뇌수술 환자의 심부정맥혈전증 예방과 피부반응에 미치는 실험연구 : 부츠형과 무릎형의 간헐적 공기 압박기 적용에 따라 (A study on the Prevention of Deep Vein Thrombosis and Skin Response of Patients after Intracranial Surgery : By Boots and Calf Intermittent Pneumatic Compression Device)

  • 본 연구는 뇌수술 후 부동 환자에게 부츠형 또는 무릎형 간헐적 공기 압박기 적용 후 심부정맥 혈전증 예방과 피부반응에 미치는 영향을 평가하기 위한 실험연구이다. 연구대상은 2015년 5월 부터 11월 까지 뇌수술 후 외과계 중환자실에 입실한 60명을 대상으로 하였다. 심부정맥 혈전증 평가는 7일 동안 대퇴정맥 혈류속도의 변화를 매일 측정하였고 피부 반응 정도도 매일 규칙적으로 관찰하였다. 그 결과 부츠형 간헐적 공기 압박기 적용군이 무릎형 간헐적 공기압박기 적용군보다 시간이 경과함에 따라 대퇴정맥 혈류속도가 증가함을 확인하였다. 즉, 7일 동안 측정한 평균 대퇴정맥 혈류속도는 시간경과에 따른 측정시기 오른쪽(F=64.41, p <.001), 왼쪽(F=58.21, p <.001)모두 통계적으로 유의한 차이를 나타내었다. 두 군 간의 발등 둘레는 측정시기와 집단 간의 상호작용에서 오른쪽(F=9.13, p <.001), 왼쪽(F=9.29, p<.001) 모두 통계적으로 유의한 차이를 보였다. 또한 부츠형 간혈적 공기 압박기 적용군은 피부 합병증도 발생하지 않았다. 이러한 결과는 뇌수술 후 부동환자에게 부츠형 간헐적 공기 압박기 적용이 심부정맥 혈전증을 예방하는데 보다 효과적임을 알 수 있다.

May-Thurner 증후군과 동반된 하지 심부정맥혈전환자에서 혈전제거술과 스텐트삽입술 (Pharmaco-mechanical Thrombectomy and Stent Placement in Patients with May-Thurner Syndrome and Lower Extremity Deep Venous Thrombosis)

  • 배경: May-Thurner 증후군은 좌측장골정맥이 우측장골동맥에 의해 눌리어 정맥 환류장해에 의해 혈전증을 일으키는 것을 말한다. 저자들은 심부 정맥혈전중의 치료를 위해 시행한 혈전 제거-용해술 및 스탠트 삽입술의 결과를 분석하고자 한다. 대상 및 방법: May-Thurner 증후군과 동반된 하지 심부정맥혈전증으로 진단받고 좌측 장골정맥에 스탠트를 삽입한 34명(평균연령 $64.6{\pm}13.7$세, 여자 25(74%)명)을 대상으로 하였다. 시술은 흡인성 혈전제거와 혈전용해 후 분지성 협착이 있는 곳에 Wall stent를 삽입하고 풍선 확장을 하였다. Multi side hole 카테타를 혈전이 있는 부위에 위치시키고 Urokinase를 시간당 8만에서 12만 International Unit을 1일 또는 2일 동안 주입하였다. 시술 중 폐동맥혈전색전증을 방지 하기 위하여 대부분 환자에서 시술 전 일시적 하대정맥 여과기(IVC Filter)를 삽입하였다. 퇴원 후 3개월간 경구용 와파린을 투여 하였으며 퇴원 전과 퇴원 후 6개월에 Multi Detector Computerized Tomography (MDCT) 혈관촬영을 이용하여 혈전의 유무를 판단하였다. 결과: 시술 48시간 이내 부종과 동통이 완전하게 소실된 환자가 2 (6%)명이었으며 증상의 완화가 있는 환자가 28 (82%)명, 증상의 호전이 없는 환자가 4 (12%)명이었다. 퇴원 시 MDCT혈관촬영에서 9 (26%)명에서 혈전없음, 21 (62%)명에서 부분 혈전, 그리고 4명(12%)에서 폐쇄소견을 보였다. 퇴원 6개월 후에 2명을 제외한 32명이 추적 MDCT혈관촬영을 하였는데 23 (72%)명에서 혈전없음을 9 (26%)명에서는 부분혈전이 관찰 되었다. 평균 5.6개월 관찰기간 동안 2 (6%)명에서 심부정맥혈전이 재발되어 재입원하였으며 하지 부종과 통증을 동반한 혈전 후 증후군은 9예(26%)에서 발생하였다. 결론: May-Thurner 증후군과 동반된 하지 심부정맥혈전환자에서 심부정맥혈전의 제거와 혈전용해술과 함께 시술된 스탠트 삽입은 효과적인 치료 방법으로 생각된다.

Deep Vein Thrombosis Due to Hematoma as a Rare Complication after Femoral Arterial Catheterization

  • Hematoma is quite a common complication of femoral arterial catheterization. However, to the best of our knowledge, there have been no previous studies regarding deep vein thrombosis (DVT) caused by compression of a vein due to a hematoma. We report a case of a hematoma developing after femoral arterial catheterization and causing extensive symptomatic DVT. A 59-year-old male was seen in our Emergency Department with right lower leg swelling 15 days after coronary stent implantation performed using right femoral artery access. Computed tomographic (CT) scanning revealed a large hematoma (45 mm in its longest diameter) compressing the common femoral vein and with DVT from the right external iliac vein to the popliteal vein. Due to the extensive DVT involvement, we decided to release the compressed common femoral vein by surgical evacuation of the large hematoma. However, even following evacuation of the hematoma, as the DVT did not resolve soon, further mechanical thrombectomy and catheter-directed thrombolysis were performed. Angiography then showed nearly resolved DVT, and the leg swelling was improved. The patient was discharged with the anticoagulation medication, warfarin.

  • 중심정맥혈전증은 흔히 관찰되는 질병이다. 그러나 그 원인 중 오른총장골정맥이 왼총장골동맥의 눌림과 협착에 의해 정맥 흐름이 방해받는 경우가 드물게 있으며, 이를 May-Thurner 증후군 또는 장골눌림증후군(Iliac compression syndrome)이라 한다. 이에 대한 치료는 중심정맥혈전증이 있을 경우에 카테터를 이용한 혈전 용해술과 혈전 용해 후 왼총장골정맥 내 스텐트(Stent) 삽입이 추천되고 있다. 저자들은 좌측 하지 부종으로 내원하여 시행한 정맥조영술상 May-Thurner syndrome으로 진단된 2예에서 혈전용해술 및 풍선확장술과 왼총장골정맥 내에 스텐트를 삽입하여 성공적으로 치료하였기에 관련 문헌과 함께 보고하는 바이다.

  • In this study, the effects of vascular parameter changes and electrodes on VOP measurement based on IPG were simulated mathematically. For the evaluation of the effects of hemodynamic changes on VOP, a mathematical model, which consists of cardiovascular system model and venous occlusion model, was developed and the model solution representing the blood flow and pressure in measuring point was found by 2nd order Runge-Kutta method. And, with sensitivity coefficients obtained from finite element solution of electric field in measuring point, the effects of electrode system on measurement were evaluated. As increasing the resistance, the venous capacitance was not changed but the venous outflows were decreased and the decreased compliance reduced the venous capacitance. And, for several configurations of round electrodes and band electrodes, the sensitivity coefficients were computed using the electric field distribution along deep vein. In conclusion, the proposed mathematical cardiovascular model could be applied to the simulation study on the effects of hemodynamic parameters on DVT diagnosis with IPG. And, also the sensitivity coefficients could provide effective electrode configuration for exact measurement of VOP.

Pulmonary Embolectomy for Acute massive Pulmonary Embolism - 1 case -

  • Pulmonary thromboembolism originated mostly from the venous thrombus, especially deep vein thrombosis in the lower extremities, which migrated upward and lodged the pulmonary vasculatures, Massive pulmonary thromboembolism usually leads to in \ulcornerhospital mortality if the patient was not treated properly. Under the cardiopulmonary bypass, a 49-year old man was treated successfully by emergent pulmonary embolectomy of pulmonary thromboembolism, which originated from the deep vein thrombosis in the right leg. Ligation or filtering device insertion of the inferior vena cava was not performed. The patient’s postoperative course was uneventful and discharged on postoperative 15th day He continued to receive oral anticoagulation with aspirin and persantin, which had been started on the third postoperative day. And he was well till recent days through the outpatient follow-up. The clinical courses of this patient are described, and massive pulmonary embolism and its management are discussed.

Deep vein thrombosis and acute hepatitis after ChAdOx1 nCov-19 vaccination in a Charcot-Marie-Tooth patient: a case report

  • Monitoring of side effects after coronavirus disease 2019 (COVID-19) vaccination has become an important issue for health systems worldwide to ensure its safety. Recently cases of deep vein thrombosis (DVT), vaccine-induced immune thrombotic thrombocytopenia, and autoimmune hepatitis have been described: the underlying pathophysiological mechanisms are still debated. We report on a patient who presented with DVT and acute hepatitis 8 days after receiving the first dose of the ChAdOx1 nCoV-19 adenoviral vector vaccine against COVID-19. The patient is a 56-year-old male who was already affected by a rare form of axonal Charcot-Marie-Tooth disease linked to MME (membrane metalloendopeptidase) gene variation and associated with mild symptoms. His blood exams did not have any evidence of thrombocytopenia but D-dimer, troponin T, alanine transaminase, and aspartate aminotransferase were abnormal, suggesting the presence of a blood clot and acute hepatitis. The patient was treated with subcutaneous enoxaparin for 15 days and with rivaroxaban for the following 8 months: his symptoms improved and his exams showed recanalization of the veins and a healed liver. The pathogenesis of thrombosis and hepatitis after vaccination is still unclear, especially in subjects affected by rare comorbidities and this may affect the safety of vaccination in this type of population. We highlight the importance of careful monitoring of side effects after vaccination: clinical suspicion must rise when patients complain of symptoms that differ from their usual presentation.

  • Purpose: Deep Vein Thrombosis (DVT) is the cause of fatal diseases such as pulmonary embolism, due to a prolonged immobility, surgery, paralysis, and injuries. This study aimed to develop an evidence-based practice guideline for DVT prevention and apply it to patients with surgery in diverse nursing sites in South Korea. Methods: A 24-staged processes of adaptation was carried out on the basis of "adaptation of nursing practical guidelines" developed by Gu et al., in 2012. Results: Developed nursing guidelines of DVT prevention are composed of 79 recommendations in 8 domains. The extent and ratings of each recommendation with its evidence were addressed along with the background information. Conclusion: The developed DVT prevention guideline is necessary to be added to the evidence-based practice guidelines for the fundamentals of nursing practice. The developed guideline is needed to be disseminated to diverse nursing clinical settings in order to prevent DVT and enhance the quality nursing care.

  • 저자들은 과거력상 반복되는 구강내 아프타성 궤양 및 결절홍반양 피부병변이 있었고, 객혈을 주소로 내원한 37세 남자환자에서 혈관조영술상 다발성 폐동맥류, 심부 정맥의 협착 및 폐색소견이 관찰되었고, 조직학적으로는 폐혈관염 소견을 보여, Hughes-Stovin 증후군으로 진단하였으며, 코일을 이용한 폐동맥색전술로 폐동맥류를 성공적으로 치료하였기에 문헌고찰과 함께 보고하는 바이다.

A New D-dimer Cutoff Value to Improve the Exclusion of Deep Vein Thrombosis in Cancer Patients

  • Objective: To find a more appropriate alternative to D-dimer cutoff value for the diagnosis of deep vein thrombosis (DVT) in cancer patients. Methods: A total of 711 cancer patients with symptoms suspicious of DVT were included in the study. D-dimer levels were assessed using ELISA. All patients were subjected to imaging procedures. Results: Among 711 patients with cancer, 466 (65.5%) were females and 245 (34.5%) were males, with an average age of $57.3{\pm}13.23$ years. The mean age in the DVT group was significantly higher than in the non-DVT group (P<0.05). The D-dimer levels of the DVT group were significantly higher than those of the non-DVT group (P<0.05). The incidence rate of DVT varied significantly according to cancer type (P<0.05). Increasing age and lung cancer were significantly correlated with D-dimer levels (P<0.05), and a one-year increase in age was associated with a 14.28 ng/ml increase in the D-dimer value. The optimal cutoff point for D-dimer was found to be 981 ng/ml, with a sensitivity of 86.4%, specificity of 79.4%, and accuracy of 82.6%. If the D-dimer cutoff point was set to 981ng/ml, the specificity would increase from 61.8% to 85.5% without loss of sensitivity in patients aged 40 years or younger. In patients aged more than 40 years, the new cutoff almost doubled the specificity with slightly reduced sensitivity. Conclusion: In cancer patients, a new cutoff value of 981 ng/ml effectively improved the exclusion of DVT, especially for patients aged more than 40 years.
