Effects of Line Dance Exercise on HOMA-IR and Energy Metabolic Factors in Postmenopausal Obese Women (라인댄스 운동이 폐경 후 비만여성의 인슐린저항성지수와 에너지대사조절인자에 미치는 영향)

  • The purpose of this study was to analyze the effects of line dance exercise on HOMA-IR and energy metabolic factors in postmenopausal obese women. The subjects were 24 obese women composed of the line dance group(n=12) trained for 12-week and the control group(n=12). The variables of body composition, HOMA-IR and energy metabolic factors were measured in all the subjects before and after the 12-week line dance exercise. The results of the study in the line dance group were as follows; The body weight, %fat, BMI, WHR and VFA had significantly decreased, but LBM had increased. The ghrelin, HDL-C had significantly increased, but leptin, insulin, glucose, HOMA-IR, TC, TG, LDL-C, SBP and DBP had decreased. And ghrelin, leptin, insulin, and glucose were the energy metabolic factors affecting the HOMA-IR in the line dance group. Therefore, regular and continuous line dance exercise will be effective for preventing the life-style diseases with improvement of the energy metabolic factors and insulin resistance by decreasing the body fat.

Garlic powder intake and cardiovascular risk factors: a meta-analysis of randomized controlled clinical trials

  • BACKGROUND/OBJECTIVES: Although preclinical studies suggest that garlic has potential preventive effects on cardiovascular disease (CVD) risk factors, clinical trials and reports from systematic reviews or meta-analyses present inconsistent results. The contradiction might be attributed to variations in the manufacturing process that can markedly influence the composition of garlic products. To investigate this issue further, we performed a meta-analysis of the effects of garlic powder on CVD risk factors. MATERIALS/METHODS: We searched PubMed, Cochrane, Science Direct and EMBASE through May 2014. A random-effects meta-analysis was performed on 22 trials reporting total cholesterol (TC), 17 trials reporting LDL cholesterol (LDL-C), 18 trials reporting HDL cholesterol (HDL-C), 4 trials reporting fasting blood glucose (FBG), 9 trials reporting systolic blood pressure (SBP) and 10 trials reporting diastolic blood pressure (DBP). RESULTS: The overall garlic powder intake significantly reduced blood TC and LDL-C by -0.41 mmol/L (95% confidence interval [CI], -0.69, -0.12) (-15.83 mg/dL [95% CI, -26.64, -4.63]) and -0.21 mmol/L (95% CI, -0.40, -0.03) (-8.11 mg/dL [95% CI, -15.44, -1.16]), respectively. The mean difference in the reduction of FBG levels was -0.96 mmol/L (95% CI, -1.91, -0.01) (-17.30 mg/dL [95% CI, -34.41, -0.18]). Evidence for SBP and DBP reduction in the garlic supplementation group was also demonstrated by decreases of -4.34 mmHg (95% CI, -8.38, -0.29) and -2.36 mmHg (95% CI, -4.56, -0.15), respectively. CONCLUSIONS: This meta-analysis provides consistent evidence that garlic powder intake reduces the CVD risk factors of TC, LDL-C, FBG and BP.

Qu-Ji-Du-Huang-Wan for Essential Hypertension : Systemic Review and Meta-analysis (본태성 고혈압에 대한 기국지황환(杞菊地黄丸)의 효과에 관한 체계적 고찰 및 메타 분석)

  • The purpose of this study is to investigate the blood pressure reducing effect of Qi-Ju-Di-Huang-Wan (QJDHW) in adults with essential hypertension by using methods of systemic review and meta-analysis. Major search engines, such as PubMed, EMBASE, Cochrane library, Web of Science, CNKI, CiNii, J-STAGE, KISS, NDSL, RISS, OASIS, DBpia and so on, were used. The search period we used is from the start date of the search engine to October 30, 2016 and no language limits were placed. Randomized controlled trials using QJDHW in adults with essential hypertension were searched and extracted by two independent researchers. Meta-analysis was performed on outcome variables of the total effective rate (TER), systolic blood pressure (SBP) and diastolic blood pressure (DBP). Risk of bias (RoB) of Cochrane was used to assess methodological quality. Thirteen studies were finally selected. We observed that the combined treatment of QJDHW and antihypertensive drug had 3.6 times the odds ratio of TER for blood pressure lowering than a single use of an antihypertensive drug. Additionally, mean differences of SBP and DBP were -8.88 mmHg (95% Confidential Interval (CI) -12.77 mmHg, -5.00 mmHg, P<0.00001), -7.09 mmHg (95% CI -9.93, -4.25, P <0.00001), respectively. Single use of QJDHW did not reduce blood pressure more than an antihypertensive drug. All items of RoB were unclear and the methodological quality was low. Our analysis suggests that the combination of QJDHW and antihypertensive drugs may be more effective in reducing blood pressure than a single antihypertensive drug. But due to low methodological quality, careful interpretation will be needed and systematic long-term clinical trials will be required.

A Case Study on Chlorine Dioxide Usage at a Conventional Water Treatment Plant (기존 정수장 이산화염소 시범도입 사례연구)

  • As the regulations on DBPs are tightened, many water treatment plants (WTPs) in Korea have already introduced or will introduce enhanced coagulation, alternative disinfectants and advanced treatments such as ozonization and granular activated carbon to improve drinking water qualify. After a phenol leakage accident at the Nakdong-River in 1991, 26 WIPs in Korea introduced carbon dioxide generators, but there has been no accumulation of significant operating data. This research summarizes things that should be considered for the introduction of carbon dioxide disinfection process to WTPs based on one year operation data from A WTP that has had high concentration of DBP during a specific period in the summer. The removal efficiency of DBP was $30{\sim}40%$, but those of 2-MIB, Geosmin were less than 10%. The generation rate of $ClO_2$ by-products such as chlorite and chlorate were $70{\sim}100%$ of input dosage, but the ratios increased over time. At the same time, strong chlorine odors may be produced in the distribution system when $ClO_2$ was used with $Cl_2$ as a result of reaction between the chlorite and residual chlorine.

The Effect of Cheonmagudeung-eum for Hypertension: A Systematic Review and Meta-analysis (고혈압에 대한 천마구등음의 효과 : 체계적 문헌고찰과 메타분석)

  • Objective: The purpose of this study is to investigate the effect of Cheonmagudeung-eum (CGE) for essential hypertension by systematic review and meta-analysis. Methods: The period of literature search was until October 30, 2016, and 14 electronic databases were utilized as search engines. The evaluation for the risk of bias (RoB) was conducted by using the Cochrane Risk of Bias Tool. The meta-analysis was performed by synthesizing outcome data, including total effective rate (TER), systolic blood pressure (SBP), diastolic blood pressure (DBP), and the incidence of adverse events. Result: There were a total of 64 RCTs using CGE on adult essential hypertension. In the RoB evaluation, most of the items were unclear, and the qualities of studies were rated low. The concurrent treatment of CGE and antihypertensive drug (AHD) showed a significant hypotensive effect since the risk ratio (RR) of TER was 1.17 times (95% CI 1.14, 1.20, p<0.01) higher than that of AHD alone. In addition, the mean difference (MD) appeared low as 8.73 mm/Hg in SBP (95% CI -11.36, -6.09, p<0.01) and 5.81 mm/Hg in DBP (95% CI -7.50, -4.12, p<0.01). Conclusion: Through this study, it was identified that the combined treatment of CGE and AHD on hypertension would be more effective than that of AHD treatment alone. However, due to the low quality of the selected original articles, the significance of this conclusion is somewhat limited, and we hope that this would be complemented through more rigorous RCTs in the future.

Clinical Significance of Abdominal Fat Distribution in Korean Male Children and Adolescents (한국 소아 청소년 남아에서 복부 지방 분포 측정의 임상적 의의)

  • Purpose: Visceral adipose tissue may be strongly linked to increased metabolic risks in adults. However, because little is known regarding the effect of visceral adipose tissue in children and adolescents, we performed this study to determine the association between abdominal fat distribution and metabolic risk factors in this population. Methods: One hundred one children and adolescents (78 males and 23 females; mean age, 10.8${\pm}$2.4 years) were enrolled. The anthropometric data and metabolic risk factors were evaluated. Theabdominal fat distribution was assessed according to the CT measurement. Age-adjusted, partial correlations were performed among the visceral adipose fat area (VFA), subcutaneous adiposefat area (SFA), metabolic risk factors, and anthropometrics. Results: The SFA increased more rapidly than the VFA with advancing years in both genders. In males, the VFA and SFA were positively correlated with anthropometrics. The VFA was correlated with low HDL-cholesterol and the SFA was correlated with diastolic blood pressure (DBP). However, there was no statistical significance between the VFA, SFA, anthropometrics, and other metabolic risk factors. The VFA and SFA were strongly linked to a number of metabolic risk factors, such as other anthropometrics. Conclusion: This study investigated how a low HDL-C was correlated with VFA and how a high DBP was associated with SFA in Korean male children and adolescents. Our results suggest that the correlation between the VFA, SFA, and metabolic risk factors was relatively weak compared to that reported in previous adult studies.

Evaluation of Obesity from BMI and Body Fat Rate, and Its Association with Coronary Risk Factors in Health Checkup Examinees (건강검진 수검자의 BMI와 체지방율에 의한 비만도 평가 및 관상동맥질환위험인자와의 관련성)

  • To evaluate the degree of obesity based on differential combinations of BMI and body fat ratio, and thereby to obtain the comprehensive determination of coronary heart disease risk factors. The present study was conducted among 3,731 individuals(male; 2,312, female; 1,419) who underwent general health checkup in the general health promotion center in a university hospital during 2-year period from Jan. 2008 to Dec. 2009. Based on the obesity categories from combinations of BMI and body fat ratio, 30.2% were "normal", 8.9% were "seemingly obese", 31.2% were "occult obese", and 29.8% were "obese". By sexual comparison, the proportion of "normal" and "seeming obese" individuals were higher in men than women, while "occult obese" and "obesity" individuals were preponderant in women compared to men. By age, the proportion of "obesity" individuals were higher in the 40' and 50's than in 30's and 60's. Based on the association between obesity categories and each of CRF, SBP, DBP, TC, HDL-C, LDL-C and TG was significantly higher in "seeming obese" and "obesity" group than in "normal" group, but SBP, TC and LDL-C was significantly higher in "occult obese" and "obesity" group than in "normal" group. For odds ratios of each of CRF for obesity categories, it was higher in "obesity" group, "seeming obese" group, "occult obese" group than in "normal" group in that order.

Effects of Mobile based-Healthcare Service for Hypertension Patients (고혈압 환자 대상의 모바일 헬스케어서비스 운영 효과)

  • This study aims to evaluate the effect of mobile healthcare service which was provided to employees diagnosed with hypertension in employee's health checkup. The study subjects were 146 employees and nurse measured their blood pressure, body mass index, and blood tests at the clinic in a workplace. The general characteristics of the participants were analyzed using descriptive statistics. After providing health care service using mobile text message for 8 weeks, blood test and blood pressure was measured. As a result, it was confirmed that blood pressure(SBP: 139.32±10.38 → 133.96±11.31, DBP: 98.13±6.21 → 94.28±8.56) and blood test(HDL: 47.90±9.79 → 51.40±9.79, HbA1C: 5.96±.66 → 5.65±.71) were more significantly reduced in 56 employees with a systolic blood pressure of 140 mmHg or diastolic blood pressure of 90 mmHg or more. It was confirmed that the program for self-management of blood pressure by receiving text messages on a mobile basis was effective. It is expected that the results of this study will be used as basic data for healthcare services that provide text information using mobile to improve drug usage, physical exercise, and eating habits.

Comparison of Physiological Variables by Age Group in Drinking Men (음주남성의 나이대별 생리학적 변수들의 비교)

  • This study was designed to investigate the differences of physiological variables by age group in alcohol-consuming men. The thirty year old-age group had the lowest waist-hip ratio (WHR), right and left cardio-ankle vascular index (R-CAVI and L-CAVI), right and left ankle-brachial index (R-ABI and L-ABI), amylase, high sensitivity C-reactive protein (hs-CRP), and creatine phosphokinase (CPK) levels, and the highest body weight (BW), interleukin-6 (IL-6), right and left intraocular pressure (R-IP and L-IP), hematocrit (Hct), hemoglobin (Hb), erythrocyte, red blood cells distribution width (RDW), alanine aminotransferase (ALT), total cholesterol (TCH), triglyceride (TG), and ferritin levels. The forty year old-age group was the highest in hs CRp and CPK levels, but the lowest in ferritin level. The fifty year old-age group possessed the highest WHR, diastolic blood pressure (DBP), R- and L-ABI, eosinophil, and amylase levels. The sixty year old-age group had the highest R- and L-CAVI, mean corpuscular volume (MCV), ALP, $\gamma$-glutamyltranspeptidase ($\gamma$-GTP), rheumatoid factor (RF), $\alpha$-fetoprotein (AFP) and prostate specific antigen (PSA) levels and the lowest BW, DBP, R-IP, L-IP, Hct, Hb, leukocyte, platelet, RDW, eosinophil, monocyte, ALT, amylase, TG, and CPK levels. These findings indicate that there may be differences of physiological variables depending on age group in alcohol-consuming men. Further studies should be focused on the physiological differences between alcohol-consuming men and women.

Ozone Inhalation with 4-(N-methyl-N-nitrosamino)-1-(3-pyridyl)- 1-butanone and/or Dibutyl Phthalate Induced Cell Cycle Alterations via Wild-type p53 Instability in B6C3F1 Mice

  • Changes in cell cycle control in the lungs and liver of the B6C3F1 mice (20 males per each group) exposed to ozone (0.5 ppm), 4-(N-methyl-N-nitrosamino)-1-(3-pyridyl)-1-butanone (NNK, 1.0 mg/kg), and dibutyl phthalate (DBP, 5,000 ppm) after 52 weeks were examined through Western, Northern blot, and immunohistochemistry based on alterations in protein expression levels of G1/S checkpoints (cyclin D1, cyclin E, and PCNA), G2/M checkpoints (cyclin B1, cyclin G, and cyclin A), negative regulators (p53, p21, GADD45, and p27), and positive regulator (mdm2). Expression levels of cyclins D1, E, G, PCNA, mutant p53, and mdm2 proteins were higher in the lungs and livers treated with combination of toxicants than in those treated with ozone only. Expression levels of the wild-type and mutant p53, p21, GADD45, p27, and mdm2 proteins and mRNAs were higher in toxicant-treated groups than those of the control. Immunohistochemical analysis revealed staining intensities of the PCNA, cyclin D1, c-myc and mdm2 protein- treated lungs and livers were stronger than those of the control group. Our results showed that combined treatment of ozone with NNK/DBP altered the cell cycle control through instability of the wild-type p53 gene. Such pivotal p53-mediated cell cycle alterations may be responsible for the toxicity observed under our experimental condition. These results may be applied to risk assessment of mixture-induced toxicity.
