Case reports: Effect of Korean Medicine Including Pharmacopuncture on Chemotherapy Induced Peripheral Neuropathy (항암제 유발 말초신경병증 한의 치험 2례)

  • Objective : Peripheral neuropathy refers to the symptoms caused by damage to peripheral fibers, inflammation and degeneration. This study reports the effects of moxibustion, electric moxibustion, and pharmacopuncture including GeonChil(Rhus verniciflua stokes, 乾漆)and Whalhyul(活血) pharmacopuncture on patients with peripheral neuropathy induced by anti-cancer drugs and chemotherapy. Method : We administered moxibustion, electric moxibustion, GeonChil and Whalhyul pharmacopuncture to two patients who showed peripheral neuropathy induced by anti-cancer drugs and chemotherapy. The symtoms were evaluated using Visual Analog Scale (VAS) and chemotherapy induced peripheral neuropathy assessment tool (CIPNAT). Results : Following observations were made after treatments. Case 1 : After nine procedures, the score of VAS was decreased. Feeling of cold and numbness were improved, and as rotation movement of ankle was also possible, gait disturbance were improved. Case 2 : After, seven procedures symptoms of both shoulder pain were improved, and the symptoms of peripheral neuropathy were eliminated. Conclusion : We found the possibility of symptom improvement after moxibustion, electric moxibustion, and pharmacopuncture treatment on peripheral neuropathy caused by anti-cancer drugs. Clinical studies of pilot study and control settings will need to be carried out later.

Rapid identification of Burkholderia glumae from diseased seeds

  • Bacterial grain rot by Burkholderia gluae cause severe damage in seedling and grain of rice after heading season. This seed-borne pathogen play a role as first infection agent that could be cause disease following cropping season. Until now the direct isolation of the bacteria has some trouble by interference of other bacteria existed inside seed. This study established convenient identification method as simple isolation with KB medium from seed showing symptom and using PCR identification. By this isolation method, B. glumae was isolated from 40 to 50% in brown rice and inner hull, however, there were saprophytic bacteria and fungi outer hull. In PCR identification with Ogf4 and Ogr3 primer to these 25 isolates, the amplified products were presented in all of the collections but not in 10 saprophytic germs. The isolation rate was constant to 3 months stored seeds. This result provide a rapid and convenient isolation and identification of B. glumae.

  • In Neijing(內經) the theory of Reverse(厥) was explained syncope causes by disharmony of emotions irregularity, deficient or excessive Qi(氣虛, 氣實) and blood disorder, it was the causes of cold hypersensitivity of hands and feet except coldness itself, include impairment of Qi circulation and the deficiency of the kidney weakened essence and blood, weakness and damage in the kidney essence, deficiency of the lower part and deficiency of the lower Qi as the major causes. In Shanghanlun(傷寒論) the theory of Reverse(厥) was divided into by disharmony of heat or cold reversal symptom. In Jingyue quanshu(景岳全書), causes of Reverse(厥) was Qi and blood disorder, damps(痰飮), alcohol and sex include syncope. In Dongeuibogam(東醫寶鑑), causes of Reverse(厥) was deficient or excessive Qi, impairment of blood circulation which means the deficiency of blood, essence(精) and blood stasis (瘀血), and disorder of gastrointestinal system which means malfunction of gastrointestine, damps(痰飮) and toxicity of alcohol. The rest of the causes include San syndrome(疝症), sun stroke(暑病) and heat reversal(熱厥).

Study on the Dossisosihotang (도씨소시호탕(陶氏小柴胡湯)에 대한 연구(硏究))

  • Objectives : 28 prescriptions with the name 'Dossi' can be found in Yixuerumen, and all these prescriptions are highly useful due to their clinical effectiveness. Among them, Dossisosihotang has an important role as the essential prescription utilizing harmonizing and releasing formula. Therefore, this paper aims to ponder upon this prescription. Methods : Dossisosihotang's origin and effectiveness were studied, and the findings served as the basis for analyzing the varied forms of this prescription which came about in the later generations in order to consider more detailed usage of this prescription. Results & Conclusions : Dossisosihotang directs its patient to consume sosihotang added with chenpi, shaoyao, chuanxiong, then simmered, and added with three spoons worth of raw wormwood juice. Its origin can be traced back to shachechuifa's 'Sihossanghaeeum,' and it is mainly effective for treating the blood aspect syndrome of lesser yang disease such as women's cold damage and heat entering the blood chamber. In treating the symptom of half-exterior and half-interior, taohua developed prescriptions depending on the syndrome, such as Sihijigyungtang, Sihohamhyoungtang, Samhosambacktang, Siryungtang, Sihosamultang, Sigalhaegitang, and Samhoseoggotang. These prescriptions became widely known for their clinical efficacy, leading to many later generations of clinical doctors to use them.

  • Hypertension is one of the most common chronic diseases in childhood and adolescence. Untreated hypertension adversely affects many organs including heart, brain, kidney and peripheral arteries. We reviewed the complication of central nervous system caused by pediatric hypertension. Cerebral blood flows are maintained constantly in response to changes in blood pressure by cerebral autoregulation. Severe hypertension which destructs cerebral autoregulation results in acute hypertensive encephalopathy syndrome, ischemic or hemorrhagic stroke. Chronic pediatric hypertension induces learning disability and cognitive defect which are subclinical symptom prior to brain damage caused by severe hypertension. We should consider the effect of hypertension on pediatric brain because appropriate antihypertensive drugs could prevent these complications.

  • "Byun Jeung Rok(辨證錄)" written by Jinsatak(陳士鐸) is composed of 14 volumes. In relation to the contents, it is organized into 126 gates(門) and 770 remaining syndromes(餘證) where internal medicine, external medicine, pediatrics, gynecology(內科 外科 小兒 婦人), etc. are divided into sub-sections of cold damage, cold stroke, wind stroke(傷寒 中寒 中風), etc. He explained the symptom, cause of disease, method of treatment, prescription, construction of prescription, instruction of medicine and prognosis(症狀 病因 治法 處方 處方構成 服用法 預後) thoroughly. This study, as an inquiry of the sec 14 volume pediatrics(小兒), deals with febrile fit cacotrophy vomiting diarrhea(驚疳吐瀉), coprozoic parasite(便蟲), smallpox(痘瘡), eruption(疹症), eating mud(喫泥), fetal toxin(胎毒). It was written very logically so it is easy to understand. The analysis of the symptoms are brief and appropriate. Therefore, it is considered to have significant clinical value for future generations and is applied by them. Finally, this topic was studied in hopes of helping later pediatrics clinical treatment.

Numb Chin Syndrome as the First Symptom of Diffuse Large B-cell Lymphoma

  • Numb chin syndrome, or mental neuropathy is a rare sensory neuropathy characterized by abnormal sensation such as hypoesthesia, paresthesia, or dysesthesia in the chin and lower lip innervated by the mental nerve. Sensory neuropathy of mental nerve is somewhat familiar to dentists because it can occur following nerve damage by trauma or dental treatment such as implant surgery or third molar extraction. It can also result from dental causes including abscess or osteomyelitis. However, it can be the first sign of the systemic disease or malignancy if it is not related to dental causes. In this study, we present the case of a patient who present with hypoesthesia and pain in chin area without other symptoms and is later diagnosed with diffuse large B-cell lymphoma.

  • Until now, occurrence of rice stripe virus (RSV) is limitted in southern part of Korea. However, recently the occurrence of RSV is increased and spreaded in central part of Korea including Chungcheong and Kyonggi province. It is very difficult to distinguish RSV symptoms on virus symptom physiological damage of rice. We detected RSV viral RNA from infected rice and its insect vector Laodelphax striatellus using specific primer of RSV-polymerase and coat protein gene with reverse transcription (RT)-PCR. The result of RT-PCR, we observed specific band including RSV-polymerase (1,,023 bp) and CP (969 bp) in both host of rice and insect vector.

Median Nerve Stimulation in a Patient with Complex Regional Pain Syndrome Type II

  • A 54-year-old man experienced injury to the second finger of his left hand due to damage from a paintball gun shot 8 years prior, and the metacarpo-phalangeal joint was amputated. He gradually developed mechanical allodynia and burning pain, and there were trophic changes of the thenar muscle and he reported coldness on his left hand and forearm. A neuroma was found on the left second common digital nerve and was removed, but his symptoms continued despite various conservative treatments including a morphine infusion pump on his left arm. We therefore attempted median nerve stimulation to treat the chronic pain. The procedure was performed in two stages. The first procedure involved exposure of the median nerve on the mid-humerus level and placing of the electrode. The trial stimulation lasted for 7 days and the patient's symptoms improved. The second procedure involved implantation of a pulse generator on the left subclavian area. The mechanical allodynia and pain relief score, based on the visual analogue scale, decreased from 9 before surgery to 4 after surgery. The patient's activity improved markedly, but trophic changes and vasomotor symptom recovered only moderately. In conclusion, median nerve stimulation can improve chronic pain from complex regional pain syndrome type II.

FAILURE OF ODONTOGENESIS AFTER CHEMO-RADIATION THERAPY FOR RHABDOMYOSARCOMA (횡문근육종의 항암제-방사선치료 후 치아발육장애)

  • This report details a case of 8-year-old girl showing failure of odontogenesis after chemo-radiation therapy for the rhabdomyosarcoma at the age of 4. The observed results were as follows : 1. Past history revealed that she had received for a total radiation dose of 4430cGy, 29 fractions in 6 weeks and chemotherapy with vincristine, actinomycin D and cytoxan, followed as maintenance phase for 2 years. 2. The patient was symptom -free and appointed for the treatment of multiple dental caries. 3. Oral examination showed hypoplastic enamel on whole erupted permanent teeth and showed retarded eruption. 4. Conventional radiograms showed failure of root development including abrupt cessation of root formation and root agenesis, and microdontia, missing teeth, irregular enamel, dislocation of the impacted teeth. Additional finding showed good healing bone pattern on the left mandibular ramus and angle area. 5. Cephalometric analysis revealed failure of bite raising due to incomplete eruption of all the first molars and made it possible to suspect entrapped mandibular growth and then Class II tendency growth. 6. There was correlation between the time of chemo-radiation therapy and the damage of the teeth.
