Nurse Educators' Experience of Developing and Implementing a High-fidelity Simulation-based Interprofessional Education Module for Medical and Nursing Students

  • Objective: Despite the recommendation of the use of high-fidelity simulation (HFS) in interprofessional education (IPE), there is little known about its work for nursing students. Thus, this study aimed to explore nurse educators' perceptions and experiences in developing and implementing the HFS-based IPE for nursing and medical students. Methods: This study used a case study, using reflective filed notes. Results: Nursing educators perceived HFS as an effective educational approach to engaging nursing and medical students actively in interprofessional collaborative practice (ICP) experiences and in evaluating their actual performance on it. In terms of their perspectives on the elements necessary for effective HFS-based IPE, four themes were identified: collaborative learning, co-facilitating debriefing, leadership commitment and active faculty involvement.

응급외상 환자 시뮬레이션 적용 효과 (Implementation Effects of Emergency Trauma Patient Simulation)

  • Purpose: The purpose of this study was to explore EMT-paramedic students' experience of simulation education and analyze the confidence before and after education, learning attitude and course evaluation. Method: Research survey was conducted on 38 EMT-paramedic students during November, 2011 and EMT-paramedic students' experience of simulation education was analyzed after applying head, spinal, and chest injury scenario. The confidence before and after education, learning attitude and course evaluation in gender were analyzed by Mann-Whitny U test and the difference of confidence before and after education was analyzed by Wilcoxon signed rank test and learning attitude & course evaluation were analyzed by evaluating frequency, percentage, mean, standard deviation by using SPSS WIN 17.0 program. Results: 1. Students experienced various advantages such as increasing interest and self-reflection on learning, critical thinking ability, and EMT-paramedic-role experience and recognition of importance of teamwork. Students also pointed out disadvantages such as gap between real situation and simulation, limit of time and equipments, and burden of demonstration. 2. The confidence between before and after education, learning attitude and course evaluation in gender were not significant different statistically. 3. Confidence mean score elevated from 5.53(before education) to 5.87(after education), but the difference in their confidence did not show significant difference statistically. 4. Total mean score in learning attitude after simulation education was 3.70 out of 5.00, which is considerably very high. 5. Total mean score in course evaluation was 3.89 with score of 3.83 in evaluation in learning environment and 3.99 in evaluation of debriefing. Conclusion: The finding of this study demonstrate that the simulation education can provide a safe and repetitive practice environment, improve problem-solving ability and critical thinking, and increase the confidence in prehospital emergency care; therefore, simulation may be the new effective EMT-paramedic education strategy.

간호대학생의 자기성찰 및 통찰력, 학업적 자기효능감이 임상추론역량에 미치는 영향 (Influence of Self-reflection and Insight, and Academic Self-efficacy on Clinical Reasoning Competence among Nursing Students)

  • Purpose: The purpose of this study was to investigate the influence of nursing students' self-reflection and insight, and academic selfefficacy on their clinical reasoning competence. Methods: Data were collected from 147 third- and fourth-year nursing students who had more than 6 months of clinical training experience. Clinical reasoning competence, self-reflection and insight, and academic self-efficacy were measured using self-reported questionnaires. Data analysis was conducted with IBM SPSS 25.0 using one-way ANOVA, independent t-test, Pearson correlation coefficient, and multiple regression analysis. Results: Clinical reasoning competence was positively correlated with self-reflection and insight (r=.24, p=.003) and academic self-efficacy (r=.30, p<.001). Academic self-efficacy (β=.23, p=.011), dissatisfaction with major (β=-.17, p=.034), and 5~8 case studies (β=-.39, p=.027) were identified as factors influencing clinical reasoning competence (adjusted R2=.15). Conclusion: To improve the clinical reasoning competence of nursing students, it is necessary to develop the academic self-efficacy of complex cases and provide critical inquiries, debriefing tailored to clinical reasoning, and sufficient reflection during clinical and simulation practice.

간호대학생의 시뮬레이션 교육 시 발생하는 불안정도가 시뮬레이션 교육만족도, 자기효능감, 임상수행능력에 미치는 영향 (Influence of Nursing Students' Anxiety during Simulation Training on Personal Satisfaction of Simulation, Self-efficacy, Clinical Competence)

  • Purpose: The purposes of this study were to identify nursing students' levels of anxiety in simulation based nursing education and to understand the relationship of anxiety and personal satisfaction of simulation, self-efficacy, and clinical competence during simulation. Methods: This study was a descriptive study with 84 senior nursing students at a private university in South Korea. Level of anxiety was measured with STAI (the State- Trait Anxiety Inventory) prior to the stimulation practice. After the debriefing, satisfaction scores were measured with SSE (Satisfaction with Simulation Experience scale), self-efficacy scores with the modified measurement developed by Roh and Park, and scores for clinical skills with a scale developed by the Korea Association of Cardiopulmonary Resuscitation. Results: Nursing students exhibited moderate to severe anxiety in simulation practice (Mean 49.51, SD 8.60). Self-efficacy was negatively correlated to anxiety (r=-.25, p=.022). Conclusion: Decreasing students' anxiety levels needs to be addressed to increase the self-efficacy of the simulation practice.

산과 간호 영역에서의 시뮬레이션 실습 교육의 효과 및 적절성 (Effects and Adequacy of High-Fidelity Simulation-Based Training for Obstetrical Nursing)

  • Purpose: Clinical training for nursing students is limited to rudimentary skills to avoid potential risks. Simulation-Based Training (SBT) can overcome the shortcomings of clinical training. We evaluated the educational effect of SBT for obstetrical nursing students using high-fidelity simulation courses. Methods: We developed a simulation program for obstetrical nursing students to practice nursing skills that are necessary to provide quality care. The program consisted of four sessions. 1st: An orientation and a preliminary test. 2nd: Learning core skills required in obstetrical nursing. 3rd: Testing each student with scenario. 4th: Providing a debriefing session. At the beginning of the program, students were surveyed about their self-confidence in obstetrical nursing care, and at the end of the program, they were surveyed about the adequacy of SBT as well as self-confidence. Results: Students' self-confidence showed a significant difference before and after simulation. Mean adequacy of SBT was $7.15{\pm}1.35$ (out of 10). Most students became more interested in Women's Health Nursing after SBT. Conclusion: The results from evaluating the effects of simulation-based obstetrical nursing training show that SBT provides invaluable clinical experience for obstetrical nursing students by overcoming the lack of actual clinical involvement in clinical training programs.

119 구급대원이 경험하는 폭력에 대한 실태조사 (Current Status of 119 EMT to Violence Experiences)

  • Purpose : The purpose of this study was to describe the current status of 119 EMT to violence experiences in Korea. Method : The number of 119 EMT in this study 2522. Self-administering questionnaire data were collected from April 27 to March 8 in 2009. Collected data were analyzed using SPSS 14.0 program. Real number, percentage, mean and standard deviation were calculated. Pearson correlation coefficient was analyzed. Result : 119 EMTs experienced 95.32%-verbal insults, 61.54%-violence aggression, 60.51%- potential violence, 38.70%-physical assault. Within 30 days from the most recent period of violence experienced verbal insults and violence aggression-51.07%, potential violence and physical assault-48.41%. The patients(50.59%) and the patients and patient's carers(38.26%) proved to be violent offender. The main reason of violent act was drug or alcohol addiction(56.15%). Recent exposure to violence was 34.82% by 7 point reward of stress level. 5-point scale showed the average of experiences of violence. Response average of emotional reactions was 2.66 points and that of physical reaction was 2.18 points. Social reaction was 2.06 points. Conclusion : This study was based on the violence experience of 119 EMT. It is necessary to prevent the violence from emergency scene. So we proposed the preventive measure against violence as well as critical incident stress debriefing(CISD). Policy for CISD and stress management will be implemented in the near future in Korea.

소방공무원의 지각한 현장출동 충격경험과 외상 후 스트레스가 신체증상 및 대처방식에 미치는 영향 (The Influence of Field-Shock Experience and Post Traumatic Stress Perceived by Fire Officials upon Somatic Symptoms and Coping Methods)

  • 본 연구는 소방공무원들의 지각한 현장출동 충격 경험과 외상 후 스트레스가 신체증상 및 대처방식에 미치는 영향을 규명하기 위하여 강원도 소재 소방안전센터에 근무하고 있는 소방공무원 남녀 362명을 대상으로 조사하였다. 수집된 자료는 Windows용 SPSS 19.0과 AMOS 19.0을 사용하여 빈도분석, 타당도와 신뢰도 분석을 위한 요인분석과 Cronbach's ${\alpha}$ 분석, 기술통계와 상관관계를 실시하였고, 가설 검정을 위해 구조방정식 모형을 이용한 분석을 실시하였다. 모든 통계분석은 유의수준은 p<.05로 설정하였다. 결과는 다음과 같다. 첫째, 지각한 현장출동 충격 경험은 외상 후 스트레스에 영향을 미치는 것으로 나타났다. 둘째, 지각한 현장출동 충격 경험이 신체증상에 영향을 미치는 것으로 나타났다. 셋째, 지각한 현장출동 충격 경험은 대처방식에 영향을 미치는 것으로 나타났다. 넷째, 외상 후 스트레스는 신체증상에 영향을 미치는 것으로 나타났다. 다섯째, 외상 후 스트레스는 대처방식에 영향을 미치는 것으로 나타났다. 여섯째, 신체증상은 대처방식에 영향을 미치는 것으로 나타났다. 이러한 결론으로 볼 때 충격적 스트레스를 해소하기 위한 제도적 방안으로 전 세계적으로 검증이 되어 실제 운영되고 있는 위기상항 스트레스 해소법(Critical Incident Stress Debriefing : CISD)을 소방서의 실정에 맞게 도입하여 운영하는 것이 필요할 것이다.

사이버불링 예방을 위한 가상체험형 시뮬레이션 적용 모형 개발 (A Development of Application Model on Virtual Simulation for Prevention of Cyberbullying)

  • 사이버불링의 피해가 점차 증가하고 있다. 이에 따라 사이버불링에 대한 법률 및 교육적 대응과 더불어, 기술(code)에 의한 대응도 요구되고 있다. 본 연구에서는 이러한 기술적인 해결 방법 중 하나로 가상 체험형 시뮬레이션의 장점을 이용하고자 하였다. 본 연구에서 개발한 적용 모형은 인지${\rightarrow}$판단${\rightarrow}$결정${\rightarrow}$행동의 각 단계 및 수준을 거치며, 사이버불링에 대한 예방 및 학습을 하게 된다. 각 단계의 세부 모형은 프리브리핑${\rightarrow}$시나리오제시${\rightarrow}$반응요구${\rightarrow}$반응참여${\rightarrow}$결과 및 피드백${\rightarrow}$성장${\rightarrow}$디브리핑으로 구성된다. 이와 같이 사이버불링 예방 및 관련 교육을 하고자 가상체험형 시뮬레이션 적용 모형을 개발하고 이를 바탕으로 실제 구현하였다. 본 연구에서 개발한 모형과 시뮬레이션이 사이버불링의 피해를 줄이는데 도움이 되기를 기대한다.

천식아동 간호 시뮬레이션 학습의 효과 (Effects of Simulation-Based Learning in the Nursing Care of Children with Asthma)

  • Purpose: The purpose of this study was to examine the effects of simulation-based learning on the knowledge about and performance confidence in the nursing care of children with asthma. Methods: A one-group pre- and post- study design was used. A total of 70 nursing students participated in the study. In groups of five, the participants experienced simulation-based learning during their child health nursing practicum. The given scenario was about nursing care for children with asthma. The simulation learning was provided for 3 hours, and included a group discussion before the simulation and a debriefing. Results: After the simulation, knowledge about and performance confidence in the nursing care of children with asthma significantly increased. The nursing students reported a high level of satisfaction with the simulation-based learning. There was a significant positive correlation between knowledge and performance confidence. Conclusion: Simulation-based learning was effective for nursing students in the nursing care of children with asthma. Our results suggest utilizing this new way of learning to strengthen the clinical experience of child health nursing in nursing students.

문제기반학습 연계의 시뮬레이션 교육이 MBTI 성격유형에 따라 비판적 사고, 문제해결능력, 학습동기에 미치는 효과 -모의 실험 (Effects of simulation integrated problem based learning (Si-PBL) on Critical thinking, Problem solving, Learning motivation according to Myers-Briggs Type Indicator (MBTI) Personality Type -Pilot study)

  • 본 논문은 문제기반학습 연계의 시뮬레이션 교육이 MBTI 성격유형에 따라 비판적 사고, 문제해결 능력 및 학습동기에 미치는 효과를 분석하고자 시도되었다. 간호 전공심화생 19명을 대상으로 첫 6주간 문제기반학습 패키지로 학습하고, 그후 5주는 문제기반학습연계의 통합시뮬레이션 교육을 받은 후 비판적 사고, 문제해결 능력, 학습동기를 평가하고 이를 MBTI 성격유형에 따라 분석하였다. 그 결과 외래근무 심화생의 학습동기와 이전에 시뮬레이션 교육경험이 없는 심화생의 비판적 사고가 유의하게 높았으며, 논리적이고 창의적인 유형(NT)에서 비판적 사고가 유의하게 높았다. 학습에 대한 태도와 동기는 성격유형과 유형들이 가지는 특징에 영향을 받을 수 있으므로, 문제기반학습 기반의 시뮬레이션 교육의 효과를 높이기 위해서는 대상자들의 성격유형을 고려하고, 특징들을 반영한 간호교육 프로그램이 개발되어야 할 것이다.
