The Study on the Korean Traditional Medical Treatment and System of Collaborative Practice Between Korean Traditional Medicine and Western Medicine for Dementia: Based on Analysis of Questionnaire Survey in Professional Group (치매의 한방치료와 한양방 협진치료에 관한 전문가 집단 설문연구)

  • Objectives : Recently, the elderly population and dementia prevalence have been rapidly increasing in Korea. Dementia is a concept of syndrome composed of characteristic symptoms. However, there have been no approved treatments for dementia with proven efficacy. Therefore, we investigated Korean traditional treatment being used in clinic and collaborative practice for dementia in the professional group. Methods : We conducted a survey using questionnaires to professors of oriental neuropsychiatry for a month. Items of the questionnaire consisted of overall Korean traditional medical treatment for dementia, which included herbal medicine, acupuncture, moxibustion, cupping, psychotherapy and others. Results : 1. It was shown that Yukmijihwangtanggamibang, Jowiseungcheongtang, Seonghyangjeonggisan have been used the most in herbal medicine. 2. Chechim, Saamchimbeop, and scalp acupuncture have been used the most in acupuncture treatment. 3. In Korean traditional psychotherapy, Ijeongbyeongi, Jieongoronyobeop, Gigongyobeop have been used, and effective in anxiety, agitation, improvement of comprehension for caregiver and environment. 4. Most professors answered that collaborative practice between Korean traditional medicine and western medicine has been needed for dementia. They suggested that herbal medicine, followed by acupuncture, and moxibustion can be used with western medicine simultaneously. Conclusions : The survey revealed that professors use herbal medicine and acupuncture as main treatments for dementia. They share perspective regarding the need of long period to treat dementia patients, and there are needs of collaborative practice for dementia.

A Study on Differences in the Caregiving Burden of Primary Caregivers by Type of Caregiving -Focused on Caregivers in Home Stay, Day Care Center, and Nursing Home Situations- (부양형태에 따른 주부양자의 부양부담의 차이 및 영향 변수에 관한 연구 -재가, 주간보호시설, 시설거주 노인의 주부양자를 대상으로-)

  • This study investigates differences by type of caregiving using data on three groups of caregivers, 177 from home stay situations, 189 from day care centers, and 138 from nursing homes. First, the result shows that characteristics of both caregivers and elders differ by type of caregiving. Second, caregivers in the home stay situation have the highest caregiving burden of the three groups. Third, from examination of the related variables, the caregiving burden of home stay caregivers is affected by family income, caregivers' health, type of job, and whether or not the elder has symptoms of dementia. Also, the study reveals that emotional services for elders reduce the aggravation of family relations and economic burden, but that instrumental services highly increase economic burden. It reveals that caregivers of elders in day care centers, especially those who are in bad health, are more likely to experience feelings of constriction, aggravation of family relations and economic burden. On the other hand, caregivers who receive more emotional services have better experiences in family relations, including relations with the elder. In case of the elders of nursing homes, if the main caregiver is a daughter-in-law, aggravation of family relations is higher than if the main caregiver is a spouse. Finally, the caregiver's burden is affected by their own health and income, and by whether the elder has symptoms of dementia or stroke.

The Relationship among Perceived Entrapment, Depression and Subjective Well-being of Women as Family Caregivers Caring for Dementia Elderly (치매노인을 돌보는 여성가족수발자의 지각된 속박감과 우울 및 주관적 안녕감의 관계)

  • Purpose: This study was designed to examine the relationship between perceived entrapment to depression and subjective well-being of women as family caregivers caring for elderly dementia patients. Methods: One hundred and sixty-nine women family caregivers were recruited from two high schools located in Seoul, Korea for this descriptive study. The instruments used were The Entrapment Scale, The Center for Epidemiology Studies Depression Scale (CES-D) and Subjective well-being scale. Results: The score of perceived entrapment significantly correlated with depression and subjective well-being. The significant predictor of depression in women caregivers was perceived entrapment, 50.3% of the variance in depression. Also, perceived entrapment was predictor of subjective well-being in women caregivers, explaining 41.4% of the variance in depression. Conclusion: This study showed that perceived entrapment is an important predictor for depression and subjective well-being. Therefore, in order to reduce depression in women caregivers, it is necessary to design an intervention program that helps with coping and reduces perceived entrapment.

  • This study aimed to investigate the role of social support to enhance posttraumatic growth in the Family Caregivers focusing on their trauma by development of the elderly with dementia. The study subjects were 204 family Caregivers for the elderly with dementia and the data were analyzed by the gender. This study utilized IBM SPSS Statistics ver. 20.0 for analysis, and the main results of this study were as follows. First, female group of the family Caregivers showed significantly higher in the level of trauma while male group showed significantly higher in the informal areas of social support and posttraumatic growth. Second, female group showed the protective effect in the formal area of social support and male group did the effect in the formal and informal area of social support. As such, this study investigated the level of trauma by gender of family Caregivers for the elderly with dementia and suggested the implementation strategies to enhance posttraumatic growth. Moreover, this study has the meaning to provide the required fundamental data to establish proper community care upon demonstrating the practical analysis results considering the characteristics of social support.

An Analysis of the Meaning of Respite for Family Caregivers of Elderly with Dementia (포커스그룹 인터뷰를 통한 치매노인 가족수발자의 휴식에 대한 의미분석)

  • Purpose: This study was a qualitative research study in which focus group interviews were used to collect data on the meaning of respite for family caregivers who are taking care of elders with dementia. Methods: The focus group interviews and participants consisted of 2 family caregiver groups, for a total of 8 people taking care of their elders and 5 professional caregivers working in a geriatrics hospital or social welfare institutions. Content analysis was used and debriefing notes were referred in order to analyze the data. Results: The meaning of respite in this research was measured using 4 main categories: 'Temporary break from routine', 'direct help', 'psychological comfort', 'valuables which cannot be taken easily' and 9 subcategories: 'Temporary diversion of attention', 'temporarily free from my duty', 'taking care of oneself', 'receiving economic help', 'empathize with others', 'comfort based on trust', 'resting together with the elder', 'no time to rest', 'cannot get out of one's obligatory duty'. Conclusion: The findings of the study show that rest means not only a temporary relief from caretaking, but also a real respite based on the patients' stable state and comfort. These results indicate a new meaning for respite, that the first step of respite program has to begin even when the caregivers do not recognize the need for respite.

An Investigation on the Status of Care-giving for the Demented Elderly and Desire for In-house Care Services (치매노인의 부양실태와 재가복지서비스 욕구조사)

  • Purpose: The purpose of this study is to examine the status of care-giving for the demented elderly and to find out the need of community in-house care services-day care services, short-term care services, home help services, and home care services. Method: It analyzed the data of 186 old people having dementia, and caregivers. Data were collected for five days, in September 2002. Results: The caregivers were mostly women and the burden for the care giving was high (87.5%). They used community care services, that is, day care services (26.5%), home care services (21.6%), home help services 00.8%), short-term care services (6.2%). Caregivers' age and education level were significant factors in the demand for day care services. Caregivers' education level was a statistically significant factor in the demand for short-term care services and home help services. Caregivers' age and education level were significant factors in the demand for home care services. Conclusions: It is necessary to expand the financial aid for the active implementation of daytime protection for dementia-patients under medical treatment at home and to promote patients' recovery. It is necessary to enhance home help services and home care services, and to establish many day care centers and short term care centers. Through this, it will prevent caregivers from becoming burnt out due to the burden of care giving.

Deep Learning-based Abnormal Behavior Detection System for Dementia Patients (치매 환자를 위한 딥러닝 기반 이상 행동 탐지 시스템)

  • The number of elderly people with dementia is increasing as fast as the proportion of older people due to aging, which creates a social and economic burden. In particular, dementia care costs, including indirect costs such as increased care costs due to lost caregiver hours and caregivers, have grown exponentially over the years. In order to reduce these costs, it is urgent to introduce a management system to care for dementia patients. Therefore, this study proposes a sensor-based abnormal behavior detection system to manage dementia patients who live alone or in an environment where they cannot always take care of dementia patients. Existing studies were merely evaluating behavior or evaluating normal behavior, and there were studies that perceived behavior by processing images, not data from sensors. In this study, we recognized the limitation of real data collection and used both the auto-encoder, the unsupervised learning model, and the LSTM, the supervised learning model. Autoencoder, an unsupervised learning model, trained normal behavioral data to learn patterns for normal behavior, and LSTM further refined classification by learning behaviors that could be perceived by sensors. The test results show that each model has about 96% and 98% accuracy and is designed to pass the LSTM model when the autoencoder outlier has more than 3%. The system is expected to effectively manage the elderly and dementia patients who live alone and reduce the cost of caring.

Characteristics of Caregivers and Services about the Adult Day Care Participants with Dementia in the U.S.A. (치매노인을 위한 주간보호시설 이용자의 가족부양현황과 서비스 이용에 관한 연구 -미국의 사례를 중심으로)

  • The purpose of this study was to understand the adult day care as a place for the elders and adults and to develop an initial understanding of the programs and their participants for adult day care for the cognitively-impaired in the U.S.A. The data was collected from 13 Adult Day Care Centers(ADC), and 318 participants from six Adult Day Care Centers from 2001 to 2002 by personal interview and the documents about the participants and their family and caregivers. Participants used ADC program average 8.15 hours In weekdays, 3 days per week. ADC programs provided primarily lunch and snack, transportation, personal care, professional health care, occupational.speech physical therapies, rehabilitation, and respite care. Participants'caregivers were mainly daughters and wives. It is the hope of this study to provide design and care professionals with a first draft of a ″sense-making″template by which they may understand adult day care in a systemic manner and engage in meaningful results as to what this place type could and should be.

Communication Aid System For Dementia Patients (치매환자를 위한 대화 보조 시스템)

  • The goat of the present research is to improve the quality of life of both the elderly patients with dementia and their caregivers. For this Purpose, we developed a communication aid system that is consisted of three modules such as speech recognition engine, graphical agent. and database classified by a nursing schedule. The system was evaluated in an actual environment of nursing facility by introducing the system to an older mail patient with dementia. The comparison study was then carried out with and without system, respectively. The occupational therapists then evaluated subject"s reaction to the system by photographing his behaviors. The evaluation results revealed that the proposed system was more responsive in catering to needs of subject than professional caregivers. Moreover we could see that the frequency of causing the utterances of subject increased by introducing the system.

A Study on the Effect of Caregiving Stress to Partner Violence among Adult Children Caregiver for the Elderly with Dementia : Mediating Effect of Depression (치매환자 자녀의 부양부담이 배우자폭력에 미치는 영향 - 우울의 매개효과 검증 -)

  • The purpose of this study was to examine the relationships among caregiving stress, depression, and partner violence by gender. Data were gathered though questionnaires surveying 223 cases living in the Seoul and Kyeonggi, Pusan areas. In analyzing precess, the researchers used t-test, ANOVA, correlation and regression. Additionally, bootstrapping was used to verify the significant mediating effect of depression. The findings are follows: First, approximately 37.3 percent of adult children caregivers reported having experienced partner violence in the past year. And the depression mean score among adult children caregivers was 1.00, higher compared to 0.73 of general population. Female caregiving burden and depression level was higher than those of male. Second, the results from regression analysis revealed that caregiving stress influenced to partner violence positively. Depression has the full mediating effect between caregiving stress and partner violence. The results suggest a necessity of intervention to reduce caregiving stress, depression and prevent partner violence in the dementia caregiver's family. The implications for social work practice were also discussed.
