Maternal, infant, and perinatal mortality statistics and trends in Korea between 2018 and 2020

  • Purpose: This study aimed to identify maternal, infant, and perinatal mortality using the national population data of South Korea between 2018 and 2020, and to analyze mortality rates according to characteristics such as age, date of death, and cause of death in each group. This study updates the most recent study using 2009 to 2017 data. Methods: Analyses of maternal, infant, and perinatal mortality were done with data identified through the supplementary investigation system for cases of death from the Census of Population Dynamics data provided by Statistics Korea from 2018 to 2020. Results: Between 2018 and 2020, a total of 99 maternal deaths, 2,427 infant deaths, and 2,408 perinatal deaths were identified from 901,835 live births. The maternal mortality ratio was 11.3 deaths per 100,000 live births in 2018; it decreased to 9.9 in 2019 but increased again to 11.8 in 2020. The maternal mortality ratio increased steeply in women over the age of 40 years. An increasing trend in the maternal mortality ratio was found for complications related to the puerperium and hypertensive disorders. Both infant and perinatal mortality continued to decrease, from 2.8 deaths per 1,000 live births in 2018 to 2.5 in 2020 and from 2.8 in 2018 to 2.5 in 2020, respectively. Conclusion: Overall, the maternal, infant, and perinatal mortality statistics showed improvements. However, more attention should be paid to women over 40 years of age and specific causes of maternal deaths, which should be taken into account in Korea's maternal and child health policies.

  • Chemical agents are classified according to their mechanism of toxicity into categories such as nerve agents, choking agents, blood agents, blister agents, etc. Among them, nerve agents cause toxic symptoms by binding with acetylcholinesterase (AChE) in the body, which breaks down neurotransmitters, thus disrupting the autonomic nervous system. In severe cases, this can lead to death, making it a critical chemical agent. Therefore, once it has penetrated into the human body, it is important to detoxify it swiftly. Antidotes used for detoxification include chemical medicines such as pretreatment agents, post-treatment agents, anticonvulsants, and bioscavengers. This review will address the uses, forms, components, and principles of detoxification of nerve agent antidotes and the association with bioscavengers.

The Development of the Analysis Program for the Electric Shock Accidents Occurred in the TN and TT System (TN 및 TT 계통에서의 감전사고 분석 프로그램 개발)

  • According to the statistics from the Ministry of labor, 70~80 people died by work-related electric shock accidents in the workplace and this number is about 10~20 times higher than those of the foreign countries such as Japan and the United Kingdom in frequencies and rates per 100,000. The electrocution deaths had decreased from several hundred in 1990s to 67 in 2002, since then there has been little change over the last 5 years. To reduce this number of the electrocution death, it is necessary to analyze the sources of the accidents and to implement the suitable measures for preventing the similar accidents from occurring. But there is not any consensus standard method or procedures for accident investigation yet. And the results solely depend on the ability of the investigator. Because of the demand of eliminating technical barrier for free trading, the IEC 60364 is adopted as a regulation for low voltage electrical installations in Korea. The facilities designed and constructed by IEC 60364 are increasing. The facilities by IEC 60364 have different features in comparison with the facilities by Korea eletro technical regulation, and these features combined with parameters of electric shock accident can greatly affect the consequences of the electric accidents. The accident analysis program for electric shock injuries in Terra Neutral(TN) and Terra Terra(TT) system was developed in this paper, which was based on KOSHA Electric Shock Analysis Program(ESAP).

Personnel's Perception toward Conducting an Autopsy in General Hospital (대학병원 직원들의 부검에 대한 인식도)

  • An autopsy is a postmortem assessment or examination of a body to determine cause of death or manner of death. The author had surveyed Personnel's perception toward conducting an autopsy in general hospital with autopsy at YeungNam University Medical Center, Daegu, Korea from April, to May, 2007. The total number of 286 personnel consisted of 121 men(42.3%), 165 women(57.7%). There were 57 doctors(19.9%), 71 nurses(24.8%), 83 medical technicians (29.0%), 58 office workers(20.3%), and 17 others(5.9%). 61.4% of doctors had an experience of education for autopsy more than 2 times, but nurses(1.4%), medical technicians(15.7%), office workers(1.7%), and others(5.9%) had little experience. Response of conducting an autopsy for sudden death of respondent or respondent's family member was 59.6% of doctors, 22.5% of nurses, 39.8% of medical technicians, 41.4% of office workers, and 35.3% of others. Response of conducting an autopsy for sudden death of respondent's companion was 66.7% of doctors, 33.8% of nurses, 39.8% of medical technicians, 43.1% of office workers, and 17.6% of others. Response of conducting an autopsy for sudden death of patients in general hospital was 50.9% of doctors, 8.5% of nurses, 19.3% of medical technicians, 24.1% of office workers, and 17.6% of others. Survey about a proper institution for autopsy showed 73.7% of doctors for department of forensic medicine in medical school, and 62.0% of nurses, 59.0% of medical technicians, 46.6% of office workers, and 58.8% of others for National Institute of Scientific Investigation. Most of the respondents agreed with the forensic pathologist as the director of autopsy:98.2% of doctors, 94.4% of nurses, 96.4% of medical technicians, 89.7% of office workers, and 88.2% of others. Survey for necessity for autopsy showed responsiveness of doctors, 23.9% of nurses, 47.0% of medical technicians, 34.5% of office workers, and 23.5% of others. Survey for donation of him- or herself after death to the medical school or institution for the death investigation revealed responsiveness of 22.8% of doctors, 11.3% of nurses, 24.1% of medical technicians, 22.4% of office workers, and 23.5% of others. The result of the survey questionnaire showed more negative awareness for autopsy in nurses, medical technicians, office workers, others and than doctors. To improve the negative awareness for autopsy and settle proper postmortem inspection system, education of professional manpower for forensic medicine and inaction of law which is adequate for the actual circumstance of Korea should be considered.

Mucopolysaccharidosis Type III: review and recent therapies under investigation

  • Mucopolysaccharidosis type III (MPS III or Sanfilippo syndrome) is a multisystem lysosomal storage disease that is inherited in an autosomal recessive manner. It consists of four subtypes (MPS IIIA, B, C, and D), each characterized by the deficiency of different enzymes that catalyze the metabolism of the glycosaminoglycan heparan sulfate at the lysosomal level. The typical clinical manifestation of MPS III includes progressive central nervous system (CNS) degeneration with accompanying systemic manifestations. Disease onset is typically before the age of ten years and death usually occurs in the second or third decade due to neurological regression or respiratory tract infections. However, there is currently no treatment for CNS symptoms in patients with MPS III. Invasive and non-invasive techniques that allow drugs to pass through the blood brain barrier and reach the CNS are being tested and have proven effective. In addition, the application of genistein treatment as a substrate reduction therapy is in progress.

  • From birth to death, every man is under the control of the three biorhythm curves that initiate from inner body. body. Those are called 'Physical rhythm, emotion I rhythm and intellectual rhythm'. These biorhythms have influence each other in human behaviour like physical endurance, creativity, record of examination. The result of investigation indicates that the students' records in low level period are lower than those of in high. Therefore, it is verified statistically whether the biorhythm has effects on human ability in scholarly record or not. And also, this research calculates the average nixed-biorhythm which is representable for a group by using mode mixed-biorhythm.

  • Injury has now replaced disease as the biggest single cause of death in children after their first birthday. Each month one child dies from preventable, unintentional injury and the medical cost of these injury is increasing remarkably. It is necessary to develop injury prevention system to manage, evaluate and analysis the information about accident. This resource manual developed as a result of literature review of child care safety project which is based on the Kidsafe Tasmanian Division in Australia and the other resources. The purposes of this study is to; 1. Develop model to enable services to focus on injury prevention 2. Develop standardized child care injury report form 3. Develop home safety checklist 4. Development of injury prevention policies The suggestions to develop injury prevention policies : 1. detail analysis of injury occurrence 2. investigation of known intervention and their effectiveness 3. analysis of policy environment 4. development of policy on implementation of intervention 5. develop protocol and materials to develop an injury prevention focus 6. increase knowledge and awareness among staff and parents of where injuries were occurring and develop.

Drug evaluation based on phosphomimetic PDHA1 reveals the complexity of activity-related cell death in A549 non-small cell lung cancer cells

  • Cancer cells predominantly generate energy via glycolysis, even in the presence of oxygen, to support abnormal cell proliferation. Suppression of PDHA1 by PDK1 prevents the conversion of cytoplasmic pyruvate into Acetyl-CoA. Several PDK inhibitors have been identified, but their clinical applications have not been successful for unclear reasons. In this study, endogenous PDHA1 in A549 cells was silenced by the CRISPR/Cas9 system, and PDHA1WT and PDHA13SD were transduced. Since PDHA13SD cannot be phosphorylated by PDKs, it was used to evaluate the specific activity of PDK inhibitors. This study highlights that PDHA1WT and PDHA13SD A549 cells can be used as a cell-based PDK inhibitor-distinction system to examine the relationship between PDH activity and cell death by established PDK inhibitors. Leelamine, huzhangoside A and otobaphenol induced PDH activity-dependent apoptosis, whereas AZD7545, VER-246608 and DCA effectively enhanced PDHA1 activity but little toxic to cancer cells. Furthermore, the activity of phosphomimetic PDHA1 revealed the complexity of its regulation, which requires further in-depth investigation.

  • Since hot toxic gas included in the fire is the most principal reason of the death and has the fluid mechanical characteristics unlike air, smoke control method appropriate for the figure and structure of a fire room is needed and each unit of the smoke control system requests high performance according to this characteristics, for which performance test procedures and evaluation criteria must be established. Domestic criteria involved with the smoke control consist of the pre-investigation by documents and the part inspection, which has lots of problems because they are far from the performance based evaluation method compared with the references of developed countries. Consequently, domestic and international references were compared and analyzed, problems being emerged and standardization scheme of the test procedure was presented.

A Case Study on the Human Error Analysis for the Prevention of Converter Furnace Accidents (전로사고 예방을 위한 인적오류 분석)

  • Occupational fatal injury rate per 10,000 population of Korea is still higher among the OECD member countries. To prevent fatal injuries, the causes of accidents including human error should be analyzed and then appropriate countermeasures should be established. There was an severe converter furnace accident resulting in five people death by chocking in 2013. Although the accident type of the furnace accident was suffocation, many safety problems were included before reaching the death of suffocation. If the safety problems are reviewed throughly, the alternative measures based on the review would be very useful in preventing similar accidents. In this study, we investigated the converter furnace accident by using human error analysis and accident scenario analysis. As a result, it was found that the accident was caused by some human errors, inappropriate task sequence and lack of control in coordinating work by several subordinating companies. From the review of this case, the followings are suggested: First, systematic human error analysis should be included in the investigation of fatal injury accidents. Second, multi man-machine accident scenario analyis is useful in most of coordinating work. Third, the more provision of information on system state will lessen human errors. Fourth, the coordinating control in safety should be performed in the work conducting by several different companies.
