직장인들의 구강건강신념과 구강건강관리 행태에 관한 연구 (A study on the oral-health belief and oral-health care of company employees)

  • The purpose of this study was to provide information on the development of an oral-health promotion program geared toward office workers. The subjects in this study were 240 employees at the headquarters of S construction company. After a survey was conducted in July and August 2008, the answer sheets from 208 respondents were gathered, and 191 answer sheets were analyzed except 17 unanalyzable ones. The findings of the study were as follows: 1. In regard to subjective oral health status, the largest number of the participants that accounted for 37.2% found their mouth to be neither healthy nor unhealthy. The greatest number of them that represented 58.1% had never visited a dentist's office to receive preventive treatment. 2. Regarding relationship between toothbrushing education experience and toothbrushing frequency, the most common toothbrushing frequency was three times a day( 46.5%) among the company employees who had ever received that education. In the event of the office workers without that experience, the most prevalent frequency was three times(63.6%). The toothbrushing frequency was statistically significantly different according to the toothbrushing education experience(p<0.05). As to the relationship of toothbrushing education experience to tooth- brushing method, the largest number(48.2%) of those who had ever received that education brushed their upper and lower teeth along with the gums by rotating the toothbrush downward from the top and upward from the bottom respectively. In the event of the office workers without that experience, 49.4 percent brushed their teeth and gums by rotating the toothbrush up and down. Thus, whether they had ever received education about toothbrushing made a statistically significant difference to their toothbrushing method(p<0.05). 3. As a result of evaluating their oral health belief by utilizing a five-point scale, they got a mean of 2.95. In each area of oral health belief, they got the best marks in benefit(3.66), salience(3.42), barrier(2.84), susceptility(2.58) and seriousness(2.23). Given the above-mentioned finding of the study, two sorts of oral health beliefs, which are benefit and salience, should be stressed in the development of oral health education programs that cater to company employees. And sustained research efforts should be channeled into finding out the relationship between oral health and actual oral health care.

미숙아의 수유 프로토콜 개발 및 적용 (Development and Effect of the Feeding Protocol for Preterm Infants)

  • 목적 본 연구는 미숙아를 위한 수유 프로토콜을 개발하고 적용하여 그 효과를 검증하기 위하여 시행되었다. 방법 미숙아 수유 프로토콜의 개발을 위해 전문가 집단을 구성하고, 문헌고찰과 협의를 통해 작성된 프로토콜에 대해 내용타당도 검증과 예비조사를 실시하여 확정하였다. 개발된 프로토콜은 비동등성대조군 전후시차설계로 B시의 P대학교병원에서 태어난 37주 미만의 미숙아 85명에게 적용되었다. 대조군(n=38)에게 2013년 1월부터 4월까지 일반적인 미숙아 수유관리가 시행되었고, 실험군(n=35)에게는 2013년 6월부터 9월까지 본 연구를 통해 개발된 미숙아 수유 프로토콜이 적용되었다. 수집된 자료는 SPSS 18.0 WIN program을 통하여 t-test와 ${\chi}^2$-test로 분석하였다. 결과 실험군은 대조군보다 첫 수유시간(t=2.22, p =.029)과 완전장관영양에 이르는 기간(t=2.28, p =.026)이 통계적으로 유의하게 감소하였다. 그러나 실험군과 대조군의 생후 7일째 체중 감소율(t=-1.23, p =.222)과 합병증 발생률(p >.05)은 통계적으로 유의한 차이가 없었다. 결론 미숙아 수유 프로토콜 적용을 통하여 미숙아들의 첫 수유시간과 완전장관영양에 이르는 기간이 단축되었다.

치매노인의 증상정도가 부양자의 자살생각에 미치는 영향에 대한 연구: 부양부담의 매개효과를 중심으로 (A Study on the Effect of Caregiver Burden on Suicidal Ideation among Caregiver for the Elderly with Dementia)

  • 본 연구는 부양자가 인지한 치매노인의 증상 정도와 치매노인 부양자의 자살생각의 관계에서 부양부담의 하위 요인을 세분화하여 매개효과를 검증하고, 치매노인 부양자를 위한 사회복지적 개입 방안 제시를 목적으로 한다. 이를 위해 2015년 현재 서울시, 경기도와 부산의 각 지역에서 데이케어센터 및 요양기관을 이용하는 치매노인 부양자를 대상으로 설문을 진행하였고 총 428부의 자료를 수집하였으며, 이들 중 응답이 부실한 13부을 제외한 415부의 설문지를 최종분석에 사용하였다. SPSS 21.0을 사용하여 매개효과와 그 유의성을 검증하였고 그 결과는 다음과 같다. 첫째, 치매노인 부양자 중 최근 1년간 자살에 대한 생각을 해본 적이 있다고 응답한 부양자는 21%로 나타냈다. 둘째, 치매노인의 증상 정도는 부양부담에 정적인 영향을 미치는 것으로 나타났다. 셋째, 부양자의 부양부담은 부양자가 인지한 치매 노인의 증상정도와 부양자의 자살생각과의 관계를 부분적으로 매개하고 있었고 이 매개효과는 통계적으로 유의미한 결과를 보여주었다. 이를 통해 부양자의 자살생각에 정적인 영향을 미치는 부양부담을 낮추기 위한 방안으로서 치매노인과 부양자에 대한 접근이 개선되어야 한다는 점을 제시하였고 이를 달성하기 위한 사회복지 중심의 프로그램 도입을 제안하였다.

통합예술치료가 초기치매노인의 인지기능과 우울에 미치는 영향에 관한 연구 (A Study on the Effect of Integrative Art Therapy on Cognitive Function and Depression in Elderly People with Early Dementia)

  • 본 연구의 목적은 통합예술치료가 초기치매노인의 인지기능과 우울에 미치는 영향을 객관적으로 살펴보고, 통합예술치료가 초기치매노인의 변화과정을 행동변화 관찰일지를 이용하여 종합적인 분석을 통해, 이를 규명하고 논의하는 데 있다. 연구방법으로 통합예술치료가 초기치매노인의 인지기능과 우울에 미치는 영향에 대한 양적연구와 함께 질적연구를 통한 심층 분석을 이용한 혼합연구를 설계하였다. 양적연구는 수원시 P 구의 주간보호센터를 이용하는 초기치매노인 20명을 실험대상으로 사전 사후검사를 하였고, 질적연구는 초기치매노인 10명을 대상으로 행동변화 관찰일지 분석을 하였다. 이러한 연구결과는 첫째, 통합예술치료가 초기치매노인의 인지기능에 유의한 영향(p<.001, t=-5.871을 미쳤다. 둘째, 통합예술치료가 초기치매노인의 우울에 유의한 영향(p<.001, t=5.325)을 미쳤다. 셋째, 통합예술치료 프로그램이 초기치매노인의 인지기능과 우울에 사전, 사후 변화에도 긍정적인 태도로 변화하였다. 본 연구결과를 통해 통합예술치료가 초기치매노인의 인지기능과 우울에 미치는 영향을 밝힘으로써 초기치매노인과 관련된 통합예술치료프로그램 등에 실효적 정보를 제공할 뿐 아니라 초기치매노인과 관련된 연구의 기초자료로 의미가 있다.

자가 및 동종 조혈모세포이식환자의 이식단계에 따른 불안과 우울 (The Levels of Anxiety and Depression according tn the Stages of Autologous and Allogeneic Hemopoietic Stem Cell Transplantation)

  • 목적: 자가 및 동종 조혈모세포 이식종류에 따라, 이식단계별 불안과 우울 정도를 파악함으로써 이식종류와 이식단계에 따라 조혈모세포이식 환자의 정서적 지지에 적합한 간호중재 개발을 위한 기초 자료를 마련하고자 시도되었다. 방법: 서울 소재 C대학병원에 2002년 8월부터 2003년 8월까지 조혈모세포이식을 위해 입원한 환자 52명을 대상으로(자가 조혈모세포이식 환자 19명, 동종 조혈모세포이식 환자 33명)조혈모세포 이식단계별 즉 입원 시, 이식 전날, 퇴원 시 총 3회 대상자를 방문하여 상태불안과 우울 정도를 조사하였다. 조사된 자료는 SAS 프로그램을 이용하여 분석하였으며, 자가 조혈모세포이식과 동종 조혈모세포이식의 이식단계별 불안과 우울은 반복측정 분산분석으로 검정하였으며, Bonferroni 다중비교로 사후검정을 실시하였다. 이식단계별 불안과 우울에 영향을 미치는 주요변인은 Stepwise multiple regression으로 분석하였다. 결과: 자가 조혈모세포이식 환자의 불안은 입원 시 42.3점, 이식전날 45.0점, 퇴원 시 40.8점이었으며, 동종 조혈모세포이식 환자의 불안은 입원 시 48.4점, 이식전날 48.7점, 퇴원 시 47.1점으로 두 군간 유의한 차이가 있었다(F=4.15, P=0.047). 자가조혈모세포이식 환자의 이식 전날 우울이 39.0점으로 입원 시 33.0점보다 유의하게 높았으며(F=21.45, P=0.0004). 동종 조혈모세포이식 환자의 우울도 이식전날 40.3점으로 입원 시 34.9점보다 유의하게 높았다(F=20.99, P=0.0002). 자가 조혈모이식단계별 불안에 영향을 미치는 주요인은 우울이었으며, 특히 입원 시에는 입원 전 직업유무가 불안에 영향을 주는 요인이었다. 동종 조혈모세포이식 환자의 입원 시 불안에 영향을 미치는 주요 요인은 우울, 성별이었으며, 퇴원 시에는 재발경험과 성별이었다. 자가 조혈모세포이식과 동종 조혈모세포이식 모두에서 이식단계별 우울에 영향을 미치는 요인은 불안으로 분석되었다. 결론: 이식전반에 걸쳐 동종 조혈모세포이식 환자의 불안과 우울이 자가 조혈모세포이식 환자보다 높았으며, 이식단계별로는 이식전날 우울이 가장 높았다. 따라서 조혈모세포이식을 위해 입원한 환자에게는 이식전날 우울에 대한 정서적 지지가 필요하며 특히 동종조혈모세포 이식환자는 입원 시, 이식 전날, 퇴원 시 등 이식 단계별로 불안과 우울을 경감시킬 수 있는 간호중재 개발과 그 적용이 요구된다.

병원 간호사의 선호근무시간대에 관한 연구 (A Study on Hoslital Nurses' Preferred Duty Shift and Duty Hours)

  • The duty shifts of hospital nurses not only affect nurses' physical and mental health but also present various personnel management problems which often result in high turnover rates. In this context a study was carried out from October to November 1995 for a period of two months to find out the status of hospital nurses' duty shift patterns, and preferred duty hours and fixed duty shifts. The study population was 867 RNs working in five general hospitals located in Seoul and its vicinity. The questionnaire developed by the writer was used for data collection. The response rate was 85.9 percent or 745 returns. The SAS program was used for data analysis with the computation of frequencies, percentages and Chi square test. The findings of the study are as follows: 1. General characteristics of the study population: 56 percent of respondents was (25 years group and 76.5 percent were "single": the predominant proportion of respondents was junior nursing college graduates(92.2%) and have less than 5 years nursing experience in hospitals(65.5%). For their future working plan in nursing profession, nearly 50% responded as uncertain The reasons given for their career plan was predominantly 'personal growth and development' rather than financial reasons. 2. The interval for rotations of duty stations was found to be mostly irregular(56.4%) while others reported as weekly(16.1%), monthly(12.9%), and fixed terms(4.6%). 3. The main problems related to duty shifts particularly the evening and night duty nurses reported were "not enough time for the family, " "afraid of security problems after the work when returning home late at night." and "lack of leisure time". "problems in physical and physiological adjustment." "problems in family life." "lack of time for interactions with fellow nurses" etc. 4. The forty percent of respondents reported to have '1-2 times' of duty shift rotations while all others reported that '0 time'. '2-3 times'. 'more than 3 times' etc. which suggest the irregularity in duty shift rotations. 5. The majority(62.8%) of study population found to favor the rotating system of duty stations. The reasons for favoring the rotation system were: the opportunity for "learning new things and personal development." "better human relations are possible. "better understanding in various duty stations." "changes in monotonous routine job" etc. The proportion of those disfavor the rotating 'system was 34.7 percent. giving the reasons of"it impedes development of specialization." "poor job performances." "stress factors" etc. Furthermore. respondents made the following comments in relation to the rotation of duty stations: the nurses should be given the opportunity to participate in the. decision making process: personal interest and aptitudes should be considered: regular intervals for the rotations or it should be planned in advance. etc. 6. For the future career plan. the older. married group with longer nursing experiences appeared to think the nursing as their lifetime career more likely than the younger. single group with shorter nursing experiences ($x^2=61.19.{\;}p=.000;{\;}x^2=41.55.{\;}p=.000$). The reason given for their future career plan regardless of length of future service, was predominantly "personal growth and development" rather than financial reasons. For further analysis, the group those with the shorter career plan appeared to claim "financial reasons" for their future career more readily than the group who consider the nursing job as their lifetime career$(x^2$= 11.73, p=.003) did. This finding suggests the need for careful .considerations in personnel management of nursing administration particularly when dealing with the nurses' career development. The majority of respondents preferred the fixed day shift. However, further analysis of those preferred evening shift by age and civil status, "< 25 years group"(15.1%) and "single group"(13.2) were more likely to favor the fixed evening shift than > 25 years(6.4%) and married(4.8%)groups. This differences were statistically significant ($x^2=14.54, {\;}p=.000;{\;}x^2=8.75, {\;}p=.003$). 7. A great majority of respondents(86.9% or n=647) found to prefer the day shifts. When the four different types of duty shifts(Types A. B. C, D) were presented, 55.0 percent of total respondents preferred the A type or the existing one followed by D type(22.7%). B type(12.4%) and C type(8.2%). 8. When the condition of monetary incentives for the evening(20% of salary) and night shifts(40% of. salary) of the existing duty type was presented. again the day shift appeared to be the most preferred one although the rate was slightly lower(66.4% against 86.9%). In the case of evening shift, with the same incentive, the preference rates for evening and night shifts increased from 11.0 to 22.4 percent and from 0.5 to 3.0 percent respectively. When the age variable was controlled. < 25 yrs group showed higher rates(31.6%. 4.8%) than those of > 25 yrs group(15.5%. 1.3%) respectively preferring the evening and night shifts(p=.000). The civil status also seemed to operate on the preferences of the duty shifts as the single group showed lower rate(69.0%) for day duty against 83. 6% of the married group. and higher rates for evening and night duties(27.2%. 15.1%) respectively against those of the married group(3.8%. 1.8%) while a higher proportion of the married group(83. 6%) preferred the day duties than the single group(69.0%). These differences were found to be statistically all significant(p=.001). 9. The findings on preferences of three different types of fixed duty hours namely, B, C. and D(with additional monetary incentives) are as follows in order of preference: B type(12hrs a day, 3days a wk): day shift(64.1%), evening shift(26.1%). night shift(6.5%) C type(12hrs a day. 4days a wk) : evening shift(49.2%). day shift(32.8%), night shift(11.5%) D type(10hrs a day. 4days a wk): showed the similar trend as B type. The findings of higher preferences on the evening and night duties when the incentives are given. as shown above, suggest the need for the introductions of different patterns of duty hours and incentive measures in order to overcome the difficulties in rostering the nursing duties. However, the interpretation of the above data, particularly the C type, needs cautions as the total number of respondents is very small(n=61). It requires further in-depth study. In conclusion. it seemed to suggest that the patterns of nurses duty hours and shifts in the most hospitals in the country have neither been tried for different duty types nor been flexible. The stereotype rostering system of three shifts and insensitiveness for personal life aspect of nurses seemed to be prevailing. This study seems to support that irregular and frequent rotations of duty shifts may be contributing factors for most nurses' maladjustment problems in physical and mental health. personal and family life which eventually may result in high turnover rates. In order to overcome the increasing problems in personnel management of hospital nurses particularly in rostering of evening and night duty shifts, which may related to eventual high turnover rates, the findings of this study strongly suggest the need for an introduction of new rostering systems including fixed duties and appropriate incentive measures for evenings and nights which the most nurses want to avoid, In considering the nursing care of inpatients is the round-the clock business. the practice of the nursing duty shift system is inevitable. In this context, based on the findings of this study. the following are recommended: 1. The further in-depth studies on duty shifts and hours need to be undertaken for the development of appropriate and effective rostering systems for hospital nurses. 2. An introduction of appropriate incentive measures for evening and night duty shifts along with organizational considerations such as the trials for preferred duty time bands, duty hours, and fixed duty shifts should be considered if good quality of care for the patients be maintained for the round the clock. This may require an initiation of systematic research and development activities in the field of hospital nursing administration as a part of permanent system in the hospital. 3. Planned and regular intervals, orientation and training, and professional and personal growth should be considered for the rotation of different duty stations or units. 4. In considering the higher degree of preferences in the duty type of "10hours a day, 4days a week" shown in this study, it would be worthwhile to undertake the R&D type studies in large hospital settings.

말기암 환자와 가족의 의료 및 간호 서비스 요구 (The Study on the Medical and Nursing Service Needs of the Terminal Cancer Patients and Their Caregivers)

  • In this study, we attempted to investigate the needs and problems of the terminal cancer patients and their family caregivers to provide them with nursing information to improve their quality of life and prepare for a peaceful death. Data was collected from August 1, 1995 to July 31, 1996 at the internal medicine unit of S hospital in Seoul area with the two groups of participants who were family members of terminal cancer patients seventy four of them were in-patients and 34 were out-patients who were discharged from the same hospital for home care. The research tool used in this study has been developed by selecting the questionnaires from various references, modifying them for our purpose and refining them based on the results of preliminary study. While general background information about the patients was obtained by reviewing their medical records, all other information was collected by interviewing the primary family caregivers of the patients using the questionnaire. The data collected were analyzed with the SPSS PC/sup +/ program. The results of this study are summarized as follows ; 1) Most frequently complained symptoms of the terminal cancer patients were in the order of pain(87%), weakness(86.1%), anorexia(83.3%) and fatigue (80.6%). 2) Main therapies for the terminal cancer patients were pain control (58.3%), hyperalimentation(47.2%) and antibiotics(21.3%). 3) Special medical devices that terminal cancer patients used most were oxygen device (11.1%), and feeding tube(5.6%). Other devices were used by less than 5% of the patients. 4) The mobility of 70.4% of the patients was worse than ECOG 3 level, they had to stay in bed more than 50% of a day. 5) Patients wanted their medical staffs to help relieve pain(45.4%), various physical symptoms(29.6%), and problems associated with their emotion(11.1%). 6) 16.7% of the family caregivers hoped for full recovery of the patients, refusing to admit the status of the patients. Also, 37% wished for the extension of the patient's life at least for 6 months. 7) Only 38.9% of the family members was preparing for the patient's funeral. 8) 45.4% of family caregivers prefer hospital as the place for the patient's death, 39.8% their own home, and 14.8% undetermined. 9) Caregivers of the patients were mostly close family members, i.e., spouse(62%), and sons and daughters or daughter-in-laws(21.3%). 10) 43.5% of the family caregivers were aware of hospice care. 46.8% of them learned about the hospice care from the mass media, 27.7% from health professionals, and the rest from books and other sources. 11) Caregivers were asked about the most difficult problems they encounter in home care, 41 of them pointed out the lack of health professionals they can contact, counsel and get help from in case of emergency, 17 identified the difficulty of finding appropriate transportation to hospital, and 13 stated the difficulty of admission in hospital as needed. 12) 93.6% of family caregivers demanded 24-hour hot line, 80% the visiting nurses and doctors, and 69.4% the volunteer's help. The above results indicate that terminal patients and their family caregivers demand help from qualified health professionals whenever necessary. Hospice care system led by well-trained medical and nursing staffs is one of the viable answers for such demands.

대전지역 직장 중년 남성의 건강 및 영양 상태 조사 (The Health and Nutritional Status of Middle Aged Men at Worksite in Taejon)

  • This study was conducted to investigate the health and nutritional status of 123 middle aged men at their worksite in Taejon. The results of this study on the factors that influence their health and nutritional status were as follows : 1) 74.8$\%$ of the subjects had history in the order of alimentary, heat, liver, diabetic and pulmonary diseases. 30.3$\%$ stopped smoking at 42.3% yrs. and 74.5$\%$ smoked more than 10 cigarets per day. Also 71.9$\%$ drank 2-3 times per week and 35.3$\%$ drank 1-2 times per week. 91.4$\%$ exercised more than 30min every day. 2) 54.4% showed concerns about their health whereas 20.3$\%$ were afraid that they night get sick. 3) 90.4$\%$ ate regularly and 54.5$\%$ worried about their cholesterol, salt, fat and MSG intakes. 48.7$\%$ ate out 1-2times per week and their favorite foods eaten outside were Korea. 4) 41.5$\%$ were classified as 'normal A', 30.9$\%$ 'normal B' group and high blood pressure and liber diseases in 'doubtful for disease' group were pointed out from their 1996 health check ups. 5) By Broca index, 39.8$\%$ were overweight and 9.8% were obese however by BMI only 23.6% were overweight. According to the relationship between calculated and self recognized obesity, 62.4$\%$ categorized themselves into the right weight range but 34.3$\%$ thought they were thinner than they were. 6) 43.9$\%$ were border line in cholesterol intake and 12.1$\%$ needed medical care for high blood cholesterol. 7) The Average energy intake was 1970.6㎉(80.9$\%$ RDA) with a 65 :19 : 16 ratio of carbohydrate : protein : fat. Protein, Fe, thiamin, riboflavin, niacin and Vit. A. 8) Occupation, regularity of meals, partner's job, income. smoking, alcohol drinking, health concerns and eating out were the factors that influenced the subject's nutrient intakes and health status. from this study, it was found that middle aged men needs to know their health and nutritional status and to be educated correct health and nutritional information through formal or informal channel. The worksite is the vest place to do this and we want these results to be used to develop the nutrition education program for middle aged men at the worksite.

치과위생사의 이직관련 요인 (Relative factors intent to leave for dental hygienists)

  • Objectives : This study was to find out the status of job satisfaction and intent to leave the job for dental hygienists, and those factors affecting their intent to leave, so as to prevent dental hygienists from leaving their jobs in advance, and to strive for extending the life span of the job, contributing to improve dental care services provided by dental hygienists. Methods : The data were collected from 280 dental hygienists working in dental clinics or hospitals in Busan and Gyeongnam area by using a structured and self-administered questionnaire through mail, from February 1 to March 15, 2009, and were analyzed by using SPSS WIN 14.0 program. Results : First, It was the most prevalent for the dental hygienists surveyed to be between 100 to 149 thousand won in the level of monthly as 42.3%, and to be the assistance of dental examination and treatment for main duty as 62.5%. Leaving job was experience by 51.6% of the subjects, with work condition as the most common reason for 32.2%. Second, the degree of job satisfaction was 3.4 points overall in the score of 5 point Likert's scale on the average, showing affirmative level above the average, and significantly different according to certain characteristics such as age, education level, total years of work career, years of work career in the current job, monthly salary level, whether to provide monthly paid leave, and the number of patients treated per day. It also was significantly correlated with some structural factors and variables consisting attitude and value, including equity in pay(r=0.612), job perception as dental hygienist(r=0.573), work environment(r=0.552), self growth(r=0.531), affirmative attitude(r=0.421), and so on. Third, the mean score of intent to leave was 2.73 overall and lower than the average, showing significant relationship with some characteristics, such as age, marital status, education level, total years of work career, years of work career in the current job, monthly salary level, whether to provide monthly paid leave, and the number of patients treated per day. Intent to leave also showed the highest correlation with job satisfaction(r=-0.708), followed by equity in pay(r=-0.482), self growth (r=-0.453), job perception as dental hygienist(r=-0.392), work environment(r=-0.362), etc. Forth, job satisfaction was significantly influenced by some factors including equity in pay, followed by job perception as dental hygienist, relationship with fellow colleagues, affirmative attitude and work environment, and so on. Fifth, the intent to leave was significantly affected not only by job satisfaction but also by some structural factors including equity in pay, self growth and job perception as dental hygienist. Conclusions : The above results showed that the equity in pay was the most important factor for the intent to leave the job as well as job satisfaction among dental hygienists, thus suggesting that systematic measures should be provided to improve the job satisfaction to prevent economic loss due to leaving job frequently, and to provide pertinent reward system as a profession to enhance the motivation for accomplishment on job for dental hygienists.

  • The problem addressed by this study was to determine the effect of nurse - patient agreement on the means to achieve nursing goals in the early postpartum period of primiparous mothers. It was hypothesized that the experimental treatment would result in hegher self-confidence and satisfaction in maternal role performance. This purpose was to contribute to the planning of nursing care to enhance self- confidence and satisfaction in maternal role performance and to the development of relevant nursing theory. Especially, the early postpartum period is crucial toward in recovery from childbirth and attainment of the maternal role. Maternal role attaintment is a complex social and cognitive process of stimulus -response accomplished by learning. Most women attain the maternal role sucessfully. But, some primiparous mothers experience difficultites in attainment of the maternal role due to lack of experience and knowledge. Self-confidence and satisfaction in maternal role performance are important factors in attainment and adjustment to the maternal role (Mercer, 1981a, 1981b ; Lederman, Weigarten, and Lederman, 1981 :Bobak and Jensen, 1985). Nursing is defined as behaviors of nurses add patients that attain nursing goals through action, reaction, interaction, and transaction. For attainment of nursing goals, active participating transactions must occur by agreement on the means to achieve those goals through nurse -patient mutual goal setting and establishment of their active relationships(King, 1981, Ha, 1977). Based on King's theory of goal attainment (1981), this stuy was planned as a non-equivalent control group, non -synchronized quasi -experimental design using agreement on the means to achieve nursing goals in early postpartum as the experimental treatment. The data were collected from July 20 to Sep. 1, 1991 by questionnaires with 60 primiparous mothers planing to breast feed after normal deliveries at W hospital in Pusan, Korea. The subjects were divided into a control group(conventional group) -those admitted from July 20 to Aug. 12, and an experimental group(agreement group) - those admitted from Aug. 13 to Sep. 1. The instument for agreement on the means to nursing goals in the early postpartum period included five steps - identification of disturbances of problems through action, reaction, and interaction with primiparous mothers : mutual early postpartal nursing goal setting : exploration of the means to achieve goals ; agreement on the means (self- care, ealry maternal -infant contact, performance of mothering behavior, and communicating about the infant's behavior and health condition) : implementation of the means. This instrument was developed on the basis of King's elements that lead to transactions in nurse-patient interactions. Lederman et al's (1981) scale for Confidence in ability to cope with tasks of motherhood and Lederman et al's(1981) scale for Mother's satisfaction with motherhood and infant care were used to measure self-confidence and satisfaction in maternal role performance ·with the subjects immediately after admission and on the day of discharge. Self-care performance in the experimental group was measured by self -evaluation tool developed by the investigator from the literature concerned. The tools to measure Pelf-confidence and satisfaction in maternal role performance, and the tool to measure self-evaluation of self-care performance were tested for internal reliability. Cronbach's Alphas were 0.94, 0.94, and 0.63. The data were analysed by using in S.P.S.S. computerized program and included percentage, x²-test, t-test, ANOVA, and Pearson Correlation Coefficient. The conclusions obtained from this study are summerized as follows : 1. The degree of self-confidence in maternal role performance of the total subjects group measured before the experimental treatment was above average with a mean score of 2.77(range 2.14-3.64). Out of 14 items, those with relatively high mean scores were ‘I would like to be a better mother than I am’(3.95), and ‘I have my doubts about whether I am a good mother’(2.87). Those with low mean scores were ‘I know that my baby wants most of the times’(2.28), ‘When the baby cries, I can tell what she /he wants’(2.37), and ‘I have confidence in my ability to care for the baby’(2;50). That is, the self - confidence of Primiparous mothers was considerably high in mothering, but rather low in activities concerning the infant care and understanding of the infant behavior. The degree of satisfaction in maternal role performance of the total subjects group measured before the experimental treatment was high with a mean score of 3.18(range 1.92-3.92). Out of 13 items, those with relatively high mean scores were ‘I am glad 1 had this baby now’(3.75), ‘I play with the baby between feedings when s/he is awake and quiet’(3.67), and ‘I enjoy being a mother’(3.27). Those with low mean scores were ‘I am upset about having too many responsibilities as a mother’(2.78), ‘It bothers me to get up for the baby at night’(2.82), and ‘I get annoyed if the baby frequently interrupts my activities’.(2.82), That is, the satisfaction of primiparous mothers was considerably high in mothering and infant care, but rather low in restraints in time or on the mother's self accomplishment and development. 2. Agreement on the means to achieve nursing goals in the early postpartum period included process of mutual goal setting, exploration of the means to achieve goals, and ahreement in concert means to achieve goals based on the mothers' condition, concerns, self-perception of the nurse - patient interactions. In the process of agreement, there was agreement that the means to achieve goals should be through trust and establishment of active relationships with the nurse through identification of problems according to planned nursing goals and active interaction, such as explanations, teaching, changing of opinions, acceptance or rejection of explanations, and proposing of questions. Therefore agreement on the means to achieve nursing goals in the early postpartum period appears to be an effective nursing intervention for primiparous mothers. 3. The degree of self- confidence in maternal role performance of the exprimental group was higher than that of the control group(t=3.95, p<0.01). Out of 14 items, those with higher score in the experimental group were ‘I would like to be a better mother than I am’(t=1.93, p<0.05), ‘I know that my baby wants most of the times’(t=2.75, p<0.01), ‘When the baby cries, 1 can tell what she/he wants’(t=2.10, p<0.05), ‘I have confidence in my ability to care for the baby’(t=3.72, p<0.01), ‘I trust my own judement in deciding how to care for the baby’(t=1.96, p<0.05), ‘I feel that I know my baby and what to do for him /her’(t=2.44, p<0.01), ‘I am concerned about being able to meet the baby's needs’(t=2.87, p<0.01), ‘I know what my baby likes and dislikes’(t=3.26, p<0.01), ‘I don't know to care for the baby as well as I should’(t=2.07, p<0.05), and ‘I am unsure about whether I give enough attention to the baby’(t=3.04, p<0.01), That is, the degree of self-confidence in mothering, activities concerning infant care, and understanding of infant behavior of the experimental group was higher than that of the control group. Therefore, the first hypothesis, that the degree of self-confidence in maternal role performance of the experimental group would be higher than that of the control group, was supported(t=3.95, p<0.01). 4. The degree of satisfaction in the maternal role performance of the exprimental group was higer than that or the control group(t=2.31, p<0.05). Out of 13 items, those with higher score in the experimental group were ‘I am glad I had this baby now’(t=2.29, p<0.05), ‘I enjoy taking care of the baby’(t=2.4g, p<0.01), ‘It is boring for me to care for the baby and do the same thing over and over’(t=2.87, P<0.01), ‘I am unhappy with the amount of time I have for activities other than childcare’(t=2.51, p<0.01), and ‘When bathing and diapering the baby, I would like to be doing something else’(t=2.43, p<0.01). That is, the degree of satisfaction in mothering, infant care, and restraints in time of on the mother's self accomplishment and development in the experimental group was higher than that of the control group. Therefore, the second hypothesis, that the degree of satisfaction in maternal role performance of the experimental group would be higher than that of the control group, was supported(t=2.31, p<0.05). 5. The third hypothesis, that the higher the degree of satisfaction in materenal role performance, the higher the degree of self-confidence in materenal role performance in the experimental group, was supported (r=0.57, p<0.01)
