농촌 지역사회노인의 만성질병수와 치매예방행위에 미치는 치매인식의 매개효과 (The Mediating Effect of Dementia Recognition on the Number of Chronic Diseases and Dementia Prevention Behaviors of Elders in Rural Communities)

  • Purpose: This study was a descriptive study to identify the factors affecting the dementia prevention behavior of elders in rural communities. Methods: The participants in this study were 125 elders aged 60 or older who lived in Eup or Myeon areas of P city. For data analysis, SPSS/WIN 22.0 was used to perform descriptive statistics, t-test, ANOVA, Pearson correlation, and linear multiple regression and mediated effects. Results: Scores for dementia recognition, dementia attitude and dementia prevention behavior averaged 5.6±2.50 points in the 0~11 range, 38.8±4.59 in 14~56 and 20.2±3.59 in 10~30 respectively. Dementia recognition (a), dementia attitude (b), dementia prevention behavior (c) and the number of chronic diseases of the elders (d) were positively or negatively correlated with each other (rab=.29, p<.01; rbc=.26, p<.01; rac=.36, p<.01; rad=-.29, p<.01; rcd=.19, p<.05). Factors affecting dementia prevention behavior were dementia recognition, dementia attitude, and degree of dementia interest. When the number of chronic diseases affects dementia prevention behavior, dementia recognition has a mediating effect. Conclusion: In order to prevent dementia among elders in rural areas, appropriate management of chronic diseases and provision of appropriate dementia-related education and information to enhance dementia recognition should be provided.

치매지식, 자기효능감 및 우울이 노인부부의 치매예방행위에 미치는 영향: 커플관계자료 분석방법적용 (Effects of Dementia Knowledge, Self-Efficacy and Depression on Dementia Preventive Behavior in Elderly Couples: Dyadic Data Analysis)

  • Purpose: This study was done to examine actor and partner effect of dementia knowledge, self-efficacy and depression on dementia preventive behavior in elderly couples. Methods: Participants were 115 couples aged 60 years or over who met eligibility criteria. All measures were self-administered. Data were analyzed using SPSS 18.0 and AMOS 18.0 program. Results: Dementia knowledge in elderly couples showed actor and partner effect on dementia preventive behavior. Self-efficacy in the wife did not have direct effects on dementia preventive behavior, but showed indirect effects through dementia knowledge. Self-efficacy in the husband showed direct effects on dementia preventive behavior and indirect effects through dementia knowledge. Wife's depression had direct actor effect on dementia preventive behavior and indirect effect through self-efficacy and dementia knowledge. Husband's depression did not have direct actor effect on dementia preventive behavior, but indirect effect through self-efficacy and dementia knowledge. Effect size of wives' dementia knowledge, self-efficacy and depression on dementia preventive behavior was larger than that of husbands'. Dementia preventive behavior, dementia knowledge and depression had a mutual effect. Conclusion: Results indicate that to promote dementia preventive activity in elderly couples, programs should be conducted for both of the couple, but focused differently for wife and husband.

요양병원 간호사의 치매 행동심리증상 부담감, 치매태도 및 감정노동이 돌봄행위에 미치는 영향 (Impacts of Burden from Behavioral Psychological Symptoms of Dementia, Dementia Attitude, and Emotional Labor on Caring Behavior of Nurses in Long-term Care Hospital)

  • Purpose: This study was a descriptive research to identify the relationships among burden from behavioral psychological symptoms of dementia, dementia attitude, emotional labor and nurse's caring behavior as well as to clarify factors that affect caring behavior of nurses in long-term care hospital. Methods: For the research, this study collected data from July 8 to July 20, in 2019 with 214 nurses who worked at 10 long-term care hospitals in Chungbuk. The data were analyzed using descriptive statistics, t-tests, Mann-Whitney U test, ANOVA, Pearson's correlation coefficient, stepwise multiple regression with the SPSS 23.0 program. Results: Factors influencing caring behavior of nurses in long-term care hospital were dementia attitudes (β=.549, p<.001), burden from behavioral psychological symptoms of dementia (β=.242, p<.001) and marital status (β=.134, p=.017). These factors explained 33.9% of caring behavior (F=37.35, p<.001). Conclusion: Dementia attitudes were confirmed to be the main variable influencing nurse's caring behavior. Therefore, to improve the caring behavior of dementia patient, it is considered that the development of programs to improve dementia attitudes and in-depth understanding regarding the behavioral and psychological symptoms of dementia is necessary.

저소득층 중년여성의 치매 지식, 태도, 자기효능과 예방행위와의 관계연구 (Relationships between Dementia Knowledge, Attitude, Self-Efficacy, and Preventive Behavior among Low Income Middle-Aged Women)

  • Purpose: This correlational study was to examine the relationships between dementia knowledge, attitude, self-efficacy and preventive behavior of low income middled-aged women. Methods: The subjects for this study were 125 low income middle aged women living in I city. The data were collected using the questionnaires for dementia knowledge, attitude, self-efficacy and preventive behavior. The data analysis was done by descriptive statistics, t-test, ANOVA, Pearson product moment correlation and stepwise multiple regression analysis. Results: The mean of dementia knowledge was 13.96 out of 20, attitude was 43.98 out of 60, self-efficacy was 54.07 out of 75 and preventive behavior was 25.98 out of 36. The positive correlations were revealed between dementia knowledge (r=.458, p=.000), attitude (r=.498, p=.000), self-efficacy (r=.573, p=.000) and preventive behavior. The influencing factors for dementia preventive behavior were self-efficacy, belief in Buddhism and attitude which accounted for 42.5% of the total variance. Conclusion: Dementia knowledge, attitude and self-efficacy were identified as variables that correlate dementia preventive behavior. Also, self-efficacy is the most influential factor affecting dementia preventive behavior. On the basis of these results, it is necessary for nurses to consider using dementia knowledge and mode of efficacy expectation in order to improve dementia preventive behavior.

중년층의 일반적 특성에 따른 치매정책인식, 치매지식, 치매태도 및 치매예방행위 차이분석 (Analysis of the Difference between Dementia Policy Perception, Dementia Knowledge, Dementia Attitude and Dementia Prevention Behavior according to the General Characteristics of Middle-aged)

  • 본 연구는 중년층의 일반적 특성에 따른 치매정책인식, 치매지식, 치매태도 및 치매예방행위 차이분석을 위한 것이다. D시와 G시에 거주하는 40세에서 64세까지 중년층 217명을 대상으로 하였고, 구조화된 설문지로 자가설문조사를 수행하였다. 연구결과 대상자의 치매정책인식 정도는 75.42±8.52, 치매지식은 8.71±1.90, 치매태도는 28.55±5.10, 치매예방행위는 27.04±3.35이었다. 대상자의 일반적 특성에 따른 치매정책인식은 유의한 차이가 없었고, 치매지식은 성별, 질환 수, 치매정보 원천에서 유의한 차이가 있었으며, 치매태도는 성별, 나이, 학력, 가족 중 치매, 치매가족 동거, 직업상태, 조기검진, 치매정보 원천에서 유의한 차이가 있었다. 또한 치매예방행위는 치매가족 동거, 조기검진에서 유의한 차이가 있었다. 치매예방행위의 세부항목 순위는 '당신은 담배를 피우십니까?(역문항)'가 가장 높았고, '당신은 평상시 혈압이나 당뇨관리 등 질환관리를 하십니까?'는 가장 낮은 것으로 나타났다. 따라서 중년층의 치매정책인식을 높이고 올바른 치매지식, 긍정적 치매태도를 형성할 수 있는 전문적이고 체계적인 치매교육 프로그램 개발과 인지활동, 사회활동, 올바른 식생활 습관, 건강관리 등을 형성 및 유지할 수 있는 맞춤형 치매예방행위 프로그램 개발이 필요하다.

요양보호사의 치매태도와 돌봄행위 간의 관계에서 공감역량의 매개효과 (Relationship between Caregiver's Dementia Attitude and Care Behavior: The Mediating Effect of Compassion Competence)

  • Purpose: This study aimed to investigate compassion competence and its mediating effects on the relationship between attitudes toward dementia and care behavior among caregivers of older adults with dementia in nursing homes. Methods: 211 participants were recruited for this cross-sectional study. Data were collected from February 1st to May 24th, 2023, through a structured questionnaire and analyzed using multiple linear regression analysis and the bootstrapping method. Results: The mean score for care behavior was 3.67±0.97 (range 1~5). Caring behaviors were significantly associated with attitudes toward dementia (r=.76, p<.001) and compassion competence (r=.75, p<.001). Using Baron and Kenny's approach and PROCESS Macro model 4, care behavior was directly affected by dementia attitude (β=.76, p<.001). Compassion competence (β=.66, p<.001) was directly affected by dementia attitude. The partial mediating effects of compassion competence (β=.47, p<.001, 95% confidence interval 0.10~0.23) on the impact of dementia attitude on care behavior was confirmed. Conclusion: The results suggest that compassion competence should be considered when developing nursing interventions to enhance dementia-related attitudes and improve care behavior among caregivers of older adults with dementia.

Factors Influencing Care Behavior of Certified Care Assistants for Older Adults with Dementia

  • Purpose: Korea is one of the most rapidly aging countries, accompanied by an increasing number of older adults with dementia. Certified-care-assistants are the primary care workers for these older adults. This cross-sectional descriptive study compared the factors influencing care behaviors among Korean and non-Korean certified-care-assistants in Korea. Methods: Participants, 113 Korean certified-care-assistants and 71 non-Korean certified-care-assistants, completed a survey questionnaire. The questionnaire was designed to assess dementia knowledge, attitude and care behaviors toward older adults with dementia. To determine the differences in these two groups, t-test was used. A stepwise multiple regression was performed to identify factors related to care behavior. Results: Significant differences in dementia knowledge, attitude and care behavior toward older adults with dementia emerged between Korean and non-Korean certified-care-assistants. Factors influencing care behavior varied by group. Dementia knowledge and education level were the significant factors of care behaviors in Korean certified-care-assistants, whereas attitude toward older adults with dementia was the significant factor in non-Korean certified-care-assistants. Conclusion: Findings indicate that different factors influence the care behavior of two groups. These findings can be utilized to develop certified-care-assistants' training curriculum that needs to be tailored by nationality to improve the quality of care for older adults with dementia.

치매환자의 의복행동에 관한 연구 (A Study on the Clothing Behavior of Dementia Inpatient)

  • This research explored the clothing behavior of dementia inpatient according to the dementia severity, dementia type and demographic characteristics. Data were collected by surveying 149 dementia inpatients and their caregivers. The collected data were analysed through frequency analysis, $X^2$-test, one way ANOVA, Duncan's multiple range test and t-test. The results were as follows: First, the dementia severity was classified into the severe, moderate and mild dementia by K-MMSE and the dementia types were composed of Alzheimer's disease, vascular dementia and the others. The severe dementia group and Alzheimer's disease group included the more aged and more female inpatients. And the severe dementia group was lower self-support of the basic activities of daily living than the mild dementia group. Second, the change motions of the dementia inpatient's clothing differed from the clothing item and dementia severity. That is, the motions for pants were more difficult than those for upper garment. And the motions for severe dementia group were more difficult than those for the mild dementia group. Third, there were significant differences among the three groups by dementia severity and between the male and female dementia inpatient in the clothing behavior. Most of abnormal clothing behaviors were found in the severe dementia group. The positive clothing behaviors appeared in the mild dementia group. And interests of clothing and appearance appeared in the female inpatients.

고혈압과 당뇨병 건강 문제를 가진 성인의 치매 예방 행위 의도에 미치는 영향 요인 (The Factors Influencing Intention of Dementia Prevention Behavior in Adults with Hypertension or Diabetes)

  • Purpose: The purpose of the present study is to examine the factors influencing intention of dementia prevention behavior in adults with hypertension or diabetes. Methods: A total of 113 community-dwelling adults with hypertension or diabetes agreed to participate in this study. Data were analyzed using descriptive statistics, t-test, ANOVA, correlation, and multiple regression. Results: Self-efficacy and cues to action toward dementia prevention behavior were factors influencing intention of dementia prevention behavior of participations. Self-efficacy toward dementia prevention behavior was the most influential factor. These factors accounted for 22% of intention of dementia prevention behavior. Conclusion: These results suggest a need for strengthening self-efficacy to increase intention of dementia preventive behaviors. And dementia prevention programs should be developed in consideration of strategies to reinforce cues to action.

노인의 치매예방 행위의도에 미치는 영향요인 (Factors Influencing Dementia Preventive Behavior Intention in the Elderly People)

  • Purpose: The purpose of this study was to identify the factors influencing dementia preventive behavior intention of the elderly people based on the Health Belief Model. Methods: The participants included 113 elderly people who met the eligibility criteria. Demographic variables, variables of the Health Belief Model (perceived susceptibility, perceived severity, perceived benefit, perceived barrier, cues to action, general health motivation, and self efficacy), dementia fear and behavioral intention of dementia prevention were examined using structured self-report questionnaires. Statistical analysis was performed by stepwise multiple regression using SPSS for Windows version 21. Results: Self efficacy, alcohol drinking, perceived barrier and education level were significant factors, which explained 32% of the variance in dementia preventive behavior intention. Multiple regression analysis demonstrated that a powerful predictor of dementia preventive behavior intention of the elderly was self efficacy. Conclusion: Developing nursing intervention to enhance self efficacy toward improvement of dementia preventive behavior among elderly people would be recommended.
