The Effect of Infant Massage on Weight Gain, Physiological and Behavioral Responses in Premature Infants

  • Purpose. The purpose of this study was to evaluate the premature infants' responses to infant massage (tactile and kinesthetic stimulation). These responses measured by weight, physiological (vagal tone, heart rate, oxygen saturation) and behavioral responses (behavioral states, motor activities, and behavioral distress). Methods. This study was conducted using an equivalent control pretest-posttest design. The sample was divided into two groups of 13 infants with gestational age less than 36 weeks at birth, birth weight less than 2000g, and no congenital anomalies. The experimental group received the massage intervention twice daily for 10 days. The data were collected for 10 minutes prior to and 10 minutes after the massage. Results. The vagal tone was significantly higher after massage than before massage in the experimental group, while no change in the control group. The experimental group had significantly higher scores for awake state and motor activity than the control group. Significantly greater awake state, more fidgeting or crying, and increased motor activity were reported after massage than before massage. Conclusions. The results of this study showed that massage therapy might enhance optimal physiological responses and behavioral organization of premature infants. Nursing staff in the NICU can use massage to promote the infant's capability to respond positively to his environment and to provide developmental support for healthy premature infants.

The Masking Effects of Introducing Sounds about Road Traffic Noise (도로교통소음에 대한 도입 연출음의 마스킹 효과)

  • This paper describes a experimental study determining the effects of the type and loudness of introducing sounds on the amenity and the masking degree of background noise. 132 relative test sounds with 11 types of introducing sounds and 3 steps of road traffic noise were presented to 29 subjects through headphone in the laboratory experiment. Seven scale of polar adjective such as 'Strong Weak', 'Pleasant Unpleasant', 'Clear Unclear', 'Suitable Unsuitable', 'Comfortable-Discomfortable' was employed for the evaluation of amenity. And Seven scale of mono polar adjective such as 'Very-Not at all' was employed for the evaluation of masking degree against road traffic noise. The results are as follows; Green music was evaluated the highest amenity among test sounds and murmurs of a stream, signal music, song of a bird, crying of insect was showed the amenity rank of order. The softer and weaker introducing sound was evaluated more suitable and comfortable under the road traffic noise. So adequate level of introducing sounds was considered for the desirable urban soundscape. Also the louder sounds compared to background noises had greater masking effects. But similar levels of introducing sounds were recommended considering both good amenity and masking effects.

An occurrence of canine herpesvirus infection in Korea (Canine herpesvirus(CHV) 감염증의 자연발생예 관찰과 감염실험)

  • Five puppies, 14 days old in the same litter showed acute symptoms such as painful crying, anorexia, abdominal pain and depression with fatal terminations. Necropsy of a puppy revealed hemorrhagic and necrotic foci in the lungs and kidney. The histological lesions were characterized with fibro-necrotizing foci in the lungs, nonsuppurative encephalitis and intranuclear inclusions of liver cells. A cytopathogenic agent was isolated from the thoracic fluid of a dead puppy by cell culture with primary dog kidney cells. The puppies inoculated with the field isolate showed the same clinical signs and lesions as those of the spontaneous cases. Viral particles were observed in suspension of the isolated agent by electron microscopy. The primary dog kidney cells infected with the field isolate showed fluorescent foci against anti-CHV monoclonal antibody after FA stain. On these findings of the disease it was diagnosed as CHV infection. The report signifies the first description of an epizootic of CHV infection in Korea.

  • Jeon-zeong(癲證) is mostly caused by anxiety and melancholy, impairment of heart and the spleen, or stasis of phlegm-Gi and heart confused by phlegm. Its manifestation are characterized by depression, expressionlessness, inclination for quietness, self muttering and hallucinating, caprice in crying and laughing, paraphasia, loss of appetite, unawareness of filth and cleanness, etc. We experienced a 50year-old man who bad brief psycbotic disorder that specified with Marked Stressor and whose condition was improved through oriental medical treatment. We treated the patient with Herbal medications and Giungoroen (至言高論)-wise saying and lofty opinion) and Relaxation therapy. After being treated, the patient showed that symptoms (insomnia, anxiety, depression) was improved considerably. This result suggests that oriental medical treatment has good effect on brief psychotic disorder due to Marked Stressor.

Operative results of coarctation of the aorta associated with ventricular defect (심실중격결손을 동반한 대동맥교약증의 수술성적)

  • The optimal surgical management of the coarctation of the aorta associated with ventricular septal defect is still debated. Sixteen patients with the coarctation of the aorta and VSD were operated upon between November, 1980 and September 1984 at Seoul National University Hospital. They were 11 males and 5 females. All presented between 5 months and 11.5 years of age [mean= 5.5 years]. Presenting symptoms were congestive heart failure in 11 [69%], cyanosis on crying in 3 [19%], and frequent upper respiratory infection in 2 [13%]. In all cases two-stage operation was applied except one in which one stage procedure was taken. Resection and end-to-end anastomosis was used in 3, Dacron graft in 5, Gortex graft in 1, and left subclavian flap angioplasty in 4. Remaining two were missed on the operating table before correction of the coarctation of the aorta. Overall operative death in repair of the coarctation of the aorta were 3[20%]. Among the 12 survivors after repair of the coarctation of the aorta, 4 required patch closure of VSD, 2 required primary closure, 2 showed spontaneous closure [17%], one [8%] showed decrease in its size, 3 were under observation. It might be safe to approach the coarctation of aorta plus VSD with initial repair of the coarctation of the aorta without banding of main pulmonary artery and later management of VSD as usual manner in simple VSD.

  • In this study, existing remote control or the cradle manually drives to recognize the voice of the way and through the app the Cradle to work with a motor. In addition, the temperature and humidity sensor was mounted in the cradle, the temperature and humidity of the cradle can be checked through the LCD. Depending on the sound size of the sound sensor, the resulting value was used to indicate a value of a, b, c, and the sum of the results over 1150, the cradle was recognized as the baby's crying, then, notificate and alarm on app.

Apraxia of Phonation: a Case Report (발성실행증 사례연구)

  • Apraxia of phonation (AOP) has often been described as a feature of apraxia of speech or of severe non-fluent type of aphasia. Pure AOP is rare and, to our knowledge, only two cases have been reported. Brain lesion sites of the reported cases were not those sites known to be responsible for apraxia of speech. This study presents a case of AOP which resulted from the secondary stroke in the left corona radiata immediately following the first stroke in the left temporoparietal lobe. A 61-year old right-handed man shwoed a global type of aphasia after the first cerebral infarction, but was able to generate spontaneously some short fragments of speech. On the day after the first infarction, he suffered from the secondary infarction, leaving him a complete loss of voluntary phonation. He did not showed any significant change in language functions. Several occurrences of involuntary phonation were observed upon laughing or crying. He was also able to cough unintentionally. A vidoe-stroboscopic examination failed to reveal any evidence of structural and functional impairment in larynx. Although this case is not of pure form of AOP, AOP appeared after the secondary stroke without significant changes of language impairment since the first stroke. Therefore, AOP may be a consequence of the brain lesion from the secondary stroke even though we cannot rule out the possibility of an additive effect of the secondary to the first stroke.

The Changes in Children's Sleeping Patterns caused by Hospitalization (입원에 따른 아동의 수면 양상의 변화)

  • The purpose of this study was to identify the children's sleeping patterns, such as the sleeping hours and the nature of sleep disruptions following hospitalization and its accompanying factors. The data were collected from December, 1997 to March, 1998 using a questionnaire developed by researchers. The subjects were 76 children in a hospital. The results of this study were summarized as follows : 1. The average sleeping hours (sleep duration) at night were 10 hours and 10 minutes and 9 hours and 9 minutes before and after hospitalization respectively. There was a significant difference (p<0.01). The average sleeping hours in the day time were 1 hour 28 minutes and 2 hours and 26 minutes before and after hospitalization respectively. There was a significant difference(P<0.01). 2. The mean bed time(sleep onset) was 10: 22 pm and 10 : 28 pm before and after hospitalization respectively. There was no significant difference. 3. The mean hour of rising(sleep termination) was 7: 54 am 7 : 08 am before and after hospitalization respectively. There was a significant difference (p<0.01). 4. The mean number of sleep disruption was 0.72 and 1.94 before and after hospitalization respectively. There was a significant difference(P<0.01). The sleep disruptions were influenced by crying of other children(53.9%), lights(28.9), nursing procedures(18.4%), noise of TV(17.l%) and noise of visitors (15.8%).

A Study on the Reasons and the Meaning of Object Attachment Occurrence in Infancy and Early Childhood (유아의 대물애착현상의 발생원인 및 의미에 관한 연구)

  • The purpose of this study is to investigate the reasons of Object Attachment Occurrence in infant and the meanings of it in his early development. For this purpose, the following hypotheses were supposed. 1. Infant's object Attachment will be a substitutional for mother attachment when he has opportunities to his mother during his first years. 2. Infant's Object Attachment will not impede the infant's normal development. In order to test this hypotheses 28 mothers whose infants show or have shown this phenomenon and 28 mothers whose infants show or have shown this phenomenon and 28 mothers whose infants do not show this one were interviewed about their child rearing practice and attitude. They were selected by the random sampling method from 310 household wives living at a delux Apt. in Seoul. The results obtained in this study are as follow ; 1. The less opportunities infant has for 1) his mother's breast feeding 2) having physical contact with his mother 3) spending time with his mother 4) receiving prompt response from his mother to his crying, the more he is apt to show this phenomenon. And infant whose first main attachment figure is not his mother also shows the same apt. 2. There was no difference between two groups in intelligence and personality. So it can be concluded that infant's Object Attachment is a substitutional phenomenon for mother attachment in the mother during his first years, and it does not impede infant's normal development.

Corrosive Esophagitis with Benzalkonium Chloride in a Two Days Old Neonate

  • Benzalkonium chloride (BAC) is a caustic agent which is used in farms, homes and hospitals for cleaning skin and wounds as an antiseptic solution. It may lead to digestive system injuries in case of ingestion. We present a two-days-old newborn case which was carried to the emergency unit with complaints of poor breastfeeding, uneasiness and crying for 4-6 hours. Her mom confessed that she had given a spoon of 10% BAC solution for her cough. Initial laboratory tests were in normal ranges. A gastroscopy performed in the second hour of her admission revealed an hyperemic and edematous mucosa in the middle third of esophagus and a circumferential ulceration followed in the distal portion. Hereupon, a conservative treatment for 10 days was administered and the control gastroscopy demonstrated that the damage was almost totally improved. She was the youngest case with this etiology and successfully treated with conservative approach.
