Background: A dense breast not only reduces the sensitivity of mammography but also is a moderate independent risk factor for breast cancer. The percentage of Western women with fat breast tissue is higher aged 40 years or older. To a certain extent, mammography as a first choice of screening imaging method for Western women of this group is reasonable. Hitherto, the frequency and age distribution of mammographic breast density patterns among Chinese women had not been characterized. The purpose of this study was to investigate the frequency and age distribution of mammographic breast density patterns among a group of Chinese screening women and breast cancer patients in order to provide useful information for age-specific guidelines for breast cancer screening in Chinese women. Methods: A retrospective review of a total of 3,394 screening women between August and December 2009 and 2,527 breast cancer patients between July 2011 and June 2012 was conducted. Descriptive analyses were used to examine the association between age and breast density. The significance of differences of breast density between the screening women and the breast cancer patients was examined using nonparametric tests. Results: There was a significant inverse relationship between age and breast density overall (r=-0.37, p< 0.01). Breast density of the breast cancer patients in the subgroups of 40-49 years old was greater compared with that of the screening women, the same in those aged 50-54 years and in those 55 years old or older, less than in the screening group. Conclusions: With regard to the Chinese women younger than 55 years old, the diagnostic efficiency of breast cancer screening imaging examinations may be potentially improved by combining screening mammography with ultrasound.
Background: Mammography is considered the gold standard of breast cancer mass screening and many countries have implemented this as an established breast cancer screening strategy. However, although the incidence of breast cancer and racial characteristics are different between Western and Asian countries, many Asian countries adopted mammography for mass screening. Therefore, the objective of this research was to determine whether mammography mass screening is cost-effective for both Western and Asian countries. Materials and Methods: A systematic review was performed of 17 national mammography cost-effectiveness data sets. Per capita gross domestic product (GDP), breast cancer incidence rate, and the most optimal cost-effectiveness results [cost per life year saved (LYS)] of a mammography screening strategy for each data set were extracted. The CE/per capita GDP ratio is used to compare the cost-effectiveness of mammography by countries. Non-parametric regression was used to find a cut-off point which indicated the breast cancer incidence rate boundary line determining whether mammography screening is cost-effective or not. Results: We found that the cost-effective cut-off point of breast cancer incidence rate was 45.04; it exactly divided countries into Western and Asian countries (p<0.0014). Conclusions: Mammography screening is cost-effective in most of Western countries, but not in Asian countries. The reason for this result may be the issues of incidence rate or racial characteristics, such as dense breast tissue. The results indicate that mammography screening should be adopted prudently in Asian countries and other countries with low incidence rates.

Background Breast aesthetics evaluation often relies on subjective assessments, leading to the need for objective, automated tools. We developed the Seoul Breast Esthetic Scoring Tool (S-BEST), a photometric analysis software that utilizes a DenseNet-264 deep learning model to automatically evaluate breast landmarks and asymmetry indices. Methods S-BEST was trained on a dataset of frontal breast photographs annotated with 30 specific landmarks, divided into an 80-20 training-validation split. The software requires the distances of sternal notch to nipple or nipple-to-nipple as input and performs image preprocessing steps, including ratio correction and 8-bit normalization. Breast asymmetry indices and centimeter-based measurements are provided as the output. The accuracy of S-BEST was validated using a paired t-test and Bland-Altman plots, comparing its measurements to those obtained from physical examinations of 100 females diagnosed with breast cancer. Results S-BEST demonstrated high accuracy in automatic landmark localization, with most distances showing no statistically significant difference compared with physical measurements. However, the nipple to inframammary fold distance showed a significant bias, with a coefficient of determination ranging from 0.3787 to 0.4234 for the left and right sides, respectively. Conclusion S-BEST provides a fast, reliable, and automated approach for breast aesthetic evaluation based on 2D frontal photographs. While limited by its inability to capture volumetric attributes or multiple viewpoints, it serves as an accessible tool for both clinical and research applications.
Malignant tumors of the breast with stromal multinucleated giant cells are rare entity of uncertain clinical significance. There have been few reports on the fine needle aspiration cytologic(FNAC) findings about these rare tumors. We report a FNAC case of invasive mammary carcinoma with osteoclast-like giant cells not only for its rare occurrence but in particular for its distinctive cytologic picture on aspirated material. The patient was a 40-year-old woman who presented with a right breast mass for one month. Mammography showed a well-demarcated rounded mass density without calcification. The aspirates of FNAC were highly cellular and two main cell types were seen; malignant epithelial cells and osteoclast-like multinucleated giant cells. The carcinoma cells occurred singly or arranged in loose clusters with ill-defined cytoplasm, oval nuclei, coarse chromatin and small but distinct nucleoli. The multinucleated giant cells showed variable number of nuclei with prominent nucleoli and abundant dense oxyphilic cytoplasm. The immunocytochemical studies suggested that osteoclast-like giant cells were not of epithelial origin, but rather of histlocytic origin.
본 연구는 디지털 유방촬영장치의 DBT(Digital Breast Tomosynthesis)와 FFDM(Full Field DigitalMammography)의 비교를 위해 유방조직등가팬텀을 이용하여 두께와 밀도를 변화시켜 선량 및 화질을 평가하여 DBT의 유용성을 평가하였다. 측정 결과 평균유선선량은 팬텀 두께 6 cm이상 밀도 70% 이상일 때, 두께 7 cm이상 밀도 50% 이상일 때 FFDM 보다 DBT가 낮은 것을 알 수 있었다. 영상 측정 결과, 섬유소는 DBT에서 우수하다고 측정 되었고, 작은 석회화 그룹과 종양에서는 FFDM 이 우수하였다. 유방의 복잡한 조직과 유사한 BR3D 팬텀에서는 모든 두께와 섬유소, 석회화 및 종양 그룹 모두 DBT 가 우수하다고 측정 되었다. DBT는 FFDM보다밀도가 높고 두께가 두꺼운 두께가 두껍고 밀도가 높은 유방에서 영상 품질 우수하고 낮은 선량을 제공함으로서, 한국 여성의 많은 분포를 차지하는 치밀유방에 더 유용할 수 있을 것이라 사료된다.
Background: Women with family history of breast cancer are more likely to be worried about having cancer and participate in breast cancer screening. However, few studies have examined relationship between family history, cancer worries, and participating in breast cancer screening in Korea. This study is to identify relationship between family history, cancer worries, and participating in breast cancer screening among women with and without family history of cancer. Methods: Respondents were 2,364 women who based on the 2013 Korean National Cancer Screening Survey which is done by National Cancer Center in Korea. Chi-square tests were performed to determine differences of cancer worries, undergoing of breast cancer screening and methods such as mammogram and ultrasonogram with and without cancer family history. Univariate and multiple logistic regression were performed to identify if family history and cancer worries are related factors on participating in breast cancer screening and methods. Stratified analysis was performed to confirm the effect of ultrasonogram on the dense breast by age. Results: Women with cancer family history frequently checked condition for conscious of having cancer (p=0.0299) and had highly perception of risk about having cancer in the future ($p{\leq}0.0001$). Women aged 30-49 did not perform significantly more ultrasonogram than women aged over 50 years old. Checking condition (moderate odds ratio [OR], 1.38; 95% confidence interval [CI], 1.20-2.08; frequently OR, 1.58; 95% CI, 1.08-1.76) and perception of risk (moderate OR, 3.12; 95% CI, 1.06-7.06; high OR, 2.74; 95% CI, 1.20-8.08) were related to participate in mammogram and ultrasonogram. A positive family history was related to 1.35 higher odds of performing only breast ultrasonogram (95% CI, 1.04-1.75). Conclusion: This study requires national education and publicity to reduce the unnecessary cost of screening, to be possible cost effective screening and to encourage women to receive more mammogram, especially women aged over 50 years old and with socioeconomic factors related to opportunistic screening.

Objective: The aim of this study was to compare the survival rates of Korean females aged 40 to 49 years with breast cancer detected by supplemental screening ultrasound (US) or screening mammography alone. Materials and Methods: This single-institution retrospective study included 240 patients with breast cancer (mean age, 45.1 ± 2.8 years) detected by US or mammography who had undergone breast surgery between 2003 and 2008. Medical records were reviewed for clinicopathologic characteristics and detection methods. Disease-free survival (DFS) and overall survival (OS) were compared between patients with breast cancer in the US and mammography groups using the log-rank test. Multivariable cox regression analysis was used to identify independent variables associated with DFS and OS. Results: Among the 240 cases of breast cancer, 43 were detected by supplemental screening US and 197 by screening mammography (mean follow-up: 7.4 years, 93.3% with dense breasts). There were 19 recurrences and 16 deaths, all occurring in the mammography group. While the US group did not differ from the mammography group in tumor stage, the patients in this group were more likely to undergo breast-conserving surgery and radiation therapy than the mammography group. The US group also showed better DFS (p = 0.016); however, OS did not differ between the two groups (p = 0.058). In the multivariable analysis, the US group showed a lower risk of recurrence (hazard ratio, 0.097; 95% confidence interval, 0.001-0.705) compared to the mammography group. Conclusion: Our study found that Korean females aged 40-49 years with US-detected breast cancer showed better DFS than those with mammography-detected breast cancer. However, there were no statistically significant differences in OS.
This study evaluates the change of computer tomography (CT) number in the case of the metal artifact reduction (MAR) algorithm, using the phantom. The images were obtained from dual CT using a gammex 467 tissue characterization phantom, which is similar to human tissues. The test method was performed by dividing pre and post MAR algorithm and measured CT values of nonmagnetic materials within the phantom. In addition, the changes of CT values for each material were compared and analyzed after measuring CT values up to 140 keV, using the spectral HU curve followed by CT scan. As a result, in the cases of N rod (trabecular bone) and E rod (trabecular bone), the CT numbers decreased as keV increasing but were constant above 90 keV. In the cases of I rod (dense bone) and K rod (dense bone), the CT numbers also decreased as keV increased but were uniform above 90 keV. The CT numbers from 40 keV to 140 keV were consistent in the cases of J rod (liver), D rod (liver), L rod (muscle), and F rod (muscle). For A rod (adipose), G rod (adipose), B rod (breast) and O rod (breast), the CT numbers increased as keV increased but were constant after 90 keV. The CT numbers from 40 keV to 140 keV were consistent in the cases of C rod (lung (exhale)), P rod (lung (exhale)), M rod (lung (inhale)) and H rod (lung (exhale)). Conclusively, because dual CT exhibits no changes in image quality and is able to analyze nonmagnetic materials by measuring the CT values of various materials, it will be used in the future as a useful tool for the diagnosis of lesions.
Mammogram is one of the important techniques for mass detection, which is the early diagnosis stage of a breast cancer. Especially, the CAD(Computer Aided Diagnosis) using mammogram improves the working performance of radiologists as it offers an effective mass detection. There are two types of CAD systems using mammogram; automatic and semi-automatic CAD systems. However, the automatic segmentation is limited in performance due to the difficulty of obtaining an accurate segmentation since mass occurs in the dense areas of the breast tissue and has smoother boundaries. Semi-automatic CAD systems overcome these limitations, however, they also have problems including high FP (False Positive) rate and a large amount of training data required for training a classifier. The proposed system which overcomes the aforementioned problems to detect mass is composed of the suspected area selection, the level set segmentation and SVM (Support Vector Machine) classification. To assess the efficacy of the system, 60 test images from the FFDM (Full-Field Digital Mammography) are analyzed and compared with the previous semi-automatic system, which uses the ANN classifier. The experimental results of the proposed system indicate higher accuracy of detecting mass in comparison to the previous systems.
목적: 기존의 Tc-99m MIBI 유방스캔은 유방암 진단에 어느 정도 유용하다고 알려져 있으나 작은 병변을 발견하는데 민감도가 낮은 것이 문제점이었다. 고해상도 유방 전용 스캔 또한 작은 병변에 대한 예민도에 한계가 있다. 본 연구에서는 1 cm이상의 촉지되는 병변에 대해서 고해상도 유방 전용 스캔의 유용성을 알아보고자 하였다. 대상 및 방법: 19명의 만져지는 병변이 있은 환자를 대상으로 Tc-99m MIBI 유방스캔을 시행하였다 약 925-1110 MBq을 투여한 10분 후에 조기 영상을 촬영하고, 30분 후에 지연 영상을 시행하였다. 결과: 19명중 23개의 병변이 조직학적으로 진단되었고, 그 중 18개는 악성으로, 5개 병변은 양성으로 진단되었다. 고해상 유방스캔이 악성 병변을 찾는데 있어 민감도 100%로 매우 유용하였다. 다섯 개의 양성 중 2개는 초기 영상에서 섭취 증가를 보였으나 지연 영상에서 섭취감소를 보여 양성으로 판단하였고 병변 진단에 지연 영상이 도움이 되었다. 유방 촬영술이 13개의 병변들에서 모호한 결과를 보였고 1개의 악성 병변을 양성으로 진단한 것에 비해 유방 전용 스캔은 악성 선별에 훨씬 우수한 결과를 보였다. 결론: 본 연구에 의하면 고해상 유방 전용 스캔은 1 cm 이상의 만져지는 병변에서 악성 병변을 선별하는데 큰 도움이 될 것으로 기대된다. 이러한 결과는 좀 더 많은 환자를 대상으로 한 연구에 의해 검증되어야 하며 특히 지연영상을 촬영하는 최적의 시간에 대해서도 비교 연구가 필요할 것으로 사료된다.
