Fine Needle Aspiration Cytology of Pulmonary Carcinosarcoma (폐에 발생한 암육종의 세침흡인 세포학적소견 -1 증례보고-)

  • Carcinsarcoma is an uncommon pulmonary malignancy characterized by carcinmatous parenchyma and sarcomatous stroma. The cytologic, immunohistochemical and ultrastructural features of a case of pulmonary carcinosarcoma suspected by fine needle aspiration cytology is presented. Only bizarre spindle cells arranged in loose groups, in microtissue fragments and in a dissociate fashion were present on the aspiration smears. They were markedly positive for vimentin. The epithelial component was not found, which was probably due to marked paucity of carcinomatous component that was proved by histologic examination of the resected tumor. The diagnosis of pulmonary carcinosarcoma should be conidered whenever poorly differentiated epithelial ceil groups with a malignant mesenchymal component set in a myxoid background are seen in a pulmonary cytology specimen.

  • The application of fine needle aspiration (FNA) cytology to the soft tissue tumors had been neglected. In recent years, however, FNA has been used increasingly in the preoperative diagnosis of these tumors due to its usefulness and accuracy. We present 3 cases of liposarcoma, myxoid, myxoid with round cell, and pleomorphic, diagnosed by FNA cytology with histologic confirmation. Good correlation between histologic and FNA cytologic findings was found. Although the cytologic appearances of liposarcomas varied with histologic type, the main criterion was the presence of atypical multivacuolated lipoblast with characteristically scalloped nuclei.

  • Bronchoalveolar lavage (BAL) has emerged as a useful technique for the study of pulmonary interstitial disorders. Several types of Information are provided by the evaluation of lavage fluid identification of cellular constituents helps to separate inflammatory process. Recently we have studied cellular constituents of BAL from three cases with histologically confirmed pulmonary sarcoidosis, idiopathic pulmonary fibrosis and hypereosinophilic syndrome. Pulmonary sarcoidosis showed a marked increase in lymphocytes, idiopathic pulmonary fibrosis revealed a predominance of neutrophils, and hypereosinophilic syndrome presented a marked increase in eosinophils in the lavage fluids.

  • We report a case of placental site trophoblastic tumor with cytologic features of cervico-vaginal smear. The smear revealed several loose clusters of atypicai cells in slightly hemorrhagic and inflammatory background. Tumor cells were large in size and polyhedral to round with abundant cytoplasm Nuclei revealed consider-able variation in size, shape, and degree of chromatin clumping. Cytologically, it was difficult to differentiate from squamous cell carcinoma of uterine cervix. Curettage and hysterectomy specimen revealed typical histologic features of placental site trophoblastic tumor composed of intermediate type trophoblasts.

  • Cryptococcosis is a systemic mycosis that most often involves the central nervous system, followed by subclinical or symptomatic pulmonary involvement. We describe a case of unsuspected pulmonary cryptococcosis in a 69-year-old woman, diagnosed by sputum cytology. Sputum smears revealed numerous yeast like organisms typical of Cryptococcus neoformans measuring from 7 to 17 um in diameter. Mucicarmine, PAS, and methenamine sliver stains showed positive reaction of capsule and cell wall of Cryptococcus neoformans.

  • Phyllodes tumor of the breast, especially malignant, is a rare tumor which has characteristic cytologic features. We recently experienced a case of fine needle aspiration cytology of malignant phyllodes tumor of the breast metastatic to the lung. The aspirate showed cellular smear with biphasic population of epithelial and stromal cells. Many individual large cells with round hyperchromatic nuclei were found in the necrotic background and stromal cell atypia was also present. Chondrosarcomatous fragments were also seen in cytologic specimens.

  • Warthin's tumor is a benign salivary gland tumor, occurring primarily in the parotid gland. It's incidence is 4% of all salivary gland tumors. The cytologic finding of Warthin's tumor is characteristic and it consists of sheets of oncocytes and scattered lymphocytes in the background of granular material. We reports three cases of Warthin's tumor showing characteristic cytologic findings. The patients consisted of two males and a female and their ages were 60, 73, and 69 years, respectively. All the lesions were in the parotid gland. The cytologic findings showed sheets of oncocytic epithelial cells and scattered lymphocytes in the back-ground of granular materials and many red blood cells. In one case, the back-ground was composed of many Inflammatory cells including lymphocytes, neutrophils and macrophages.

Fine Needle Aspiration Cytology of Parasitic Infestation in Soft Tissue (연부조직의 기생충 감염의 세포학적 소견)

  • In the past, parasitic diseases were a major problem in public health in Korea. In recent years, however, nematodiasis that used be prevalent are no longer a serious problem. Instead some cestodiasis, particularly cysticercosis and sparganosis have become comparatively more important in recent years. Parasitic infestation of soft tissue is presented as a subcutaneous nodule or mass with nonspecific clinical manifestations. We experienced 4 cases of parasitic infestation in the soft tissue diagnosed by fine needle aspiration (FNA). Three out of four cases were histologically confirmed, two out of four cases were cysticercosis and one case was confirmed as sparganosis. FNA may be useful in providing a diagnosis in cases of parasitic infestation.

  • A case of presacral chordoma in a 55-year-old male diagnosed by aspiration biopsy cytology Is reported. Cytologically, three cell types were recognized in a mucoid background. large, mononucleated or binucleated physaliphorous cells with vacuolated bubbly cytoplasm; small, uniform and rounded non-vacuolated cells; and cells with microvacuolated and plump cytoplasm. The diagnosis of chordoma was possible because typical radiological and cytomorphological features were supported by the results of special staining and immunohistochemical staining with the cell block specimen obtained from the fine needle aspiration.

  • Amyloid tumor is a tumorlike localized deposit of amyloid which is encountered occasionally in association with multiple myeloma and various chronic inflammatory diseases. This report describes a case of solitary amyloid tumor of the neck which was the presenting symptom arising in association with multiple myeloma. A 56-year-old woman complained of a palpable neck mass and fine needle aspiration was done. Multiple myeloma was diagnosed on the basis of the bone marrow biopsy and monoclonality of kappa light chain. The histologic and cytologic features of the amyloid appear to be characteristic and may allow a definitive diagnosis to be made on needle aspiration biopsy.
