A rare development of tumoral calcinosis of the ear auricle

  • Tumoral calcinosis is a condition characterized by deposition of calcium salts in the skin and subcutaneous tissue, commonly found around the joints. However, tumoral calcinosis of the auricle is extremely rare. We present the case of a 13-year-old boy with tumoral calcinosis of the helix of the ear auricle. A 13-year-old boy presented with a 10-year history of an enlarging mass on the left auricle. The mass was hard, non-tender, and non-compressible. The patient had no history of trauma. Complete surgical excision and pathological examination of the specimen was performed. The final diagnosis of the excised mass was tumoral calcinosis. After 9 months of follow-up, there were no signs of recurrence of the tumor and the patient was satisfied with the surgical results. Tumoral calcinosis of the auricle is extremely rare and may be misdiagnosed as other tumors. Pathological examination is essential for definitive diagnosis and complete surgical excision should be considered as the treatment of choice.

Primary non-Hodgkin lymphoma of the parotid gland: a case report

  • Most malignant lymphomas of the head and neck region are non-Hodgkin lymphomas (NHL), and diffuse large B-cell lymphoma is the most common subtype. The prevalence of malignant lymphoma among parotid tumors is low, approximately 1% to 4%. The most common symptom of parotid lymphoma is a unilateral, non-tender, firm mass that slowly grows in size over time. As its clinical manifestations are nonspecific, a comprehensive assessment is required for an accurate diagnosis. The initial work-up includes imaging tools, such as computed tomography and magnetic resonance imaging. However, NHL of the parotid gland is difficult to distinguish from other types of benign tumors prior to biopsy; histopathological evaluation and subsequent immunohistochemical staining are needed for the final diagnosis. Once a definitive diagnosis is established, patients should be referred to an oncologist for staging. Treatment is mainly based on systemic chemotherapy, whereas radiotherapy is indicated for certain cases. Here, we report the case of a 53-year-old man who presented with a progressively enlarging mass in the right parotid area, which was later diagnosed as malignant lymphoma of the parotid gland after superficial parotidectomy.

A Case of Solitary Fibrous Tumor Presenting as Lower Neck Mass (하경부 종물로 발현한 고립성 섬유종 1예)

  • Solitary fibrous tumor (SFT) is rare mesenchymal tumor usually arising from pleura. SFT can be found at all anatomic site in our body but incidence of SFT is much lower in head and neck region especially at lower neck area. We found a case of SFT that presented as a lower neck mass in a 41-year old woman. Ultrasonography showed a 3×1cm sized hypoechoic mass in the intermuscular fat plane of left lower neck, and computed tomography showed a well circumscribed, low-density mass with contrast enhancement. Fine needle aspiration showed no malignant cells with abundant red blood cells, but it was not possible to completely rule out malignant tumors or nodules clinically. Surgery was performed to make a definitive diagnosis and histopathology showed tightly packed, round to fusiform cells with staghorn shaped vessels at microscopic examination. The tumor cell were positive for CD34 but negative for CD31 and S-100 protein.

  • Flabby tissue is not rare for denture wearers. Mucostatic impression technique is necessary due to compromised retention and stability of denture resulting from distortion of mobile flabby tissue. In this report, individual tray was fabricated by model-scanning and 3D printing treatment denture. And then, mucostatic impression for flabby tissue was obtained by using individual tray modified with window technique. Definitive denture was fabricated based on information of treatment denture including incisal pontic arrangement, jaw relationship and occlusion.

Investigation of Wood Species and Conservation Status of Wooden Seated Amitabha Buddha Triad and Wooden Amitabha Buddha Altarpiece of Yongmunsa Temple, Yecheon, Korea (Treasure)

  • The Wooden Seated Amitabha Buddha Triad and Altarpiece have not been the subjects of definitive species identification and scientific analysis. In this study, visual investigation, portable X-ray fluorescence (p-XRF), species identification, and lacquer layer observations were carried out to determine the original materials and conservation status. Cracks, exfoliation and discoloration were detected during the visual investigation. The p-XRF data indicated that chrome oxide green, malachite, gold, cadmium red, cinnabar, minium, hematite, lead white, ink stick (Carbon), and copper were used for pigments and the coronet accessory. Tilia spp., Abies spp., and Pinus spp. were identified from both the Buddha Triad and Altarpiece. Finally, lacquer layer analyses of the base layer, lacquer layer, orange pigment layer, and gold leaf layer observed modern synthetic pigments likely used in previous conservation. As the Korean Cultural Heritage Charter and the International Charter for the Conservation and Restoration of Monuments and Sites clearly state that objects of cultural heritage must be conserved using their original materials, future conservation of these objects should utilize the data obtained in this study to employ traditional materials. Furthermore, a deterioration map diagnosis can be applied together with the obtained analysis data to understand the conservation status of and inform an appropriate and authentic conservation treatment for the Buddha Triad and Altarpiece.

An Analysis of the Efficiency of Agricultural Social Enterprises Using the Stochastic DEA Model (농업·농촌 기반 사회적기업의 부트스트래핑 효율성 분석)

  • This paper analyzes the efficiency of social enterprises by analyzing bootstrapping data envelopment analysis. Unlike the definitive DEA model, we analyze the confidence intervals of efficiency estimates through the DEA model, which takes into account stochastic factors. Major analysis results are summarized as follows: First, the results of the bootstrapping DEA analysis of social enterprises estimated that the technical efficiency was 0.459 and the 95% confidence interval was 0.389 to 0.601. Second, the number of inefficient social enterprises with efficiency values of less than 0.5 was found to be 15 (55.56%) in technical efficiency, 5 (18.52%) in pure technical efficiency, and 8 (29.63%) in scale efficiency. It can be seen that a significant number of social enterprises are operating in an inefficient state. Third, looking at the returns of scale of social enterprises, 25 (67.57%) are currently in the increasing returns of scale, 10 (27.02%) are in the constant returns of scale, and 2 (5.41%) are in decreasing returns of scale. In other words, it can be seen that social enterprises are under-invested in terms of input factors.

Primary malignant mucosal melanoma of the maxillofacial area

  • Objectives: We aimed to collect and report data from all patients who have been diagnosed with mucosal malignant melanoma to obtain the epidemiology and principles of current treatments. Materials and Methods: Between January 2008 and December 2018, 20 patients underwent surgery or follow-up observations at Yonsei University Dental Hospital. The patients' clinical information was reviewed retrospectively. Results: Seventeen of 20 patients had undergone definitive surgery, while only 6 patients received adjuvant radiotherapy or systemic therapy. Eight of 20 patients, including those that had recurrent lesions, were provided immunotherapy. The 3-year survival for all stages was 50%, with a local recurrence rate of 75% and a metastasis rate of 65%. Conclusion: The overall survival of patients receiving surgical treatment was longer than that of patients who did not undergo surgical resection. Eight of 20 patients received immunotherapy as the first-line regimen at our clinic, and those patients exhibited longer overall survival compared to patients in reported keynote studies.

A Case of Squamous Cell Carcinoma arising from an Odontogenic Keratocyst (우측 하악의 치성 각화성 낭종 수술 후 발견된 암으로 내원한 63세 환자 1예)

  • Odontogenic keratocyst (OKC) accounts for 3-11% of all odontogenic cysts. OKC is a benign intra-osseous odontogenic tumor, but what makes this cyst special is its aggressive behavior and high recurrence rate. OKC is relatively aggressive compared to other odontogenic cysts, but its malignant transformation is considered extremely rare. Squamous cell carcinoma associated with odontogenic keratocysts have rarely been reported in the medical literature. We recently experienced a case of a 63-year-old man finally confirmed with squamous cell carcinoma of the mandible, which was initially diagnosed as a benign odontogenic keratocyst. Surgical resection was performed as definitive treatment. Therefore, we present this unique case with a review of the literature.

  • The definitive and effective method for tetanus prevention has contributed to decreased incidence and mortality of tetanus infections in developed countries. On the other hand, there has been a recent increase in incidence due to the lack of awareness of the disease and lack of adherence to preventive measures. Tetanus infections can be fatal. Therefore, prevention must be the utmost priority, followed by aggressive surgical management along with supportive care. The authors present a case of a 59-year-old male patient diagnosed with a generalized tetanus infection from a foot injury. The patient recovered successfully after aggressive surgical debridement and amputation of the infected toe at the metatarsal level, followed by two months of supportive care.

  • A public health concern emerging from a zoonotic disease. Monkeypox is caused by the orthopoxvirus specie Monkeypox virus (MPXV). Monkeypox was identified as the most common orthopoxvirus infection in humans following the eradication of smallpox. Monkeypox has a similar clinical presentation to smallpox. The MPXV is now considered a high-threat pathogen that causes a serious public-health problem. The continuous spread of Monkeypox virus from West Africa to all other places around the world throughout 2018 to 2022, have raised concerns that MPXV could have emerged to acquire the immunological and ecological niche vacated by smallpox virus. This review highlights the current knowledge about Monkeypox evolution, infection biology, and epidemiology around the world since from 1970 to 2022, with a focus on the human, viral, and cellular factors that influence virus emergence, infection, spread, and maintenance in nature. This paper also discusses the current therapeutic options for Monkeypox treatment and control. Under the right conditions, with limited smallpox vaccination and very little orthopoxvirus immunity in some areas of the world, MPXV could become a more efficient human pathogen. Finally, the review identified knowledge gaps, particularly in terms of identifying a definitive reservoir host for monkeypox and proposes future research endeavors to address the unanswered questions.
