other factors. A total of 128 patients scheduled to undergo scaling at a dental clinic of a general hospital located in the Gyeonggi Province, between July 2014 and July 2015, were enrolled in the study. The participants underwent scaling following the application of 20% benzocaine gel or placebo gel anesthetic within the gingival sulcus, and the data was collected using a questionnaire. There was a significant difference in the severity of pain, participant satisfaction, perceived sensitivity, overall discomfort, and fear of scaling between the two groups. The two groups were compared in terms of perceived need for gel anesthesia, willingness to pay for anesthesia costs, and willingness to receive scaling in the future. There were significant differences in all the three parameters depending on whether gel anesthesia was used or not. There were significant differences between the two groups in perceived sensitivity immediately after scaling and one day after scaling, with no difference seen one week after scaling. With regards to overall discomfort over time, there were significant differences between the two groups immediately after scaling. Based on these findings, we expect that application of gel anesthetic within the gingival sulcus during scaling will reduce pain, perceived sensitivity, overall discomfort, and fear of scaling with increased satisfaction.

Comparison of anxiety and pain perceived with conventional and computerized local anesthesia delivery systems for different stages of anesthesia delivery in maxillary and mandibular nerve blocks

  • Background: Fear of local anesthesia (LA) is a significant impediment to dental care as many patients delay or avoid treatment to avert pain. Computer-controlled local anesthetic delivery system (CCLAD), with constant and controlled rate of flow, present a painless alternative. The present study aimed to compare anxiety and pain perceived with conventional and computerized systems, for different stages of anesthesia delivery when administering various nerve blocks. Methods: One hundred patients requiring bilateral LA participated in the study. One side was anesthetized using one system and the contralateral side was anesthetized using the other, in two separate appointments. Patients assigned anxiety scores on a 5-point scale and used the visual analogue scale (VAS) for pain determination at needle insertion, during delivery of anesthetic solution, immediately after injection, and at the end of the periodontal procedure. Each patient's preference for the delivery system of future injections was also recorded. Results: Patients reported significantly lower anxiety levels with CCLAD compared to the syringe. Significantly lower mean VAS scores for anesthesia deposition, pain immediately after, and at the end of the periodontal procedure were also noted. However, pain at needle insertion was comparable between the two systems, with no statistical significance. Overall, 64.4% patients preferred CCLAD for future anesthesia. Conclusion: Lower pain perceived with CCLAD and higher preference for the system suggest that CCLAD should replace conventional syringes to allow pain-free dental treatment.

치과내원 환자의 레이저치료 실태 및 만족도 (The laser therapy conditions and satisfaction of the dental patients)

  • Objectives : This study was progressed a research with the aim of using it as basic data necessary for a plan in order to increase qualitatively medical level for patients who feel anxiety about dental treatment, and of offering basic data for improving the whole medical services by grasping conditions and satisfaction level with laser therapy in patients who visited dental clinic. Methods : The statistical analysis was carried out t-test and ANOVA by using SPSS(PASW statistics) 18.0. When there is statistically significant difference in analysis of variance, post-test was carried out by using Duncan multiple range test. Results : In therapy kinds of using laser, gingival treatment was the largest with 37.0%. It was the highest in likely seeming to be not painful with 32.9% as for a decisive factor, which came to receive laser therapy, and in having been enough to endure even if having been slightly tingling as for pain level. Conclusions : This study can be considered to have significance in changing recognition of medical cure by surveying patients satisfaction with using laser as a method for slightly solving elements of fear and anxiety about dental treatment and to have its value as basic data for seeking improvement plan for diverse medical services in the future.

치주임상에서의 정주진정법의 적용 (Intravenous Sedation in Periodontal Practice)

  • Chronic periodontitis is one of the most common disease in clinical dentistry, which needs various surgical interventions to treat the moderate to severe destruction of supporting periodontium. Most patients have fear and anxiety to these surgical procedures and some dentists also have problems to deal with these patients. Applying the conscious sedation technique in outpatient units, periodontists have become to manage their patients successfully without any fear and anxiety and increased the level of patients' satisfaction. Particularly, periodontal treatments require relative long chair-time and patients are usually reluctant to the periodontal instruments used in the oral cavity. This study is focused on the sedation procedure with intravenous midazolam infusioon performed in Department of Periodontology, Ewha Womans University Hospital. The whole procedure of intravenous midazolam sedation is briefly reviewed from patient selection to patient discharge.

향기흡입법이 발치 전·후에 미치는 불안과 통증에 관한 연구 (A Study on the Effect of Aroma Therapy on Anxiety and Pain Before and After Tooth Extraction)

  • The purpose of this study was to examine the effect of aroma therapy on anxiety and pain before and after tooth extraction. The subjects in this study were 60 patients who felt severe anxiety and pain due to tooth extraction. The experiment was conducted from January through March, 2004, by organizing an experimental group and a control group with 30 patients each. The experimental group was asked to keep wearing lavender-containing necklaces from two days before tooth extraction to inhale lavender, and no such an action was taken to the control group. The collected data were analyzed with SPSS 10.0 program to obtain statistical data. and ${\times}2$ test and t-test were implemented. The findings of this study were as follows: 1. Regarding whether or not the experimental and control groups were homogeneous, men outnumbered women, and the largest number of the patients were in their 30s. Those who were married were more than the others who were unmarried in number, and those who lived with their spouses under the same roof together outnumbered the others who didn't, as the rate of the former stood at 65 percent in the experimental group and 86.4 percent in the control group. They expressed high satisfaction at their spouses, since 45 percent of the experimental group and 31 percent of the control group did it, but the difference between them and those who were unsatisfied was insignificant (pE0.347). By occupation, the largest number of people in the experimental group, which numbered 16.7 percent, were self-employed, and lots of patients in the control group were government workers. As for blood type, type A was most prevailing, which recorded 43.3 percent. By religion, 43 percent of the experimental group had no religion, whereas 36.7 percent of the control group were Christian. The most common monthly income ranged from 2 million to 2.5 million won. 2. There was little disparity in past pain experience between the two groups before aroma therapy was applied. The experimental group underwent more pain (6.15) than the control group (5.78), but the difference wasn't significant. The experimental group (90%) experienced more anxiety and fear than the control group(83%), but the difference was insignificant. This fact showed that there was little gap between the two groups in anxiety and fear caused by tooth extraction. Contrary to earlier expectation that pre-anxiety might not be the same. little significant difference was found. 3. After aroma therapy was applied, 50 percent of the experimental group and 23.3 percent of the control group suffered significantly less anxiety and fear about tooth extraction(${\times}2$=4.59, pE.05). And the experimental group exposed to aroma therapy was less nervous(3.0) than the control group(4.39), and the gap between the two was significant (t=13.37, pE.001). Therefore, aroma therapy had a good effect on alleviating their anxiety. During tooth extraction. 73.3 percent of the experimental group and 93.3 percent of the control group felt pain. The former group suffered Significantly less pain(${\times}2$=4.32, PE.05). Concerning the extent of pain, the experimental group(2.53) found it less painful to have their teeth extracted than the control group(5.50), and the gap between the two was significant(t=5.89, PE.05). 4. As to the effect of aroma therapy on alleviating anxiety or fear, the experimental group(33.3%) felt that aroma therapy let them more relieved. Every member of that group was willing to use aroma therapy again in the future, and 86.7 percent of that group perceived that aroma therapy made a difference to dental treatment. The experimental group responded to aroma therapy favorably, as every member of it had an intention to advise others to use that therapy.

치과 환자의 진료 만족도 증진을 위한 비약물적 접근으로서 최면 요법의 분석 (Use of hypnosis in dentistry for improving patient satisfaction: as a means of non-phamaceutical approach)

  • 본 연구의 목적은 치과 치료를 받는 환자들의 불안, 통증, 공포, 스트레스를 줄이기 위해 최면의 효과를 알아보고 환자의 만족도를 높이기 위해 어떤 방법으로 도입하는 것이 바람직할지 알아보기 위함 이었다. Medline을 이용하여 치과 및 의학 영역의 최면과 관련된 연구를 검색하여 분석하였다. 치과 영역에서 최면 요법의 이용은 환자의 불안, 공포, 통증, 스트레스 등을 줄여주어 편안한 치료를 받을 수 있도록 도움을 주는 것으로 나타났다. 또한 면역 체계에 영향을 끼쳐 치유에 도움을 주고, 약물의 사용을 줄여 부작용을 감소시킬 수 있는 효과도 나타났다. 따라서, 최면 요법을 활용한 치과 진료 시 환자는 정신적, 육체적으로 편안한 진료가 가능하며, 이로써 환자의 진료의 만족도가 높아질 것으로 예상된다.

스케일링 전과 후의 불안 및 Vital Sign 변화 (Changes in Anxiety and Vital Signs of Scaling Patients)

  • 본 연구는 일개 대학 치위생과에 스켈링을 받으러 내원한 환자를 대상으로 스켈링 전과 후의 불안 및 공포와 vital sign 변화를 알아보기 위해 2015년 8월부터 11월까지 실시되었다. 자기기입식 설문지 작성과 Vital Sign을 측정하여 진행하였으며, 다음과 같은 결론을 얻었다. 불안 및 공포에 대한 항목 중 '마취용 주사바늘을 볼 때 두려움을 느낀다.'가 가장 높은 점수를 보였으며, 스케일링 전과 후에 각각 2.84점, 2.51점으로 조사되었다. 스케일링 전 맥박은 분당 77.48회에서 스케일링 후 분당 74.36회, 혈압은 스케일링 전 수축기 혈압이 124.86mmHg에서 122.34mmHg로 유의하게 감소하였다. 스케일링 전과 후의 불안 및 공포와 Vital Sign 변수들 간의 상관관계는 모두 수축기 혈압과 이완기 혈압이 가장 높은 양의 상관관계를 보였으며, 일반적 특성에 따른 불안의 경우 스케일링 전에 여자는 2.40점, 남자는 1.86점, 스케일링 후에는 여자 2.16점, 남자 1.65점으로 각각 조사되었다. 일반적 특성에 따른 Vital Sign의 경우 스케일링 전 수축기혈압이 남자가 133.59mmHg, 여자가 118.31mmHg으로 조사되었으며, 스케일링 후에는 수축기혈압이 남자가 129.36mmHg, 여자가 117.87mmHg으로 나타나 통계적으로 유의한 차이를 보였다. 연령에 따른 Vital Sign은 스케일링 후의 이완기 혈압의 경우 10대 69.63mmHg, 20대 76.48mmHg, 30대 이상은 79.29mmHg으로 연령이 증가할수록 이완기 혈압이 상승하는 것으로 조사되었다. 이상의 결과를 정리해보면 스케일링 전과 후의 불안 및 공포와 Vital Sign은 변화가 있으며, 성별, 거주지, 연령 직업 등이 불안과 Vital Sign 변화에 영향을 미치는 것으로 나타났다. 이에 본 연구를 바탕으로 치과에 방문하는 환자들의 불안과 공포를 줄일 수 있는 방안에 대한 연구가 필요할 것으로 사료된다.

Analysis of the annual changes in dental institutions that claimed dental sedatives in Korea and the types of sedatives using health care big data

  • Background: Dentists make various efforts to reduce patients' anxiety and fear associated with dental treatment. Dental sedation is an advanced method that dentists can perform to reduce patients' anxiety and fear and provide effective dental treatment. However, dental sedation is different from general dental treatment and requires separate learning, and if done incorrectly, can lead to serious complications. Therefore, sedation is performed by a limited number of dentists who have received specific training. This study aimed to investigate the proportion of dentists who practice sedation and the main sedatives they use in the context of the Republic of Korea. Methods: We used the customized health information data provided by the Korean National Health Insurance. We investigated the number of dental hospitals or clinics that claimed insurance for eight main sedatives commonly used in dental sedation from January, 2007 to September, 2019 at the Health Insurance Review and Assessment Service. We also identified the changes in the number of dental medical institutions by region and year and analyzed the number and proportion of dental medical institutions prescribing each sedative. Results: In 2007, 302 dental hospitals prescribed sedatives, and the number increased to 613 in 2019. In 2007, approximately 2.18% of the total 13,796 dental institutions prescribed sedatives, increasing to 3.31% in 2019. In 2007, 168 institutions (55.6%) prescribed N2O alone, and in 2019, 510 institutions (83.1%) made claims for it. In 2007, 76 (25.1%) hospitals made claims for chloral hydrate, but the number gradually decreased, with only 29 hospitals (4.7%) prescribing it in 2019. Hospitals that prescribed a combination of N2O, chloral hydrate, and hydroxyzine increased from 27 (8.9%) in 2007 to 51 (9%) in 2017 but decreased to 38 (6.1%) in 2019. The use of a combination of N2O and midazolam increased from 20 hospitals (6.6%) in 2007 to 51 hospitals (8.3%) in 2019. Conclusion: While there is a critical limitation to the investigation of dental hospitals performing sedation using insurance claims data, namely exclusion of dental clinics providing non-insured treatments, we found that in 2019, approximately 3.31% of the dental clinics were practicing sedation and that N2O was the most commonly prescribed sedative.
