Sevoflurane is familiar to anesthesiologists as an inhaled anesthetic to induce and maintain anesthesia; however, it has not been widely used for sedation. Recently, the use of sevoflurane for inhalational sedation has been increasing. Moreover, in Korea, the use of sevoflurane for conscious or deep sedation in dental care for children and the disabled is increasing, primarily by dental anesthesiologists. In this article, we reviewed a sedation method using sevoflurane.
Objectives: This study was conducted to assess dental health states of disabled people and analyze association between perception and awareness toward dental health and dental health status. Methods: The survey was performed from June 25 through October 30, 2004. A total of 548 disabled people participated in the study with details of 419 living in eight residential care centers located in Gyeongsangbuk-do and 129 children from a special school and two day-care centers. All subjects underwent oral examination and surveyed through a questionnaire. Parents of 129 children with disabilities were also surveyed through a separate questionnaire. Results: The dental caries experience rate was 82.1% of total 548 subjects. By age, those in their 20s experienced a high rate of dental caries with 87.5%. By educational level, those with a middle school education experienced a high dental caries rate with 91.8% (p<0.05). Of total subjects who experienced dental caries, 78.9% had experience in dental caries treatment. By age, those in their 10s showed a high rate of dental caries treatment with 87.4%(p<0.05). By educational level, those with a high school education showed a high rate of dental caries treatment with 87.7%(p<0.05). Those in residential care centers had a high rate of dental caries treatment with 82.1%, which is significantly higher than 68.8% of those who used day-care centers. A tooth extraction rate was 38.0% of total subjects. Those in their 40s had a higher rate of tooth extraction(p<0.01). Those in residental care centers had a significantly higher rate of extraction with 43.4%, compared with 20.2% of those in day-care centers. Of total subjects, 61.5% had plaque. A high rate of plaque formation was observed in those in their 40s(92.0%), those with a high school education(84.0%) and those with multiple disabilities(77.8%)(p<0.01). Among total subjects, 47.6% maintained healthy periodontal tissue. Those in their 40s and those with multiple disabilities had diseased periodontal tissue(p<0.01). Of 129 disabled children, 43.8% had plaque with parents who were not oral health-conscious while 18.6% had plaque with parents who were oral health-conscious, showing a significant difference(p<0.05) Conclusion: The results of the study suggest the need for educating parents with disabled children about oral health and strengthening programs for oral health for teachers working at special schools and day-care centers.

Background: Patients were subjected to post-discharge follow-up (by telephone) in order to investigate the potential complications of outpatient general anesthesia or deep sedation that could develop in disabled dental patients discharged from the hospital. The ultimate aim of this study was to establish an appropriate response measure for such complications. Methods: The caregivers of 79 disabled patients who underwent dental procedures under general anesthesia at our outpatient clinic were interviewed over telephone. Necessary care instructions were provided during the phone calls when required. The patient satisfaction level regarding the telephonic follow-up care was surveyed by additional telephone calls. Results: Most of the patients did not suffer any serious complications; however, some reported fever and bleeding. The data obtained in this study can be utilized towards the development of caregiver education pertaining to the ambulatory general anesthesia of dental patients with disabilities. Conclusions: Additionally, we hope that the findings of this study will help minimize the effects of complications experienced by disabled dental patients undergoing ambulatory general anesthesia, as well as increase the overall patient satisfaction level.

General anesthesia or sedation is an essential for a successful dental treatment for the disabled. The aim of this study is to assess the patients who had dental management under general anesthesia or sedation at Chungnam dental clinic for the disabled, who received dental treatment under general anesthesia, sevoflurane sedation and intravenous sedation from January, 2011 to September 2012. Of the 426 patients studied, 389 patients received dental care under general anesthesia, 20 patients received dental care under sevoflurane sedation and 17 patients received dental care under intravenous sedation. The Rate of general anesthesia was higher than that of sevoflurane sedation, intravenous sedation. Sevoflurane sedation is a useful method for short time treatment such as traumatic pediatric patients. Intravenous sedation is an option for patients who had anxiety and fear such as dentally disabled patients. Sevoflurane or intravenous sedation not only gradually reduces the use of general anesthesia but also useful methods themselves for the dentally disabled.

In Korea, the elderly is defined as the person aged over 65 and the disabled is defined as the person who is registered with Ministry of Health and Welfare. In 2008, the elderly with disabilities was about 770,000. Korea was already poised for an aging society in 2000 and is ahead of and aged society in 2018 and a super aged society in 2026. So, the number of the elderly with disabilities will continue to grow. Various oral diseases can be seen in the elderly with disabilities due to the systemic disease and impaired self oral care, in addition to the physiological aging. Particularly, we should be care for the aspiration pneumonia, which is the major cause of the death of the elderly. As a professional for the special care dentistry, we should understand the general and dental, individual characteristics of the elderly with disabilities and do proper oral care and treatment. Thereby, the quality of life of the patients might be improved.

Dentally disabled person means one who have difficulty in cooperating voluntarily with a dentist for routine dental care. Seoul National University Dental Hospital (SNUDH) Dental Clinic for Disabled has worked for them under outpatient anesthesia concept. The aim of this study was to determine the anesthetic characteristics of the patients attending for dental treatment at SNUDH Dental Clinic for Disabled in order to establish better future treatment plans. The data were drawn from the patients who visited SNUDH Dental Clinic for Disabled from January 1999 to October 2002. Total 93 dental treatments for 83 patients were enrolled in this study. Most patients had mental retardation and the conservative treatments were carried out under general anesthesia. Thiopental and vecuronium were mostly used for induction agent and neuromuscular blocker, respectively. Enflurane, with oxygen and nitrous oxide, was mostly used to maintain the anesthesia. Mean total anesthetic time was $164.4{\pm}57.2$ minutes and mean duration at the post-anesthetic care unit was $106.2{\pm}50.5$ min. Serious postoperative complications were not observed. This study aims to help utilize the dental treatment institutions for the disabled and provide high quality of medical services in the future by identifying the satisfaction factors for the patients of the dental clinics for the disabled. As a study method, surveys were completed 200 patients who visited the dental hospital for the disabled. Among these, the surveys from 187 patients were statistically analyzed. The main findings of this study are as follow. Since the study showed that there were differences in the satisfaction factors between the disabled and non-disabled people, subsequent studies needs to be done to develop measuring tools to accurately accommodate the satisfaction factors of the disabled. In addition, as the low spatial accessibility in terms of distance and location was found in this study, more dental treatment institutions for the disabled are required. In the meantime, the survey results found that the most satisfactory aspect was the lower treatment cost compared to other clinics, and that the most important factor when choosing a clinic was also treatment. In addition, given the fact that the group that took a prosthetic treatment, which was non-insured treatment, showed higher satisfaction compared to the group that did not, it suggests that the economic burden plays an important role for the dental treatment of the disabled.
