Dental panoramic radiography is an imaging technique which shows the information of teeth, jaws and superficial structures on a single image. In this study, we propose the clinical dose reference for dental panoramic radiography. Dental panoramic radiography is an application which can increase the radiation does of oral cavity. It is very important to study the real condition of management for these panoramic X-ray equipments. Since there was no researches on dental panoramic equipments in domestic and foreign study groups, we measured and analyzed the dose such as effective dose, DAP and DWP of panoramic radiography.
Purpose : To compare the diagnostic accuracy of proximal caries detection between Kodak Insight film and the Biomedisys CDX2000HQ digital (CCD) sensor. Materials and Methods: 156 proximal surfaces of extracted teeth, 78 of which had chemical artificial caries, were used in this study. Four observers interpreted the radiographs using a five-point confidence rating scale to record their diagnoses. The results were analyzed by receiver operating characteristic curves, ANOVA and Kappa values. Result: Analysis using receiver operating characteristic curves revealed the areas under each curve which indicated a diagnostic accuracy of 0.951 in Insight and 0.952 in CDX2000HQ digital sensor. ANOVA revealed no significant differences between the two images with respect to caries detection. Kappa values indicated that the mean intra-observer agreement was 0.85 and inter-observer agreement 0.71 in conventional radiography. In digital radiography, the mean intra-observer agreement was 0.84 and inter-observer agreement 0.72. Conclusion: The results suggest that no significant difference exists between the two modalities for artificial caries detection and that CDX2000HQ was as good as Insight film for this purpose.
Purpose : Computed radiography (CR) has been used in cephalometric radiography and many studies have been carried out to improve image quality using various digital enhancement and filtering techniques, During CR image acquisition, the frequency rank and type affect to the image quality. The aim of this study was to compare the diagnostic quality of conventional cephalometric radiographs to those of computed radiography. Materials and Methods : The diagnostic quality of conventional cephalometric radiographs (MO) and their digital image counterparts were compared, and at the same time, six modalities (M1-M6) of spatial frequency-processed digital images were compared by evaluating the reproducibility of 23 cephalometric landmark locations. Reproducibility was defined as an observer's deviation (in mm) from the mean between all observers. Results and Conclusion: In comparison with the conventional cephalometric radiograph (MO), Ml showed statistically significant differences in 8 locations, M2 in 9, M3 12, M4 in 7, M5 in 12, and M6 showed significant differences in 14 of 23 landmark locations (p < 0.05). The number of reproducible landmarks that each modality possesses were 7 in M6, 6 in M5, 5 in M3, 4 in M4, 3 in M2, 2 in Ml, and 1 location in MO. The image modality that observers selected as having the best image quality was M5.

Purpose : The aim of this study was to assess the validity of standard deviation of gray scale histogram in digital subtraction radiography as a test parameter for superimposition error. Materials and Methods : Twenty periapical radiographs were used as baseline images and they were copied to exclude the influence of exposure geomety and contrast differences. These subsequent images were linearly displaced by 0.1-0.5 mm in the x-. y- and xy-directions, rotated by 0.5-3° and distorted by angular contraction of 1-5° in x- and y-axis before subtraction. The standard deviations of gray levels in the subtraction images were obtained and paired t-tests were performed. Pearson correlation coefficients(r) were calculated between the standard deviations and the superimposition errors. Results : Linear displacement showed high correlation coefficients of 0.997, 0.997 and 0.995 in x-. y- and xy-axis respectively. Statistically significant different standard deviation existed among all linearly displaced groups(p<0.05). Distortion showed relatively low correlation coefficients of 0.982 and 0.959 in x- and y-axis. The standard deviations between the two distortion groups were statistically significant different(p<0.05). Conclusion : Standard deviation of gray level distribution in digital subtraction images is satisfactory but not perfect similarity measure to assess the superimposition errors.
Purpose : To determine the more valuable information to detect the mandibular canal and the mental foramen in panoramic radiographs of a selected Korean population for the implant. Materials and Methods : This study analysed 288 panoramic radiographic images of patients taken at the Dental hospital of Chosun University retrospectively. Indirect digital panoramic X-ray machine (ProlineXC, PLANMECA, Finland) with processing by using Directview $CR950^@$ (Kodak, U.S.A.) and Direct digital panoramic X-ray machine (Promax, PLANMECA, Finland) were used for all exposures. All images were converted into Dicom format. Results : The common position of the mental foramen was in line with the longitudinal axis of the second premolar (68.1%). The mental foramen was symmetrical in 81.8% of cases. The mandibular canal was not identified at anterior portion and discontinued with the mental foramen in 27.8% of all cases, in 42.4% identified with lower border line continued with the mental foramen, in 14.6% with both upper and lower border lines, and in 15.3% unilaterally identified with lower border line. Conclusion : Clinicians can estimate the upper border line of the mandibular canal from the confirmation of the mental foramen and the lower border line of the mandibular canal symmetrically on the panoramic radiography taken in adjusted midsaggital plane of patient's head.
Conventional intraoral radiography continues to be the most widely used image modality for the diagnosis of dental caries. But, conventional intraoral radiography has several shortcomings, including the difficulty of exposing and processing intraoral film of consistently acceptable quality. In addition, radiographic retaking that was the result of processing errors, may result in increased discomfort and radiation dose to the patient. Recently, various digital radiographies substitute for conventional intraoral radiography to overcome these disadvantages. The advantages of digital radiography are numerous. One of advantages Is the elimination of processing errors. In addition, the radiation dose for digital system is approximately 20% to 25% of that required for conventional intraoral radiography Another potential advantage of digital imaging is the ability to perform image quality enhancements such as contrast and density modulation, which may increase diagnostic accuracy. The purpose of this study was to compare the diagnostic ability of artificial proximal defects to conventional intraoral radiography, direct digital image(CDX2000HQ$^{\circledR}$) and indirect digital image(Digora$^{\circledR}$). Artificial defects were made in proximal surfaces of 60 extracted human molars using #1/2, #1, #2 round bur. Five dentists assessed proximal defects on conventional intraoral radiography, direct digital image(CDX2000HQ$^{\circledR}$) and indirect digital image(Digora$^{\circledR}$). ROC(Receiver Operating Characteristic) analysis and Two-way ANOVA test were used for the evaluation of detectability, and following results were acquired. 1. The mean ROC area of conventional intraoral radiography, direct digital image(CDX2000HQ$^{\circledR}$) and indirect digital Image(Digora$^{\circledR}$) were 0.6766, 0.7538, 0.6791(Grade I), 0.7176, 0.7594, 0.7361(Grade II), and 0.7449, 0.7608, 0.7414(Grade III), respectively. 2. Diagnostic ability of direct digital image was higher than other image modalities. But, there was no statistically significant difference among other imaging modalities for Grade I, II, III lesion(p>0.05). In conclusion, when direct and indirect digital system are comparable with conventional intraoral radiography. these systems may be considered an alternative of conventional intraoral radiography for the diagnosis of proximal surface caries.
Purpose: This study evaluated the diagnostic accuracy of the reverse contrast mode in intraoral digital radiography for the detection of proximal dentinal caries, in comparison with the original digital radiographs. Materials and Methods: Eighty extracted premolars with no clinically apparent caries were selected, and digital radiographs of them were taken separately in standard conditions. Four observers examined the original radiographs and the same radiographs in the reverse contrast mode with the goal of identifying proximal dentinal caries. Microscopic sections $5{\mu}m$ in thickness were prepared from the teeth in the mesiodistal direction. Four slides prepared from each sample used as the diagnostic gold standard. The data were analyzed using SPSS (${\alpha}=0.05$). Results: Our results showed that the original radiographs in order to identify proximal dentinal caries had the following values for sensitivity, specificity, positive predictive value, negative predictive value, and accuracy, respectively: 72.5%, 90%, 87.2%, 76.5%, and 80.9%. For the reverse contrast mode, however, the corresponding values were 63.1%, 89.4%, 87.1%, 73.5%, and 78.8%, respectively. The sensitivity of original digital radiograph for detecting proximal dentinal caries was significantly higher than that of reverse contrast mode (p<0.05). However, no statistically significant differences were found regarding specificity, positive predictive value, negative predictive value, or accuracy (p>0.05). Conclusion: The sensitivity of the original digital radiograph for detecting proximal dentinal caries was significantly higher than that of the reversed contrast images. However, no statistically significant differences were found between these techniques regarding specificity, positive predictive value, negative predictive value, or accuracy.
Purpose: The purpose of the experiment was to evaluating the diagnostic ability of dental caries detection using digital subtraction in the artificial caries activity model. Materials and Methods: Digital radiographies of five teeth with 8 proximal surfaces were obtained by CCD sensor (Kodak RVG 6100 using a size #2). The digital radiographic images and subtraction images from artificial proximal caries were examined and interpreted. In this study, we proposed novel caries detection method which could diagnose the dental proximal caries from single digital radiographic image. Results: In artificial caries activity model, the range of lesional depth was $572-1,374{\mu}m$ and the range of lesional area was $36.95-138.52mm^2$. The lesional depth and the area were significantly increased with demineralization time (p<0.001). Furthermore, the proximal caries detection using digital subtraction radiography showed high detection rate compared to the proximal caries examination using simple digital radiograph. Conclusion: The results demonstrated that the digital subtraction radiography from single radiographic image of artificial caries was highly efficient in the detection of dental caries compared to the data from simple digital radiograph.
Purpose: The aim of this study was to evaluate the radiopacities of various types of restorative materials with different thicknesses compared with enamel, dentin, and aluminum. Materials and Methods: Four bulk-fill resins, 2 hybrid ceramics, 2 micro-hybrid resin composites, 6 glass ionomer-based materials, 2 zinc phosphate cements, and an amalgam were used in the study. Twelve disk-shaped specimens were prepared from each of 17 restorative materials with thicknesses of 1 mm, 2 mm, and 4 mm (n=4). All the restorative material specimens with the same thickness, an aluminum (Al) step wedge, and enamel and dentin specimens were positioned on a phosphor storage plate and exposed using a dental X-ray unit. The mean gray values were measured on digital images and converted to equivalent Al thicknesses. Statistical analyses were performed using 2-way analysis of variance and the Bonferroni post hoc test(P<0.05). Results: Radiopacity was significantly affected by both the thickness and the material type (P<0.05). GCP Glass Fill had the lowest radiopacity value for samples of 1 mm thickness, while Vita Enamic had the lowest radiopacity value for 2-mm-thick and 4-mm-thick samples. The materials with the highest radiopacity values after the amalgam were zinc phosphate cements. Conclusion: Significant differences were observed in the radiopacities of restorative materials with different thicknesses. Radiopacity was affected by both the material type and thickness.
Purpose: To compare the diagnostic accuracy of cone-beam computed tomography (CBCT) with that of parallel(PPA) and oblique projected periapical(OPA) radiography for the detection of different types of peri-implant bone defects. Materials and Methods: Forty implants inserted into bovine rib blocks were used. Thirty had standardized bone defects(10 each of angular, fenestration, and dehiscence defects), and 10 were defect-free controls. CBCT, PPA, and OPA images of the samples were acquired. The images were evaluated twice by each of 2 blinded observers regarding the presence or absence and the type of the defects. The area under the receiver operating characteristic curve (AUC), sensitivity, and specificity were determined for each radiographic technique. The 3 modalities were compared using the Fisher exact and chi-square tests, with P<0.05 considered as statistical significance. Results: High inter-examiner reliability was observed for the 3 techniques. Angular defects were detected with high sensitivity and specificity by all 3 modalities. CBCT and OPA showed similar AUC and sensitivity in the detection of fenestration defects. In the identification of dehiscence defects, CBCT showed the highest sensitivity, followed by OPA and PPA, respectively. CBCT and OPA had a significantly greater ability than PPA to detect fenestration and dehiscence defects(P<0.05). Conclusion: The application of OPA radiography in addition to routine PPA imaging as a radiographic follow-up method for dental implantation greatly enhances the visualization of fenestration and dehiscence defects. CBCT properly depicted all defect types studied, but it involves a relatively high dose of radiation and cost.
