하인두 재건을 위한 전외측 대퇴부 유리 피판의 변형된 도안 (Modified Design of Anterolateral Thigh Free Flap for Hypopharyngeal Reconstruction)

  • Purpose: Defect after ablation of hypopharyngeal cancer often requires reconstruction by free tissue transfer. Since neo-hypopharynx is totally buried, various methods have been suggested for monitoring. We propose a modified design of anterolateral thigh (ALT) free flap for reconstruction of pharyngolaryngectomy defect, which has an exteriorized part for clinical monitoring and allows for primary closure. Materials and Methods: Three consecutive patients with hypopharyngeal cancer were reconstructed with ALT flap with modified design: 1) distal part of flap was elongated into fusiform shape and used as exteriorized monitoring segment with a deepithelized bridge and 2) proximal part was designed as curve so the maximum width of the flap was reduced to less than 10 cm. Results: Patient 1, 2 had uneventful postoperative course with healthy skin color and fresh pin prick bleeding. In patient 3, defect after cancer ablation was shorter than usual and deepithelized bridge was longer. When the general hemodynamic status of the patient was aggravated in postoperative course, the color of monitoring skin was changed. Viability of the whole flap was confirmed by endoscopy. However, leakage developed after 3 weeks and repair was necessary. In all patients the donor sites were closed primarily. Conclusion: By the modified design of ALT flap, clinical monitoring can be possible by examining exteriorized monitoring flap and also donor site can be closed primarily. However possibility of false positive exists and technical caution and patient selection is needed because of danger of leakage.

위상배열 초음파 기법을 이용한 TOFD 검사에 관한 연구 (A Study on TOFD Inspection Using Phased Array Ultrasonic Technique)

  • 용접부 및 구조물에 발생된 결함의 깊이를 정밀하게 측정하기 위한 노력은 현재까지 계속되어 왔으며 앞으로도 계속되어야 할 연구분야중의 하나이다. TOFD 검사기법은 현재까지 결함의 깊이를 측정하기 위한 여러 가지 검사 방법중에 가장 정밀한 것으로 알려져 있다. 그러나 사용하는 초음파 굴절각에 따른 빔의 도달범위에 의하여 결함을 놓칠 우려가 있다. 본 여구에서는 일반적인 TOFD 기법대신 위상배열 초음파 기법을 이용한 TOFD 검사를 적용하여 두께가 두꺼운 검사대상에 대하여 결함의 검출능 및 깊이 측정결과를 비교하였다. 위상배열 초음파를 이용한 TOFD 검사를 사용함으로써 다양한 결함에 대하여 검출능 및 정확한 깊이 측정이 가능함을 실험적으로 확인하였다. 본 연구에서 개발된 위상배열 TOFD 기법은 발전설비의 압력용기와 같은 두꺼운 검사대상에 적용될 경우 검사 신뢰도 향상에 기여할 것으로 판단된다.

누설자속탐상의 수치해석 (Numerical Analysis of Magnetic Flux Leakage Inspection)

  • 본 논문에서는 누설자속탐상에 대한 전자기 수치해석을 수행하였다. 수치해석방법으로 2차원 유한요소법을 이용하였다. 자기벡터포텐셜을 미지수호 사용하였으며, 자기비선형(磁氣非線型)을 고려한 자장해석을 수행하여 자기포화에 의한 영향을 고려하였다. 수치해석 결과를 검증하기 위하여 실험장치를 자체 제작하여 실험을 수행하였다 시료로는 SM 45C 탄소강을 사용하였으며, 표면에 몇 가지 결함을 생성하였다. 시편과 실험장치를 이용하여 결함을 검출하는 비파괴검사를 수행하였다. 결함의 깊이 변화 및 결함의 형상변화에 대하여 결과를 비교 검토하였다. 수치해석 및 실험에서 유사한 결과를 얻었으며, 수치해석을 통하여 누설자속탐상의 시뮬레이션이 가능함을 보였다.

초대형 컨테이너선의 해치 코밍 용접부의 내부 균열에 대한 파괴역학적 연구 (A Study on Embedded Crack at the Hatch Coaming FCA Butt Weldment in an Ultra Large Containership on the Basis of Fracture Mechanics)

  • The purpose of this study is to prevent the unstable fracture at the FCA butt weldment of hatch coaming deck in the ultra large containership during service life. In order to do it, the behavior of the embedded crack at the weldment under design loading conditions was evaluated in accordance with BS7910. Here, the level of primary stress induced by ship motion was evaluated by the design code of classification society and FEA. The level of residual stress as secondary stress was calculated in consideration of the restraint degree of weldment and welding heat input by using the predictive equation proposed by authors in the previous study. The fatigue crack growth rate at the weldment was evaluated using CT specimen in accordance with ASTM E647. According to the results, although the allowable defect for embedded crack specified in the classification society exists at the weldment, the occurrence possibility of unstable fracture at the weldment could not be negligible, regardless of CTOD value given in this study. So, in this study, the effect of initial defect size, welding heat input, restraint degree and CTOD on the fracture mechanical characteristics of embedded crack at the weldment was evaluated by the comprehensive fracture assessment. Based on the results, the design criteria including allowable defect, residual stress level and CTOD value was established to prevent the unstable fracture at the FCA butt weldment of hatch coaming deck in an ultra large containership during service life of 20years.

웨이블릿변환이 접목된 포락처리를 이용한 저속 회전하는 구름요소베어링 결함 진단 (Low Speed Rolling Bearing Fault Detection Using AE Signal Analyzed By Envelop Analysis Added DWT)

  • Acoustic Emission (AE) technique is a non-destructive testing method and widely used for the early detection of faults in rotating machines in these days, because the sensitivity of AE transducers is higher than normal accelerometers. So it can detect low energy vibration signals. The faults in the rotating machines are generally occurred at bearings and gearboxes which are the principal parts of the machines. It was studied to detect the bearing faults by envelop analysis in several decade years. And the researches showed that AE had a possibility of the application in condition monitoring system(CMS) using the envelope analysis for the rolling bearing. And peak ratio (PR) was developed for expression of the bearing condition in condition monitoring system using AE. Noise level is needed to reduce to take exact PR value because the PR is calculated from total root mean square (RMS) and the harmonics peak levels of the defect frequencies of the bearing. Therefore, in this paper, the discrete wavelet transform (DWT) was added in the envelope analysis to reduce the noise level in the AE signals. And then, the PR was calculated and compared with general envelope analysis result and the result of envelope analysis added the DWT. In the experiment result about inner fault of bearing, defect frequency was difficult to find about only envelop analysis. But it's easy to find defect frequency after wavelet transform. Therefore, Envelop analysis added wavelet transform was useful method for early detection of default in signal process.

혈소판 유래 섬유소(Platelet Rich Fibrin(PRF))를 이용한 골결손부의 치료 : 증례보고 (Clinical use of Platelet Rich Fibrin(PRF) in Various Hard and Soft Tissue Defect : Case Report)

  • Recently several studies have been developed not only to apply bone materials to bony defect, but also to use osteogenic and osteoinductive materials to form bone more effectively. In 1998 Mark et al applied gel formation of PRP(platelet-rich plasma) in bony transplantation for mandibular reconstruction as one of the method of stimulating bone formation in maxillofacial area, which is contain of varies growth factors. After he reported that PRP accelerate bone formation, which is used in varies bone transplantation and augmentation with a good result. Especially there are amount of growth factors in PRP, and PRP increase angiogenesis, cell division, and mesenchymal cell growth. Moreover it is capable of osteoconduction, hemostatitis, anti-infection, forming the shape at transplantation, ease of handling, and recipient site stability. So it is known that success rate is high in bone transplantation. However PRP need tissue adhesive to make plasma to solid form. Thrombin and calcium chloride, component of PRP, is extracted from autogenic donor. So it is expensive to extract and there is possibility of hepatitis, AIDS, and hematogenous metastasis. After all, tissue adhesive have the limitation and danger of use. So we are willing to introduce that we had get some idea after using PRF(platelet-rich fibrin) in the various hard and soft tissue bony defect, which is self extracted simply and contain growth factors.

공동주택 타일공사의 하자 유형 분석 및 저감 대책에 관한 연구 (A Study on Analysis of Defect Types and Measures for Reduction of Tile Construction for Apartment Houses)

  • 국내의 주택 부족 문제가 해소되면서 공동주택 건설시장이 고품질을 원하는 소비자 위주의 시장으로 변화되었고, 마감 품질에 대한 기대가 높아졌다. 한국토지주택공사가 공급한 공동주택의 하자 민원 발생 접수 현황에 따르면, 타일 관련 민원이 가장 많이 발생하였다. 이와 관련된 하자 민원을 줄이기 위해 국토교통부에서는 「공동주택 하자의 조사, 보수 비용 산정 및 하자판정기준」 개정안을 입안하는 등 지속적인 노력을 하고 있으나, 예방적 처리 방안에 대한 마련은 미흡한 실정이었다. 또한, 국내·외 선행연구를 살펴보며 전문가의 정성적 지식이 반영된 타일공사 하자 유형 특징을 도출한 후 분류 체계를 구성하고, 유형별 원인에 따른 대책을 제시하는 연구가 부족하였다. 따라서 본 연구는 정성적 지식을 반영할 수 있으며 다양한 지표와 하자 유형 간 관계 파악이 용이한 AHP 분석 기법을 활용해 건설 전문가에게 설문조사를 실시한 후, 도출된 두 가지 정성적 지표인 '하자 보수 후 재발 가능성', '하자 보수의 어려움 정도'의 가중치와 정량적 지표인 '하자 발생빈도'를 방사형 그래프로 시각화하여 타일공사 하자 유형의 특징을 분석하고 원인에 따른 저감 대책을 수립한다. 이는 거주환경의 품질을 높일 뿐만 아니라 공동주택 타일공사의 원활한 하자 관리를 위한 시스템 구축 및 공동주택 타일공사 하자 저감에 기여할 것이다.

Massive Rhabdomyolysis Following Cardiopulmonary Bypass

  • Here, we report a case of massive rhabdomyolysis following an uncomplicated repair of a ventricular septal defect in a five-month-old baby. Postoperatively, the patient was hemodynamically stable but metabolic acidosis continued, accompanied by fever and delayed mental recovery. The next day, he became comatose and never regained consciousness thereafter. The computed tomography of the brain revealed a diffuse brain injury. The patient followed a downhill course and eventually died on postoperative day 33. An unusually high level of creatine phosphokinase was noticed, peaking (21,880 IU/L) on postoperative day 2, suggesting severe rhabdomyolysis. The relevant literature was reviewed, and the possibility of malignant hyperthermia obscured by cardiopulmonary bypass and hypothermia was addressed.

안전한 제품을 설계하기 위한 새로운 제품위험분석 방법 (A New Approach to Product Risk Analysis for Safe Product Design)

  • Today we are observing a lot of injuries, casualties, and property losses that are mainly caused by the defects of products. In order to derive safety designs, which minimize the possibility of such product liability-related accidents, we need to take into account the user-product interaction as an important part of the danger factor analysis. Existing risk analysis techniques, however, have some limitations in detecting comprehensive danger factors that are peculiarly involved in human errors and the functional defects of products. Researches on danger factor analysis regarding the user-product interaction have been carried out actively in ergonomics. In this paper, we suggest a novel product risk analysis technique, which is more objective and systematic compared to the previous ones, by combining a modified TAFEI (Task Analysis For Error Identification) technique with SASA (Systematic Approach to Accident Scenario Analysis) technique. By applying this technique to the product design practice in industry, corporations will be able to improve the product safety, consequently strengthening the competitiveness.

  • 산업체에서 이미 보편화되어 온 FMEA기법의 목적은 발생가능성이 있는 하자형태를 파악하고, 고객에게 미칠 고장 발생 요인을 우선 순위별로 사전에 제거하도록 이행하는 예방조치의 한 수단이다. FMEA는 초기 단계(Design 개념 설정시)에서 작성하는 Design FMEA와 양산되기 전에 시기 적절하게 작성하는 Process FMEA의 두 종류가 있다. (설계기능이 없는 공급자에게는 Process FMEA만 해당) 본 연구에서는 Process FMEA를 건설업의 시공분야에 적용하여 품질향상의 신뢰성을 높이고 Life Cycle Analysis, Risk Analysis 등의 기초자료로 활용이 가능한 기본자료(DATA)를 축적할 수 있는 방안을 연구하였다.
