Evaluating the Accuracy of Blood Pressure Measurement (혈압측정의 정확성 평가)

  • Background : Blood pressure is an important indicator in diagnosis and assessing treatment of a patient. Clinical staffs use blood pressure on the assumption that measured value is accurate and reliable. However, whether measured blood pressure is accurate has been rarely investigated in Korea. Objectives : The aims of this study are to evaluate clinical staffs' knowledge and technique as well as accuracy of sphygmomanometer. Also the program to improve the measurement is developed. Methods : Seventy-three registered nurses were asked nine multiple choice questions including Korotkoff sound, cuff size, and deflation rate. Simultaneously characteristics of nurses were examined, age, working place, duration of employment and academic degree. A testing videotape(Standardizing Measurement Video-Tutored Course) was used for evaluating the accuracy of measurement. Testees were to read and record the 12 cases of blood pressure measurement, watching a falling mercury column and hearing Korotkoff sounds. After 10 minutes' education, they were again tested with the same cases. Additionally, 83 mercury sphygmomanometers were checked to find defects such as inaccurate calibration and zero setting, leaky bladder, etc. Results: For the knowledge testing correct response rate was 41.1%. They were the lowest in selecting the proper cuff size and Korotkoff sound. In examining accuracy of blood pressure with videotape, nurses had 67.7% correct response rate. The correct response rate was significantly improved by a session of education. About 23% of sphygmomanometers was without discernable defects. Conclusion : The knowledge and skill of clinical staffs along with the accuracy of equipment have to be improved. A properly designed education program would contribute to the accuracy improvement of blood pressure measurement. Also, more concerns should be given to the precision and maintenance of equipment.

Plurality and intrinsic value of Truth (진리의 다원성과 본래적 가치)

  • This paper research an answer to the question what is the relation between intrinsic value of truth and substantiality of truth. The concept of substantiality can be variably defined, but I characterize it as being an opaque concept. Michael P. Lynch, who is one of the most influential pluralists about truth, accept both that truth is substantial and that truth is intrinsically valuable. This paper analyzes Lynch's argument for the intrinsic value of truth, and will show that his argument is independent of his theory which insists that truth is manifested plurally. Moreover I will discuss how Paul Horwich who is a deflationist denying substantiality of truth explains the intrinsic value of truth. And comparing these two views, I will elaborate the reason why intrinsic value of truth and its substantiality are logically independent each other. Finally, I will suggest a brief case which shows an implication included in the conclusion that plural manifestation of truth does not generally entails plurality of value of truth.

Balloon Valvuloplasty for Congenital Pulmonary Valve Stenosis (풍선 카테터에 의한 폐동맥 판막 성형술)

  • Percutaneous pulmonary valvuloplasty was performed in 19 patients of congenital pulmonary valve stenosis. Pulmonary annulus diameter was estimated by cross sectional echocardiography and right ventricular cineangiography. The size of balloon dilatation catheter was chosen by the same size of the estimated pulmonary annulus in the first 3 patients and 20 \ulcorner30% greater than the annulus in the last 17 patients. After valvuloplasty a satisfactory results was obtained in most patients. Before dilatation, the right ventricular systolic pressure was 91.7 mmHg[range 58-150 mmHg] and it fell to 49.2mmHg[25-85 mmHg] after dilatation. The transvalvular gradient was 67.7 mmHg[33 \ulcorner120 mmHg] before dilatation and it fell to 23.7mmHg [5 \ulcorner62] after dilatation. Repeat cardiac catheterization has been scheduled in all patients 3 months after the initial valvuloplasty but follow up recatheterization was performed in only two patients; in one of them residual gradient of 50 mmHg was reduced to 30 mmHg by repeat valvuloplasty. The other patient showed no evidence of restenosis with transvalvular gradient of 20 mmHg The balloon used for valvuloplasty was single balloon for the first 10 cases and for the later 10 cases it was replaced by Trefoil balloon which was easier for inflation and deflation. There were no significant complications during and after the procedure. From our results, we conclude that balloon valvuloplasty for congenital pulmonary valve stenosis is the treatment of choice in most patients.

  • A single dose of pilocarpine was subcutaneously injected into the rabbit in order to clarify the effects of cholinergic activities on the secretion of the pulmonary surfactant. The animal was sacrificed four hours after the injection, and the pressure-volume curve of the excised lung was obtained. The surfactant activities of the lung washings were also measured, and a comparison was made with the normal. The pressure-volume curve of the excised lung by the injection and ejection of air formed a loop (hysteresis) and the mean intra-alveolar pressure at 35 ml of air was 220 $mmH_20$ in the normal. The mean deflation curve of the pilocarpine treated rabbit lungs was shifted to the left compared with the normal. The surfactant activities were slightly increased than the normal. The above results suggest that pilocarpine stimulates the secretion of the pulmonary surfactant from the Type II alveolar epithelial cell.

Surface deformation monitoring of Augustine volcano, Alaska using GPS measurement - A case study of the 2006 eruption - (GPS를 이용한 미국 알래스카 어거스틴 화산의 지표변위 감시 - 2006년 분화를 중심으로 -)

  • Augustine is an active stratovolcano located in southwest of Cook Inlet, about 290 kilometers southwest of Anchorage, Alaska. Between January 11 and 28, 2006, the volcano erupted explosively 14 times. We collected twelve permanent GPS stations operating by Plate Boundary Observatory (PBO) from 2005 to 2011. All data processing was carried out using Bernese GPS Software V5.0 with IGS precise orbit. Static baseline processing by fixing AC59 station was applied for the volcano activity monitoring. AC59 is the nearest (about 24.5 km) station to Augustine volcano, and located on North America Plate including Augustine Island. The test results show inflation (9.7 cm/yr) and deflation (-9.2 cm/yr) of volcano before and after eruption around crater clearly. After volcano activity has reached a plateau, some of the GPS stations installed north of the volcano show ground subsidence phenomenon caused by compaction of pyroclastic flows. These results indicate the possibility of using surface deformation observed by GPS for monitoring and prediction of volcano activity.

A Study on the Change of Magma Activity from 2002 to 2009 at Mt. Baekdusan using Surface Displacement (지표변위를 활용한 백두산의 2002-2009년 마그마 활동 양상 변화 연구)

  • There have been a number of observed precursors of volcanic activities- such as volcanic earthquake, surface inflation, specific volcanic gas emission, temperature of hot spring- at Mt. Baekdusan since 2002. We identified the increase of the volume of magma chamber beneath Mt. Baekdusan as we observed an inflation trend of vertical and horizontal surface displacement around Cheonji caldera lake by using precise leveling data from 2002 to 2009. The surface displacement trend changed to deflation in 2010, and the trend changed to inflation again after a while. Utilizing the data of inflated surface (46.33 mm) on the northern slope of Mt. Baekdusan from 2002 to 2003, we calculated the volume change of magma chamber beneath the Mt. Baekdusan. The volume change was about 0.008 $km^3$ ($7.7-8.0{\times}10^6m^3$) from 2002 to 2003. It indicated that a new magma (0.008 $km^3$) injected to the magma chamber 5 km below Mt. Baekdusan.

Similarity between the dispersion parameter in zero-altered model and the two goodness-of-fit statistics (영 변환 모형 산포형태모수와 두 적합도 검정통계량 사이의 유사성 비교)

  • We often observe count data that exhibit over-dispersion, originating from too many zeros, and under-dispersion, originating from too few zeros. To handle this types of problems, the zero-altered distribution model is designed by Ghosh and Kim in 2007. Their model can control both over-dispersion and under-dispersion with a single parameter, which had been impossible ever. The dispersion type depends on the sign of the parameter ${\delta}$ in zero-altered distribution. In this study, we demonstrate the role of the dispersion type parameter ${\delta}$ through the data of the number of births in Korea. Employing both the chi-square statistic and the Kolmogorov statistic for goodness-of-fit, we also explained any difference between the theoretical distribution and the observed one that exhibits either over-dispersion or under-dispersion. Finally this study shows whether the test statistics for goodness-of-fit show any similarity with the role of the dispersion type parameter ${\delta}$ or not.

Typology of Deteriorated Hiking Trails in Mountain National Parks of Korea (산악 국립공원 등산로의 훼손 유형과 요인)

  • Hiking trails in Mt Jiri, and Mt Halla, National Park have been examined in terms of their degrading factors. The trails are deteriorated by natural erosion processes as well as human trampling. Trail deterioration is classified into tread lowering, sidewall retreat, path widening and divergence based upon a place where erosional processes occur. Tread lowering and sidewall retreat is generally produced by natural erosion factors, whereas path widening and divergence is generated by human trampling. Rainwash is the most contributing process to tread lowering. By contrast, several processes such as rainwash, needle ice action, deflation, tree falling and animal activity play a major role in sidewall retreat according to physical conditions of a hiking trail. Path widening and divergence could be classified by a factor producing human trampling. There are lots of cases related to rainwash such as the tree root, gravel, and bedrock, exposed by a surface flow lowering a tread and the riser produced by tread scouring. A puddle of rainwater on a flat tread and a fallen tree of Abies koreana in a forest region are also major factors to cause path widening and divergence. A paved tread with stones encourages a hiker to walk out of a trail. Taking a shortcut also results in path widening and divergence without a factor giving a hiker inconvenience on a trail.

An Analysis on the Effect of Japanese Monetary Policy in 21C (21c 일본 통화정책 효과에 대한 분석)

  • The expansionary monetary policy was practiced after 2001 in Japan to treat the deflation spiral, and reduced only the nominal interest rates and domestic household demand. One of the most serious factors for this failure was the change of private sector's expectancy. This paper has studied the effect of Japanese monetary policy in 21c., with empirical research based on a renewed macroeconomic model and the VAR. The empirical analysis shows that the effect of monetary policy on the national income during 2001.01-2015.03 is weaker than that of 1985.01-1994.04. Money volume has a diminutive effect on the growth of GDP within a short term after 2001. The change in the expectations of the private sectors might have been the cause of ineffectiveness of the expansive monetary policy. Economic agents learned from the past Japanese financial crisis that an expansive monetary policy increased the inflation rate and caused the 'bubbles to burst' afterwards. The VAR analysis says that the effectiveness of monetary policy on the economic depression declined over the past 20 years and the expansion of money volume has no influence on exchange rate and net export. This means that the expansive monetary policy lost its effect on net export and national income steadily. Monetary policy makers have to recognize this fact, and to consider another anti-cycle political instrument, i.e. the fiscal policy with government debt.

Sleep Deprivation Attack Detection Based on Clustering in Wireless Sensor Network (무선 센서 네트워크에서 클러스터링 기반 Sleep Deprivation Attack 탐지 모델)

  • Wireless sensors that make up the Wireless Sensor Network generally have extremely limited power and resources. The wireless sensor enters the sleep state at a certain interval to conserve power. The Sleep deflation attack is a deadly attack that consumes power by preventing wireless sensors from entering the sleep state, but there is no clear countermeasure. Thus, in this paper, using clustering-based binary search tree structure, the Sleep deprivation attack detection model is proposed. The model proposed in this paper utilizes one of the characteristics of both attack sensor nodes and normal sensor nodes which were classified using machine learning. The characteristics used for detection were determined using Long Short-Term Memory, Decision Tree, Support Vector Machine, and K-Nearest Neighbor. Thresholds for judging attack sensor nodes were then learned by applying the SVM. The determined features were used in the proposed algorithm to calculate the values for attack detection, and the threshold for determining the calculated values was derived by applying SVM.Through experiments, the detection model proposed showed a detection rate of 94% when 35% of the total sensor nodes were attack sensor nodes and improvement of up to 26% in power retention.
