자동차 조립 작업자들에서 상지 근골격계의 인간공학적 작업평가(Rapid Upper Limb Assessment) 결과와 자각증상과의 연관성 (The Relation Between Work-Related Musculoskeletal Symptoms and Rapid Upper Limb Assessment(RULA) among Vehicle Assembly Workers)

  • 본 연구는 Rapid Upper Limb Assessment(RULA)를 이용하여 자동차 조립작 업에 대한 인간공학적 작업평가를 하고, 이 결과와 작업관련 상지 근골격계 질환의 자각증상율 및 작업특성 변수들의 연관성을 분석함으로써, RULA결과가 관련 신체부위의 통증이나 불편함으로 보고되는 근골격계 부하의 좋은 지표가 될 수 있는지, RULA평가체계를 이용하여 작업 위험도가 적합한지 여부를 평가할 수 있는가를 보고자 하였다. 자동차 제조업 작업자 314명을 대상으로 근골격계질환 자각증상설문과 RULA를 이용한 작업평가를 하였으며, 다음과 같은 결과를 얻었다. 1. NIOSH의 작업관련 근골격계 질환의 감시기준(surveillence criteria)에 따라 자각증상을 조사한 결과, 연구대상자 중 목 32.8%, 어깨 26.4%, 팔 10.5%, 손 29.3%, 허리 41.4%였으며, 한가지 이상 의 증상을 보이는 경우는 62.4%의 분포를 보였다. 2. 품질관리부에 비해 조립부서 작업자 틀이 전체 증상과 어깨, 허리, 팔 및 손부 위 증상에서 더 높은 유증상률을 보였다(p<0.05). 3. 부서별 RULA를 이용한 작업평가 결과, 서브/조립반과 의장반의 공정이 높은 위험점수를 보였다. 전체 조립부서에 대한 평가결과 적합한 작업은 3.02%였고, 62.8%가 부적합한 작업으로 작업전환이나 중재조치가 필요한 경우였다. 자각증상과 RULA를 이용한 작업평가 결과를 비교한 결과 RULA점수가 클수록 자각증상율이 높음을 보였다.(p<0.05) 4. RULA평가지표와 자각증상사이의 관련성을 보기 위하여, RULA지표와 기타 작업관련요인들을 독립변수로, 자각증상유무를 종속변수로 하여 다중 로지스틱 회귀분석을 실시하였다. 최종적으로 증상유무를 가장 잘 설명하는 회귀모형에 대한 분석결과는 다음과 같다. 모든 증상군에 대해서는 반복작업(OR 2.183), 진동공구 사용(OR 2.775)과 총 위험도 점수(OR 2.250); 목 부위 증상군에서는 상완의 자세점수(OR 1.786), 상완 및 손부 위의 총점수(OR 1.634) ; 어깨 증상군은 상지의 근사용(OR 3.076), 상완 및 손부 위의 총점수(OR 1.798); 어깨와 목 부위를 합한 증상군은 상완 및 손부위의 총점 수(OR 1.715)와 상지의 근사용 점수(OR 2.057); 팔 증상군에서는 상지의 근사용 점수(OR 10.662) ; 손 부위 증상군에 대해서는 손목의 자세/손목 비틀림 지표 (OR 2.068)와 상지의 근사용 점수(OR 2.215); 허리부위 증상군에서는 하지의 근사용 점수(OR 2.601)가 통계적으로 유의한 지표였다.(p<0.05) 이상의 결과에서 작업관련 상지근골격계 자각증상과 RULA 점수사이에는 연관성이 있음이 관찰되었다. 이는 RULA가 직업성 질환을 유발할 수 있는 근골격계 부하에 폭로된 작업자들을 평가하는, 일차적인 작업 위험도 평가도구(Screening tool)로서 쓰일 수 있음을 말해준다. 다만 향후 RULA 사용에 있어서 상지(upper limb)와 허리부위(back)의 평가를 구분하고, 기타 다른 작업관련 요인들에 대한 평가체계를 보완하는 것이 필요하다.

일부 농촌과 도시의 건강선별조사 자료로 본 백혈구수와 고혈압과의 관계 (Leukocyte count and hypertension in the health screening data of some rural and urban residents)

  • 백혈구수와 고혈압과의 관계를 알아보기 위해 1985년에 일부 농촌 거주자와 1986년에 일부 도시 거주자를 대상으로 혈액학적인 검사와 장기별로 기능검사를 통해서 건강선별조사를 했던 자료를 이용했다. 농촌거주자는 1985년에 경상북도 경산군에서 남자 206명, 여자 203명을 마을, 통, 반을 기준으로 다단계 집락추출법으로 추출했다. 도시 거주 대상자는 1985년 12월에 대구시에 거주하고 있는 20세 이상의 주민의 성별, 연령별 구성비율에 부합되게 농촌과 같은 표본추출방법으로 600명을 선정했으나 600명중 384명만이 본 조사에 응해주어서 나머지 인원은 피보험대상자들의 정기 신체검사를 위해 대학병원 건강관리과에 내원한 사람들 중에서 성, 연령을 짝지어서 추출했다. 이들 자료에서 백혈구수 측정이 빠진 사람, 고혈압력을 포함한 질병 유소견자와 검사상 극도의 비정상적인 수치를 보이는 자들을 농촌 거주자에서 36명 (8.8%), 도시 거주자에서 29명 (4.8%)을 제외시켜서 최종분석에 이용된 자는 도시가 571명, 농촌이 373명이었다. 백혈구수 측정은 ELT-8 Laser shadow method 를 이용했으며 단위는 $cells/mm^3$ 였다. 혈압 측정은 Aneroid 혈압계를 사용해서 표준화한 후 측정했다. 고혈압자의 정의는 수축기혈압 140 mmHg/확장기혈압이 90 mmHg 이상인 자로 했다. 농촌과 도시를 합한 총대상자에대해 고혈압자와 정상 혈압자간의 백혈구수의 평균을 비교하면 정상 혈압자에서 $6490.61{\pm}1941.32$, 고혈압에서 $6965.93{\pm}1997.01$로서 고혈압자에서 백혈구수의 평균이 통계적으로 유의하게 더 높았으며 (P=0.00), 농촌에서 동일한 방향의 유의한 차이를 보였다 (P=0.03). 그 외의 거주지와 성별로 층화한 각 층에서는 유의한 평균차이를 보이는 결과는 없었다. 총대상자에서 백혈구수 1/5분위수를 기준으로 했을 때 2/5분위수의 비차비가 0.99 (95% confidence interval, CI 0.62-1.59), 3/5분위수가 1.41 (95% CI 0.90-2.21), 4/5분위수가 1.76(95% CI 1.14-2.72), 5/5분위수가 1.80 (95% CI 1.15-2.82)로서 백혈구수가 증가 함에따라 고혈압의 유병율이 일정하게 상승하는 것을 알 수 있었으며 경향분석에서도 유의한 경향($X^2_{trend}=5.53,\;df=1,\;P<0.05$)을 나타내었다. 나머지 층에서는 유의한 비차비도 없었으며 유의한 경향 역시 보이지 않았다. 그러나 거주지와 성별로 층화한 각 층에서 연령, 흡연과 음주량을 다중지수회귀분석으로 통제를 했을 때 단일분석시 통계적인 유의성을 보였던 전체 대상자의 비차비는 통계적으로 유의하지 않게 되었고 그 크기 역시 작아지고 불안정해졌다. 전체적으로 비차비의 크기가 조금씩 줄었으며 경향분석에서도 유의한 경향을 보인 층은 없었다. 이러한 사실은 백혈구수와 고혈압과의 가설을 처음으로 제시했던 Friedman등의 연구가 고혈압의 예측변수를 찾기위해 800여개의 변수와 고혈압발생과의 관계를 보는 과정에서 우연히 발견된 통계적인 1종오차의 결과 일지도 모른다는 것을 시사해주는 결과이다.

대학생의 라이프스타일 유형에 따른 신체계측, 식행동, 건강관련 생활습관 및 영양소 섭취상태에 관한 연구 (Anthropometric Measurement, Dietary Behaviors, Health-related Behaviors and Nutrient Intake According to Lifestyles of College Students)

  • 본 연구는 2003년에 행해진 연구이나 일반적으로 라이프스타일은 미시적 차원인 특정개인으로부터 거시적 차원인 사회전체에 이르기까지 여러 규모의 차원으로 파악되고 있으며 이는 개인의 전반적인 활동이나 인식에 영향을 주는 생활양식의 개념으로 여겨지는 점과 다른 선행연구들을 살펴볼 때 일반적으로 10년을 주기로 라이프스타일에 따른 변화를 살펴본 연구들이 많은 점으로 미루어 볼 때 본 연구 결과가 현재의 라이프스타일을 반영할 수 있는 것으로 사료된다. 본 연구에서는 전국 남녀 대학생의 라이프스타일에 따른 신체계측, 식행동, 건강관련 생활습관 및 영양섭취실태를 비교하고자 설문조사를 실시하였으며 조사결과는 다음과 같다. 조사대상자의 연령은 남학생 평균 23.7세, 여학생 평균 21.6세였다. 남학생의 경우, 체중은 '개방적 유행추구형'군이 64.1 kg으로 다른 군에 비해 유의적으로 낮은 것으로 조사되었으며, 여학생의 경우, 신장과 체중은 '성취적 가족추구형'군이 평균 160.9 cm, 50.7 kg으로 '소극적절약 외모지향형'군의 평균 162.7 cm, 52.7 kg과 '개방적 유행추구형'군의 평균 163.1 cm, 53.0 kg에 비해 유의적으로 낮은 것으로 조사되었다. 균형 잡힌 식사 여부에 관한 질문에서 "항상 균형 잡힌 식사를 한다"고 응답한 비율은 '적극적 건강추구형'군이 23.3%로 다른 군에 비해 유의적으로 높은 응답율을 보였다. 신용카드로부터 식비 지출금액의 경우 '개방적 유행추구형'군이 월 평균 23.7만원을 사용한데 비해 '비소비 대인관계추구형'군은 월평균 8.5만원을 식비로 지출한다고 응답하여 유의적인 차이를 보였다. 건강관련 생활습관과 관련하여 '적극적 건강추구형'군이 다른 군에 비해 운동량은 많은 반면 음주와 흡연은 적게 하는 것으로 조사되었다. 결식의 경우 '소극적절약 외모지향형'군이 유의적으로 가장 적은 결식율을 보였으나 라이프스타일 유형과 상관없이 모든 대학생들의 하루 중 3끼 이상 결식하는 경우가 80% 이상으로 나타나 대학생들의 올바른 식습관 형성을 위한 영양교육이 필요한 것으로 사료된다. 체중조절을 하는 이유로는 '비소비 대인관계추구형'의 16.2%가 "건강을 위해서"라고 응답한 반면 '소극적절약 외모지향형'과 '개방적 유행추구형'의 24.8%와 26.3%가 "외모를 위해서"라고 응답하여 유의적인 차이를 보였으나 체중조절 방법으로는 대부분이 "약물복용", "운동요법", "비만클리닉" 등의 순인 것으로 조사되었다. 라이프스타일 유형에 따른 영양소 섭취상태는 남학생의 경우 '개방적 유행추구형'군이 '적극적 건강추구형'과 '성취적 가족추구형'군에 비해 유의적으로 높은 비타민 C의 섭취량을 보였으며 여학생의 경우 '적극적 건강추구형'군이 '성취적 가족추구형'군에 비해 유의적으로 낮은 비타민 $B_2$, 칼슘 및 철분 섭취량을 보였다. 그러나 전반적으로 볼 때 라이프스타일에 관계없이 남녀 대학생 모두 에너지와 칼슘 섭취량이 한국인 영양섭취기준의 70% 수준으로 나타났으며 특히 여학생의 경우 철분의 섭취량이 한국인 영양섭취기준의 80% 수준으로 매우 낮은 섭취율을 보였다. 이상의 결과로 볼 때 경제발전과 사회구조의 변화에 따라 대학생들의 식행동, 건강관련 생활습관 및 영양소 섭취실태도 라이프스타일에 따라 다르게 나타나는 것을 볼 수 있다. 그러므로 대학생 시기의 올바른 라이프스타일의 확립 및 영양밀도가 높은 식품섭취를 통해 건강한 생활을 영위할 수 있도록 학교나 지역사회 및 정부차원에서의 지속적이고 체계적인 영양교육이 필요하다. 또한 일단 성립된 바람직하지 않은 라이프스타일은 쉽게 변화하지 않으므로 이론 위주의 영양교육보다는 실제적인 행동변화를 유도할 수 있는 적극적인 영양교육 프로그램의 개발이 필요한 것으로 사료된다.

  • The wall shear stress in the vicinity of end-to end anastomoses under steady flow conditions was measured using a flush-mounted hot-film anemometer(FMHFA) probe. The experimental measurements were in good agreement with numerical results except in flow with low Reynolds numbers. The wall shear stress increased proximal to the anastomosis in flow from the Penrose tubing (simulating an artery) to the PTFE: graft. In flow from the PTFE graft to the Penrose tubing, low wall shear stress was observed distal to the anastomosis. Abnormal distributions of wall shear stress in the vicinity of the anastomosis, resulting from the compliance mismatch between the graft and the host artery, might be an important factor of ANFH formation and the graft failure. The present study suggests a correlation between regions of the low wall shear stress and the development of anastomotic neointimal fibrous hyperplasia(ANPH) in end-to-end anastomoses. 30523 T00401030523 ^x Air pressure decay(APD) rate and ultrafiltration rate(UFR) tests were performed on new and saline rinsed dialyzers as well as those roused in patients several times. C-DAK 4000 (Cordis Dow) and CF IS-11 (Baxter Travenol) reused dialyzers obtained from the dialysis clinic were used in the present study. The new dialyzers exhibited a relatively flat APD, whereas saline rinsed and reused dialyzers showed considerable amount of decay. C-DAH dialyzers had a larger APD(11.70$\pm$1.32mmHg/min)compared to CF dialyzers(4.32$\pm$0.55mmHg/min)(p<0.05). However, there was no observable difference in the UFR between the two dialyzers. Neither APD nor UFR showed any significant increase with an increasing number of reuses for up to more than 20reuses. A substantial number of failures observed in APD(larger than 20mmHe/min)on the reused dialyzers(2 out of 40 CP and S out 26 C-DAK) were attributed to the Possible damage on the fibers. The CF 15-11 HFDs which failed APD test did not show changes in the UFR compared to normal dialyzers indicating that APD is a more sensitive test than UFR test to evaluate the integrity of the fibers. 30527 T00401030527 ^x For quantitative measurement of reflected light from a clinical diagnostic strip, a prototype old reflectance photometer was designed. The strip loader and cassette were made to obtain more accurate reflectance parameters. The strip was illuminated at 45˚c through optical fiber and the intensity of reflected light was determined at rectanguLat angle using a photodiode. The kubelka-munk coefficient and reflection optical density were determined ar four different wavelengths(500, 550, 570 and 610nm) for blood glucose strip. For higher concentration than 300mg/41 about glucose, a saturation state of abforbance was observed at 500, 550 and 570nm. The mice were divided into two groups. One group was injected with Photofrin II, intravenously (2.5mg/kg body weight). The other group received no Photofrin II. Mice from both groups were irradialed for approximately 15 minutes at 100, 300, or 500 mW/cm2 with the argon (488nm/514.5 nm), dye(628nm) and gold vapor (pulsed 628 nm) laser light. A photosensitizer behaved as an added absorber. Under our experimental conditions, the presence of Photolfrin II increased surface temperature by at least 40% and the temperature rise due to 300 mW/cm2 irradiation exceeded values for hyperthermia. Light and temperature distributions with depth were estimated by a computer model. The model demonstrated the influence of wavelength on the thermal process and proved to be a valuable tool to investigate internal temperature rise. 30536 T00401030536 ^x We investigated the structural geometry of thirty-eight Korean femurs. The purpose of this study is to identify major geometrical differences between Korean femurs 3nd others that we believe belong to Caucasians so that we would be able to get insights into the femoral component design that fits Asians including Koreans. We utilized computerized tomography (CT) images of femurs extracted from cadavers. The CT images were transformed into bitmap data by using a film scanner, and then analyzed by using a commercially available software called Image v.1.0 and a Macintosh IIci computer.The resulting data were compared with already published data. The major results show that the geometry of the Korean femurs is significantly different from that of Caucasians: (1) the anteversion angle and the canal flare index are greater by the amount of approximately 8˚ and 0.5, respectively, (2) the shape of the isthmus cross section is more round, and (3) the distance between the teaser trochanter and the proximal border of the isthmus is shelter by about 15 mm. The results suggested that the femoral component suitable for Asians should be different from the currently-used components designed and manufactured mostly by European or American companies. 30537 T00401030537 ^x It is well known that nonlinear propagation characteristics of the wave in the tissue may give very useful information for the medical diagnoisis. In this paper, a new method to detect nonlinear propagation characteristics of the internal vibration in the tissue for the low frequency mechanical vibration by using bispectral analysis is proposed. In the method, low frequency vibration of f0( = 100Hz) is applied on the surface of the object, and the waveform of the internal vibration x (t) is measured from Doppler frequency modulation of silmultaneously transmitted probing ultrasonic waves. Then, the bispectra of the signal x (t) at the frequencies (f0, f0) and (f0, 2f0) are calculated to estimate the nonlinear propagation characteristics as their magnitude ratio, w here since bispectrum is free from the gaussian additive noise we can get the value with high S/N. Basic experimental system is constructed by using 3.0 MHz probing ultrasonic waves and the several experiments are carried out for some phantoms. Our department has developed an information system for routine hematology using down-sized computer system. We have used an IBM 486 compatible PC with 16MB main memory, 210 MB hard disk drive, 9 RS-232C port and 24 pin dot printer. The operating system and database management system were SCO UNIX and SCO foxbase, respectively. For program development, we used Xbase language provided by SCO foxbase. The C language was used for interface purpose. To make the system use friendly, pull-down menu was used. The system connected to our hospital information system via application program interface (API), so the information related to patient and request details is automatically transmitted to our computer. Our system interfaced with fwd complete blood count analyzers(Sysmex NE-8000 and Coulter STKS) for unidirectional data tansmission from analyzer to computer. We propose a simple adaptive algorithm to increase capability of noise cancelation in QRS complex detection with two stage adaptive filter.   • The wall shear stress in the vicinity of end-to end anastomoses under steady flow conditions was measured using a flush-mounted hot-film anemometer(FMHFA) probe. The experimental measurements were in good agreement with numerical results except in flow with low Reynolds numbers. The wall shear stress increased proximal to the anastomosis in flow from the Penrose tubing (simulating an artery) to the PTFE: graft. In flow from the PTFE graft to the Penrose tubing, low wall shear stress was observed distal to the anastomosis. Abnormal distributions of wall shear stress in the vicinity of the anastomosis, resulting from the compliance mismatch between the graft and the host artery, might be an important factor of ANFH formation and the graft failure. The present study suggests a correlation between regions of the low wall shear stress and the development of anastomotic neointimal fibrous hyperplasia(ANPH) in end-to-end anastomoses. 30523 T00401030523 ^x Air pressure decay(APD) rate and ultrafiltration rate(UFR) tests were performed on new and saline rinsed dialyzers as well as those roused in patients several times. C-DAK 4000 (Cordis Dow) and CF IS-11 (Baxter Travenol) reused dialyzers obtained from the dialysis clinic were used in the present study. The new dialyzers exhibited a relatively flat APD, whereas saline rinsed and reused dialyzers showed considerable amount of decay. C-DAH dialyzers had a larger APD(11.70${\pm}$1.32mmHg/min)compared to CF dialyzers(4.32${\pm}$0.55mmHg/min)(p<0.05). However, there was no observable difference in the UFR between the two dialyzers. Neither APD nor UFR showed any significant increase with an increasing number of reuses for up to more than 20reuses. A substantial number of failures observed in APD(larger than 20mmHe/min)on the reused dialyzers(2 out of 40 CP and S out 26 C-DAK) were attributed to the Possible damage on the fibers. The purpose of this study is to identify major geometrical differences between Korean femurs 3nd others that we believe belong to Caucasians so that we would be able to get insights into the femoral component design that fits Asians including Koreans. We utilized computerized tomography (CT) images of femurs extracted from cadavers. The CT images were transformed into bitmap data by using a film scanner, and then analyzed by using a commercially available software called Image v.1.0 and a Macintosh IIci computer.The resulting data were compared with already published data. The major results show that the geometry of the Korean femurs is significantly different from that of Caucasians: (1) the anteversion angle and the canal flare index are greater by the amount of approximately 8˚ and 0.5, respectively, (2) the shape of the isthmus cross section is more round, and (3) the distance between the teaser trochanter and the proximal border of the isthmus is shelter by about 15 mm. Then, the bispectra of the signal x (t) at the frequencies (f0, f0) and (f0, 2f0) are calculated to estimate the nonlinear propagation characteristics as their magnitude ratio, w here since bispectrum is free from the gaussian additive noise we can get the value with high S/N. The procedure has the following three distinct stages. First, the initial center is estimated by the initial center estimation algorithm which is applied to decimated image. The main unit includes a 12.1" TFT color LCD, a main CPU board, and peripherals such as a module controller, Ethernet LAN card, video card, rotate/push button controller, etc. The main unit can connect at maximum three module cases each of which can accommodate up to 7 parameter modules. They include the modules for electrocardiograph, respiration, invasive blood pressure, noninvasive blood pressure, temperature, and SpO2 with Plethysmograph.

사회치위생학의 학습목표 제안: 구강보건행정 영역 (Suggestion of Learning Objectives in Social Dental Hygiene: Oral Health Administration Area)

  • 본 연구는 공중구강보건학 구강보건행정 영역의 국가시험 A항목 학습목표 48개의 '치위생 직무관련성', '치위생역량관련성', '교육목표 설정의 가치판별성', '시의성'을 검토하여, 최종으로 제안한 사회치위생학 구강보건행정 영역의 국가시험 A항목 학습목표는 총 75개였다. 전체 학습목표 중 18개를 삭제하였고, 15개를 수정보완하였으며, 기존 학습목표 15개를 유지하였고, 새로운 학습목표 45개를 추가하였다. 학습목표 주제는 I. 사회보장과 의료보장, II. 구강보건진료제도, III. 구강보건행정, IV. 구강보건정책으로 구분하여 고찰하였다. 최근 건강보험 및 노인장기요양보험의 확대 등 의료보장제도는 국가정책의 변화 등을 반영한 의료보장과 사회보장제도에 대한 내용을 제공할 수 있도록 수정보완이 필요하며 더 나아가 제도의 문제점과 해결방안을 제시할 수 있는 '옹호자'로서의 사회적 역할을 수행할 수 있도록 사회보장과 의료보장 분야의 학습목표를 개선할 것을 제안하였다. 구강보건진료제도와 구강보건행정 분야의 기존 학습목표는 치과위생사로서 현장의 직무와 관련성이 높은 개념과 내용으로 수정보완이 필요함을 제시하였고, 구강보건정책 분야는 치과위생사로서 정책에 참여하고 치위생 정책의제를 발굴할 수 있는 역량을 강화하여 변화주도자, 옹호자의 역할을 수행할 수 있도록 국내 보건의료정책, 치위생 관련 정책 이슈의 변화, 정책과정, 정책참여, 정책평가 등의 내용을 중점으로 학습목표를 신설할 것을 제안하였다. 본 연구결과, 변화하는 시대적 상황에 부응하여 사회치위생학 분야의 학습목표를 개정할 필요성이 제기되었다. 또한, 교육내용을 개편함에 있어 우선순위를 두어 개선하여야 한다는 것에 의견이 모아졌다. 첫째, 사회치위생학 학습목표를 개정하고, 실제 직무와 관련성이 높은 역량을 개발해야 할것이다. 둘째, 사회치위생학 학습내용은 지식, 태도, 행동을 향상시킬 수 있는 학습목표를 적극 개발해야 할 것이다. 셋째, 개정된 학습목표와 역량을 기준으로 사회치위생학 교재와 교육자료의 개발이 필요할 것이다. 넷째, 개정된 사회치위생학 학습목표를 바탕으로 치과위생사 국가시험을 개선해야 할 것이다. 이러한 교육의 변화를 통하여 치위생(학)계는 학습내용이 지식중심에서 그치는 것이 아니라 지식을 통해 다양한 활동으로 전환될 수 있도록 치과위생사의 역량을 강화하고, 치위생학 학문의 질적 수준을 향상시켜야 할 것이다. 따라서 사회에 진출 한 후 임상과 지역사회 현장에서 치과위생사로서 사회적 역할과 책임을 다할 수 있는 내실 있는 교육을 운영해야 할 것이다.
