Purpose: Nurses' management of Nursing Home residents' remaining functional ability is phenomena emphasizing as the main practice focus in long term care and has specific meanings within the context of quality of life for fragile and disabled elderly people in nursing homes. This study was conducted to clarify and to conceptualize the phenomena of nurses' management of nursing home residents' remaining functional ability. Method: The Hybrid Model of concept development was applied to develop a concept of nurses' management of residents' remaining functional ability. Results: This concept of nurse' management emerged as a phenomenon having meanings in two different dimensions, deliberation and enactment for five types of residents: residents with declining function and dementia requiring nursing assistance, bed-ridden residents with moderate dementia, residents with advanced dementia and behavior problems, but good physical health, residents at the end of life stage, and bed-ridden residents with intact mental health. Conclusions: Results indicate that nurses' management of remaining functional ability of residents in nursing homes is a concept having types of needs that should be treated in specified ways. Interventions should have a positive impacts on practical applications in nursing homes and on enhancing residents' remaining function.
The demands of nursing home for the dementia are rapidly increasing. However, there are no enough nursing homes and design guidelines for it. This study was intended to examine the needs of the staff for the nursing home design and suggest the design guidelines of the Korean nursing home for the dementia. The subjects were 15 nurses and 18 care givers who worked in D hospital for the elderly with dementia in Busan. Data were collected by the self-administered questionnaire. The findings were as follows; First, the most important design element in nursing home for the dementia which the staff mentioned was 'healing environment' and then 'function' and 'maintenance' Second, the care-givers were more satisfied with their facility design than the nurses. Third, the staff pointed out that 'the relation between individual room and nursing station', namely, short distance between them, and 'enough wandering space' had to be considered at first in design process of nursing home with dementia. Also, the comfort and intimate interior mood, fresh air and sunlight were also pointed out as the important elements in nursing home design for the dementia.

Purpose: This study aimed to explore the subjective perception structures and types of empathy among nurses caring for patients with behavioral and psychological symptoms of dementia (BPSD) using Q-methodology. Methods: Thirty-eight Q-samples (statements) were derived from in-depth interviews with 10 nurses working in long-term care hospitals and nursing homes. The Q-sorting was conducted in rank order (the responses obtained by each of the 30 nurses working at 3 long-term care hospitals and 4 nursing homes) into a normal distribution grid (from -4 to +4). The types of empathy among participants were analyzed using the PC-QUANL program. Results: Five types of empathy accounting for 48.5% of the total variance were categorized as follows: (1) taking the patients' personality into consideration while helping, (2) interacting closely and emotionally, (3) supporting the patient as a companion, (4) performing the duty in a defensive manner, and (5) resolving patients' problems by focusing on their needs. Conclusion: This study shows that there are various types of empathy in nurses caring for patients with BPSD. Therefore, it is necessary to develop strategies and educative programs to enhance empathy competency and deal with burnout based on the type of empathy.
Purpose: This study aimed to investigate compassion competence and its mediating effects on the relationship between attitudes toward dementia and care behavior among caregivers of older adults with dementia in nursing homes. Methods: 211 participants were recruited for this cross-sectional study. Data were collected from February 1st to May 24th, 2023, through a structured questionnaire and analyzed using multiple linear regression analysis and the bootstrapping method. Results: The mean score for care behavior was 3.67±0.97 (range 1~5). Caring behaviors were significantly associated with attitudes toward dementia (r=.76, p<.001) and compassion competence (r=.75, p<.001). Using Baron and Kenny's approach and PROCESS Macro model 4, care behavior was directly affected by dementia attitude (β=.76, p<.001). Compassion competence (β=.66, p<.001) was directly affected by dementia attitude. The partial mediating effects of compassion competence (β=.47, p<.001, 95% confidence interval 0.10~0.23) on the impact of dementia attitude on care behavior was confirmed. Conclusion: The results suggest that compassion competence should be considered when developing nursing interventions to enhance dementia-related attitudes and improve care behavior among caregivers of older adults with dementia.
The purpose of this study was to describe the overall physical environment's features of nursing homes for Korean American elders in the United States. This investigation was conducted in Korean American nursing homes on the East Coast and in the Midwest with multiple qualitative data collection methods, such as interviews, observations, document review, and field notes. The findings of this study are as follows: 1) Although almost all respondents were unable to speak and understand English at all, most written materials in nursing homes were in only English; 2) The nursing homes were placed near or within the Korean communities; thus, it contributed to close ties with religious Korean American groups and other groups in the ethnic communities; 3) Whether the facility's physical features looked like those of a hospital or a home, many residents did not consider a nursing homes as a real home; 4) A fenced garden in the nursing home in the Midwest was the residents' favorite public space; 5) Due to being forced to room with residents who had dementia and were bedridden, they were more likely to have conflicts with their roommates and feel uncomfortable living in their room; and 6) The facilities seemed to be designed without consideration to protect residents' privacy.
Purpose: The purpose of the study was to explore and describe the experience of adult Korean children who are caregivers for parents institutionalized with dementia. Methods: Participants were fourteen adult children caregivers of elders institutionalized with dementia. Data were collected through in-depth unstructured interviews with individual participants from August to November, 2012. Theoretical sampling was used to the point of theoretical saturation. Data were analyzed using Strauss and Corbin's Grounded Theory Method. Results: From open coding, 67 concepts, 29 sub-categories, and 14 categories were identified. Analysis revealed that the core category of the experience of adult children caring for their parents institutionalized with dementia was 'enduring the role of a prop' consisting of four phases: initial turmoil, exploration, role adjustment, and acclimation. To manage the role of a prop, participants utilized various action/interactional strategies such as overcoming the unfamiliarity, overseeing the nursing home care, and counterbalancing the caring roles. As a result, participants experienced ambivalence towards the existence of parents with dementia, changes in family relationships, altered viewpoint towards nursing homes, and restructuring of life. Conclusion: In-depth understanding of the experience will guide nurses to promote effective interventions in order to better support the Korean family caregivers of parents institutionalized with dementia.
The purpose of this study was to clarify spatial composition of the staffs at nursing homes for the elderly with dementia. For this, the data was taken from the staffs in 2 nursing homes in Tokyo by the questionnaire from December4 to 28 in 2004. Replicated MDS (Multi-dimensional Scaling) was employed for data analysis. The results of the study were following. Staffs perceived nursing home by four spatial compositions; facility spaces as like sanitation room, laundry, bathroom, washroom, washbowl, social interaction spaces as like training room, inner court/balcony, living room, staff related spaces as like room for staff, treatment room, fundamental living spaces as like bedroom, dining room. According to two-dimensional perceptual map, sanitation room was wanted near by laundry, treatment room was wanted near by room for staff, bedroom was wanted near by dining room, and training room, living room were wanted near by inner court/balcony. On the other hand, washbowl and washroom were desired somewhere near by every spaces. Therefore, simple shaped washbowls are desired to install in dining room, living room, hallways. And sanitation room and laundry were desired somewhere remote by dining room, bedroom, and living room. Responding upper results, it is recommended planning for visual and sense of smell isolation in case of having difficulty in floor structure and circulation.
Purpose: We aimed to describe and compare the cognitive function, anxiety, and quality of life for the demented elderly in day care centers or nursing homes. Methods: We selected 44 subjects from day care centers and 63 subjects from nursing homes by performing convenience sampling. The Korean version of Mini Mental Status Examination was used for measuring cognitive status; Rating Anxiety in Dementia, for assessing anxiety; Korean Version of Quality of Life-Alzheimer's Disease, for assessing the quality of life. The data were collected from May to June 2008 and analyzed by SPSS/WIN 14.0. Results: 1) The cognitive function of the subjects in the day care center was higher than that of the subjects in the nursing home. Anxiety in the subjects in the nursing home was higher than that in the subjects in the day care center. 2) The cognitive function and quality of life of the subjects in the day care center showed statistically significant correlation, 3) The anxiety and quality of life of the subjects in the nursing home showed statistically significant correlation. Conclusions: Nursing interventions that improve the quality of life of the demented elderly should be considered and applied according to the degree of cognitive function and anxiety.
The purpose of this study was to identify the participation in activities, emotions, and problem behaviors of elderly people residing in nursing homes. In this observational study targeting 81 elderly patients with dementia, each patient's participation in activities, emotion, and problem behaviors were observed for 4 hours per day with 20 minutes interval, which leads to 6,804 data based on 12 times a day for 7 days. According to this study, 90.1% of the participants were suffering from severe cognitive impairment. Their emotions were scored 4 out of 7. They mainly participated in activities such as sitting or lying down, which had no specific purpose, but they showed the most positive emotion when involved in social activities. The most frequent types of problem behaviors were repetitive behaviors and making noises, and the frequency of their behaviors were different depending on the time. There were significant correlation between cognitive functions, emotions, and problematic behaviors. Therefore, it is necessary to find effective ways to increase participation in activities, induce participation in positive emotional activities, and reduce problem behaviors manifested most prominently each hour of the elderly patients with dementia residing in nursing homes.

This study aims to obtain basic data to develop clothes for dementia patients by classifying types of dementia patients. Data was collected from those dementia patients who entered a nursing home. This study analyzed a total of 221 sheets. Furthermore, descriptive statistics, cross-tabulation, and K-means clustering were performed for statistical processing using Minitab 14. As a result, dementia patients were classified into four types: inactive-dependent, active-problematic behavior, activity-autonomy, and inactive-offensive. Inactive-dependent type was a group with the most severe disability in cognitive and daily activity functions; however, they lacked behavioral and psychological symptoms and problematic behavior with clothes. Active-problematic behavior type showed the most behavioral and psychological problems and problematic behavior with clothes. Activity-autonomy type was a group without any problematic behaviors. Moreover, the inactive-offensive type had very good cognitive function toward humans. The study imply that it is necessary to provide clothes with proper functions based on the types of patients rather than providing them uniform clothes because clinical and clothes behaviors differ significantly depending on the types of dementia patients.
