Reconstruction of Abdominal Wall of a Chronically Infected Postoperative Wound with a Rectus Abdominis Myofascial Splitting Flap

  • Background If a chronically infected abdominal wound develops, complications such as peritonitis and an abdominal wall defect could occur. This could prolong the patient's hospital stay and increase the possibility of re-operation or another infection as well. For this reason, a solution for infection control is necessary. In this study, surgery using a rectus abdominis muscle myofascial splitting flap was performed on an abdominal wall defect. Methods From 2009 to 2012, 5 patients who underwent surgery due to ovarian rupture, cesarean section, or uterine myoma were chosen. In each case, during the first week after operation, the wound showed signs of infection. Surgery was chosen because the wounds did not resolve with dressing. Debridement was performed along the previous operation wound and dissection of the skin was performed to separate the skin and subcutaneous tissue from the attenuated rectus muscle and Scarpa's fascial layers. Once the anterior rectus sheath and muscle were adequately mobilized, the fascia and muscle flap were advanced medially so that the skin defect could be covered for reconstruction. Results Upon 3-week follow-up after a rectus abdominis myofascial splitting flap operation, no major complication occurred. In addition, all of the patients showed satisfaction in terms of function and esthetics at 3 to 6 months post-surgery. Conclusions Using a rectus abdominis myofascial splitting flap has many esthetic and functional benefits over previous methods of abdominal defect treatment, and notably, it enabled infection control by reconstruction using muscle.

Defect Formatìon and Annealìng Behavìor in MeV Si Self-Implanted Silicon (MeV Si 자기 이온주입된 단결정 Silicon내의 결함 거동)

  • In this study MeV Si self ion implantations were done to reveal the intrinsic behavior of defect formation by excluding the possibility of chemical interactions between substrate atoms and dopant ones. Self implantations were conducted using Tandem Accelerator with energy ranges from 1 to 3 MeV. Defect formation by high energy ion implantation has a significant characteristics in that the lattice damage is concentrated near Rp and isolated from the surface. In order to investigate the energy dependence on defect formation, implantation energies were varied from 1 to 3 MeV under a constant dose of $1{\times}10^{15}/cm^2$. RBS channe!ed spectra showed that the depth at which as-implanted damaged layer formed increases as energy increases and that near surface region maintains better crystallinity as energy increases. Cross sectional TEM results agree well with RBS ones. In a TEM image as-implanted damaged layer appears as a dark band, where secondary defects are formed upon annealing. In the case of 2 MeV $Si^+$ self implantation a critical dose for the secondary defect formation was found to be between $3{\times}10^{14}/cm^24$ and $5{\times}10^{14}/cm^2$. Upon annealing the upper layer of the dark band was removed while the bottom part of the dark band did not move. The observed defect behavior by TEM was interpreted by Monte Carlo computer simulations using TRIM-code. SIMS analyses indicated that the secondary defect formed after annealing gettered oxygen impurities existed in silicon.

Effects of Recombinant Human Bone Morphogenetic Protein-2 loaded Acellular Dermal Matrix on Bone Formation (재조합 골형성 단백질 2형(rh-BMP-2) 함유 무세포성 진피조직(acellualr dermal matrix)의 골재생 효과)

  • Introduction : The purpose of this study was to evaluate the possibility of the acellular dermal matrix (ADM) as a barrier membrane for bone regeneration, and to evaluate the osteogenic effect of ADM as a carrier system for rhBMP-2 in the rat calvarial defect model. Materials and Methods: An 8-mm, calvarial, critical-size osteotomy defect was created in each of 60 male Spraque-Dawley rats(weight $250{\sim}300g$). Three groups of 20 animals, each received either rhBMP-2(0.025mg/ml) in an ADM carrier, ADM only, or negative surgical control. And each group was divided into 2- and 8-weeks healing intervals. The groups were evaluated by histologic and histomorphometric parameters(10 animals/group/healing intervals). Data were expressed as $means{\pm}standard$ deviations($m{\pm}SD$). Comparisons between experimental and control groups were made using two-way ANOVA and post hoc t-test. Comparisons between 2 weeks and 8 weeks were made using paired t-test. The level of statistical difference was defined as P< 0.05. Results : The ADM group and rhBMP-2/ADM group results in enhanced local bone formation in the rat calvarial defect at both 2 and 8 weeks. The amount of defect closure and new bone formation were significantly greater in the rhBMP-2/ADM group relative to ADM group(P<0.05). At 8 weeks, the majority of ADM in the defect was contracted, and integrated with surrounding host tissues. In addition, host cell infiltration and neovascularization of the ADM in the absence of an inflammatory response were observed, and the newly formed bone around ADM showed a continuous remodeling and consolidation. Conclusion : The results of the present study indicated that ADM may be used as a barrier membrane for bone regeneration and that may be employed as a delivery system for BMPs.

Evaluation of Bone Resorption Rate after Nonvascularized Iliac Bone Graft for Mandibular Discontinuity Defect (하악골의 불연속 결손부 재건 시 비혈행화 장골이식술의 골흡수율에 관한 연구)

  • Purpose: Mandible resection and discontinuity defect created lead to aesthetic and functional problems. The iliac crest bone graft exhibits relative ease for bone harvesting, possibility of two team approach, ability to close the wound primarily, large amount of corticocancellous bone and relatively few complications. Whereas the use of free vascularized flaps has donor site morbidity and worse-fitting bone contour, the use of nonvascularized iliac bone graft has advantages in the operation time and patients' recovery time. So, nonvascularized iliac bone graft could be an attractive option. Methods: Twenty-one patients (M:F=1:1.1) underwent iliac crest bone harvesting for reconstruction of mandibular discontinuity defect (mean length : $61.6{\pm}17.8$ mm), from May 2005 to October 2011 at the Department of Oral and Maxillofacial Surgery in Kyungpook National University. The average age was $44.1{\pm}16.4$ years and the mean follow up periods was $28.2{\pm}22.7$ months. Bone resorption rate, according to age, sex, primary lesion, location and distance of defect, type of fixation plate, time of graft and pre-operative radiation therapy, were measured in each patient. Results: The mean bone resorption rate was $16.1{\pm}9.0%$. Bone resorption rate was significantly increased in mandibular defect that is over 6 cm in size (P=0.015, P<0.05) and the cases treated pre-operative radiation therapy (P=0.017, P<0.05). All was successfully fixed and maintained for the long-term follow-up. There were a few donor site complications and almost all patients were shown favorable outcome without severe bone resorption in this study. Conclusion: The nonvascularized iliac bone graft seems to be a reasonably reliable treatment option for reconstruction of mandibular discontinuity defects.

Classifying Sub-Categories of Apartment Defect Repair Tasks: A Machine Learning Approach (아파트 하자 보수 시설공사 세부공종 머신러닝 분류 시스템에 관한 연구)

  • A number of construction companies in Korea invest considerable human and financial resources to construct a system for managing apartment defect data and for categorizing repair tasks. Thus, this study proposes machine learning models to automatically classify defect complaint text-data into one of the sub categories of 'finishing work' (i.e., one of the defect repair tasks). In the proposed models, we employed two word representation methods (Bag-of-words, Term Frequency-Inverse Document Frequency (TF-IDF)) and two machine learning classifiers (Support Vector Machine, Random Forest). In particular, we conducted both binary- and multi- classification tasks to classify 9 sub categories of finishing work: home appliance installation work, paperwork, painting work, plastering work, interior masonry work, plaster finishing work, indoor furniture installation work, kitchen facility installation work, and tiling work. The machine learning classifiers using the TF-IDF representation method and Random Forest classification achieved more than 90% accuracy, precision, recall, and F1 score. We shed light on the possibility of constructing automated defect classification systems based on the proposed machine learning models.

Analysis of Major Factors of Window Work in Construction Phase Considering Recurrence of Defects in the Maintenance Phase (유지관리단계의 하자 재발생을 고려한 창호공사 시공단계의 중점관리요소 분석)

  • As the construction standards for energy-saving eco-friendly housing have recently been strengthened, the proportion of window work has increased with the demand for high-efficiency housing. Windows have high frequency of use, and there is the potential for many defects to occur depending on the characteristics of construction. According to a government agency's survey of defects in public rental apartment housing, defects in the windows work accounted for the highest portion of complaints received. Accordingly, related previous studies were considered, and it was found that the existing studies in Korea lacked research that reflected the construction characteristics of window work and the importance of maintenance. In addition, existing overseas studies considered both the constructor and the resident's position, considering the cost aspect together, and showed a trend of structuring the relationship between defects and causes. Therefore, this study will analyze the causes of defects that can occur in the construction phase of the windows work, reflect the construction characteristics, and derive major factors that consider the importance of maintenance based on the possibility of recurrence after repairing defects. Ultimately, this research will contribute to preventing defects in the construction phase and reducing maintenance costs by presenting a highly effective defect management plan through selecting the major factors for each defect type that can be intuitively judged by analyzing the causal relationship between defect types and causes.

The Natural Course and Size Change of Atrial Septal Defect Secundum (단순 이차성 심방 중격 결손의 자연 경과와 성장에 따른 결손의 크기 변화)

  • Purpose : This study was performed to follow the natural course and size change of isolated atrial septal defect(ASD) secundum. Methods : Among the newly diagnosed with ASD secundum at the division of pediatric cardiology in Yonsei cardiovascular hospital from January 1996 to December 2000, 89 patients with pure ASD secundum were checked by the serial echocardiographic evaluation to measure the size change. Results : There was a statistical difference(P<0.05) in the rate of spontaneous closure and size change of the defect between the group of bigger defect(>8 mm) and the group of smaller defect (${\leq}8mm$). There was also a statistical difference(P<0.05) in the rate of spontaneous closure and size change of the defect between the group of younger than three years of age and the group of older than three years of age. The initial size of the defect and change of size were the significant influencing factors in the rate of spontaneous closing of ASD. In the group of patients who have persistent ASD, the size of defect showed a tendency of increasing size of defect with the age(P<0.005); however in the correction of these values by the body surface area of each patients, there was no statistical difference. Conclusion : The possibility of spontaneous closure of pure ASD secundum was higher in the group of patients who have smaller defect and who were younger. The ASD secundum that did not have spontaneous closure showed an increase in size with the growth of the patients; however when this size was corrected by the body surface area, there was no statistical difference. The closure of the defect was at 26.2 months of age so close follow-up of the patients is important.

A Study on the Detection of Defects from Parallel Cylindrical Objects Using Spectral Analysis of Acoustic Impact Signal (타격음 주파수 분석법에 의한 원통 병렬 구조물의 파손 여부 식별에 관한 연구)

  • This paper describes principles and experimental results in conjunction with examining the possibility of the detection of effects from a cylindrical objects. Cylindrical objects have its own resonance frequency. The frequency varies according to the diameter, length etc. And acoustic sound, whose frequency and the harmonic components corresponding to resonance frequency, is radiated from the object. When an object have defect in itself, the radiated acoustic sound is different from normal one. So we can detect its defect by analysing frequency components of acoustic sound. We proved that detection of defective objects by acoustic signal analysis is possible automatically. Also the result can be applied to other kinds of objects.

A Case of Traumatic Ventral Hernia Repair with a Porcine Dermal Collagen Graft (Permacol) (외상성 복벽 탈장 환자에서 biologic mesh를 이용한 치료)

  • Resection of the bowel is necessary for the repair of a ventral hernia after recovery from trauma in some cases. In such instances, polyester or polypropylene meshcannot be used due to the possibility of infection; we had to use biological mesh instead. We report a case in which a traumatic hernia was repaired with Permacol (Covidien, Norwalk, CT, USA). A 42-year-old male patient had been injured by a factory machine seven months prior to admission. At that time, he had abdominal wall injury and small bowel perforation. His abdominal wall had been a defect after operation. A CT scan of the abdomen showed that the left abdominal wall, which is lateral to left rectus abdominis muscle had only one muscle layer, an external oblique muscle, and that a previous abdominal incision had a defect along the entire incision. During the exploration, 10 cm of small bowel was removed due to firm adhesion to the previous surgical scar. Permacol mesh was applied and fixed with transfascial fixations and tacks by using the intraperitoneal onlay mesh technique. There were no complications after the surgery and the patient was discharged without any problems.

Pullout Bond Characteristics of Polymer Cement Slurry Coated Rebars (폴리머 시멘트 슬러리 도장철근의 인발부착 특성)

  • Recently, epoxy-coated re-bar used to the structure partly and put to practical use step, but not economical and appeared to the defect of deterioration of long term bond strength between concrete. The method for complement the defect of epoxy coated re-bar, study of polymer cement slurry coated re-bar started and basic properties appeared to excellent, but study of bond properties embedded in concrete specimens insufficient until now. This study attempts to examination of using possibility for bond strength of polymer cement slurry coated re-bar between concrete specimens compare to ACI Code and KS Code through pull-out test of 15cm$\times$15cm$\times$15cm specimens with polymer cement slurry coated re-bar as polymer cement ratio 50%, 100%, 150%, coating thickness 250${\mu}{\textrm}{m}$, 440${\mu}{\textrm}{m}$ and curing age. In the results of this study, the bond strength of polymer cement slurry coated re-bar compare to plain re-bar, epoxy coated re-bar decreased St/BA-modified polymer cement slurry coated re-bar, but bond strength of PA-modified polymer cement slurry coated re-bar appeared to excellent results. The bond properties of polymer cement slurry coated re-bar between concrete will be obtain more precise results according to compressive strength change of concrete and re-bar diameter size.
